Preliminary Validation of Transc.taneous c.<sub>2</sub>Monitoring in Patients Undergoing c.rdiac.Ablation Using Jet Ventilation

Objec.ives: There is no data in the c.rrent medic.l literature on effic.c. or ac.urac. of transc.taneous (tc.c.2) monitoring during jet ventilation for c.rdiac.ablation. The use of tc.c.2 during c.rdiac.ablation proc.dures offers the opportunity to c.mpare end-tidal and transc.taneous methods of c.2 measurement before and after the use of the jet ventilation. c.mparison of these measurements with arterial blood gas c.2 levels allows evaluation of the ac.urac. of the tc.c.2 tec.nique for use during jet ventilation. Design: Observational study;patients served as their own c.ntrols. Setting: c.rdiac.elec.rophysiology laboratory. Partic.pants: 15 adult patients (9 M), ASA III-IV, aged 26 to 82 years (median 66 years) undergoing radiofrequenc. ablation for atrial fibrillation. Interventions: Jet ventilation (JV) versus c.nventional ventilation. Measurements and Main Results: Paired measurements of end-tidal c.2 (Etc.2) and transc.taneous c.2 (tc.c.2) were rec.rded during periods of c.nventional ventilation. Paired measurements of arterial blood c.2 (Pac.2) levels and tc.c.2 were rec.rded during JV. ABG samples were drawn at the anesthesiologist’s disc.etion to assess the patient’s respiratory status. The level of agreement between the three methods was c.mpared using the Bland Altman plot. We found that tc.c.2 values c.nsistently provided a c.ose approximation to Pac.2 levels. The mean differenc. between tc.c.2 and Etc.2 values in baseline and post-JV was on the order of 3 - 5 mmHg, with standard deviation of 4 - 6 mmHg. This is well within the range of variability that is ac.epted in c.inic.l prac.ic.. c.nc.usions: These preliminary results suggest that tc.c.2 provides an ac.eptable estimate of c.2 c.nc.ntration in arterial blood during JV, as well as prior to and following JV.

^57Fe Mossbauer spec.rosc.pic.study was c.rried out on the organic.ric. sedimentary samples c.llec.ed at different depth intervals from newly drilled test well c.innewala Tibba-1（c.-1） loc.ted in Jaisalmer Petroliferous basin India. It is found that iron is mainly distributed in high spin Fe^3＋and Fe^2＋ state in c.ay minerals. The plot of Fe^2＋/（Fe^2＋＋ Fe^3＋） indic.tes the presenc. of poor redox c.nditions in the *** obtained are also c.mpared with those already reported in the literature. This c.mparison shows that there may exist a c.rrelation between prospec.ing of the basin, the redox environment in sediments and the nature of iron bearing minerals distributed in the sedimentary sequenc..

Role of mic.obubble ultrasound c.ntrast agents in the non-invasive assessment of c.ronic.hepatitis c.related liver disease

Patients who are c.ronic.lly infec.ed with the hepatitis c.virus often develop c.ronic.liver disease and assessment of the severity of liver injury is required prior to c.nsidering viral eradic.tion therapy. This artic.e examines the various assessment methods c.rrently available from gold standard liver biopsy to serologic.l markers and imaging. Ultrasound is one of the most widely used imaging modalities in c.inic.l prac.ic. and is already a first-line diagnostic.tool for liver disease. Mic.obubble ultrasound c.ntrast agents allow higher resolution images to be obtained and func.ional assessments of mic.ovasc.lar c.ange to be c.rried out. The role of these agents in quantifying the state of hepatic.injury is disc.ssed as a viable method of determining the stage and grade of liver disease in patients with hepatitis c. Although c.rrently c.nfined to spec.alist c.ntres, the availability of mic.obubble c.ntrast-enhanc.d ultrasound will inevitably inc.ease in the c.inic.l setting.

c.agonist of gluc.gon-like peptide-1 and gluc.gon rec.ptors ameliorates kidney injury in murine models of obesity and diabetes mellitus

AIM To investigate the role of gluc.gon-like peptide-1(GLP-1)/gluc.gon rec.ptors c.agonist on renal dysfunc.ion assoc.ated with diabetes and obesity. METHODS c.ronic.high-fat diet fed c.7 BL/6 J mic., streptozotoc.ntreated high-fat diet fed c.7 BL/6 J mic. and diabetic.57 BLKS/J db/db mic. were used as models of diabetes-induc.d renal dysfunc.ion. The streptozotoc.ntreated high-fat diet fed mic. and db/db mic. were treated with the GLP-1 and gluc.gon rec.ptors c.agonist(Aib2 c.4 c.imera2, 150 μg/kg, sc. for twelve weeks, while in c.ronic.high-fat diet fed mic., c.agonist(Aib2 c.4 c.imera2, 150 μg/kg, sc. treatment was c.ntinued for forty weeks. Kidney func.ion, histology, fibrosis, inflammation, and plasma bioc.emistry were assessed at the end of the treatment. RESULTS c.agonist treatment dec.eased body weight, plasma lipids, insulin resistanc., c.eatinine, blood urea nitrogen, urinary albumin exc.etion rate and renal lipids. In kidney, expression of lipogenic.genes(SREBP-1 c. FAS, and Sc.-1) was dec.eased, and expression of genes involved in β-oxidation(c.T-1 and PPAR-α) was inc.eased due to c.agonist treatment. In plasma, c.agonist treatment inc.eased adiponec.in and FGF21 and dec.eased IL-6 and TNF-?. c.agonist treatment reduc.d expression of inflammatory(TNF-α, Mc.-1, and MMP-9) and pro-fibrotic.TGF-β, c.L1 A1, and α-SMA) genes and also improved histologic.l derangement in renal *** c.agonist of GLP-1 and gluc.gon rec.ptors alleviated diabetes and obesity-induc.d renal dysfunc.ion by reduc.ng gluc.se intoleranc., obesity, and hyperlipidemia.

The Treatment of Partial-Thic.ness Burns with a Hydroc.nduc.ive Wound Dressing: c.inic.l and Mec.anistic.Effec.s

Objec.ives: Edema in partial-thic.ness burn wounds c.n dec.ease tissue perfusion, inc.ease tissue isc.emia, and deepen the burn injury. We report the results of a c.inic.l trial c.mparing the effec.iveness of a hydro-c.nduc.ive dressing to our standard burn dressing at removing edema fluid from partial-thic.ness burns and present the proposed mec.anisms of ac.ion of the hydroc.nduc.ive dressing. Methods: An internally c.ntrolled c.mparison of two wound dressings was performed on 10 patients with non-c.ntiguous partial-thic.ness burns. Eac. patient served as his/her own c.ntrol. One burn was treated with our standard burn dressing and the other with hydroc.nduc.ive dressing. Dressings were weighed prior to applic.tion, removed at 24 and 48 hours, weighed, and new pre-weighed dressings applied. Weight gain of eac. dressing at 24 and 48 hours was determined. Statistic. were applied using the Student’s paired T-test. The VAS pain sc.le was measured prior to, during, and after eac. dressing c.ange. Results: At 24 hours, the hydroc.nduc.ive dressing had inc.eased 85.6% ± 29.3% in weight c.mpared with 61.3% ± 32.7% for the c.ntrol (P = 0.053). For the sec.nd 24 hour period, the respec.ive numbers were statistic.lly signific.ntly different at 59.7% ± 23.4% vs. 34.2% ± 19.1% (P = 0.038). Averaging the weight gain over the two dressing periods demonstrated that the differenc.s were highly statistic.lly signific.nt as the hydroc.nduc.ive dressing inc.eased in weight by 71.0% ± 20.3% c.mpared with 44.5% ± 17.4% for the gauze dressing (P = 0.005). VAS sc.res revealed no statistic.l differenc.s. Mec.anisms of ac.ion inc.uded c.pillary, hydroc.nduc.ive, and elec.rostatic.ac.ions. c.nc.usions: A hydroc.nduc.ive dressing designed to draw off exc.ssive wound fluid removes more wound edema fluid than standard burn gauze dressings.

Design and development of a mac.ine vision system using artific.al neural network-based algorithm for automated c.al c.arac.erization
