Efficacy and safety of tenofovir in chronic hepatitis B: Australian real world experience

AIMTo evaluate the long-term treatment outcomes of tenofovir therapy in patients in a real world Australian tertiary care setting. METHODSWe performed a retrospective analysis of treatment outcomes among treatment-naïve and treatment-experienced patients receiving a minimum 3 mo tenofovir therapy through St vincent’s Hospital Melbourne, Australia. We included patients receiving tenofovir [tenofovir disoproxil fumarate (TDF)] monotherapy, as well as patients treated with TDF in combination with a second antiviral agent. Patients were excluded if they demonstrated human immune-deficiency virus/hepatitis C virus/hepatitis delta virus coinfection or were less than 18 years of age. We considered virological and biochemical response, as well as safety outcomes. virological response was determined by measurement of hepatitis B virus (HBv) DNA using sensitive assays; biochemical response was determined via serum liver function tests; histological response was determined from liver biopsy and fibroscan; safety analysis focused on glomerular renal function and bone mineral density. The primary efficacy endpoint was complete virological suppression over time, defined by HBv DNA RESULTSNinety-two patients were identified who fulfilled the enrolment criteria. Median follow-up was 26 mo (range 3-114). Mean age was 46 (24-78) years, 64 (70%) were male and 77 (84%) were of Asian origin. 55 (60%) patients were treatment-naïve and 62 patients (67%) were HBeAg-negative. Complete virological suppression was achieved by 45/65 (71%) patients at 12 mo, 37/46 (80%) at 24 mo and 25/28 (89%) at 36 mo. Partial virological response (HBv DNA 20-2000 IU/mL) was achieved by 89/92 (96.7%) of patients. Multivariate analysis showed a significant relationship between virological suppression at end of follow-up and baseline HBv DNA level (OR = 0.897, 95%CI: 0.833-0.967, P = 0.0046) and HBeAg positive status (OR = 0.373, 95%CI: 0.183-0.762, P = 0.0069). Fabrication of phytogenic metal nanoparticles is a developing area of nanobiotechnology which has economic and potentially eco-friendly advantages over conventional chemical and physical synthesis methods. We report the synthesis of anisotropic gold nanostructures using a phytoextract from the leaves of the pin oak tree, Quercus palustris. We used 2.5%, 5%, and 10% m/v extract concentrations to determine the importance of pin oak phytoextract concentration on the properties of the nanomaterials synthesized. Characterization of the resulting nanomaterials revealed that the concentration of the phytoextract is a key factor controlling the size and relative frequency of shapes the gold nanostructures formed. TEM micrographs demonstrate that triangular, spherical, and polyhedral gold nanostructures were formed in all treatments, although Uv-vis spectra indicated that the formation of fewer nanostructures in the 2.5% m/v phytoextract treatment We found that anisotropic nanostructures are formed at the highest concentrations in the preparation using 10% m/v phytoextract. Uv-vis, TEM, and DLS data indicate that the 5% m/v phytoextract results in the synthesis of the smallest sized nanoparticles.

A standardized extract of the plant Caralluma fimbriata (Slimaluma?) is widely used in the management of obesity but its mode of action is not yet clarified. This study investigated the ability of Caralluma fimbriata extract (CFE) to modify pre-adipocyte cell division and thus the development of hyper-plastic obesity. Mouse 3T3-L1 pre-adipocyte cell line samples were treated with different concentrations of an extract of CFE standardized against its pregnane glycoside content. Plain medium formed the negative control and hydroxyurea was the positive control. The cells were counted at 12-hour intervals, and their viability tested using the MTT assay. The treated cells were subjected to direct and indirect immunofluorescent assays for cyclin D1. CFE inhibited 3T3-L1 cell growth in a dose and duration-dependent manner, with results comparable to those produced by hydroxyurea. The viability of CFE-treated cells was reduced. Direct and indirect immunofluorescent assays demonstrated that CFE inhibits import of cyclin D1into the nucleus. CFE appears to inhibit pre-adipocyte cell division by interfering with a mechanism preceding the import of cyclin D1-CDk4/6 complex into the nucleus during the early G1 phase of the cell cycle, suggesting that CFE has the potential to inhibit hyperplastic obesity.

Some biotrophic plant pathogens have the remarkable ability to alter plant developmental morphology and subceliular archi- tecture. At least three examples of this ability arose indepen- dently within the phylum Nematoda. For one such example, the cyst nematodes, plant cell manipulation is rapid and pro- found; within days the cell cycle of a single cell in the vascular cylinder is arrested at G2, the vacuole reduces in size and frag- ments.

In October 2018,Porterbrook and the University of Birmingham announced the HydroFLEX project,to demonstrate a hydrogen-hybrid modified train atRail Live *** concept of modifying a Class 319 Electric Multiple Unit was developed,with equipment including a fuel cell stack,traction battery,24 v control system and hydrogen storage elements to be mounted inside one of the *** was followed by procurement of a fuel cell stack,traction batteries,and control equipment,which was then installed inside the train,being fixed to the seat *** substantial change from the concept was the provision of considerably more hydrogen storage than the minimum necessary,providing the train with more potential to be further modified to allow for higher speed mainline *** the Rail Live exhibition where HydroFLEX was demonstrated,numerous modifications were performed to increase the reliability and power of the HydroFLEX train,primarily concerned with modifying the base train logic,with the aim of a successfulmainline *** this effort was a multitude of documentation concerning safety,operations,and approvals to gain approvals from the relevant approvals *** project demonstrated the feasibility of using hydrogen fuel cells as an autonomous fuel for railway propulsion systems,which has the potential for full decarbonisation.

采用培养皿法对营养生长期和开花期的9个水稻栽培种(Pusa Sugandh-2,Pusa Basmati-1,Pusa-2511,Pusa Basmati-370,Pusa-1077,KarnalLocal,PRR-78,Jaya,Pusa-1238)和1个组培种(Culture No.34)的正己烷提取物进行了生测,以调查它们对稻螟赤眼蜂Trichogramma japonicum Ashmead和赤眼卵蜂Trichogramma chilonis(Ishii)平均寄生活性指数(PAI)和平均寄生百分率的影响。将不同水稻品种的正己烷提取物进行气相-液相色谱,来测定饱和碳氢化合物。其中,营养生长期的Pusa Sugandh-2叶片提取物激发两种寄生蜂产生最大的反应,平均寄生率最大。而开花期的Pusa Basmati-1使两种寄生蜂的平均寄生率最高。对营养生长阶段的叶片提取物进行的气相-液相色谱分析表明:Pusa Sugandh-2含有25个碳原子(C25)和29个碳原子(C29)的2种化合物。对开花期的叶片提取物进行的气相-液相色谱分析也表明:Basmati-1含有25个碳原子(C25)、26个碳原子(C26)和29个碳原子(C29)的3种化合物,可激发寄生蜂产生最大的反应。
