Background and Obj.ctives: The aim of this study is to evaluate the usefulness of Magnetic Resonance Imaging (MRI) for planning surgery and postoperative control of complex cases of abdominal wall hernia. Methods: Included in a prospective data base with more than 350 patients treated in two centers, a prospective study over ten patients with complex incisional hernias was designed. In all cases, preoperative diagnosis study and postoperative imaging control by MR Imaging were established. In the early and late follow-up controls, local and general complications were specifically noted and studied by MRI in each case. Results: The preoperative MRI was effective method for an adequate understanding of the defects and hernia sack content. In the postoperative control, the MRI was useful in the follow-up of the mesh integration, detection of complications and diagnosis of the relapsing. Conclusion: In our experience the MRI test is specially recommended in the preoperative workup of complex cases of incisional hernia.

This paper aims to assess the use of the sentinel node technique in squamous cell carcinoma (SCC) of the trunk and extremities in a Latin American oncology reference center. The descriptive retrospective cohort study included 60 patients diagnosed with SCC of the trunk and extremities, submitted to surgical treatment of the primary tumor and sentinel node biopsy at the breast and soft tissue tumor services of the National Institute of Cancerology (Bogotá, Colombia) over a period of 6 years. The sentinel node was identified in 96.6% (58/60) of the patients. The sentinel node pathology report was negative in 81.7% (49), positive in 15% (9). There were no complications due to the procedure in 85% of the patients. The mean recurrence-free survival time was 8.3 months (CI 95% 5.0 to 11.5 months) in patients with positive sentinel node and 58.6 months (CI 95% 47.8 to 69.3 months) in patients with negative results. Only 4 of 49 patients (8.1%) with negative sentinel node had regional relapse. The study evidenced that the sentinel node technique in patients with high-risk SCC of the trunk and extremities is an adequate staging tool for the lymph node chain, with a low rate of associated complications. This opens an interesting opportunity for prospective cohort studies that can demonstrate statistically significant differences.

Freshwater biota are more comprehensive and direct indicators of biological impacts,and more meaningful to the public than water quality or physical habitat *** biotic data and the multiple biological indicators developed from them offer a much richer array of data for assessing the impacts of pollution controls than a limited set of physical or chemical *** recent decades,assemblage-based assessments by ecologists,environmental scientists,and water quality agencies have been employed globally for determining the condition of,and threats to,freshwater ecosystems.A key step in this advance has been the development of multimetric indices(MMIs)or indices of biotic integrity(IBIs)based on quantitative assessments of algae,macrophyte,macroinvertebrate,fish or riparian bird *** Europe,where biological assemblages are mandated for assessing freshwater ecosystem health,many indices are ***,the proliferation of MMIs globally has not always occurred through the application of rigorous study designs and monitoring protocols,nor have they always effectively incorporated functional metrics,stressor assessments,and statistical ***,in this review,we discuss eleven maj.r concerns with the development and application(including logistical limitations)of multimetric indicators based on freshwater biota to encourage more rigorous and widely applicable(transferable)MMI use and ***,our concerns focus on reference conditions;sampling effort,methods,and season;trophic guild definition;metric comprehensiveness,options,screening and scoring;and MMI *** could also benefit from increased attention to ecological mechanisms and metric development,to further improve our understanding of anthropogenic impacts as well as rehabilitation effects on freshwater ecosystems *** closer attention to study designs,ecological mechanisms and metric development should further improve our understa

This paper presents a CFD study about the effect of the V and U grooves in the flow over four cylinders in diamond shape configuration at subcritical flow conditions（Re= 41 000）. The k- ε Realizable turbulence model was implemented to fully structured hexahedral grids with near-wall refinements. Results showed that the numerical model was able to reproduce the impinging flow pattern and the repulsive forces present in the lateral cylinders of the smooth cylinder array. As a consequence of the flow alignment induced by the grooves, a j.t-flow is formed between the lateral cylinders, which could cause an important vortex induced vibration effect especially in the rear cylinder. The magnitudes of the shear stresses at the valleys and peaks for the V grooved cylinders were lower than those of the U grooved cylinders, but the separation points were delayed due the U grooves presence. It is discussed the presence of a blowing effect caused by counter-rotating eddies located near the grooves peaks that cause a decrease of the shear stresses in the valleys, and promote them at the peaks.

High sensitivity radio searches of unassociated γ-ray sources have proven to be an effective way of finding new pulsars. Using the Five-hundred-meter Aperture Spherical radio Telescope(FAST) during its commissioning phase, we have carried out a number of targeted deep searches of Fermi Large Area Telescope(LAT) γ-ray sources. On February 27, 2018 we discovered an isolated millisecond pulsar(MSP), PSR j.318+0253, coincident with the unassociated γ-ray source 3 FGL j.318.1+0252. PSR j.318+0253 has a spin period of 5.19 ms, a dispersion measure(DM) of 26 pc cm-3 corresponding to a DM distance of about 1.3 kpc, and a period-averaged flux density of(~11±2) μj. at L-band(1.05-1.45 GHz). Among all high energy MSPs, PSR j.318+0253 is the faintest ever detected in radio bands, by a factor of at least ~4 in terms of L-band fluxes. With the aid of the radio ephemeris, an analysis of 9.6 years of Fermi-LAT data revealed that PSR j.318+0253 also displays strong γ-ray pulsations. Follow-up observations carried out by both Arecibo and FAST suggest a likely spectral turn-over around 350 MHz. This is the first result from the collaboration between FAST and the Fermi-LAT teams as well as the first confirmed new MSP discovery by FAST, raising hopes for the detection of many more MSPs. Such discoveries will make a significant contribution to our understanding of the neutron star zoo while potentially contributing to the future detection of gravitational waves, via pulsar timing array(PTA) experiments.

AIM To investigate the rates and determinants of success of repeat percutaneous coronary intervention(PCI) following an initial failed attempt at recanalising the chronic total occlusions(CTO) *** In 445 consecutive first attempt CTO-PCI procedures in our institution,procedural failure occurred in 149(33.5%). Sixty-four re-PCI procedures were performed in 58 patients(39%) all had a single CTO. Procedural and outcome data in the re-PCI population was entered into the institutional database. A retrospective analysis of clinical,angiographic and procedural data was performed. RESULTS Procedural success was achieved in 41(64%) procedures. Univariate analysis of clinical and angiographic characteristics showed that re-PCI success was associated with intravascular ultrasound(IVUS) guidance(19.5% vs 0%,P = 0.042),while failure was associated with severecalcification(30.4% vs 9.7%,P = 0.047) and a j.TO score > 3(56.5% vs 17.1% P = 0.003). Following multiple regression analysis the degree of lesion complexity(j.CTO score > 3),IVUS use,involvement of an experienced CTO operator and LAD CTO location were significant predictors of successful re-PCI. Overall the complication rate was low,with the only MACCE two periprocedural MI's neither of which required intervention. CONCLUSION Re-PCI substantially increases the overall success rate of CTO revascularization. Predictors of re-PCI success included the use of IVUS,the involvement of an experienced CTO operator in the repeat attempt and the location of the CTO.

Aim: Advanced age and fragility often lead to dependence, making caregiver assistance necessary for performing the activities of daily living. Patients with diabetes are characterized by an elevated presence of comorbidities and subsequent frailty—a relevant consideration when making decisions regarding treatment goals. Health professionals may overlook the caregiver experience when assessing the circumstances surrounding frail elderly patients with diabetes, but this factor can have a large impact on relevant caregiver and patient outcomes. Methods: An observational, retrospective study was carried out to assess the impact of improved management in diabetes on caregiver burden. We assessed patients’ performance of activities of daily living using the Barthel scale. Our primary outcome was caregiver burden, as assessed using the Zarit scale. Results: At baseline, 81.4% of the caregivers reported severe overburden (>55 points on the Zarit scale). At three months, the proportion of caregivers reporting this level of burden had dropped to 76.7% and remained stable at six months (77.1%). In contrast, 8.5% of the caregivers reported moderate overburden (47 to 55 points) at baseline, compared to 13.3% at three months and 12.5% at six. The proportion of caregivers reporting no overburden (Conclusions: Caregiver burden remained persistently high, decreasing only slightly after six months.
