Despite percutaneous laser thermal ablation(LTA)of liver tumors being regarded as a safe technique,major complications can *** report the first case of hemorrhagic cardiac tamponade after LTA of a colorectal metastasis in segment II of the *** heat diffusion causing indirect thermal injury to the pericardium with resultant hemorrhagic reaction was hypothesized as the most likely cause of tamponade.A pericardial drain was emergently placed,200 mL of bright red blood were drained,and the patient showed rapid hemodynamic *** lesions located in segment II of the liver and strictly close to the pericardium,a careful risk/benefit analysis should be made by the multidisciplinary team to identify the best treatment option,taking into account both effectiveness and complications of each available technique.

A comprehensive picture of the initial stages of silicene growth on graphite is *** is shown that quasiparticle interferences play a crucial role in the formation of the observed silicene *** propose,on one hand,that the charge modulations caused by those quantum interferences serve as templates and guide the incoming Si atoms to self-assemble to the unique(√3 x√3)R30°honeycomb atomic *** the other hand,their limited extension limits the growth to about 150 Si atoms under our present deposition *** here proposed electrostatic interaction finally explains the unexpected stability of the observed silicene islands over time and with *** the robust guiding nature of those quantum interferences during the early growth phase,we demonstrate that the window of experimental conditions for silicene growth is quite narrow,making it an extremely challenging experimental ***,it is shown that the experimentally observed threedimensional silicon clusters might very well be the simple result of the end of the silicene growth resulting from the limited extent of the quasi-particle interferences.

We report a case of Krukenberg tumor of gastric origin with adnexal metastasis,in which ultrasonography(US) and contrast-enhanced US(CEUS) played a key diagnostic *** 64-year-old female patient was referred to our department for abdominal pain,nausea and *** examination was performed as first line diagnostic imaging approach,confirming the presence of ascites and detecting marked thickness of the gastric wall and a right adnexal *** was immediately performed and showed arterial enhancement followed by wash-out in the venous phase of both the gastric wall and the adnexal mass,suggesting the diagnosis of gastric cancer with right adnexal metastasis(Krukenberg syndrome).The patient underwent USguided paracentesis and esophagogastroduodenoscopy that showed linitis *** examination of the peritoneal fluid revealed the presence of signetring cells,and histologic examination of the specimen obtained by endoscopic biopsy showed primary gastric mucus-producing adenocarcinoma with signetring *** transvaginal US is undoubtedly the method of choice to evaluate ovarian tumors,abdominal US and CEUS can provide key diagnostic elements,supporting clinicians in the first steps of the diagnostic work-up of abdominal and pelvic masses.

Predicting wild boar damages to croplands in a mosaic of agricultural and natural areas

Crop damages by wildlife is a frequent form of human-wildlife conflict. Identifying areas where the risk of crop damages is highest is pivotal to set up preventive measures and reduce conflict. Species distribution models are routinely used to predict species distribution in response of environmental changes. The aim of this paper was assessing whether species distribution models can allow to identify the areas most at risk of crop damages, helping to set up management strategies aimed at the mitigation of human-wildlife conflicts. We obtained data on wild boar Sus scrofa damages to crops in the Alta Murgia National Park, Southern Italy, and related them to landscape features, to identify areas where the risk of wild boar damages is highest. We used MaxEnt to build species distribution models. We identified the spatial scale at which landscape mostly affects the distribution damages, and optimized the regularization parameter of models, through an information-theoretic approach based on AIC. Wild boar damages quickly increased in the period 2007-2011; cereals and legtmaes were the crops more affected. Large areas of the park have a high risk of wild boar damages. The risk of damages was related to low cover of urban areas or olive grows, intermediate values of forest cover, and high values of shrubland cover within a 2-km radius. Temporally independent validation data demonstrated that models can successfully predict damages in the future. Species distribution models can accurately identify the areas most at risk of wildlife damages, as models calibrated on data collected during only a subset of years correctly predicted damages in the subsequent year [Current Zoology 60 （2）： 170-179, 2014].

Cardiac primary tumors are uncommon with an estimated prevalence between 0.17%and 0.19%.[1,2]Cardiac lipoma are extremely rare representing only 10%?19%of primary cardiac tumors and only few are symptomatic,depending on their location within the heart.[3,4]They originate from the subendocardium(50%),subepicardium(25%)or myocardium(25%)and with different sizes and locations.[5]Few cases of sudden death due to primary cardiac tumors are reported in literature(0.0025%);in these cases,conductive and haemodinamic abnormalities have been indicated as the cause of death.

Bloating is one of the most common and bothersome symptoms complained by a large proportion of patients. This symptom has been described with various definitions, such as sensation of a distended abdomen or an abdominal tension or even excessive gas in the abdomen, although bloating should probably be defined as the feeling (e.g. a subjective sensation) of increased pressure within the abdomen. It is usually associated with functional gastrointestinal disorders, like irritable bowel syndrome, but when bloating is not part of another functional bowel or gastrointestinal disorder it is included as an independent entity in Rome III criteria named functional bloating. In terms of diagnosis, major difficulties are due to the lack of measurable parameters to assess and grade this symptom. In addition, it is still unclear to what extent the individual patient complaint of subjective bloating correlates with the objective evidence of abdominal distension. In fact, despite its clinical, social and economic relevance, bloating lacks a clear pathophysiology explanation, and an effective management endorsement, turning this common symptom into a true challenge for both patients and clinicians. different theories on bloating etiology call into questions an increased luminal contents (gas, stools, liquid or fat) and/or an impaired abdominal empting and/or an altered intra-abdominal volume displacement (abdomino-phrenic theory) and/or an increased perception of intestinal stimuli with a subsequent use of empirical treatments (diet modifications, antibiotics and/or probiotics, prokinetic drugs, antispasmodics, gas reducing agents and tricyclic antidepressants). In this review, our aim was to review the latest knowledge on bloating physiopathology and therapeutic options trying to shed lights on those processes where a clinician could intervene to modify disease course.

Biological and Nutritional Aspects of Human Milk in Feeding of Preterm Infants

Breastfeeding and human milk are the normative standards for feeding and nutrition of both term and pre-term infants. Fresh mother’s own milk is recognized as the optimal choice for feeding all newborns, including preterm and very low birth weight infants. Evidence documents short and long-term metabolic, immunologic and neurodevelopmental advantages of breastfeeding when compared to formula. Moreover, benefits of breastfeeding on psychological and relational aspects have to be considered. Currently, human milk supplementation is usually performed to meet the specific nutritional requirements of preterm infants. When mother’s milk is unavailable or in short supply, donor milk represents the best alternative, although some nutritional elements are inactivated by the necessary pasteurization process. Aim of this review is to briefly summarize the main biological and nutritional factors that contribute to the beneficial effects of human milk feeding for preterm infants.

A Retrospective Study on Hepatitis C Virus Infected Individuals Lost to Follow-Up in Ticino: The Hub and Spoke Approach

Background: Approximately 71.1 million individuals are chronically infected with hepatitis C virus (HCV). The global incidence of HCV was 23.7 cases per 100,000 population in 2015, with an estimated 1.75 million new HCV infections diagnosed in 2015. In Switzerland, it is estimated that 37,000 patients have chronic HCV infection. In the Southern part of Switzerland liver disease patients are mainly followed at the six outpatient clinics of Epatocentro Ticino (EPT) connected with its hub in Lugano. The aim of the study was to identify lost to follow-up patients and reconnect them to specialist care. Methods: We used the database from EPT to identify HCV positive patients, who were lost to follow-up in the years 2007-2017. We consider lost to follow-up patients who had their last visit more than 2 years before closing date of analysis. We contacted the patients on the phone or by mail. Results: Overall 1271 patients have been at EPT between 2007 and 2017;of those 74 were lost to follow-up at the time of our screening. We received an answer from 60 of them. The results we collected were that 12 (6%) have died, 31 (42%) were currently followed by the General Practitioner (GP) or a different specialist, 9 (12%) were cured from HCV infection and did not need any further visit, and 4 (5%) refused to come for a check-up visit. In total, we rescued 4 (5%) patients and follow them at the EPT. Conclusion: Our look-back study revealed that our “Hub and Spoke-approach” had a high retention rate such that only 6% of our HCV patients were lost to follow-up. The look-back effort was still useful: the number of patients to recall identifying one patient needing treatment was 19.
