The Impact of Health Information Technology on Hospital performance: A Systematic Integrative Literature Review

Objective: To review, categorise, and synthesise findings from literature on health information technology (HIT) functionalities, HIT use, and the impact of HIT on hospital performance. Materials and Methods: We conducted a systematic integrative literature review based on a compre-hensive database search. To organise, categorise and synthesise the ex-isting literature, we adopted the affordance actualization theory. To align the literature with our research framework, we used four categories: 1) the functionalities of HIT and how these functionalities are measured;2) use and immediate outcomes of HIT functionalities;3) different perfor-mance indicators and how HIT functionalities affect them;and 4) what hospital characteristics influence the outcome of hospital performance. Results: Fifty-two studies were included. We identified four types of HIT. Only ten studies (19.2%) define the use of HIT by explicitly meas-uring the use rate of HIT. We identified five dimensions of hospital per-formance indicators. Every dimension showed mixed results;however, in general, HIT has a positive impact on mortality and patient readmis-sions. We found several hospital characteristics that may affect the rela-tionship between HIT and hospital-level outcomes. Discussion: Further efforts should focus on embedded research on HIT functionalities, use and effects of HIT implementations with more performance indicators and adjusted for hospital characteristics. Conclusion: The proposed framework could help hospitals and researchers make decisions regard-ing the functionalities, use and effects of HIT implementation in hospi-tals. Given our research outcomes, we suggest future research opportuni-ties to improve understanding of how HIT affects hospital performance. .

目的评估3.0T三维膝关节软骨延迟钆增强MR成像(dGEMRIC)在早期关节炎诊断中的可重复性。方法 20例病人在7d内行两次3.0T三维膝关节dGEMRIC。为校正在采集过程中病人的运动,所有影像都严格进行三维刚性标注。在每例病人的两种影像上对8个关节软骨解剖学上的感兴趣区进行分析。基于T1影像的dGEMRIC的可重复性检测在每例病人每一层面的2个感兴趣区之间分别进行评估,T1影像可在空间上分辨关节软骨的质量。使用同类相关系数(ICC)和Bland-Altman图法对可重复性进行评估。结果 ICC的范围为0.87～0.95,表明具有较好的重复性。T1影像显示软骨质量空间差异具有可重复性(ICC为0.79)。基于Bland-Altman法,可确定在纵向研究中使用dGEMRIC判断结果的变化,可以将95ms作为具有差异的阈值。结论膝关节软骨三维延迟钆增强3.0TMR成像与三维图像标记结合,在早期的膝关节炎诊断中显示软骨的质量具有较高的可重复性。因此,在有早期关节炎或有局灶性软骨缺陷病人的纵向研究中,dGEMRIC可以作为一种对评价关节软骨质量有价值的无创性的工具。要点①膝关节软骨延迟钆增强MR成像(dGEMRIC)可以评估关节炎。②对早期膝关节炎病人,dGEMRIC可产生具有较高可重复性的T1值。③建立的阈值可以确定dGEMRIC结果的显著性改变。④在纵向研究中,dGEMRIC可以用来评估软骨质量。

Assessment of food supplementation and surveillance as techniques to reduce damage caused by black capuchin monkeys Sapajus nigritus to forest plantations

Black capuchin monkeys Sapajus nigritus are among several mammalian species that damage forest plantations, pri- marily plantations of pine trees （pinus spp.） in South Brazil. Management practices to reduce these damages include supplemen- tary food offerings and plantation surveillance. To assess the efficiency of these techniques, four treatments were evaluated： feeding （F, with supplementary bananas and maize）, surveillance （S, in situ, wherein men patrolled the plantations, deterring the monkeys）, feeding ＋ surveillance （FS） and control （no procedures）（C）. The efficiency assessment of each treatment involved the number of recorded primate sightings in the treatments with the presence of guards （S and FS） and a damage inventory of the outskirts of the experimental points immediately before and after the experiment. These data were complemented by a simulta- neous study of the availability of fruit, the primary item in the diet of this primate species, in the remnant native forest in the study area. Most sightings of S. nigritus occurred early in the morning, and in general, sightings were inversely proportional to the availability of native fruits. This trend was most evident in treatment FS, most likely due to the presence of food. Treatments F and FS exhibited higher percentages of damage, indicating that these are inefficient management techniques. Treatment S exhi- bited the lowest damage rate, but at a high cost, making this technique difficult to apply over large expanses. The development of other management techniques, such as environmental enrichment and the use of deterrents, is recommended to minimize the damage caused by capuchins in forest plantations [Current Zoology 60 （5）： 581-590, 2014].

With tremendous weight saving potential,magnesium alloy high pressure die casting components have been widely used for automotive *** fastening technology is thus becoming increasingly important to design *** as-cast holes of magnesium high pressure die casting components with thread forming fasteners provides significant advantages for the assembly process,overall cost benefit and joint *** type of joint is thus preferred for structural *** the thread forming fasteners with as-cast holes follows the general rules for designing for the machined holes,including carefully designing the variables such as the assembly torque range,the length of thread engagement,the hole diameter and required failure *** addition,special attention needs to be paid to the draft angles of the magnesium cast components that are required for the die casting *** this paper,the effects of above key factors,individually and combined,on the joint performance of thread forming fasteners with as-cast blind holes of AM60B magnesium components are studied.A joint design philosophy was proposed to optimize both joint performance(the prevailing torque,the failure torque and the failure mode)and manufacturing easiness(the hole diameter and corresponding draft angle).The detailed design considerations for as-cast holes of magnesium HpDC are discussed and explained through a hypothetical example.

Corrections to the scaling of the second-order structure function in isotropic turbulence

The approach of Obukhov assuming a constant skewness was used to obtain analytical corrections to the scaling of the second order structure function, starting from kolmogorov＇s 4/5 law. These corrections can be used in model applications in which explicit expressions, rather than numerical solutions are needed. The comparison with an interpolation formula proposed by Batchelor, showed that the latter gives surprisingly precise results. The modification of the same method to obtain analytical corrections to the scaling law, taking into account the possible corrections induced by intermittency, is also proposed.
