Ventriculo-atrial shunt with occlusion of the internal jugular vein: operative experience and surgical technique

Background Ventricular shunts are one of the most frequent techniques used for the management of hydrocephalus. The ventriculoperitoneal shunt (VPs) is the most commonly performed procedure, and the ventriculoatrial shunt (VAs) is the second option in most medical centers. The main objective of this study is to introduce and describe a surgical approach for VAs outlining our experience and comparing it with traditional shunting *** In this retrospective cohort comparison study, we included patients with hydrocephalus treated with a surgical procedure between January 2010 and February 2021 at a single academic institution. We categorized the procedures into two groups: patients with VPs and conventional VAs grouped together into the conventional technique (CT) group, and the second group was patients with whom we performed VAs with complete internal jugular vein occlusion (IJVOT). We compared the surgical time, postoperative complications, and occurrence of shunt failure among the groups by performing univariate analysis using the Fisher exact *** Out of the 106 patients included in the analysis, IJVOT was performed in 66 patients, and CT in 40 patients. The median surgical time was 60 min (IQR 60-90) for IJVOT versus 100 min (IQR 60-120) for CT (p s with IJVOT and 62.5% of patients with CT did not require shunt removal or shunt revision (p shunt revision rates were 12.5% and 1.5% for CT while 1.5% and 2.5% for IJVOT at 1 and 6 months after the *** Our findings demonstrate that VAs with IJVOT is a safe method that exhibited shorter surgical times and outcomes comparable to CT. The water scarcity in quality and quantity is becoming more noticeable and an urgent concern around the world. In Uruguay, these issues become exacerbated by the need to obtain drinking water in coastal areas, influenced by the climate change. Basic and structural geologies are strong conditioners in heterogeneous coastal aquifers. The objective of this study is to characterize the hydrochemistry of the fractured aquifers after having identified the main bearing fractures and the causes of aleatoreous water scarcity and quality problems, for hydric resources sustainable management. Identification of water bearing fracture, hydrogeochemical analysis and water quality evaluation are specific objectives. some strategies were performed: 1) a base map in QGIs software;2) fracture photointerpretation;3) geological correlation;4) statistical analysis of the background geochemistry data;5) ions analysis of strategically located wells. There were found water bearing fractures corresponding to 28 msup>3sup>/h maximum flow rate for the NW-sE and 12 msup>3sup>/h maximum flow rate for the NE-sW fracture direction, respectively. Besides, there could be a problem related to the high sodium (Na) and Chloride (Cl) levels. In this respect, having previous data from 25 well samples, ions geochemical analysis has been carried out for 14 wells from Costa Azul and surrounding to have a first approach about the possible cause for the high values of Na (max. 385 mg/L) and Cl (max. 381 mg/L). The selected area for this study has a particular characteristic, because it corresponds to a heterogeneous fractured aquifer, which makes it difficult to catch water with enough flow rates and water quality to meet the population demand.

Influence of statins and Fibrates Drugs on Bone Health and Regeneration

In the medical and dental field, the importance and need for the study of materials and drugs for use as bone grafts or regeneration in injured areas due to the presence of fractures, infections or tumors that cause extensive loss of bone tissue is observed. Bone is a specialized, vascularized and dynamic connective tissue that changes throughout the life of the organism. When injured, it has a unique ability to regenerate and repair without the presence of scars, but in some situations, due to the size of the defect, the bone tissue does not regenerate completely. Thus, due to its importance, there is a great development in therapeutic approaches for the treatment of bone defects through studies that include autografts, allografts and artificial materials used alone or in association with bone grafts. Pharmaceuticals composed of biomaterials and osteogenic active substances have been extensively studied because they provide potential for tissue regeneration and new strategies for the treatment of bone defects. statins work as specific inhibitors of 3-hydroxy-3-methyl-glutaryl coenzyme A reductase (HMG-CoAreductase). They represent efficient drugs in lowering cholesterol, as they reduce platelet aggregation and thrombus deposition;in addition, they promote angiogenesis, reduce the β-amyloid peptide related to Alzheimer’s disease and suppress the activation of T lymphocytes. Furthermore, these substances have been used in the treatment of hypercholesterolemia and coronary artery disease. By inhibiting HMG-CoAreductase, statins not only inhibit cholesterol synthesis, but also exhibit several other beneficial pleiotropic effects. Therefore, there has been increasing interest in researching the effects of statins, including simvastatin, on bone and osteometabolic diseases. Five Year Follow up after surgical Treatment of Type 2 Diabetes with Laparoscopic sleeve Gastrectomy Associated with a Duodenal Ileal Interposition

Introduction: Type 2 diabetes mellitus (T2D) is a chronic disease, primarily caused by a combination of defective insulin secretion and the inability of insulin-sensitive tissues to respond to insulin. Laparoscopic sleeve gastrectomy associated with duodenal ileal interposition (sGDII) has been shown to be a feasible treatment option for patients with T2D, as it provides improvement and control of glycemia, dyslipidemia and arterial hypertension. The aim of this study was to evaluate the mid and long-term results of sGDII for the treatment of diabetic patients, considering diabetic remission, weight loss and postoperative complications. Materials and Methods: Retrospective study with 96 patients with T2D submitted to sGDII, between 2010 and 2016. The glycated hemoglobin (HbA1c) value Results: sixty-one patients (62.8%) were male, and 36 patients (37.2%) were female. The average age was 50.9 years. Median BMI was 33.43 kg/m2. The incidence of major postoperative complications in the first 30 days was 2.1%, with no mortality. Dyslipidemia control was sustained, after five years, in 62% of the patients. The five years follow-up demonstrated that 80% of patients had T2D remission at one year, 74.5% at three years and 61.8% at five years. Univariate analysis demonstrated that preoperative values of HbA1c and BMI, preoperative use of insulin, gender, and 30-day complication were not predictors of remission at all study intervals. The average duration of the disease was nine years and the mean glycated hemoglobin before surgery was 8.95%. Conclusion: sGDII resulted in good glycemic control at 5-years follow-up and represents a valid alternative for the treatment of T2D.

Objective: This case report aimed to demonstrate a possible neuromuscular effect of Latarjet nerves transection or truncal vagotomy, in association with sleeve gastrectomy plus antrojejunostomy, in order to reproduce a Roux-en-Y gastric bypass (RYGB) mechanistic principles, in patients with previous sleeve Gastrectomy (sG) who had had weight regain, with or without concomitant gastroesophageal reflux disease (GERD). Background: sleeve gastrectomy (sG) is one of the most frequently performed bariatric operations worldwide. Nevertheless, weight regain and gastroesophageal reflux disease (GERD) have been consistently demonstrated, in association with this technique, which may require a revisional procedure. RYGB is an option in such a situation but, implies in gastrointestinal exclusions, which represents a shortcoming of this revision. surpassing this inconvenient would be of great value for the patients. Methods: We describe herein two cases of sG revision for weight regain and GERD, with a follow-up of one year. Gastroesophageal reflux disease was evaluated by validated questionnaire, upper endoscopy, seriography, high resolution manometry (HRM) and impedance pHmetry (I-pHmetry), in the pre and postoperative periods. A re-sleeve Gastrectomy with antrojejunal anastomosis was done in both cases, after informed consents. The Latarjet nerves were transected in one case, due to a bleeding in the left gastric vessels and a truncal vagotomy was required in the other, to appropriately treat an associated hiatal hernia. Results: In the postoperative evaluation it was observed a pyloric spasm in both cases, during seriography and endoscopy, kept until the one-year follow-up. There was unidirectional contrast flow to the gastrointestinal anastomosis, filling the jejunal limb, in radiologic contrast study. No contrast passed through the pylorus. 3D City Models as a Visual support Tool for the Analysis of Buildings seismic Vulnerability: The Case of Lisbon

3D city models associate a database of a city to a rigorous geospatial representation that is close to the visible reality by combining appearance, geometry, and semantics. As such, these models may include tools that convey attribute analysis, not only at the statistical level but also in terms of visual appearance of the objects, thus allowing the generation of new knowledge about a phenomenon or its effects. Lisbon is a city with a moderate level of seismic risk, and has been hit in the past by strong earthquakes. several seismic vulnerability studies of buildings have been carried out in Lisbon. These studies generate different scenarios that consider the magnitude and source location of potential earthquakes. The results of these studies were presented as maps obtained from crossing information contained in 2D layers and presented in a GIs environment. In the present study a seismic vulnerability assessment of Lisbon's buildings held in 2D is extended to the third dimension, which refines the resolution and the set of parameters and explores the gains of spatial analysis in 3D representations. The numerous parameters that contribute to assess seismic vulnerability of buildings may be analyzed individually or simultaneously as well as in multiple seismic scenarios. Although covering all residential buildings in Lisbon, the application of the 3Dcity model is demonstrated with more detail in three pilot areas.

Obesity is a significant and escalating health issue both in Brazil and globally, with over 650 million overweight adults worldwide. The treatment of obesity can be performed clinically, endoscopically or surgically;surgical treatment proves to be safe and more effective in terms of weight loss and long-term maintenance. Objective: This study aimed to monitor the progress of weight loss and comorbidity control in patients undergoing sleeve gastrectomy with duodenal bipartition. Methods: This pilot project involved 8 patients divided into 2 arms. In the first arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenoileal transit bipartition (s-RYDITB), while in the second arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenojejunal transit bipartition (s-RYDJTB). Both procedures involved Roux-en-Y reconstruction without duodenal exclusion. In s-RYDITB, the duodenal-ileal anastomosis was performed 300 cm from the ileocecal valve (ICV), creating a 250 cm common channel and a 50 cm alimentary channel. In s-RYDJTB, a biliopancreatic loop was created 200 cm from the angle of Treitz, with a 1 m alimentary channel. Results: Five patients underwent the procedures, with one undergoing s-RYDITB and four undergoing s-RYDJTB. No adverse events such as hospitalizations, readmissions, reoperations, fistulas, bleeding, pulmonary embolism, diarrhea, dumping syndrome, or hypoglycemia occurred during the study period. The mean length of hospital stay was 2 days. The average BMI decreased from 37.27 kg/msup>2sup> preoperatively to 29.48 kg/msup>2sup> after 6 months. The significant percentage of weight loss was 21.22%, with excess weight loss of 63.6%. Ninety-five percent remission of comorbidities, including hypercholesterolemia, hypertriglyceridemia, diabetes, hypertension, steatosis, and pre-diabetes. screening of Environmental Bacterial strains for Wild Aedes Mosquito Larval Biocontrol

The effectiveness of current control measures against Aedes mosquitoes remains low, resulting in persistent epidemics in urban areas. The emergence of resistant mosquito populations to chemical insecticides highlights the need for novel, environmentally friendly, cost-effective control strategies. This study explored the potential of environmental bacterial isolates to biocontrol wild Aedes larvae. Initially, we collected bacterial samples from infectious masses of Aedes fluviatilis larvae. The isolated bacteria were identified using biochemical, enzymatic, and molecular methods, including 16s rRNA sequencing and MALDI-TOF. Previously, Aeromonas hydrophila and Bacillus thuringiensis isolated from these infectious masses showed limited Aedes larval inhibition. Consequently, we screened additional environmental isolates from the bacteriotheque. six isolates previously identified were tested: Chromobacterium violaceum, Enterobacter cloacae, Bacillus cereus, Bacillus sphaericus, and two strains of Bacillus thuringiensis israelensis. Among these strains, B. thuringiensis and C. violaceum exhibited significant inhibitory activities against wild Aedes larvae. Bacillus thuringiensis cultures grown under daylight conditions showed a slight ability to inhibit Aedes larvae. The potential of B. thuringiensis and C. violaceum strains studied, along with optimized culture growth conditions, will be further investigated to develop bioinsecticide products to provide safer and more sustainable alternatives for controlling larvae of Aedes mosquitoes.
