Climate variability impacts on cocoa production are evaluated for the first time using 31 years (1979-2010) of data from SODEXAM (climate ground- based observations) and the ex-CAISTAB in three main cocoa production regions (Goh, Marahoué, and Haut-Sassandra) in the west-central part of Cote d’Ivoire. The work is a contribution to improving the quality of climate services dedicated to cocoa cultivation to ensure producers’ income and improve the yield of the production in the west-central part of Cote d’Ivoire. The results show that cocoa production is affected by the changes and variability in climate conditions (i.e. rainfall and temperature). In the Goh region, the increase in cocoa production seems to be mostly related to the augmentation of rainfall amount while in Marahoué, the increase in temperature is identified to have a more significant impact. Over the Haut-Sassandra region, both temperature and rainfall seem to have a considerable effect on the changes in cocoa production. The analysis based on linear regression by least-squares fit shows two characteristic modes (low and high-frequency variability) in the relationships between cocoa production and meteorological conditions suggesting a strong temporal signal impact related to the changes in the emblazoned surfaces. This leads to an important impact of the short-term variations of climate in cocoa production whereas, the influence of the long-term variability in climate on the cocoa yield seems marginal.

目的讨论肺结核的发病现状及有效预防控制策略。方法于2018年1月至2019年2月,选取我州的110例肺结核患者成为此次研究观察对象,继而将其随机分为对照组(给予常规的病情监测及用药指导,不实施预防控制策略)与研究组(实施预防控制策略)各55例,评判标准:并发症发生情况、临床症状缓解率及健康知识知晓率。结果研究组患者肺气肿及肺心病并发症的发生率明显低于对照组,ppp<0.05。结论预防控制策略的实施可有效的降低肺结核患者发生并发症的机率,改善其临床症状,提高其健康知识知晓情况,值得推广。

＜正＞ Major cytochrome p450 （ CYp）, drug metabolism enzymes and transporters are regulated by nuclear receptors: aryl hydrocarbon receptor （AhR）, constitutive androstane receptor （CAR） and pregnane X receptor （pXR）. These receptors establish cross-talk with other signalling pathways. For example. CAR and pXR are regulated by the glucocorticoid receptor （GR） and the hypothesis of a GR-[ CAR/ pXR]-CYp cascade has been proposed （pascussi et al. Mol pharmacol 2000;Mol Endocrinol 2003）. Consistent with this hypothesis, we showed that the repressive effect of interleukin-1β on drug metabolism is linked to the inhibitory effect of NFkB on GR transcriptional activity （Assenat et al. Hepatology 2004）. Another important cross-talk concerns the vitamin D receptor （VDR）: VDR binds to and transactivates pXR/CAR responsive elements of CYp2B6/2C9/3A4 （dro-court et al. J Biol Chem 2002）, while pXR activates VDR-target genes including CYp24 encoding a mitochondrial p450 involved in catabolism of vitamin D （pascussi et al. J Clin Invest 2005）. Other examples will be presented. These crosstalks provide new views for understanding how physiopathological stimuli affect drug metabolism and how drugs might excert adverse effects.

BACKGROUND Haemoglobin drop is common in acute coronary syndrome(ACS)patients and correlates with poor ***,the association between mild haemoglobin drop and adverse clinical outcome remains insufficiently *** study aimed to examine the association between in-hospital haemoglobin drop and risk for adverse clinical outcomes in ACS patients,especially those with mild *** Included patients from the BleeMACS(Bleeding complications in a Multicenter registry of patients discharged after an Acute Coronary Syndrome)registry were categorized into three groups by the presence and amount of in-hospital haemoglobin drop(non-drop,mild drop and severe drop).The cut-off point between mild drop and severe drop is≥3 g/*** Cox regression was used to assess the association between haemoglobin drop and major adverse cardiac endpoints(MACE).patients taking potent p2Y_(12) inhibitors were selected for the additional *** score matching was used to avoid selective bias in the additional *** Of 6911 patients,4949 patients(71.6%)experienced in-hospital haemoglobin *** with non-drop group,patients with haemoglobin drop had higher risk of MACE[adjusted hazard ratio(HR)=1.36,95%CI:1.03–1.80 for mild drop group;adjusted HR=1.70,95%CI:1.07–2.68 for severe drop group].patients in mild drop group were less likely to receive potent p2Y_(12) inhibitors at discharge(mild drop group *** drop group ***-drop group:10.9%vs.10.7%vs.23.8%).After propensity score matching adjustment among patients with potent p2Y_(12) inhibitors,patients in mild drop group were not associated with an increased risk of MACE than those in non-drop group(adjusted HR=1.52,95%CI:0.49–4.72).CONCLUSIONS In-hospital haemoglobin drop was common in ACS patients and associated with a higher risk for adverse *** prescription for potent p2Y_(12) inhibitors may be responsible for poor prognoses among patients with mild haemoglobin dro

目的:探讨复方余甘子散对糖尿病大鼠血糖及血液流变学的影响。方法:大鼠50只随机分为模型对照组、实验组及正常对照组;其中实验组分为:高剂量组(2 g/kg)、中剂量组(1 g/kg)、低剂量组(0.5 g/kg),每组10只。模型对照组及正常对照组,均按l mL/100 g体重每天灌胃生理盐水一次,实验组予以复方余甘子散按照分组进行不同剂量灌胃给药,每日一次,连续21天;分别从实验开始的第0、7、14、21天检测所有大鼠的空腹血糖;21天末次给药后lh腹腔注射10g/kg乌拉坦麻醉,从腹主动脉抽血,置加肝素抗凝剂的洁净干燥试管中,抗凝血于血流变仪测定血液流变学各项指标。结果:用药21天后,实验组的大鼠的多饮、多食、多尿、消瘦等糖尿病症状较模型对照组有一定改善;实验组中各剂量组空腹血糖水平,随给药时间呈现持续下降趋势,21天后实验组中各剂量组空腹血糖水平较模型对照组明显降低,差异有统计学意义(ppp<0.05)。结论:复方余甘子散能降低STZ诱导的糖尿病大鼠的空腹血糖水平,同时具有改善糖尿病大鼠血液流变学"粘、凝、聚"的倾向,提示该药物可用于糖尿病及其并发症的临床治疗。

目的分析接受连续性肾脏替代治疗(CRRT)的急性肾损伤(AKI)患者血小板(pLT)减少与其短期预后的相关性.方法回顾性分析2016年1月至2019年2月南通大学附属医院ICU收治的130例诊断为AKI且接受CRRT患者的临床资料,根据院内预后分为存活组(n=72)和死亡组(n=58),单因素分析比较两组临床数据差异.多因素Logistic回归分析患者死亡的独立危险因素.绘制受试者工作特征(ROC)曲线,评估血小板变化率(△pLT)对院内死亡的预测价值.根据△pLT的最佳截断值进行分组,对各组患者进行Kaplan-Meier生存曲线分析,比较患者机械通气时间、ICU住院时间、总住院时间及28 d累积生存率的差异.结果①58例患者住院期间死亡,病死率为44.6%.死亡组患者急性生理学及慢性健康状况评估系统Ⅱ(ApACHEⅡ)评分、序贯器官衰竭评分(SOFA)、接受有创机械通气、使用升压药及输血的比例高于存活组.②69例患者(53.0%)存在基线血小板减少症(Tp).CRRT后新发Tp 36例(27.7%).死亡组患者CRRT后血小板最低值低于存活组(24.00×109/L vs.58.50×109/L,ppLT预测患者住院期间死亡的曲线下面积(AUC)为0.711(95%CI0.621~0.802,ppLT≥51.28%是接受CRRT的AKI患者院内死亡的独立危险因素(OR=3.349,95%CI1.500~7.474,p=0.003).⑤Kaplan-Meier曲线分析显示,CRRT后pLT下降51.28%以上的患者机械通气时间、ICU住院时间及总住院时间延长,28 d累积生存率降低.结论血小板减少在接受CRRT的AKI患者中常见,且与患者短期不良预后相关.
