我出生在美国,父亲在美行医。古巴革命胜利后,我的家人于1962年回到了古巴。我们全家都希望为创造一个新的、更公正的社会作出贡献。20世纪80年代末,古巴神经科学中心与中国建立合作20世纪60年代末,作为一名医学生,我与纽约大学的Erwin Roy John教授一起,利用古巴的第一台计算机(1970年)开创了量化脑电图(qEEG)技术。时至今日,我们的目标仍是创造一种在群体层面协助诊断和管理大脑疾病的工具。1977年,我们在《自然》上发表了一篇神经计量学的论文,这即是我们本次合作的成果。我需要继续强化医学训练,这促使我从医学专业毕业后开始学习数学。后来,我领导的团队为世界上首批qEEG设备之一,即MEDICID系统构建了硬件和软件。

Absence epilepsy is believed to be highly associated with the abnormal interactions between the cerebral cortex and *** the direct coupling,anatomical evidence indicates that the cerebral cortex and thalamus also communicate indirectly through an important intermediate bridge-basal *** has been thus postulated that the basal ganglia might play key roles in the modulation of absence seizures,but the relevant biophysical mechanisms still not be completely *** a computational model(see Figure 1),we demonstrate here that the typical absence seizure activities can be controlled and modulated by the direct GABAergic projections from the substantia nigra pars reticulata(SNr)to either the thalamic reticular nucleus(TRN)or the specific relay nuclei(SRN)of thalamus,through different biophysical *** certain conditions,these two types of seizure control are observed to coexist in the same *** importantly,due to the competition between the inhibitory SNr-TRN and SNr-SRN pathways,we find that both decreasing and increasing the activation of SNr neurons from the normal level may considerably suppress the generation of typical 2-4 Hz spike-and-slow wave discharges in the coexistence region(see Figure 2).Taken together,these results highlight the bidirectional functional roles of basal ganglia in controlling and modulating absence seizures,and might provide novel insights into the therapeutic treatments of this brain disorder.

The N400 component is commonly associated with the detection of linguistic incongruity. A few studies have shown that the N400 can also be elicited by non-linguistic stimuli. Different spatiotemporal patterns were observed between the typical Linguistic N400 and the Non-linguistic N400, suggesting distinct brain generators. The aim of this study was to investigate the presence of an N400 in response to linguistic and non-linguistic stimuli, and to specify anatomical sources of both N400s using a novel analysis method: the Bayesian Model Averaging (BMA) distributed source model. Picture-word and environmental soundpicture associations, either congruent or incongruent, were presented to ten young healthy adults while highdensity ERP recordings were made. Standard electrophysiological analyses confirmed that the N400 was not specific to linguistic incongruity but was also elicited by environmental sound-picture incongruities. Different topographic distributions were obtained for the Linguistic N400 and Non-linguistic N400. BMA analysis showed that the Linguistic N400 generators were mostly located in the left superior temporal gyrus, whereas the sources of the Non-linguistic N400 were identified mostly in the right middle and superior temporal gyri. Detection of linguistic incongruities recruited cerebral areas commonly associated with language processing, whereas non-linguistic incongruities recruited right cerebral regions usually associated with auditory processing of non-linguistic stimuli. The Linguistic and Non-linguistic N400s appear to be elicited by similar cognitive mechanisms assumed by different cerebral areas depending on the type of material to be processed. The present findings support the existence of parallel pathways for the processing of linguistic and non-linguistic incongruities.

Background: Leptospirosis is an important zooanthroponotic disease spread worldwide which infection is recognized as a re-emergent disease. Leptospirosis is a systemic disease of humans and domestic animals, mainly dogs, cattle and swine, characterized by fever, renal and hepatic insufficiency, pulmonary manifestations and reproductive failure. Objective: To study the seroprevalence of anti-Leptospira antibodies in an urban canine population. Materials and Methods: The study was performed in March 2014 in stray dogs that had been captured in the districts of Coyoacan, Iztapalapa, Iztacalco and Benito Juarez in Mexico City;117 dogs were analyzed, from each of which 5 ml of blood were obtained. Serology was performed using a MAT test (Microscopic Agglutination Test), finding that the antigens corresponded to 13 serotypes of Leptospirainterrogans. Three age groups were formed: Group 1) younger than a year (n = 28), Group 2) 1 to 6 years (n = 75), and Group 3) older than 6 years (n = 14). Results: Of the analyzed sera, 28.2% were seropositive to one or more Leptospira serotypes, 74% of the positive seracoagglutinated with two or more serotypes. The most frequent serotypes were: Canicola, Icterohaemorrhagiae and Portland-vere. Serotypes Canicola, Pyrogenes, and Bratislava resulted statistically significant (p 0.001). The presence of anti-Leptospira antibodies in the blood of animals of 1 to 6 years resulted with a p 0.001. The sera from Iztapalapa reacted to nine serotypes. Conclusion: The 28.2% of seropositivity indicates Leptospira transmission in the canine population that was studied could exhibit a potential public health risk.

Introduction: To perform a Latin-American multicentric study for the prediction of benign and malignant thyroid nodules using Alpha Score, and to compare it with ACR TIRADS® and Bethesda®. Materials and Methods: A prospective multicentric study in 10 radiological hospitals and institutions of Latin America was performed and 818 thyroid nodules were analyzed by ultrasound and classified by using both ACR TIRADS® and Alpha Score;fine-needle aspiration biopsy was performed when needed and classified with Bethesda. The relationships between predictors were analyzed by using binary logistic regression, statistical significance was defined by a p-value of 0.05, with an error margin of 4% and 95% confidence intervals. Results: Alpha Score 2.0 establishes five types of malignant predictors: microcalcifications, irregular borders, taller-than-wide shape, predominant solid texture and hypoechogenicity;a diameter equal to or greater than 1.5 cm adds an extra point to the final score. Resulting classification divides TNs into 4 categories: benign (1.9%), low suspicion (8.7%), mild suspicion (13.6%) and high suspicion (75.7%) of malignancy probability;sensitivity of 82%, specificity of 74%, the positive predictive value of 94%, the negative predictive value of 51%, the statistical accuracy of 81%, odds ratio of 108.89 and correlation with ACR TIRADS of 0.77 and Bethesda of 0.91. Conclusions: Alpha Score 2.0 has superior diagnostic accuracy and performance compared to the previously published Alpha Score and is able to classify a benign TN in a precise, safe and accurate way, avoiding unnecessary FNABs or determining the necessity of FNAB in cases of moderate to high suspicion of malignancy.

Background: This document includes recommendations and guidelines issued by a group of Mexican researchers and specialists gathered in the First National Colloquium for the Diagnosis and Management of Chronic Myeloid Leukaemia (CML) by initiative of Instituto Nacional de Cancerología and with the support of the Leukaemia Department of the MD Anderson Cancer Center. Mexico lacks of updated information taken from its own reality on the diagnosis and treatment of CML and other haematological disorders;besides, there are no national guidelines. Aim: To publish a consensus document with guidelines for the management of CML adjusted to the national environment and overall characteristics. Method: The participants answered a DELPHI questionnaire about the overall aspects of the disease, aiming to target controversial topics, discuss them in the colloquium, and to agree on the best ones. After those meetings, a final document was drawn up. Results: The group presents recommendations for definition, diagnosis, prognosis, monitoring, and treatment of CML in Mexico. Conclusions: Having consensus guidelines for the clinical management of CML in our country will enable the consensual practice of Mexican specialists regarding the clinical approach to CML, as well as optimize the resources which allow the rational planning of the medical care strategies.
