The carbonate lithofacies of the Nganzi Exploratory Block located in the Lukula Territory in the Central Kongo Province were described in the field, sampled and subjected to microscopic, mineralogical, physico-chemical and petrophysical analyses. The aim is to increase the probability of discovery of deposits following the drilling of exploration wells in the most appropriate structures. These carbonates, limestones (white, greyish and greenish) and dolomites (massive and platelets) are presented in packstone, grainstone and wackestone textures. In addition to the essential minerals of these rocks, calcite and dolomite, we find quartz, plagioclase and kaolinite. With a porosity between 11% and 19% and a permeability of 138 to 155 mD, the Nganzi carbonates, which contain up to 23% oil, constitute good oil reservoirs. The characterization of the detrital facies and the acquisition of a seismic survey of the wells is essential to have a more detailed knowledge of the oil resources of the Nganzi Block.

Background： Plasmodium falciparum malaria remains a major life-threatening disease. Recently, the Plasmodium apoptosis-linked pathogenicity factors （PALPF） have been identified. These antigens PALPF are expressed only by P falciparum-infected erythrocytes triggering endothelial cell apoptosis （apoptogenic）. Methods： We designed ten synthetic peptides （PI to P10） from PALPF： PF07 0032, PF10_0226, PFI0130c, PFD0875c and MAL13P1.206, and analyzed their antigenicity with an ELISA method using plasma samples from subjects living in Dienga, Gabon. Results： Four peptides showed good reactivity with human antibodies. The prevalence rate of specific IgG was 61%, 51%, 44% and 34% for P5, P6, P4 and P2, respectively. The median optical density of total IgG anti-P2 was higher than that directed against P4 and P6 （P = 0.009; P = 0.012 respectively）. The prevalence rate oflgG subclasses determined with plasma samples recognizing peptide 5 for IgGl, 2, 3 and 4 isotypes was 69%, 45%, 76% and 62%, respectively. All the subjects had at least one immunoglobulin subclass, while 13 （44%） had both IgG1 and IgG3 antibodies. There was no significant difference in the prevalence rate of anti-P5 IgG1, IgG3 and IgG4. Conclusion： These results warrant further immunogenicity studies of peptides 2, 4, 5 and 6 with a view of a tentative to antimalarial vaccine development.

目的为更新1999年法国共识"麻醉中肌肉松弛药与逆转剂的应用"特召集16名专家成立共识委员会。编写开始时宣布庄重的利益声明与分歧监督原则,并在编写过程中予以执行,本指南编写全过程不依赖任何商业资助。所有作者均需遵循推荐分级的评价、制定与评估系统的原则(Grading of Recommendations,Assessment,Development and Evaluation,GRADE?),以评估这些推荐意见的证据质量,规避基于低质量证据做出强推荐的可能性。故有少数推荐意见仍未分等级。方法专家组的讨论主要集中在以下8个问题:(1)在无面罩通气困难标准的情况下,是否有必要在注射肌肉松弛药前检查面罩通气的可行性?面罩通气是否有必要使用肌肉松弛药?(2)为便于气管插管是否有必要使用肌肉松弛药?(3)是否有必要使用肌肉松弛药以便于声门上气道装置的置入及管理相关并发症?(4)是否有必要应用神经肌肉阻滞监测来进行气道管理?(5)是否有必要应用肌肉松弛药以利于手术的进行?如有必要,是哪种手术?(6)术中是否有必要监测神经肌肉阻滞程度?(7)防治肌肉松弛药残留阻滞作用的策略是什么?(8)在特殊人群中(如电休克治疗、肥胖、儿童、神经肌肉疾病、肝/肾功能衰竭、老年患者)使用肌肉松弛药和逆转剂的适应证及注意事项是什么?所有问题都采用研究对象、干预、对照和预后(Population,Intervention,Comparison and Outcome,PICO)模型中的临床问题予以阐明,并生成证据档案,最后采用GRADE?系统评估文献分析的结果并进行推荐。结果SFAR指南专家组提出了31项关于麻醉中应用肌肉松弛药与逆转剂的推荐意见。在这些推荐意见中,有11项是属于强推荐(GRADE 1±),有20项是属于弱推荐(GRADE 2±),有5项应用GRADE?系统无法给出推荐意见,其中2个问题生成了专家意见。经过2轮讨论和1次修订后,所有指南和方案都获得了专家组的一致同意。结论关于麻醉中应用肌肉松弛药与逆转剂的多项强推荐意见,专家间意见基本一致。SFAR推荐在整个麻醉过程中使用神经肌肉功能监测仪来监测神经肌肉阻滞程度。

To establish consensual definitions of anoperineal lesions of Crohn’s (APLOC) disease and assess interobserver agreement on their diagnosis between experts. METHODSA database of digitally recorded pictures of APLOC was examined by a coordinating group who selected two series of 20 pictures illustrating the various aspects of APLOC. A reading group comprised: eight experts from the Société Nationale Française de Colo Proctologie group of study and research in proctology and one academic dermatologist. All members of the coordinating and reading groups participated in dedicated meetings. The coordinating group initially conducted a literature review to analyse verbatim descriptions used to evaluate APLOC. The study included two phases: establishment of consensual definitions using a formal consensus method and later assessment of interobserver agreement on the diagnosis of APLOC using photos of APLOC, a standardised questionnaire and Fleiss’s kappa test or descriptive statistics. RESULTSTerms used in literature to evaluate visible APLOC did not include precise definitions or reference to definitions. Most of the expert reports on the first set of photos agreed with the main diagnosis but their verbatim reporting contained substantial variation. The definitions of ulceration (entity, depth, extension), anal skin tags (entity, inflammatory activity, ulcerated aspect), fistula (complexity, quality of drainage, inflammatory activity of external openings), perianal skin lesions (abscess, papules, edema, erythema) and anoperineal scars were validated. For fistulae, they decided to follow the American Gastroenterology Association’s guidelines definitions. The diagnosis of ulceration (κ = 0.70), fistulae (κ = 0.75), inflammatory activity of external fistula openings (86.6% agreement), abscesses (84.6% agreement) and erythema (100% agreement) achieved a substantial degree of interobserver reproducibility. AIM: To assess whether a liver specific nitric oxide (NO) donor (V-PYRRO/NO) would prevent the development of portal hypertension and liver fibrosis in rats with bile duct ligation (BDL). METHODS: Treatment (placebo or V-PYRRO/NO 0.53 μmol/kg per hour) was administered i.v. to rats 2 d before BDL (D-2) and maintained until the day of hemodynamic measurement (D26). Intra-hepatic NO level was estimated by measuring liver cGMP level. Effects of V-PYRRO/NO on liver fibrosis and lipid peroxidation were also assessed. RESULTS: Compared to placebo treatment, V-PYRRO/ NO improved splanchnic hemodynamics in BDL rats: portal pressure was significantly reduced by 27% (P < 0.0001) and collateral circulation development was almost completely blocked (splenorenal shunt blood flow by 74%, P = 0.007). Moreover, V-PYRRO/NO significantly prevented liver fibrosis development in BDL rats (by 30% in hepatic hydroxyproline content and 31% in the area of fibrosis, P < 0.0001 respectively), this effect being probably due to a decrease in lipid peroxidation by 44% in the hepatic malondialdehyde level (P = 0.007). Interestingly, we observed a significant and expected increase in liver cGMP, without any systemic hemodynamic effects (mean arterial pressure, vascular systemic resistance and cardiac output) in both sham- operated and BDL rats treated with V-PYRRO/NO. This result is in accordance with studies on V-PYRRO/NO metabolism showing a specific release of NO in the *** in BDL rats improved liver fibrosis and splanchnic hemodynamics without any noxious systemic hemo- dynamic effects.
