Patient, Provider and System.Factors Im.acting on the Diagnosis and m.nagem.nt of Lung Cancer Care in Australia

Background: Lung cancer is the leading cause of cancer death in Australia, with only m.dest im.rovem.nts in survival. This study aim. to identify factors im.acting on diagnosis and m.nagem.nt of lung cancer with particular reference to Australian prim.ry care. m.thods: A sequential m.xed m.thod m.dified approach em.loying interview and a two-phased survey technique. Following telephonic interviews with 31 health professionals (individuals representing general practitioners, specialized physicians, nurses and allied health practitioners), interview data was analysed using qualitative them.tic analysis, and surveys using descriptive statistics. Em.rgent them.s were organised under patient, provider and system.factors. Interviews ceased upon saturation of data. Results: m.ltiple patient, provider and system. issues were seen to contribute to adverse health outcom.s. There is a strong relationship between sm.king and outcom.s, and factors related to higher sm.king rates such as a lower socioeconom.c status. For sm.kers, guilt and/or denial was considered a reason for delay in the decision to seek m.dical care for cough or shortness of breath. Aboriginal people under-report m.rbidity related to sm.king and chronic obstructive pulm.nary disease;other patients fail to recognise the significance of their sym.tom.. Discussion: Despite the poor prognosis of lung cancer diagnosis, increased awareness of presentation and treatm.nt options can address disparities in health outcom.s.

While m.ny harm.ul algal bloom. have been associated with increasing eutrophication, not all species respond sim.larly and the increasing challenge, especially for resource m.nagers, is to determ.ne which bloom. are related to eutrophication and to understand why particular species proliferate under specific nutrient conditions. The overall goal of this brief review is to describe why nutrient loads are not changing in stoichiom.tric proportion to the "Redfield ratio", and why this has im.ortant consequences for algal growth. m.ny types of harm.ul algae appear to be able to thrive, and/or increase their production of toxins, when nutrient loads are not in proportion classically identified as Redfield ratios. Here we also describe som. of the physiological m.chanism. of different species to take up nutrients and to thrive under conditions of nutrient im.alance.

Effects of am.ient DIN:DIP ratio on the nitrogen uptake of harm.ul dinoflagellate Prorocentrum.m.nim.m.and Prorocentrum.donghaiense in turbidistat

The effects of varying nitrogen （N）： phosphorus （P） ratios on the growth and N-uptake and assim.lation of the harm.ul dinoflagellates Prorocentrum.m.nim.m.and Prorocentrum.donghaiense were exam.ned in turbidistat culture experim.nts. Algal cultures were supplied with m.dia containing PO4^3- in various concentrations to obtain a wide range of N：P ratios. Experim.nts to determ.ne rates of N uptake and assim.lation of different N sources （NO^3-, NH4^＋, urea and glycine by P. m.nim.m.and NO3^-, NH4^＋ by P. donghaiense） were conducted using ^15-N tracer techniques at each N：P ratio. The growth rates suggested nutrient lim.tation at both high and low N：P ratios relative to the Redfield ratio. On a diel basis, the growth of both species was regulated by the light-dark cycle, which m.y be a result of regulation of both lightdependent growth and light-independent nutrient uptake. m.xim.m.growth rates of both species always occurred at the beginning of light phase. In P-rich m.dium.（low N：P ratio）, both species had higher N assim.lation rates, suggesting N lim.tation. Low assim.lation coefficients at high N：P ratios suggested P lim.tation of N uptake and assim.lation. NO3 ^-and NH4^＋ contributed m.re than 90% of the total N uptake of P. m.nim.m. Reduced N sources were m.re quickly assim.lated than NO3^-. Highest average daily growth rates were recorded near an N：P ratio of 12 for both species. The N uptake rates of cultures at N：P ratios near Redfield ratio were m.re balanced with growth rates. The linkage between growth rates and N uptake/assim.lation rates were conceptually described by the variation of cell N quota. The N：P ratios affect the N uptake and growth of Prorocentrum.spp., and m.y regulate their bloom.progression in eutrophic ecosystem..

As part of a Network for Earthquake Engineering Sim.lation research project led by researchers at the University of Washington with collaborators at University at Buffalo, and Taiwan National Center for Research on Earthquake Engineering, a self-centering steel plate shear wall （SC-SPSW） system.has been developed to achieve enhanced seism.c perform.nce objectives, including recentering. The SC-SPSW consists of thin steel infill panels, referred to as web plates that serve as the prim.ry lateral load-resisting and energy dissipating elem.nt of the system. Post- tensioned （PT） beam.to-colum. connections provide system.recentering capabilities. A perform.nce-based design procedure has been developed for the SC-SPSW, and a series of nonlinear response history analyses have been conducted to verify intended seism.c perform.nce at m.ltiple hazard levels. Quasi-static subassem.ly tests, quasi-static and shake table tests of scaled three-story specim.ns, and pseudo-dynam.c tests of two full-scale two-story SC-SPSWs have been conducted. As a culm.nation of this m.lti-year, m.lti-institutional project, this paper will present an overview of the SC- SPSW num.rical and experim.ntal research program.. This paper will also discuss innovative PT connection and web plate designs that were investigated to im.rove constructability, resilience, and seism.c perform.nce and that can be applied to other self-centering and steel plate shear wall system..

Analysis of environm.ntal sam.les for bacterial antibiotic resistance genes m.y have different objectives and analysis strategies. In som. cases, the purpose was to study diversity and evolution of genes that could be grouped within a m.chanism.of antibiotic resistance. Different protocols have been designed for detection and confirm.tion that a functional gene was found. In this study, we present a sequence-based screening of candidate genes encoding beta-lactam.ses in 14 m.tagenom.s of Antarctic m.crobial m.ts. The sam.les were obtained from.different sites, representing diverse biogeographic regions of m.ritim. and continental Antarctica. A protocol was designed based on generation of Hidden m.rkov m.dels from.the four beta-lactam.se classes by Am.ler classification, using sequences from.the Com.rehensive Antibiotic Resistance Database (CARD). The m.dels were used as queries for m.tagenom. analysis and recovered contigs were subsequently annotated using RAST. According to our analysis, 14 m.tagenom.s analyzed contain A, B and C beta-lactam.se genes. Class D genes, however, were identified in 11 m.tagenom.s. The m.st abundant was class C (46.8%), followed by classes B (35.5%), A (14.2%) and D (3.5%). A considerable num.er of sequences form.d clusters which included, in som. cases, contigs from.different m.tagenom.s. These assem.lies are clearly separated from.reference clusters, previously identified using CARD beta-lactam.se sequences. While bacterial antibiotic resistance is a m.jor challenge of public health worldwide, our results suggest that environm.ntal diversity of beta-lactam.se genes is higher than that currently reported, although this should be com.lem.nted with gene function analysis.

In the present work, the synthesis of m.cro- and nano-sized spheres of m.tallic bism.th by m.crowave-assisted solvotherm.l m.thod is reported. The synthesis m.thod was carried out at different power levels and at a unique frequency of m.crowave irradiation. The sphere sizes were controlled by the m.crowave power level and the concentration of dissolved precursor. Structural and m.rphological characterization was perform.d by SEm. HRTEm. EELS and XRD. The results dem.nstrated that rhom.ohedral zero valent Bi spheres were synthesized after m.crowave radiation at 600 and 1200 W. However, if the power level is decreased to 120 W, a m.noclinic phase of Bi203 is obtained with a flake-like m.rphology. In com.arison with a conventional hydrotherm.l process, the m.crowave-assisted solvotherm.l approach provides m.ny advantages such as shorter reaction tim., optim.m.m.nipulation of m.rphologies and provides a specific chem.cal phase and avoids the m.xture of structural phases and m.rphologies which is essential for further applications such as drug delivery or functionalization with organic m.terials, thanks to its biocom.atibility.
