地点:巴西东南部3个分枝杆菌参比实验室。目的:评估自动化分枝杆菌生长指示管(MGIT)检测结核分枝杆菌药物敏感性的能力。设计:用自动BECTEC MGIT 960(M960)对95份结核分枝杆菌临床分离株进行链霉素(SM),异烟肼(INH)、利福平(rMP)、乙胺丁醇(EMB)的敏感性检测,并与BACTEC 460 (B460)、比例法(PM)、抗性比率法(rrM)的检测结果进行比较,其中88株以BACTEC 460、比例法、抗性比率法中的至少两种方法获得一致结果作为最终结果,并与M960的结果相比较。结果:M960与B460的符合率为95.2%,与PM为96.6%,与rrM为93.4%;其中与rrM在SM上符合率最低,与B460在EMB上符合率最低。M960结果与最终结果的符合率为97.9%, INH与rMP的符合率为99.2%,SM与EMB的符合率分别为96.2%和96.9%,M960报告结果的平均时间为6.9d。结论:与比例法和抗性比率法相比,M960显示出了极大的优势,并且有助于患者的治疗。

本研究的目的是应用超声弹性成像显示乳腺肿块并分析其表现，尤其是乳腺纤维腺瘤。本研究最初包括235例病人共计302个乳腺病变，均经针吸活检取得病理证实。其中270个病变在常规超声检查中呈实质性肿块，作为本研究资料。其中115个（42．6％）组织学证实为纤维腺瘤，155个（57．4％）为非纤维腺瘤。这些肿块又被进一步分为纤维囊性病变、潜在低度恶性病变和恶性病变。纤维腺瘤也根据组织学表现分为3个亚型，以便根据弹性成像评分进行对照分析。

AIM:To study if impaired renal function is associated with increased risk of peri-infarct heart failure (HF) in patients with preserved ejection fraction (EF).METHoDS:Patients with occluded infarct-related arteries (IrAs) between 1 to 28 d after myocardial infarction (MI) were grouped into chronic kidney disease (CKD) stages based on estimated glomerular filtration rate (eGFr).rates of early post-MI HF were compared among eGFr *** regression was used to explore independent predictors of ***:reduced eGFr was present in 71.1% of 2160 patients,with significant renal impairment (eGFr l/min every 1.73 m2) in 14.8%.The prevalence of HF was higher with worsening renal function:15.5%,17.8% and 29.4% in patients with CKD stages 1,2 and 3 or 4,respectively (P ll absolute difference in mean EF across eGFr groups:48.2 ± 10.0,47.9 ± 11.3 and 46.2 ± 12.1,respectively (P=0.02).The prevalence of HF was again higher with worsening renal function among patients with preserved EF:10.1%,13.6% and 23.6% (P relationship was not significant among patients with depressed EF:27.1%,26.2% and 37.9% (P= 0.071).Moreover,eGFr was an independent correlate of HF in patients with preserved EF (P=0.003) but not in patients with depressed EF (P=0.181).CoNClUSIoN:A significant proportion of post-MI patients with occluded IrAs have impaired renal *** renal function was associated with an increased rate of early post-MI HF,the association being strongest in patients with preserved *** findings have implications for management of peri-infarct HF.

Aim:To determine the tolerance and acceptance of hepatic venous pressure gradient(HVPG)measurements in patients with liver ***:This prospective international multicenter study included 271 patients with cirrhosis who were scheduled to undergo HVPG measurement between october 2019 and June *** related to the tolerance and acceptance of HVPG measurements were collected using descriptive ***:HVPG measurements were technically successful in all 271 patients,with 141(52.0%)undergoing HVPG measurement *** complication rate was 0.4%.Postoperative pain was significantly lower than preoperative expected pain(praoperative pain(praoperative pain was also significantly lower than preoperative expected pain(p=0.036).No,mild,moderate,severe,and intolerable discomfort scores were reported by 36.9%,44.6%,11.1%,6.3%,and 0.4%of these patients,respectively,during HVPG measurement and by 54.6%32.5%,11.4%,1.5%,and 0%,respectively,after HVPG *** these patients,39.5%had little understanding and 10%had no understanding of the value of HVPG measurement,with 35.1%and 4.1%regarding HVPG measurements as being of little or no help,*** patients reported that they would definitely(15.5%),probably(46.9%),or possibly(29.9%)choose to undergo additional HVPG measurements again,and 62.7%regarded the cost of the procedure as ***:HVPG measurement was safe and well‐tolerated in patients with cirrhosis,but patient education and communication are warranted to improve the acceptance of this procedure.
