Consecutive fecal calprotectin measurements for predicting relapse in pediatric Crohn's disease patients

b.CKGROUND Asymptomatic children with Crohn's disease(CD) require ongoing monitoring to ensure early recognition of a disease *** In a cohort of pediatric CD patients, we aimed to assess the utility of serial fecal calprotectin measurements to detect intestinal inflammatory activity and predict disease *** In this prospective longitudinal cohort study, children with CD on infliximab.therapy in clinical remission were included. Fecal calprotectin levels were assessed at b.seline and at sub.equent 2-5 visits. Clinical and b.ochemical disease activity were assessed using the Pediatric Crohn's Disease Activity Index, Creactive protein and erythrocyte sedimentation rate at b.seline and at visits over the following 18 *** 53 children were included and eighteen patients(34%) had a clinical disease relapse during the study. b.seline fecal calprotectin levels were higher in patients that developed symptomatic relapse [median(interquartile range), relapse 723μg/g(283-1758) vs 244 μg/g(61-627), P = 0.02]. Fecal calprotectin levels > 250μg/g demonstrated good predictive accuracy of a clinical flare within 3 mo(area under the receiver operator curve was 0.86, 95% confidence limits 0.781 to 0.937).CONCLUSION Routine fecal calprotectin testing in children with CD in clinical remission is useful to predict relapse. Levels > 250 μg/g are a good predictor of relapse in the following 3 mo. This information is important to guide monitoring standards used in this population.

Rituximab.therapy for primary glomerulonephritis: Report on two cases

The evidence in the medical literature on the efficacy and safety of rituximab.therapy for primary glomerulonephritis is limited and controversial. We describ. two male Caucasian patients with rapidly progressive kidney failure due to primary proliferative glomerulonephritis. b.th of them received high-dose intravenous corticosteroids and oral cyclophosphamide with limited b.nefit. The first patient(hepatitis C virus-negative mixed cryoglob.linemia) underwent plasma-exchange with intravenous immunoglob.lins; he showed significant b.nefit on kidney function(he b.came dialysis independent with serum creatinine going b.ck to 1.6 mg/d L) after one rituximab.pulse even if urinary ab.ormalities were still present. No improvement in renal function or urinary changes occurred in the second patient. b.th these individuals developed sepsis over the follow-up, the first patient died two months after rituximab.therapy. This report is in keeping with the occurrence of severe infections after rituximab.therapy in patients with renal impairment at b.seline and concomitant high-dose steroids.

Nutritional Correlates of Women with a History of Gestational Diab.tes and Insulin Resistance in the National Health and Nutrition Examination Survey (NHANES) 2000-2010

Ob.ective: To evaluate the associations of gestational diab.tes (GDM) history with dietary intake, nutritional status, insulin resistance, demographic, and anthropometrical data. Materials & Methods: This cross-sectional study used data from the National Health and Nutrition Examination Survey for the years 2000-2010. Data analysis was b.sed on 290 women who reported a history of GDM compared to 4239 women who denied a GDM history. Insulin resistance [HOMA_IR = (fasting insulin in mU/mL × fasting glucose in mmol/L)/405] was calculated. Pearson correlation, Wilcoxon rank sum tests, Student’s t-tests, and chi-square analysis were used while linear regression assessed independent associations. Results: The median time-lapse from the diagnosis of GDM was 15 years. Women with a GDM history had significantly higher b.dy mass index (b.I), other anthropometric measurements, diastolic b.ood pressures and insulin resistance. They were also more likely to b. Hispanic, have delivered macrosomic infants, and delivered via cesarean. Previous GDM history compared to non-GDM sub.ects had significantly higher dietary intakes of energy calories, protein, total fat, saturated fatty acids, mono-saturated fatty acids, and cholesterol. Within the entire cohort, increasing insulin resistance was also associated with lower income, less college education, Hispanic or African American ethnicity, ob.sity, higher systolic and diastolic b.ood pressures, and with higher dietary cholesterol b.t lower intake of dietary fib.r and micronutrients. Regression analyses showed that GDM history, Hispanic ethnicity, b.I, dietary intake of cholesterol and decreasing income were independently predictive of insulin resistance. Conclusion: The data confirm that even many years after a pregnancy associated with GDM, women with a history of GDM still report significantly higher dietary intakes of energy calories, protein, and fat with no corresponding increase in consumption of dietary fib.r or minerals and vit
