Promoting recovery from severe mental illness: Implications from research on metacognition and metacognitive reflection and insight therapy

Research indicates that individuals with schizophrenia recover. Recovery, however means different things to different individuals and regardless of what kind of experiences define recovery, the individual diagnosed with the serious mental illness must feel ownership of their recovery. This raises the issue of how mental health services should systematically promote recovery. This paper explores the practical implications for research on metacognition in schizophrenia for this issue. First, we present the integrated model of metacognition, which defines metacognition as the spectrum of activities which allow individual to have available to themselves an integrated sense of self and others as they appraise and respond to the unique challenges they face. Second, we present research suggesting that many with schizophrenia experience deficits in metacognition and that those deficits compromise individuals' abilities to manage their lives and mental health challenges. Third, we discuss a form of psychotherapy inspired by this research, Metacognitive Reflection and Insight Therapy which assists individuals to recapture the ability to form integrated ideas about themselves and others and so direct their own recovery. The need for recovery oriented interventions to focus on process and on patient's purposes, assess metacognition and consider the intersubjective contexts in which this occurres is discussed.

Evaluation of the use of surrogate Laminaria digitata in eco-hydraulic laboratory experiments

Inert surrogates can avoid husbandry and adaptation problems of live vegetation in laboratories. Surrogates are generally used for experiments on vegetation-hydrodynamics interactions, but it is unclear how well they replicate field conditions. Here, surrogates for the brown macroalgae Laminaria digitata were developed to reproduce its hydraulic roughness. Plant shape, stiffness and buoyancy of L. digitata were evaluated and compared to the properties of inert materials. Different surrogate materials and shapes were exposed to unidirectional flow. It is concluded that buoyancy is an important factor in low flow conditions and a basic shape might be sufficient to model complex shaped plants resulting in the same streamlined shape.

Enabling intercalation-type TiNb_(24)O_(62)anode for sodiumand potassium-ion batteries via a synergetic strategy of oxygen vacancy and carbon incorporation

The key to develop earth-abundant energy storage technologies sodium-and potassium-ion batteries(SIBs and PIBs)is to identify low-cost electrode materials that allow fast and reversible Na^(+)/K^(+)***,we report an intercalation-type material TiNb_(24)O_(62)as a versatile anode for SIBs and PIBs,via a synergistic strategy of oxygen vacancy and carbon incorporation to enhance ion and electron *** TiNb_(24)O_(62-x)/reduced graphene oxide(rGO)composite anode delivers high reversible capacities(130 mA h g^(-1)for SIBs and 178 mA h g^(-1)for PIBs),great rate performance(54 mA h g^(-1)for SIBs and 37 mA h g^(-1)for PIBs at 1 A g^(-1)),and superior cycle stability(73.7%after 500 cycles for SIBs and 84%after 300 cycles for PIBs).The performance is among the best results of intercalation-typemetal oxide anodes for SIBs and *** better performance of TiNb_(24)O_(62-x)/rGO in SIBs than PIBs is due to the better reaction kinetics of the ***,mechanistic study confirms that the redox activity of Nb4+/5+is responsible for the reversible intercalation of Na^(+)/K^(+).Our results suggest that TiNb_(24)O_(62-x)/rGO is a promising anode for SIBs and PIBs and may stimulate further research on intercalation-type compounds as candidate anodes for large ion batteries.

Modeling of Soybean Plant Sap Flow

Soybean (Glycine max. (L.) Merr.) sap flow during the growth stages in relation to soil moisture, nutrition, and weather conditions determine the plant development. Modeling this process helps to better understand the plant water-nutrition uptake and improve the decisions of efficient irrigation management and other inputs for effective soybean production. Field studies of soybean sap flow took place in 2017-2021 at Marianna, Arkansas using heat balance stem flow gauges to measure the sap flow during the reproductive growth stages R3-R7. Plant water uptake was measured using the lysimeter-container method. The uniform sap flow-based hydraulic system in the soil-root-stem-leaf pathway created negative water tensions with osmotic processes and water surface tensions in stomata cells as water evaporation layers increase are the mechanism of the plant water uptake. Any changes the factors like soil water tension, solar radiation, or air relative humidity immediately, within a few seconds, affect the system’s balance and cause simultaneously appropriate reactions in different parts of the system. The plant water use model was created from plant emergence, vegetative to final reproductive growth stages depending on soil-weather conditions, plant morphology, and biomass. The main factors of the model include solar radiation, air temperature, and air relative humidity. The effective sap flow uptake occurs around 0.8 KPa VPD. Further research is needed to optimize the model’s factors to increase the plant growth dynamics and yield productivity.

Simulation and Visualisation of Functional Landscapes:Effects of the Water Resource Competition Between Plants

Vegetation ecosystem simulation and visualisation are challenging topics involving multidisciplinary aspects. In this paper, we present a new generic frame for the simulation of natural phenomena through manageable and interacting models. It focuses on the functional growth of large vegetal ecosystems, showing coherence for scales ranging from the individual plant to communities and with a particular attention to the effects of water resource competition between plants. The proposed approach is based on a model of plant growth in interaction with the environmental conditions. These are deduced from the climatic data （light, temperature, rainfall） and a model of soil hydrological budget. A set of layers is used to store the water resources and to build the interfaces between the environmental data and landscape components： temperature, rain, light, altitude, lakes, plant positions, biomass, cycles, etc. At the plant level, the simulation is performed for each individual by a structural-functional growth model, interacting with the plant＇s environment. Temperature is spatialised, changing according to altitude, and thus locally controls plant growth speed. The competition for water is based on a soil hydrological model taking into account rainfalls, water runoff, absorption, diffusion, percolation in soil. So far, the incoming light radiation is not studied in detail and is supposed constant. However, competition for light between plants is directly taken into account in the plant growth model. In our implementation, we propose a simple architecture for such a simulator and a simulation scheme to synchronise the water resource updating （on a temporal basis） and the plant growth cycles （determined by the sum of daily temperatures）. The visualisation techniques are based on sets of layers, allowing both morphological and functional landscape views and providing interesting tools for ecosystem management. Institutional analysis of intra-and post-operative tracheostomy management for risk reduction

Objectives:Determine variability in intra-and post-operative management of tracheostomies(trachs)at our institution as existing literature suggests that trachs are a frequent trigger for airway-related *** the development of an institution-wide protocols for trach ***:A 39-question online survey was sent to 55 providers who perform open and percutaneous trachs at three of the hospitals within our large,urban,academic medical *** providers were identified by surveillance of the operating room schedules for 1 ***:The survey was completed by 40 of the 53 eligible providers(75.5%).Response rate by question *** included members of all departments that perform trachs at our institution(Otorhinolaryngology,Trauma Surgery,Thoracic Surgery,General Surgery,Cardiovascular Surgery and Interventional Pulmonology).While most responses demonstrated uniformity in practice,notable variations included the following:80%of percutaneous trach providers stated that morbid obesity was not a contraindication to performing a trach outside of the operating room(n=20)while 58%of open trach providers stated that morbid obesity was a contraindication;only 35%of open trach providers perform a Bjork flap(n=350).The survey also identified significant variability in practice with regards to timing of trach suture ***:Lack of uniformity was identified in several practices related to intra-and post-operative tracheostomy *** did,however,trend toward consensus in many *** results are being used to establish a more consistent approach to tracheostomy management across our institution to ensure standardization of practice amidst the rapidly evolving practices of trach *** for practice:With ongoing evolution in the methods of trach placement and its management,the concepts put forth here will be a resource for health care providers at other institutions to consider intra-institutional analysis and

The idea of using ultrashort X-ray pulses to obtain images of single proteins frozen in time has fascinated and inspired *** was one of the arguments for building X-ray free-electron *** to theory,the extremely intense pulses provide sufficient signal to dispense with using crystals as an amplifier,and the ultrashort pulse duration permits capturing the diffraction data before the sample inevitably *** was first demonstrated on biological samples a decade ago on the giant *** then,a large collaboration has been pushing the limit of the smallest sample that can be *** ability to capture snapshots on the timescale of atomic vibrations,while keeping the sample at room temperature,may allow probing the entire conformational phase space of *** we show the first observation of an X-ray diffraction pattern from a single protein,that of Escherichia coli GroEL which at 14 nm in diameter is the smallest biological sample ever imaged by X-rays,and demonstrate that the concept of diffraction before destruction extends to single *** the pattern,it is possible to determine the approximate orientation of the *** experiment demonstrates the feasibility of ultrafast imaging of single proteins,opening the way to single-molecule time-resolved studies on the femtosecond timescale.

Preoperative Medication Management, Compliance and Adverse Events in Adult Patients Undergoing Elective Surgery: A Historical Chart Review

Purpose: Medication compliance for chronic medications has been well studied, but there is a gap in the literature regarding compliance within the perioperative period. Our objective was to determine the incidence of patient non-compliance with preoperative medication instructions for adult non-emergent surgery. Additional objectives were to identify predictors of compliance, describe medication instructions by drug type, and explore the impact of non-compliance. Patients and Methods: This historical chart review evaluated preoperative compliance to medication instructions in 393 adults undergoing non-emergent surgeries at Hamilton Health Sciences between May 1, 2012, and April 30, 2013. Seven patient factors (age;sex;American Society of Anesthesiologists class;number of medications;type of surgery;time between preoperative appointment to surgery;the individual collecting the medication list) were evaluated as potential predictors of non-compliance and analyzed using logistic regression analysis. Consequences of non-compliance were assessed by impact on intraoperative blood pressure, blood glucose level, drop in hemoglobin, bronchospasm, and case delays. Results: One hundred forty-six (37.2%) patients were non-compliant with one or more medication reconciliation instructions provided by the anesthesiologist. No significant associations were observed for any patient risk factors and non-compliance. Non-compliance was not associated with any clinically significant consequences. Conclusions: Our study shows that 37.15% of adult patients undergoing non-emergent surgery were non-compliant with medication instructions, although patients did not receive any written instructions for 46% of their medications. We did not identify any predictive patient factors or adverse outcomes associated with non-compliance.
