Dexamethasone intravitreal implant(Ozurdex) in diabetic macular edema: real-world data versus clinical trials outcomes

AIm: To investigate the safety and efficacy of intravitreal dexamethasone implants(Ozurdex?/DEX) in patients with diabetic macular edema(DmE) either na?ve or nonna?ve to anti-VEGF therapies who switched to DEX implant independent of response to anti-vascular endothelial growth factors(anti-VEGFs).mETHODs: This was an audit retrospective review of medical records of patients with DmE who switched to the DEX intravitreal implant. Patients were divided into 2 groups: patients na?ve to antiangiogenic therapy and patients who were previously treated with anti-VEGFs. Data regarding demographics, changes in mean best-corrected visual acuity(BCVA), central macular thickness(CmT), and intraocular pressure(IOP) was collected over 6 mo. The demographic data mean changes in BCVA, CmT, and IOP were compared. six-month follow-up data of 47 patients(57 eyes), who either switched to DEX implant irrespective of response to previous treatments or were treatment na?ve before receiving DEX implant, was ***: Improvement in mean BCVA was observed from 1-4 mo after injection with a decreased effect at month 6 as expected, with better outcomes in na?ve compared to non-na?ve patients. A statistically relevant decrease in mean CmT was observed during the follow-up period. An increase in mean IOP was observed in the first 2 mo after DEX therapy. The mean number of injections of the overall population during the 6 mo was 1.3. A subgroup analysis showed no relevant difference between phakic versus pseudophakic patients relative to measured outcomes. There was no cataract progression during the follow-up period and no adverse events ***: This real-life setting study shows that intravitreal DEX implant is effective and safe. The timings of greater therapeutic impact are concordant with previous studies and suggest that earlier treatment with corticosteroids may have an additional benefit in na?ve patients.

Colorectal cancer represents the third most diagnosed malignancy in the *** liver is the main site of metastatic disease,affected in 30%of patients with newly diagnosed *** resection is considered the only potentially curative treatment for colorectal liver metastasis(CRLm),with a 5-year survival rate ranging from 35%to 58%.However,up to 80%of patients have initially unresectable disease,due to extrahepatic disease or bilobar multiple liver *** availability of increasingly effective systemic chemotherapy has contributed to converting patients with initially unresectable liver metastases to resectable disease,improving long-term outcomes,and accessing tumor *** recent years,response to preoperative systemic chemotherapy before liver resection has been established as a major prognostic *** studies have demonstrated that patients with regression of hepatic metastases while on chemotherapy have improved outcomes when compared to patients with stabilization or progression of the *** if disease progression during chemotherapy represents an independent negative prognostic factor,some patients may still benefit from surgery,given the role of this modality as the main treatment with curative intent for patients with *** selected cases,based on size,the number of lesions,and tumor markers,surgery may be offered despite the less favorable prognosis and as an option for non-chemo responders.

The main purpose of this paper is to implement a system capable of detecting faults in railway point mechanisms. This is achieved by developing an algorithm that takes advantage of three empirical criteria simultaneously capable of detecting faults from records of measurements of force against time. The system is dynamic in several respects： the base reference data is computed using all the curves free from faults as they are encountered in the experimental data; the algorithm that uses the three criteria simultaneously may be applied in on-line situations as each new data point becomes available; and recursive algorithms are applied to filter noise from the raw data in an automatic way. Encouraging results are found in practice when the system is applied to a number of experiments carried out by an industrial sponsor.

This study aims at investigating the effects of foliar application of aqueous solutions of La3+and Ce3+nitrates on soybean plants(Glycine max).First,we observe that Triton HW 1000 surfactant at 0.01 vol%reduces droplets contact angle and increases their drying *** greenhouse conditions,the foliar treatments do not affect chlorophyll content,plant height,number of leaves,number of pods,number of seeds per pod,and average seed ***,the treatments induce phytotoxicity since foliar injuries appear after the *** X-ray fluorescence spectroscopy combined to scanning electron microscopy shows that the leaf lesions are positively correlated to accumulation of Ce and La on the leaf surface and also promote structural alteration to the epidermal cells.X-ray absorption near edge structure shows that the La and Ce nitrates are partially bio transformed into oxides by the leaves which might explain the harmful effects.

In this report,we present a systematic study on the preparation of Co:ZnO ceramics via a standard solid-state route from different Co precursors(Co_(3)O_(4),CoO,and metallic Co)and atmospheres(O_(2)and Ar).Particular emphasis is given to defect chemistry engineering and the sintering growth ***-principles calculations based on density functional theory are employed to determine the formation en-ergy of the main point defects in ZnO and Co:ZnO *** on the theoretical results,a set of chemical reactions is *** microstructural characterization is also performed to determine the degree of Co incorporation into the ZnO *** samples prepared in Ar atmosphere and metal-lic Co present the highest Co solubility limit(lower apparent Co incorporation activation energy)due to the incongruent ZnO *** of the parameters of the sintering growth kinetics reveals that Co_(3)O_(4)is the best sintering additive to achieve larger grain sizes,and possible higher densi-ties,in both sintering atmospheres,while metallic Co is the best to achieve the smallest grain size with higher Co concentration and homogeneous spatial distribution for a subsequent reduction of *** results show that the sintering in O_(2)effectively promotes zinc vacancies in the ZnO structure,while the sintering in Ar promotes zinc interstitial *** findings contribute to understanding the preparation of Co-doped ZnO ceramics and the sintering growth kinetics,which may improve the state of the art in processing the material at both bulk and nanometric scales.

Cuba has been leading marine protected area(mPA)designation in the Caribbean region to ensure conservation of its valuable marine ***,an efficient monitoring program for mPAs is still to be implemented and will benefit from pre-existing *** highly regulated mPA of Punta Francés National Park is one of the first Cuban mPAs and was established mainly to protect species and ecosystems for tourism *** of protection effects on commercial fish species was lacking in this *** study aimed at increasing local scientific knowledge by providing a baseline study about the most commercially fished families(Haemulidae,Lutjanidae and serranidae)around Punta Francés *** collected represent only a limited period but can be used as a base point to support future *** abundance in number and biomass,and size were collected to test for differences between inside and adjacent areas outside the Punta Francés mPA,in different coral reef *** main result of this study was the significantly larger size and biomass of snappers and groupers outside the mPA where intense fisheries *** observation consistent with a large spawning aggregation was also recorded outside the *** with a limited set of data,these results suggest that,at least temporarily,the most targeted species[and sizes]are still highly vulnerable to *** habitats in the adjacent area,that are apparently missing within the mPA,may support some of the results *** the limits of the Punta Francés mPA to include those habitats outside may be critical to ensure the effectiveness of this mPA in contributing to protect the most commercial species of the *** should be done together with efficient fisheries management measures in the region,such as the significantly increase of minimum legal sizes and temporal closures during spawning migrations.
