本文是《PLoS Medicine》关于卫生系统知证政策指导三篇系列文章的第一篇,阐述了应如何将证据转化为指导,以形成卫生系统政策,并促进临床和公共卫生干预的实施。其要点如下:①薄弱的卫生系统阻碍了有效干预的实施,加强此系统的政策需要基于当前可得的最佳证据;②卫生系统证据最好以政策制定过程参考的指导的形式呈现,但目前卫生系统指导制定质量较差;③将问题、干预及实施相关的研究转化为影响卫生系统组织方式的决策和政策是卫生系统指导制定面临的一个挑战;④制定及时且适用于广大卫生系统利益相关者的指导,并确定评价卫生系统指导质量的方法是另一个挑战;⑤改编现有方法(如用于临床指南的那些方法)制定有意义的建议来应对卫生系统、政策系统及背景的复杂性,尚需进一步研究。

目的　评价中国人参对减少辐射诱导的人周围血 G0 期淋巴细胞 (PBL)微核量的作用。方法　采用细胞因子阻滞的微核法检测健康志愿者的血标本。体外 1 37Cs照射之前 ,每份血标本的单个核细胞培养以不同浓度 (0～ 2 0 0 0μg· m l- 1 )的人参根水提取物孵育 2 4 h。结果　 (1)在 0 Gy时 ,每 10 0 0个双核 (Bn )细胞的微核量是 11.3± 2 .7,人参水提取物≥ 5 0 0μg· ml- 1 时微核量下降 ,但微核量和人参水提取物的浓度之间的差异是显著的。 (2 )单独射线照射显著增加微核量 ,当暴露于 1Gy的照射时 ,随着培养介质中人参水提取物浓度的增加 ,微核量呈显著的线性降低 (R2 =0 .0 5 ,P =0 .0 0 2 ) ;对暴露于 1Gy辐射诱导的微核量 ,人参水提取物浓度在 15 0 0μg· m l- 1 时减少 4 6 .6 % ,浓度在 2 0 0 0μg·m l- 1时减少 5 7.6 % ;对暴露于 2 Gy辐射诱导的微核量的减少呈更强的线性相关 (R2 =0 .7,P=0 .0 0 0 1) ;(3)微核数据分析的结果呈现出与 Poisson模型相关的过度离散趋势。结果表明 :(1)人参水提取物浓度达 2 0 0 0 μg· ml- 1时 ,对 PBL 细胞毒作用无明显影响 ;(2 )人参水提取物浓度在 5 0 0 μg· ml- 1和 2 0 0 0 μg· ml- 1之间时 ,对抗 1 37Cs照射诱导的 Mn发挥保护作用。

Isotopic Composition of Waters in the Dem?novská dolina Valley and Its Underground Hydrologic System During Winter and Spring of 2010/2011

In the Dem?novská dolina Cave system（Slovakia） and its vicinity,32 sampling places for regular observation（in 2-months interval） of δ18O and δ2H in water were *** monitoring included precipitation waters,waters in the surface streams,waters of the underground hydrological system as well as the dripping seepage waters of the cave *** extent of the area was from 800 m a.s.l.（lowermost cave entrance） to 2024 m a.s.l.（Chopok *** the top of the crystalline range）.Initial results show some similarities but also differences within the analyzed water *** precipitation,a high variability of isotopic composition was confirmed,from quite depleted up to more enriched waters(δ18O from-16.8‰ up to-5.7‰;δ2H from-121.6‰ to-32.7‰).During the recharge process and groundwater/surface water formation,precipitation water is homogenized,what is reflected in much more stable isotope *** most depleted(δ18O ≈-11.7‰ to-10.8‰;δ2H ≈-78.9‰ to-73.4‰) were the waters of surface streams,running from the northern slopes of the nízke Tatry Mts.,formed by crystalline rocks,alochtonous to the underground hydrological *** autochtonous surface water streams（formed in the side valleys of the main karstic canyon） are slightly enriched(heavier,as δ18O ≈-11.4‰ to-10.6‰;δ2H ≈-78.3‰ to-71.5‰),what reflects lower altitudes of their *** is the distribution of the isotope content of the underground streams in the cave *** most depleted are the underground streams directly（visibly） communicating with surface waters(δ18O≈-11.33±0.13‰;δ2H≈-76.88±1.68‰).Extent of the relationship of underground streams to the autochtonous seepage waters（slow circulation through the fissures） is manifested by respective portion of isotopically enriched waters–as the underground streams show different isotope *** combination of the alochtonous water components（from surface streams reaching the karstic area from the adjacent crystalline via swa

目的：短暂性脑缺血发作（TIA）是常见和重要的卒中先兆。其处理方法多样，大多数发表的指南在近年来均未被更新。我们试图制定一个全面的、无偏倚的、循证的TIA处理指南。方法：根据一种客观标准（可以预测在该研究领域中执业医师对专家提名的文献测量学方法）挑选出15名专家组成员。通过系统回顾检索到过去发表的指南，由专家对其中的推荐意见的质量进行独立评价。选择出质量最高的推荐意见，然后让专家组采用改良Delphi法通过多次问卷调查的方式进行修订，从而对新的修改达成共识。给专家们提供近期临床研究的系统评价，要求专家根据新的证据判断措辞修改的合理性并且根据证据等级和质量对最终的推荐意见进行评定。不允许专家在预期有可能存在任何利益冲突的专题方面提出推荐意见。结果：通过系统回顾检索到257个指南，其中有13篇文献包括的137条推荐意见符合所有纳入标准。需要6次重复的问卷调查对53条最终推荐意见的措辞达成共识。最终的推荐意见涉及TIA的初步处理、评价、内科治疗、外科治疗和危险因素控制。结论：对TIA患者医疗诊治的最终推荐意见强调了紧急评价和治疗的重要性。这种用于制定本指南的新方法是可行的，考虑到了快速更新并能减少偏倚。

Importance:In remote communities of the northern Territory,Australia,children experience high rates of otitis media(OM),commonly caused by non-typeable Haemophilus influenzae(nTHi).Few data exist on antibiotic susceptibility of nTHi from ***:To determine whether population-level nasopharyngeal nTHi antibiotic susceptibility data could inform antibiotic treatment for ***:nTHi isolates(n=92)collected from ear discharge between 2003 and 2013 were selected to time-and age-match nTHi isolates from the nasopharyngeal carriage(n=95).Antimicrobial susceptibility were *** trees and a genome-wide association study(GWAS)were performed to determine the similarity of nasopharyngeal and ear isolates at a population ***:Among 174 nTHi isolates available for antimicrobial susceptibility testing,10.3%(18/174)were resistant to ampicillin and 9.2%(16/174)were resistant to *** numbers of isolates(≤3)were resistant to tetracycline,chloramphenicol,or amoxicillin-clavulanic *** was no statistical difference in the proportion of ampicillin-resistant(P=0.11)or trimethoprim-sulfamethoxazole-resistant isolates(P=0.70)between ear discharge and nasopharynx-derived nTHi *** multi-drug resistant nTHi isolates were *** trees showed no clustering of 187 Haemophilus influenzae isolates based on anatomical niche(nasopharynx or ear discharge),and no genetic variations that distinguished nTHi derived from ear discharge and nasopharyngeal carriage were evident in the ***:In this population-level study,nasopharyngeal and ear discharge isolates did not represent distinct microbial *** results support tracking of population-level nasopharyngeal nTHi antibiotic resistance patterns to inform clinical management of OM in this population.

The Tribbles(TRIB) family of pseudokinase proteins has been shown to play key roles in cell cycle, metabolic diseases, chronic inflammatory disease, and cancer development. A better understanding of the mechanisms of TRIB pseudokinases could provide new insights for disease development and help promote TRIB proteins as novel therapeutic targets for drug discovery. At the 2 nd International Symposium on Tribbles and Diseases held on May 7–9, 2018 in Beijing, China, a group of leading Tribbles scientists reported their findings and ongoing studies about the effects of the different TRIB proteins in the areas of immunity, metabolism, fundamental cell biology and cancer. Here, we summarize important and insightful overviews from 4 keynote lectures, 13 plenary lectures and 8 short talks that took place during this meeting. These findings may offer new insights for the understanding of the roles of TRIB pseudokinases in the development of various diseases.
