Climatic and non-climatic factors driving the livelihood vulnerability of smallholder farmers in Ahafo Ano North District,Ghana

Smallholder farmers in Ahafo Ano North District,Ghana,face multiple climatic and non-climatic *** study assessed the factors contributing to the livelihood vulnerability of smallholder farmers in this district by household surveys with 200 respondents and focus group discussions(FGDs)with 10 *** Mann–Kendall trend test was used to assess mean annual rainfall and temperature trends from 2002 to *** relative importance index(RII)value was used to rank the climatic and non-climatic factors perceived by *** socioeconomic characteristics affecting smallholder farmers’perceptions of climatic and non-climatic factors were evaluated by the binary logistic regression *** showed that mean annual rainfall decreased(P>0.05)but mean annual temperature significantly increased(P<0.05)from 2002 to 2022 in the *** key climatic factors perceived by smallholder farmers were extreme heat or increasing temperature(RII=0.498),erratic rainfall(RII=0.485),and increased windstorms(RII=0.475).The critical non-climatic factors were high cost of farm inputs(RII=0.485),high cost of healthcare(RII=0.435),and poor condition of roads to farms(RII=0.415).Smallholder farmers’perceptions of climatic and non-climatic factors were significantly affected by their socioeconomic characteristics(P<0.05).This study concluded that these factors negatively impact the livelihoods and well-being of smallholder farmers and socioeconomic characteristics influence their perceptions of these ***,to enhance the resilience of smallholder farmers to climate change,it is necessary to adopt a comprehensive and context-specific approach that accounts for climatic and non-climatic factors.

Cardiac Surgery at Yaoundé General Hospital by a Local Team: Activity Report from September 2022 to January 2024

Introduction: The practice of cardiac surgery (CS) in sub-Saharan Africa faces a number of challenges. We report on the surgical activities of the first 17 months of the program launched at the Yaoundé General Hospital (YGH), describing the types of interventions and the early results. Patients and Methods: This was a descriptive cross-sectional study including patients who underwent CS from September 2022 to January 2024 at the YGH. Clinical, operative and postoperative data were collected and analysed retrospectively by R software. Results: A total of 37 patients underwent CS during the study period, divided into 23 (62.2%) open-heart and 14 (37.8%) closed-heart cases. Males predominated (sex ratio 1.17). The median age and interquartile range (IQR) were 36 years (9 - 51). Isolated valve surgery, in 15 (40.5%) cases, was the most common procedure in open-heart surgery cases. Closed-heart surgery involved closure of the patent ductus arteriosus in 9 (24.3%) cases, coronary artery bypass grafting in 3 (8.1%) cases, and pericardectomy in 2 (5.4%) cases. The median duration of extracorporeal circulation and aortic cross-clamping was 101 min (IQR 84 - 143) and 74 min (IQR 54 - 112) respectively. The median duration of mechanical ventilation was 2 hours (IQR 2 - 3). The median intensive care unit stay and median hospital stay were 2 days (IQR 2 - 3) and 7 days (IQR 5 - 8) respectively. Operative mortality was 2 (5.4%). Conclusion: Cardiac surgery remains a major challenge for African populations. The early results achieved by the local team are satisfactory. An increase in surgical activity should be strongly encouraged to facilitate local training and ensure the sustainability of the activity.

Laparoscopic Surgery of Incisional Hernia: Technique and Short-Term Results in Three Surgical Units in Cameroon

Introduction: Incisional hernia is a common medical situation and its treatment has always been a challenge for general surgeons. If laparoscopic repair has become a gold standard elsewhere, it is increasingly done in our milieu, but no data has been found. The aim of this study was to review this surgery done in our country so far, to describe the operative technique and to evaluate the postoperative outcomes. Patients and Methods: We conducted an observational descriptive study with retrospective collection of data from the 1st of July 2018 to 30th of June 2022, in three hospitals in Cameroon. Data on socio-demographic and clinical characteristics, surgical technique and postoperative outcomes of patients who had a laparoscopic repair of their incisional hernia were collected. Results: We reviewed 20 files. There were 14 women with an average age of 54.6 years. The average BMI was 32.8 kg/m2. Thirteen patients had a moderate size hernia and the hernia was localized at the midline in 18 cases. Only composite biface meshes were used with dimensions chosen to obtain at least 5 cm overlap. Spiral tacks were the only fixation means used. We registered 3 complications. One case of generalized peritonitis secondary to missed digestive perforation, one case of seroma and one case of postoperative ileus. We registered no death. Conclusion: Laparoscopic repair of incisional hernias is feasible in a resource-limited setting like ours. Compliance with the operative technique and recommendations is important to obtain short-term results closed to that put forward by literature.

Effect of Biofertilizers on the Agromorphological Parameters of Three Okra Cultivars in Southern Côte d’Ivoire

Okra (Abelmoschus esculentus L.) is an herbaceous plant of the Malvaceae family. In Côte d’Ivoire, okra production is estimated to be over 193,000 tons. This low production is largely due to poor soils and hardly covers the needs of the population. To remedy this, growers systematically use mineral fertilizers. However, these fertilizers pollute the environment. To find an alternative to chemical fertilization and increase production, the effect of biofertilizers (Spaawet, Retone, Super Gro) compared with NPK mineral fertilizer was evaluated on Divo, Teriman, and Djonan F1 cultivars. The trial was set up in a factorial block design with three replications. Plant height, number of functional leaves, and crown diameter were assessed at 60 days after sowing (DAS). The time to 50% flowering, production time, and fruit yield were calculated. The results showed that the biofertilizer Retone induced the highest heights and number of functional leaves, with averages of 61.89 cm and 29.88 leaves, respectively. The diameter at the crown (17.77 mm) was highest with the NPK mineral fertilizer, and the shortest 50% flowering time, with an average of 47.61 days, was also obtained with the biofertilizer Retone. The NPK mineral fertilizer produced the longest production time, with an average of 35.25 days. The highest yields were obtained using Retone (11.07 t/ha) and NPK (9.52 t/ha) fertilizers. The “Divo*Retone” interaction produced the highest yield with an average of 12.19 t/ha. The biofertilizer Retone could therefore be used as an alternative fertilizer to chemical fertilization in okra crops, given its effect on the parameters assessed.

The Influence of the Exchange-Correlation Functional on the Non-Interacting Kinetic Energy and Its Implications for Orbital-Free Density Functional Approximations

Antimony(Sb)was utilised over several millennia as the prime material to opacify or decolour glass and glazes,as well as an accompanying element in copper(Cu)*** antimony objects are rare,and mostly confined to Chalcolithic Central Italy and to the first millennia BCE in the southern *** innovation of antimony use in metallurgy seems to be confined to the southern Caucasus,and the invention of it might be even more specifically situated in the Great *** knowledge of mining set the pathway for the initial stage of antimonial copper alloys in the first half of the third millennium BCE and for metallic antimony ornaments in the second half of the third millennium ***,the first major expansion of antimony in the metallurgy of the Racha-Lechkumi district in the southern Caucasus(present-day Georgia)started around 1700 BCE,while its spreading in glassmaking occurred in the Late Bronze Age(LBA).Explanations that place antimony adoption within its broader social context are favoured over those that consider material or geological properties in isolation.A recurring theme is the importance of comparative analysis,both geographically and between the different pyrotechnologies,including the precious metals and glass industries,to explore how social,political,climatic and economic conditions affected adoption and innovation *** these factors are considered to explain why the extraction of antimony blossomed in the Late Bronze Age in the southern Caucasus and to reconstruct a framework of exploitation,distribution/trade and use of antimony in the Caucasus and its neighbouring regions in the south and east.

Optimal p-Ary Codes from One-Weight and Two-Weight Codes over IF_p+ vIF_p

This paper is devoted to determining the structures and properties of one-Lee weight codes and two-Lee weight projective codes Ck1,k2,k3 over p IF+ v IFp with type p2k1pk2pk3. The authors introduce a distance-preserving Gray map from( IFp + v IFp)nto2np. By the Gray map, the authors construct a family of optimal one-Hamming weight p-ary linear codes from one-Lee weight codes over IFp+ v IFp, which attain the Plotkin bound and the Griesmer bound. The authors also obtain a class of optimal p-ary linear codes from two-Lee weight projective codes over IFp + vIFp, which meet the Griesmer bound.

Pneumonia, a respiratory infection induces acute or chronic inflammation, characterized by increased activity of lymphocytes and neutrophils, thus generating oxygen-free radicals that decrease the endogenous antioxidants defence system. The aim of this experimental study focused on the capacity of nontoxic aqueous, hydroethanolic and ethanolic extracts of Greenwayodendron suaveolens (Engl. & Diels) Verdc. subsp. suaveolens to regulate free reactive species and protein inflammation generated by infectious disease. The phytochemical screenings of G. suaveolens extracts were carried out according to precipitation and colorimetric methods. The total antioxidant and flavonoid contents were determined by the Folin-Ciocalteu and Aluminium Chloride ethanolic methods. The efficiency of G. suaveolens extracts on free radicals was evaluated using DPPH•, ABTS+•, and FRAP methods. The anti-inflammatory properties of extracts were evaluated according to in vitro protein (BSA) denaturation, Proteinase Inhibitory Action, and Red Blood Cell Membrane stabilization assays. The G. suaveolens aqueous, hydroethanolic and ethanolic extracts were used for the acute toxicity assessment according to the OECD protocol. The obtained results showed the presence of flavonoids, phenols, polyphenols, tannins, anthocyanins, alkaloids, terpenoids, and sterols as secondary metabolites families in G. suaveolens extracts. The highest contents of total antioxidants and flavonoids were highlighted in the hydroethanolic extract. However, it’s the G. suaveolens aqueous extract that showed the best free radical DPPH• and ABTS+• scavenging activities (SC50) of 11.06 μg/mL and 15.16 μg/mL respectively. The highest ferric-reducing activity was found in G. suaveolens ethanolic extract with 866.23 μg EGA/mg of dry weight. Introduction: Urethroplasty remains the gold standard for the management of urethral stricture. However, the treatment of stricture disease in the elderly tends to be less invasive due to the presumption that they might not be able to stand long hours of surgery and might have higher rates of recurrence due to poor wound healing from microangiopathy. We present our experience with the outcomes of urethroplasty among elderly men seen at the Komfo Anokye Teaching Hospital from January 2012 to December 2021. Methods: This was a retrospective review of data captured in the urology database on all patients 65 years and above who underwent urethroplasty at the hospital over the study period. Data was obtained on patients’ demographics, stricture characteristics, urethroplasty technique, and outcome. A successful outcome was defined as peak flow rate > 15 mls/s, a patent urethra on retrograde urethrogram, patient satisfaction with urine stream, or restoration of the normal stream of urine with only one attempt at urethral calibration or internal urethrotomy postoperatively. Data was analyzed using PASW Statistics for Windows, Version 18.0. Results: Overall, 43 urethroplasties were done over the study period in elderly men. The age range was 65 to 87 years. The commonest aetiology was catheterization (62.79%) followed by urethritis (32.56%). Stricture length ranged from 0.5 cm to 16 cm with a mean of 3.93 cm. Most patients (60.46%) had bulbar urethral strictures. The repair methods employed were anastomotic urethroplasty (62.80%), fasciocutaneous flap (FCF) ventral onlay (13.95%), buccal mucosa graft (BMG) ventral onlay urethroplasty (4.65%), and staged urethroplasty (4.65%). Three of the patients (6.98%) had a combination of anastomotic and tissue transfer urethroplasty. The overall success rate was 88.37%.
