Effect of the Covid-19 Pandemic on Prenatal Consultations for Pathological Pregnancies at the Maternity Unit of Abass Ndao Hospital, Dakar, Senegal

Introduction: The COVID-19 pandemic has led to changes in health systems. The objective was to assess the effect of Covid-19 on the frequency of prenatal consultations (PNCs) for pathological pregnancies at the Abass NDAO Hospital Centre. Materials and Methods: This was primarily a retrospective study conducted at the Abass NDAO Hospital maternity ward. The target was the medical records of women followed for a pathological pregnancy at maternity from July 2019 to November 2020. Second, the department’s qualified staffs were asked about their experiences with the Covid-19 pandemic. Data collection was carried out between October 2021 and February 2022. Results: A total of 690 prenatal charts for pathological pregnancy were collected: 375 (54%) before the pandemic compared to 315 during Covid-19 (46%). The average age of women was 28.15 years. The age group 25 and 35 was the most represented (36.81%). Reasons for consultation were dominated by threats of premature delivery (51%), followed by severe pre-eclampsia (14%) and premature membrane rupture (11%), respectively. The frequency of prenatal consultations (PNCs) was inversely related to pandemic dynamics and significant monthly variations were observed. Qualified health personnel were midwives (71.40%) and gynecologists (28.60%). During the pandemic, more than the majority of staff (67%) worked 3 days a week, and 24% of health workers 5 days. The majority of staff (60%) reported experiencing overwork during the pandemic. Non-compliance with consultation days was noted in 38% of health providers. Protective equipment was not provided to 14% of the staff. Staff difficulties during prenatal consultations were fear of occupational exposure (12%) and patient reluctance (88%). Conclusion: The Covid-19 pandemic is having a major impact on prenatal consultations for pathological pregnancies at Abass NDAO Hospital. Native of West Africa, Saba senegalensis belongs to the family of Apocynaceae, and is a wild plant that is mainly exploited in the diet. It grows mainly along riverbanks, in wooded savannah areas in humid areas, in gallery forests and in rocky ravines and hills. Saba senegalensis is a large woody liana with white latex, dark gray bark, can reach more than 40 m high and a trunk that can measure up to 47 cm in diameter. The fruit is a globose shell, 7 to 10 cm long, 6 to 8 cm wide. The leaves are opposite, elliptical, dark green in color, about 8 to 15 cm long and 4 to 6 cm wide. This fruit is characterized by its richness in antioxidant molecules especially vitamin C (480 mg/100 g) and total polyphenols (945.83 mg/100 g). The fruit contents 41.43 until 80% water with a low pH (2.24). In Senegal, the fruit is eaten as it or transformed into puree, nectar, syrup, canned etc. It is a plant that has enormous therapeutic virtues, from the fruit to the roots through the leaves, bark, and latex. Despite its high nutritional potential, its many therapeutic virtues and its economic potential, Saba senegalensis remains under-exploited. Further researches on the nutritional and therapeutic properties deserve to be conducted.

The baobab, Adansonia digitata L. plays an important role in the economy of local populations. The oil from the seeds of the baobab fruit is nowadays highly prized because of its numerous cosmetic and therapeutic applications and its composition of unsaturated fatty acids, sterols and tocopherols. However, unlike refined oils, locally extracted baobab oil has not undergone purification operations to ensure its quality. Only a filtration on special cloths is carried out after decantation. Indeed, the oil obtained after pressing is cloudy because of the presence of various impurities. It therefore requires treatment operations to make it more attractive and of higher quality. Therefore, in order to provide innovative solutions to local companies to improve the quality of vegetable oils, a study of clarification (treatment) of crude oil is necessary. An experimental device has been developed in the laboratory. It includes a glass column and a filter bed of dune sand and activated carbon. This study has shown the efficiency of the experimental device. Indeed, the activated carbon, thanks to its adsorbing power, has allowed a significant decrease in turbidity at the 5% threshold, from 14.61 NTU for the raw oil to 0.08 NTU for the oil filtered on 3% carbon and 0.033 NTU for the oil filtered on 5% carbon. That is to say an abatement higher than 95%. This decrease in turbidity could be correlated with the decrease in brown index from 187.39a for the initial crude oil to 128.53d for the oil treated with 3% activated carbon versus 187.59a for the oil filtered on cloths. The lowest brown index was observed with the filtration using 5% activated carbon (35.99b). Thus, for the yellowness index, only the filtration on 5% charcoal allowed to obtain a significant decrease in yellowness. The Health Project—Community Health USAID is a community-based intervention that has been implemented by a group of NGOs, among which there is Africare, for a period of 5 years (2006-2011). It seeks to strengthen the community health system through a package of basic services offered at the health center, and as well as to improve the skills and knowledge of community actors. The main objective of this study is to capture the perceptions of beneficiaries and stakeholders about the services offered at the PSSC health posts located in the health district Goudomp. A qualitative cross-sectional study on the perception of the community of the USAID PSSC and quality of services offered by the health huts was conducted from the 2nd to 5th of May 2009 in the Goudomp health district. To achieve the objectives of this study, we conducted focus group discussions with all the community leaders and had individual interviews with the community health workers (CHWs) and the supervisor of the NGO Africare. From most of the discussions, it can be concluded that the activities of the health hut are not well known by the populations. The best known activities are those performed by the nurse: immunization and prenatal care. The existence of health huts at the community level was considered beneficial by all the community leaders. For the majority of the participants to the focus group discussions, the health huts “provided access to basic care”. Some participants, however, pointed out the “frequent drugs shortages” which, according to them, would explain the low attendance of the health huts in favor of the health post. The majority of CHWs and matrons felt that supervision activities were done on a regular basis. During the interviews, they mentioned many difficulties. However, the most common problem is the fact that the CHWs and the matron “who have no other source of income” were not salaried workers. 秀丽隐杆线虫的神经系统结构表明,在机器人领域应用动物智能是一种简单的方法。本文提出了一种基于Pybullet框架的四足蚂蚁运动控制方法。虽然增广随机搜索算法已经取得了显著的效果,并成功地应用于各种形式的大规模任务中,但仍有一些有待改进的地方。本文针对四足蚂蚁机器人在连续时间环境下的控制问题,提出了一种将随机搜索算法与蠕虫神经网络相结合的控制方法,该方法使蚂蚁的运动具有了一定的可解释性。本文主要研究内容如下:首先,对蠕虫(秀丽隐杆线虫)进行了介绍,给出了与蠕虫相关的背景,主要描述了蠕虫运动学假说。本文还讨论了仿生学在四足机器人领域的应用。其次,介绍了强化学习的理论和技术,并利用仿生蠕虫神经网络对蚂蚁四足机器人进行了训练。第三,我们回顾了机器人中四足动物的进化历程。描述了四足蚂蚁机器人在PyBullet环境中的结构和运动的数学模型。第四,以蚂蚁四足机器人为例,说明了抽头提取电路的功能和映射过程。蚂蚁四足机器人由111个输入和8个输出组成,为了控制蚂蚁四足机器人,我们需要对它的8个输出进行控制。首先,我们利用一个线性层,将111个输入变量映射为连续的2个变量,然后将它们输入抽头提取电路(TW电路)的2个感测神经元,最后再利用一个线性层,将2个运动神经元的神经电位映射到8个控制输出,即完成了对蚂蚁四足机器人的8个输出的控制。最后,我们通过观察PyBullet蚂蚁控制过程中的结果,展示了蠕虫神经网络的可解释性,蠕虫神经网络作为一种简单自然的人工神经网络结构,表现出了较好的性能。实验结果表明,将增广随机搜索算法与蠕虫网络相结合形成的神经电路策略比单独使用增广随机搜索算法的蚂蚁控制具有更好的性能。

Lifestyle Habits and Dietary Diversity among Medical Students at Université Gaston Berger (UGB) of Saint-Louis in 2023

Introduction: At university, habits, particularly eating habits, can negatively impact well-being. This study aimed to describe lifestyle habits and dietary diversity among medical students. Methodology: This was a cross-sectional, descriptive study, carried out at the UFR 2S of the University Gaston Berger from May 23 to June 3, 2023. The collection tool focused on sociodemographic characteristics, lifestyle habits and dietary diversity. Statistical analyzes were carried out using R software. Results: A total of 471 students participated in this study. Which corresponds to a participation rate of 89.2% with a M/F sex ratio of 1.66. The age of participants, varying from 18 to 32 years, had a median of 23 years and a mean of 22.4 years with a standard deviation of 2.2 years. Among the participants, 67.2% skipped breakfast, and 28.5% did not practice any physical sports activity. Alcohol and tobacco were consumed by 2.5% and 0.6% of participants, respectively. The most consumed foods were starchy foods (97.5% of men and 98.4% of women);and the least consumed were dark green leafy vegetables (21.9% of men and 15.1% of women) and offal (16.9% of men and 11.9% of women). The dietary diversity score was low for 31.9% of men and 28.7% of women. Conclusion: Certain lifestyle habits (skipping breakfast, dining late at night, lack of sports practice) of Medicine students at UGB can prove deleterious to their well-being. These students are at risk of developing several deficiencies in terms of food groups that are underrepresented in their diet. Awareness sessions on good lifestyle habits and diet would be relevant recommendations for improving the health of these students.

Coronary Angiography: Indications, Results and Cost-Effectiveness in the Diagnosis of Stable Angina Pectoris in Two Hospitals in Senegal

Background: The current gold standard for the diagnosis of stable coronary artery disease (CAD) is invasive coronary angiography. But a large proportion of patients undergoing coronary angiography don’t have obstructive coronary artery disease. Objective: The aim of the present study was to evaluate the diagnostic performance of invasive coronary angiography for patients without known coronary artery disease presenting with stable chest pain syndrome at two hospitals structures in Senegal. Method: We conducted a prospective, descriptive, and analytical study from March 1, 2019, to December 31, 2020 in the Cardiology Departments of General Hospital Idrissa Pouye (HOGIP) and Aristide Le Dantec Hospital (HALD). During the study period a cohort of patients referred to angiography coronary for diagnostic CAD because of suspected stable angina were enrolled. Demographic characteristics, risk factors, symptoms, and noninvasive test results were correlated with the presence of obstructive coronary artery disease. Results: A total of 143 patients were included in our study with a median age of 60.91 ± 10.58 years;men were 96 (67.13%) and women 47 (32.87%). The prevalence of hypertension was 60.84%;diabetes was 34.27%;dyslipidemia was 32.17% and sedentary was 26.57% in our study population. Typical and atypical angina symptoms were present in 37.76% (n = 54) and 49.65% (n = 71) respectively, while 10.49% had dyspnea. Coronary angiography revealed 59 (41.26%) patients with no CAD as well as 27 (18.88%) with one-vessel;28 (19.58%) with two-vessel, and 29 (20.28%) with three-vessel disease. Introduction The main objective of any healthcare establishment must be to ensure the quality of patient care and customer satisfaction. It is necessary to regularly assess patient satisfaction. The aim of this study was to assess the level of satisfaction of customers aged over 18 years attending the emergency department of the health center. Methodology This was a descriptive and analytical cross-sectional study of patients aged 18 years and over, who attended the Samu Municipal emergency department between 02 and 30 May 2023. The satisfaction index was determined using the adapted 2009 SAPHORA-MCO questionnaire and the Likert satisfaction scale. Results A total of 400 patients were surveyed. The average age was 35 years, with a standard deviation of 14.7. Of those surveyed, 51% were women, 87% were educated, 50% lived in Grand Yoff and 59.5% were unemployed. Satisfaction levels linked to perception of the cost of care (72%), waiting time (64.3%), information given to patients (69.1%) and pain management (74 .5%) are fair. On the other hand, the levels of satisfaction linked to administrative procedures (82.5%), staff attitudes towards patients (84%), staff availability (86.4%), patient privacy (89.2%), general atmosphere (87.2%), staff competence (87.3%), and the effectiveness of care (89.4%) were satisfactory. The average waiting time was 38 minutes. However, 32% of patients waited less than 30 minutes and 92% less than an hour. The satisfaction index linked to administration and reception was 72.9% and 79.85%, respectively. The satisfaction index linked to the administration and technical quality of care is equal to 85.8% and 83.7%, respectively. The overall satisfaction index is equal to 80.6%;the level of satisfaction of users of the health structure is satisfactory. Conclusion Patient satisfaction is an essential part of quality care.
