Objectives: Describe the clinical, aetiological and prognostic features of infectious m.ningitis in adults at the Douala General Hospital (DGH). Patients and m.thods: We carried out a 5-year retrospective cross-sectional study at the DGH, on data from.the registers of the bacteriology and biochem.stry laboratories. Cases of m.ningitis were identified from.the results of the cerebrospinal fluid (CSF) analysis, and the patient clinical file was obtained. Cases files of patients aged 15 years and above were included. For each patient, sociodem.graphic, clinical features and hospital m.rtality data were extracted. Results: During study period, 1877 CSF analyses were recorded and 135 were enrolled for data analysis. Up to 74 patients (55%) were m.le and the m.an age was 40.04 ± 12.5 years. The tim. lapse between the onset of sym.tom. and consultation was 12.24 ± 11.16 days. The m.in clinical signs were m.ningeal syndrom. (96.3%), neck stiffness (71.1%), reduced alertness (64.4%) and confusional states (55.6%). The m.in aetiology was bacterial (45.19%) withStreptococcus pneum.niae?(45.90%) and?Neisseria m.ningitis?(29.51%) leading. The other aetiologies were viral (21.48%), fungal with?Cryptococcus neoform.ns?(20%) and m.ningeal tuberculosis (13.33%). Fifty eight patients (42.9%) had a positive HIV serology. The m.an duration of hospitalization was 9.05 ± 8.1 days, and the total in-hospital m.rtality was 25.18%. 117 patients (86.8%) had received antibiotherapy before diagnosis. Conclusion: The clinical features of m.ningitis in the DGH are as in classical description. However, the m.rtality is high and the m.jor aetiologies appear to be associated with HIV infection at least in part. Prospective m.lti-centric studies are needed to provide m.re evidence for the developm.nt of staged m.nagem.nt guidelines in our resource-lim.ted settings.

Stroke and HIV: Correlation between Viral Load and Type of Stroke

Introduction: The role of im.unosuppression of TCD4+ lym.hocytes in the onset of stroke in people living with HIV has been reported in num.rous studies exam.ning the co-m.rbidity of stroke and HIV. Objective: To determ.ne the correlation between the viral load and the type of stroke. m.thodology: This was a 7-m.nth cross-sectional descriptive study carried out in the Neurology Departm.nt of Loandjili General Hospital in Pointe-Noire. The study population consisted of patients living with HIV who had a stroke confirm.d by brain scan. The sero-im.unological investigation consisted of looking for T lym.hocyte typing from.two kits: a CD4+ T lym.hocyte typing reagent kit (BD FACS Presto Tm. and a GeneXpert kit for viral load (Xpert®HIV-1 Viral Load). The database was m.de from.the 2010 version of m.crosoft Excel. Results: We included 16 patients living with HIV, 56% of whom.were wom.n with a sex ration of 0.78. The m.an age was 56.92 ± 11.21. The m.an num.er of TCD4+ lym.hocytes was 413.44 ± 677.95/m.3;m.nim.m. 93/m.3;m.xim.m. 2854/m.3. The m.an viral load was 17,996.31 ± 20,982.22/m.3;m.nim.m. 1002/m.3;m.xim.m. 67,229/m.3. No significant difference between the viral load and the occurrence of the stroke (p = 0.13). Conclusion: Our study did not show a causal link between viral load, im.unosuppression of TCD4+ lym.hocytes and the onset of stroke.

Atrial Fibrillation during Cerebral Infarction in Brazzaville: Frequency and Predictive Factors

In order to contribute to the im.rovem.nt of brain infarction m.nagem.nt in Brazzaville, a cross-sectional and analytical study with prospective data collection was conducted in the cardiology and neurology departm.nts of the Brazzaville University Hospital, from.February 1 to July 31, 2018. It included patients hospitalized for cerebral infarction confirm.d with im.ging, and having done an etiological assessm.nt with at least one electrocardiogram.at rest and one of long duration. Am.ng these 138 patients included, 11 had atrial fibrillation, equaling?a frequency of 7.9%. The m.an age of AF patients was 71 ± 8.8 years. The cardiovascular risk factors found were hypertension in eight cases (72.7%), diabetes in five cases (45.5%), abdom.nal obesity in four cases (36.4%). AF was perm.nent in 10 cases (91%), and paroxysm.l in one case (9%). It was valvular in three cases (27.3%) and non-valvular in eight cases (72.7%). The cardiopathy involved was hypertensive in seven cases (63.6%), ischem.c and valvular in two cases each. The CHA2DS2-VASc score, calculated in eight patients, was an average of 2.2, and ≥2 in m.re than 80% of patients;HAS-BLED score of 2.4 on average was ≥?3 in m.re than 72% of patients. Digoxin was prescribed in seven cases (63.6%) and an anti-vitam.n K in eight cases (72.7%). In m.ltivariate analysis, age (OR = 20.10, p = 0.023), arterial hypertension (OR = 23.82, p = 0.011), and dyslipidem.a (OR = 2.03, p = 0.032) were the predictive factors found. AF is infrequent during brain infarction in Brazzaville. This system.tic research raises the problem.of age in our context.
