Introduction: post-hospital syndrome (pHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of pHS. Objective: The objective is to estimate the prevalence of pHS that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. Material and Methods: It is a descriptive observational study of patients with hyponatremia who were discharged from 1 September 2010 to 2 February 2020 at the Internal Medicine Service of the Hospital University of San Juan (Alicante, Spain). Results: Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [SD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. Conclusion: Hyponatremia may be the cause of pHS, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on pHS.

Using TiO2-Biocharcoal and TiO2-Diatomite for photodisinfection in Washing Machine Wastewater

The photodisinfection process using biomolded semiconductor photocatalysts can inactivate bacteria in wastewater washing machine samples. The comparative study evaluated the photocatalyst material titanium dioxide (TiO2) synthesized with diatomite and biocharcoal biotemplate (TiO2-Biocharcoal and TiO2-Diatomite) in photodisinfection processes using domestic washing machine wastewater samples, the results of bacterial inactivation were above 96%. The efficiency of the photodisinfection process was evaluated by counting the number of colonies of the bacteria. Experiments under LED solar lamps presented similar bacterial inactivation, and a correlation with kinetic models. The kinetic study demonstrated a curved regression, indicating a better fit with the Hom model. A tail at the end of the modeling curve indicates the presence of a high concentration of inactive bacteria in the medium, while a shoulder at the beginning of the curve suggests a heterogeneous sample with a high concentration of gram-positive bacteria. The toxicity tests performed with wastewater samples without light exposure indicated low toxicity for both materials. The study presented promising disinfection results for an accessible and efficient photo-sterilization process of water contaminated with bacteria using abundant solar and renewable energy throughout the national territory. .

Development of a modified predicted heat strain model for hot work environments

Excessive exposure to heat can lead to injuries,illness,and death among *** actual cost of heat-related injuries and illnesses is unknown because of underreporting and lack of symptom ***-factorial,evidence-based,and field-ready guidelines for identifying–and predicting–physiolo gical markers of heat strain are currently *** predicted heat strain(pHS)model,is the latest attempt by mining companies to aid in the evaluation and management of occupational heat *** adopted algorithm relies on worksite environmental measurements and an estimate of individual metabolic rate for mine workers to provide an estimate of the workers’core temperature during a work *** are several known limitations of the pHS model,including the assumption that the subject worker is hydrated and fit.A modified pHS model was presented based on eight physical parameters that are measured at different intervals during a work shift;these parameters are air temperature,relative humidity,air velocity,radiation,metabolic rate,acclimatization,clothing insulation and *** validate the results,the predictions from the modified pHS model were compared with direct physiological measurements obtained from ingestible pills and heat stress monitors under different environmental and working conditions.

The inflammato ry response plays an important role in neuroprotection and regeneration after ischemic *** use of non-ste roidal anti-inflammatory drugs has been a matter of debate as to whether they have beneficial or detrimental *** this context,the effects of the anti-inflammatory agent meloxicam have been scarcely documented after stro ke,but its ability to inhibit both cyclooxygenase isoforms(1 and 2) could be a promising strategy to modulate postischemic *** study analyzed the effect of meloxicam in a transient focal cerebral ischemia model in rats,measuring its neuroprotective effect after 48 hours and 7 days of reperfusion and the effects of the treatment on the glial scar and regenerative events such as the generation of new progenitors in the subventricular zone and axonal sprouting at the edge of the damaged *** show that meloxicam’s neuroprotective effects remained after 7 days of reperfusion even if its administration was restricted to the two first days after ***,meloxicam treatment modulated glial scar reactivity,which matched with an increase in axonal ***,this treatment decreased the formation of neuronal progenitor *** study discusses the dual role of anti-inflammatory treatments after stro ke and encourages the careful analysis of both the neuroprotective and the regenerative effects in preclinical studies.

Newly Described Components and Regulatory Mechanisms of Circadian Clock Function in Arabidopsis thaliana

The circadian clock temporally coordinates plant growth and metabolism in close synchronization with the diurnal and seasonal environmental changes. Research over the last decade has identified a number of clock components and a variety of regulatory mechanisms responsible for the rhythmic oscillations in metabolic and physiological activities. At the core of the clock, transcriptional/translational feedback loops modulate the expression of a significant proportion of the genome. In this article, we briefly describe some of the very recent advances that have improved our understanding of clock organization and function in Arabidopsis thaliana. The new studies illustrate the role of clock protein complex for- mation on circadian gating of plant growth and identify alternative splicing as a new regulatory mechanism for clock function. Examination of key clock properties such as temperature compensation has also opened new avenues for func- tional research within the plant clockwork. The emerging connections between the circadian clock and metabolism, hor- mone signaling and response to biotic and abiotic stress also add new layers of complexity to the clock network and underscore the significance of the circadian clock regulating the daily life of plants.

Hepatitis C virus NS5A region mutation in chronic hepatitis C genotype 1 patients who are non-responders to two or more treatments and its relationship with response to a new treatment

To determine the number of mutations in the NS5A region of the hepatitis C virus (HCV) and its relationship to the response to antiviral therapy in patients with chronic hepatitis C genotype 1 who are non-responders to two or more treatments. METHODSSequences within HCV NS5A [pKR binding domain (pKRBD) and the interferon-sensitivity-determining region (ISDR)] were analysed via direct sequencing in a selected cohort of 72 patients, with a total of 201 treatments [interferon-alpha (IFN-α), n = 49; IFN-α + ribavirin (RBV), n = 75; pegylated (peg) IFN-α + RBV, n = 47; first-generation direct-acting antivirals (DAAs), n = 13; and second-generation DAAs, n = 17]. Of these, 48/201 achieved a sustained virological response (SVR) and 153/201 achieved no virological response (NVR). RESULTSFor both regions, treatments resulting in SVR were associated with more baseline mutations than were treatments resulting in NVR (SVR vs NVR; pKRBD: 5.82 ± 3 vs 4.86 ± 2 mutations, p = 0.045; ISDR: 2.65 ± 2 vs 1.51 ± 1.7 mutations, p = 0.005). A decrease or no change in the number of mutations over time between treatments in the pKRBD or ISDR, as shown by sequencing, was associated with patients who usually failed to respond to treatment (pKRBD, p = 0.02; ISDR, p = 0.001). Moreover, patients showing a post-treatment baseline viral load > 600000 IU/mL and increased ISDR mutations with respect to the previous treatment were 9.21 times more likely to achieve SVR (p = 0.001). CONCLUSIONThe obtained results show that among patients who have shown no response to two or more antiviral treatments, the likelihood of achieving SVR increases with the genetic variability in the ISDR region (≥ 2 mutations or number of substitutions from the HCV-J and HCV-1 prototype), especially when the viral load is greater than 600000 IU/mL.
