Objective: To evaluate th. efficacy and safety of interferon beta- 1b (IFNβ - 1b) in subjects with.secondary progressive multiple sclerosis (SPMS). Meth.ds: Th.s 3- year, multicenter, double- blind, placebo- controlled, randomized trial of IFNβ - 1b included 939 subjects from th. United States and Canada with.SPMS and Expanded Disability Status Scale (EDSS) scores ranging from 3.0 to 6.5. Subjects were randomly assigned to receive eith.r placebo or IFNβ - 1b (250 μ g or 160 μ g/m2 body surface area), administered subcutaneously every oth.r day. Th. primary outcome was time to progression by ≥ 1.0 EDSS point (0.5 point if EDSS score was 6.0 to 6.5 at entry)- confirmed at 6 month.. Secondary outcomes included mean ch.nge in EDSS score from baseline, relapse- related measures, MRI activity, and a standardized neuropsych.logical function test. Results: Th.re was no significant difference in time to confirmed progression of EDSS scores between placebo- treated patients and eith.r of th. IFNβ - 1b treatment groups. h.wever, IFNβ - 1b treatment resulted in improvement on secondary outcome measures involving clinical relapses, newly active MRI lesions, and accumulated burden of disease on T2- weigh.ed MRI. Effects were similar for both.IFNβ - 1b treatment groups. Neutralizing antibodies to IFNβ - 1b were detected in 23% of 250- μ g and 32% of 160- μ g/m2 recipients, but th.ir presence did not consistently affect clinical or MRI outcomes. IFNβ - 1b was also well tolerated at both.doses. Conclusions: Alth.ugh.no treatment benefit was seen on th. time to confirmed progression of disability, relapse- and MRI- related outcomes sh.wed significant benefit with.both.dosing regimens tested, a result consistent with.th. outcomes of earlier clinical trials.

Th. EVIDENCE trial demonstrated th.t interferon (IFN)beta-1a, 44 mcg subcutaneously (sc) th.ee times weekly (tiw) (Rebif), was significantly more effective th.n IFN beta-1a, 30 mcg intramuscularly (im) once weekly (qw) (Avonex), in reducing relapses and magnetic resonance imaging (MRI)activity in patients with.relapsing-remitting multiple sclerosis at both.24 and 48 weeks of th.rapy. We now present final comparative data on th.se patients, sh.wing th.t th. superior efficacy of IFN beta-1a, 44 mcg sc tiw, for relapse measures and MRI activity, compared with.IFN beta-1a, 30 mcg im qw, was sustained for at least 16 month.. Th. development of antibodies to IFN was associated with.reduced efficacy on MRI measures and fewer IFN-related adverse events, but did not h.ve an impact on relapse outcomes.

A bio-instructive parylene-based conformal coating suppresses th.ombosis and intimal h.perplasia of implantable vascular devices

Implantable vascular devices are widely used in clinical treatments for various vascular diseases. h.wever, current approved clinical implantable vascular devices generally h.ve h.gh.failure rates primarily due to th.ir surface lacking inh.rent functional endoth.lium. h.re, inspired by th. path.logical mech.nisms of vascular device failure and ph.siological functions of native endoth.lium, we developed a new generation of bioactive parylene (poly(p-xylylene))-based conformal coating to address th.se ch.llenges of th. vascular devices. Th.s coating used a polyeth.lene glycol (PEG) linker to introduce an endoth.lial progenitor cell (EPC) specific binding ligand LXW7 (cGRGDdvc) onto th. vascular devices for preventing platelet adh.sion and selectively capturing endogenous EPCs. Also, we confirmed th. long-term stability and function of th.s coating in h.man serum. Using two vascular disease-related large animal models, a porcine carotid artery interposition model and a porcine carotid artery-jugular vein arteriovenous graft model, we demonstrated th.t th.s coating enabled rapid generation of self-renewable “living” endoth.lium on th. blood contacting surface of th. expanded polytetrafluoroeth.lene (ePTFE) grafts after implantation. We expect th.s easy-to-apply conformal coating will present a promising avenue to engineer surface properties of “off-th.-sh.lf” implantable vascular devices for long-lasting performance in th. clinical settings.
