Abnormal Umbilical Doppler: Incidence and Neonatal Outcomes among Hypertensive Disorders in Pregnancy at KCMC: A Hospital-Based Prospective Cohort Study

Hypertensive disorders of pregnancy (HDP) are the most common maternal and perinatal health challenges. Globally, the incidence of HDP increased from 16.30 million to 18.08 million, with a total increase of about 10.9% from 1990 to 2019. Umbilical Doppler study in hypertensive disorders of pregnancy helps to predict neonatal outcomes and prevent neonatal and maternal morbidity and mortality. Objective: This study aims to determine the incidence of abnormal umbilical Doppler among hypertensive pregnant women, to identify the adverse neonatal outcomes associated with abnormal umbilical Doppler, and also to detect the diagnostic predictive values of umbilical Doppler to neonatal outcomes at KCMC. Material and methods: A hospital-based prospective cohort study included women with hypertensive disorders of pregnancy from the gestational age of 28 weeks and above, followed up to delivery during the study period from August 2022 to March 2023. Multivariate logistic regression analysis was used to determine the association between AUD and neonatal outcomes. Results: Out of 112 women with HDP, the incidence of abnormal umbilical Doppler was 38 (33.93%). Abnormal umbilical Doppler was associated with neonates with low birth weight aOR (95% of CI) of 4.52 (1.59 - 12.83) p = 0.005 and neonatal ICU admission 9.71 (2.90 - 32.43) p Conclusion: The incidence of abnormal umbilical Doppler is high in hypertensive disorders of pregnancy which is associated with an increase in neonatal low birth weight and neonatal ICU admissions, the sensitivity of abnormal umbilical Doppler in prediction of low birth weight and neonatal ICU admission is significant hence the routine use of umbilical Doppler assessment among hypertensive pregnant women is crucial.

Alcoholic liver injury:Influence of gender and hormones

This article discusses several subjects pertinent to a consideration of the role of gender and hormones in alcoholic liver injury (ALI). Beginning with an overview of factors involved in the pathogenesis of ALI, we review changes in sex hormone metabolism resulting from alcohol ingestion, summarize research that points to estrogen as a cofactor in ALI, consider evidence that gut injury is linked to liver injury in the setting of alcohol, and briefly review the limited evidence regarding sex hormones and gut barrier function. In both women and female animals, most studies reveal a propensity toward greater alcohol-induced liver injury due to female gender, although exact hormonal influences are not yet understood. Thus, women and their physicians should be alert to the dangers of excess alcohol consumption and the increased potential for liver injury in females.

Introduction: Adolescence is a period of transition to adulthood, including for HIV-infected adolescents (HIA), when sexual problems emerge. Few studies have been carried out on the sexuality of HIA. This study aimed to assess the sexual behavior of HIV-infected adolescents in Cameroon. Methodology: A cross-sectional study was conducted in three hospitals in the cities of Yaoundé and Douala, from November 2019 to June 2020. All HIA aged 13 to 19 years followed in the study who knew their HIV status were included in the study after obtaining their assent and their parent’s consent. Socio-demographic and clinical characteristics were collected, as well as sexual practices and the determinants of their sexual activity. A multivariate analysis was performed to explore the relationship between the different variables studied and the sexual activity of these adolescents. Results: Of 204 HIA enrolled in the study, 64 (31.7%) were sexually active, the mean age at first sexual intercourse was 15.6 ± 2.8 years and the sex ratio was 0.94. Of the adolescents in the study, 75.7% of girls and 61.2% of boys had regular sexual activity, while 21.3% of girls and 35.4% of boys had multiple sexual relationships in the six months preceding the study. Nearly 16 (25.0%) of the sexually active HIA had more than 2 sexual encounters per month, 6 (9.3%) of these encounters had been paid for and 9 (27.2%) of the girls had already had at least one pregnancy. More than 7 out of 10 HIA (79.6%) had used a condom the last time they had sex. Age less than 18 years [OR = 11.1 (95% CI: 3.1 - 39.4), p = 0.001], lack of remuneration [OR = 9.8 (95% CI: 2.0 - 47.4), p = 0.001] and self-care were significantly associated with sexual activity in HIA, while school attendance was a protective factor. Self-funded HIAs were 21 times more likely to be sexually active [OR = 21.6 (95% CI: 2.3 - 179), p = 0.004]. Synchronous waving in fiddler crabs: a review

Many animals that use acoustic communication synchronize their mate attract!on signals: individuals precisely time their calls to overlap those of their neighbors. In contrast, synchrony in the mate attraction displays of species with visual/motion-based signals is rare. It has only been documented in five species of fiddler crabs. In all of them, small groups of males wave their single large claw in close synchrony. Here, I review what we know about synchrony in fiddler crabs, comparing the five species with each other to determine whether similar mechanisms and functions are comm on to all. I also propose future research questions that, if answered, would shed light on synchronous behavior in both visual and acoustic signallers.

Orbit orientation in didelphid marsupials （Didelphimorphia： Didelphidae）

Usually considered a morphologically conservative group, didelphid marsupials present consider- able variation in ecology and body size, some of which were shown to relate to morphological structures. Thus, changes on orbit morphology are likely and could be related to that variation. We calculated orbit orientation in 873 specimens of 16 Didelphidae genera yielding estimates of orbits convergence （their position relative to midsagittal line） and verticality （their position relative to frontal plane）. We then compared similarities in these variables across taxa to ecological, morpho- logical and phylogenetic data to evaluate the influencing factors on orbit orientation in didelphids. We found an inverse relation between convergence and verticality. Didelphids orbits have low ver- ticality but are highly convergent, yet orbit orientation differs significantly between taxa, and that variation is related to morphological aspects of the cranium. Rostral variables are the only morpho- logical features correlated with orbit orientation： increasing snout length yields more convergent orbits, whereas increase on snout breadth imply in more vertical orbits. Size and encephalization quotients are uncorrelated with orbit orientation. Among ecological data, diet showed significant correlation whereas locomotion is the factor that less affects the position of orbits. Phylogeny is uncorrelated to any orbital parameters measured. Ecological factors seemingly play a more import- ant role on orbit orientation than previously expected, and differentiation on orbit orientation seems to be more functional than inherited. Thus, despite the apparent homogeneity on didelphid morphology, there is subtle morphological variability that may be directly related to feeding behavior.

Dependence of Upper Atmosphere Photochemistry on the Shape of the Diurnal Cycle of the Photolysis Rates

A photochemical model of the atmosphere constitutes a non-linear, non-autonomous dynamical system, enforced by the Earth＇s rotation. Some studies have shown that the region of the mesopause tends towards non-linear responses such as period-doubling cascades and chaos. In these studies, simple approximations for the diurnal variations of the photolysis rates are assumed. The goal of this article is to investigate what happens if the more realistic, calculated photolysis rates are introduced. It is found that, if the usual approximations-sinusoidal and step fiunctions—are assumed, the responses of the system axe similar： it converges to a 2-day periodic solution. If the more realistic, calculated diurnal cycle is introduced, a new 4-day subharmonic appear.

Epididymosomes： transfer of fertility-modulating proteins to the sperm surface

A variety of glycosylphosphatidylinositol （GPI）-linked proteins are acquired on spermatozoa from epididymal luminal fluids （ELF） during sperm maturation. These proteins serve roles in immunoprotection and in key steps of fertilization such as capacitation, acrosomal exocytosis and sperm-egg interactions. Their acquisition on sperm cells is mediated both by membrane vesicles （epididymosomes, EP） which were first reported to dock on the sperm surface, and by lipid carriers which facilitate the transfer of proteins associated with the membrane-free fraction of ELF. While the nonvesicular fraction is more efficient, both pathways are dependent on hydrophobic interactions between the GPI-anchor and the external lipid layer of the sperm surface. More recently proteomic and hypothesis-driven studies have shown that EP from several mammals carry transmembrane （TM） proteins, including plasma membrane Ca^2＋-ATPase 4 （PMCA4）. Synthesized in the testis, PMCA4 is an essential protein and the major Ca^2＋ efflux pump in murine spermatozoa. Delivery of PMCA4 to spermatozoa from bovine and mouse EP during epididymal maturation and in vitro suggests that the docking of EP on the sperm surface precedes fusion, and experimental evidence supports a fusogenic mechanism for TM proteins. Fusion is facilitated by CD9, which generates fusion-competent sites on membranes. On the basis of knowledge of PMCA4＇s interacting partners a number of TM and membrane-associated proteins have been identified or are predicted to be present, in the epididymosomal cargo deliverable to spermatozoa. These Ca^2＋-dependent proteins, undetected in proteomic studies, play essential roles in sperm motility and fertility, and their detection highlights the usefulness of the hypothesis-driven approach.

Membrane distillation(MD)is a thermal-based separation technique with the potential to treat a wide range of water types for various applications and *** challenges remain however,which prevent it from becoming commercially widespread including moderate permeate flux,decline in separation performance over time due to pore wetting and high thermal energy ***,its attractive characteristics such as high rejection(ca.100%)of nonvolatile species,its ability to treat highly saline solutions under low operating pressures(typically atmospheric)as well as its ability to operate at low temperatures,enabling waste-heat integration,continue to drive research interests *** particular interest is the class of carbon-based nanomaterials which includes graphene and carbon nanotubes,whose wide range of properties have been exploited in an attempt to overcome the technical challenges that MD *** low dimensional materials exhibit properties such as high specific surface area,high strength,tuneable hydrophobicity,enhanced vapour transport,high thermal and electrical conductivity and *** use in MD has resulted in improved membrane performance characteristics like increased permeability and reduced fouling *** have also enabled novel membrane capabilities such as in-situ fouling detection and localised heat *** this review we provide a brief introduction to MD and describe key membrane characteristics and fabrication *** then give an account of the various uses of carbon nanomaterials for MD applications,focussing on polymeric membrane *** research directions based on the findings are also suggested.
