New Approach to Measuring the Ankle and Toe Brachial Indices as New Markers for Early Detection of Lower Extremity peripheral Artery Disease

Background: Lower extremity peripheral artery disease (pAD) is caused by atherosclerosis, or plaque buildup, that reduces the blood flow to the legs and feet. pAD affects approximately 230 million adults worldwide and is associated with an increased risk of coronary heart disease, stroke, and leg amputation. The first-line method for diagnosis of pAD is the Ankle Brachial Index (ABI), which is the ratio of ankle to brachial higher systolic pressure measured in ankles and arms. The Toe Brachial Index (TBI), which is the ratio of the toe systolic pressure to brachial higher systolic pressure measured in both arms, is considered to be an alternative to the ABI in screening for pAD. The ABI and TBI are measured on the right and left side, and the lower of these numbers is the patient’s overall ABI and TBI. Clinical studies and meta-analysis reviews have shown that the conventional ABI measurement, which uses a cuff, and handheld sphygmomanometer and continuous-wave Doppler tracings, provides an acceptable-to-high specificity level but low sensitivity when compared with vascular color Doppler ultrasound, and/or angiography methods. Another study has shown that the TBI measurement has greater sensitivity but lower specificity than the ABI when compared with vascular color Doppler ultrasound diagnostic based on waveforms. The aim of this clinical study was to evaluate the specificity and sensitivity of the Vasopad System comparing its results to the vascular color doppler ultrasound waveforms. Materials and Methods: The Vasopad System is an automated device using the pulse wave method to measure the arms and ankles dorsalis and tibial posterior artery blood pressures, the photoplethysmography second derivative (pTGSD) to estimate the toe systolic pressure, a patented photoplethysmography (pTG) index marker and volume plethysmography via cuffs during deflation. Geostatistical Analyst for Deciding Optimal Interpolation Strategies for Delineating Compact Zones

Variability maps of Hydraulic conductivity (K) were generated by using geo statistical analyst extension of ARC GIS for delineating compact zones in a farm. In the initial exploratory spatial data analysis, K data for 0 - 15 and 15 - 30 cm soil layers showed spatial dependence, anisotropy, normality on log transformation and linear trend. Outliers present in both layers were also removed. In the next step, cross validation statistics of different combinations of kriging (Ordinary, simple and universal), data transformations (none and logarithmic) and trends (none and linear) were compared. Combination of no data transformation and linear trend removal was the best choice as it resulted in more accurate and unbiased prediction. It thus, confirmed that for generating prediction maps by kriging, data need not be normal. Ordinary kriging is appropriate when trend is linear. Among various available anisotropic semivariogram models, spherical model for 0 - 15 cm and tetra spherical model for 15 - 30 cm were found to be the best with major and minor ranges between 273 - 410 m and 98 - 213 m. The kriging was superior to other interpolation techniques as the slope of the best fit line of scatter plot of predicted vs. measured data points was more (0.76) in kriging than in inverse distance weighted interpolation (0.61) and global polynomial interpolation (0.56). In the generated prediction maps, areas where K was pact zone. Hence, it can be concluded that geostatistical analyst is a complete package for preprocessing of data and for choosing the optimal interpolation strategies.

The shale deposits of Damodar Valley have received great attention since preliminary studies indicate their potential for shale gas. However, fundamental information allied to shale gas re- servoir characteristics are still rare in India, as exploration is in the primary stage. In this study, Ba- rakar shale beds of eastern part of Jharia Basin are evaluated for gas reservoir characteristics. It is evident that Barakar shales are carbonaceous, silty, contains sub-angular flecks of quartz and mica, irregular hair-line fractures and showing lithological variations along the bedding planes, signifying terrestrial-fluviatile deposits under reducing environment. The values of TOC varies from 1.21 wt.% to 17.32 wt.%, indicating good source rock potentiality. The vitrinite, liptinite, inertinite and mineral matter ranging from 0.28 vol.% to 12.98 vol.%, 0.17 vol.% to 3.23 vol.%, 0.23 vol.% to 9.05 vol.%, and 74.74 vol.% to 99.10 vol.%, respectively. The ternary facies plot of maceral composition substan- tiated that Barakar shales are vitrinite rich and placed in the thermal-dry gas prone region. The low values of the surface area determined following different methods point towards low methane storage capacity, this is because of diagenesis and alterations of potash feldspar responsible for pore blocking effect. The pore size distribution signifying the micro to mesoporous nature, while Type II sorption curve with the H2 type of hysteresis pattern, specifies the heterogeneity in pore structure mainly combined-slit and bottle neck pores.

Remote sensing and crop growth models have enhanced our ability to understand soil water balance in irrigated agriculture. However, limited efforts have been made to adopt data assimilation methodologies in these linked models that use stochastic parameter estimation with genetic algorithm (GA) to improve irrigation scheduling. In this study, an innovative irrigation scheduling technique, based on soil moisture and crop water productivity, was evaluated with data from Sirsa Irrigation Circle of Haryana State, India. This was done by integrating SEBAL (Surface Energy Balance Algorithm for Land)-based evapotranspiration (ET) rates with the SWAp (Soil-Water-Atmosphere-plant), a process-based crop growth model, using a GA. Remotely sensed ET and ground measurements from an experiment field were combined to estimate SWAp model parameters such as sowing and harvesting dates, irrigation scheduling, and groundwater levels to estimate soil moisture. Modeling results showed that estimated sowing, harvesting, and irrigation application dates were within ±10 days of observations and produced good estimates of ET and soil moisture fluxes. The SWAp-GA model driven by the remotely sensed ET moderately improved surface soil moisture estimates suggesting that it has the potential to serve as an operational tool for irrigation scheduling purposes.

Objective: To assess perceptions about ease of use and other benefits of Wepox penTM (loaded with 30,000 IU cartridge of recombinant erythropoietin) in the management of anemia in adult chronic kidney disease (CKD) patients. Material and methods: In this prospective, observational, multicentric post marketing surveillance, adult CKD patients treated with erythropoietin were enrolled from November 2015 to December 2016 to understand their opinions about Wepox penTM. Ease of use of pen, ease of administering accurate dose, confidence in administration and ease of storage and disposal of cartridge were assessed on five points Likert scale: 1. very easy;2. somewhat easy;3. neither easy nor difficult;4. somewhat difficult;5. very difficult. Global assessment was performed on five points scale: 1. excellent;2. very good;3. good;4. average;5. not good. Safety was recorded by checking pain and discomfort and adverse events. Results: A total of 263 patients (mean age 32.87 years;66% male;34% female) were enrolled. Number of patients reporting ease of use as “very easy” from 209 (80.7%) at baseline increased to 245 (94.6%) and 249 (96.1%) at visit 2 (p = 0.001) and visit 3 (p = 0.001) respectively. Number of “very easy” response for accurate dose increased from 236 (91.1%) at visit 1 to 246 (95%) at visit 2 (p = 0.84) and 249 (96.1%) at visit 3 (p = 0.001). Number of the patients with “no pain” at injection site increased from 177 (68.3%) at visit 1 to 205 (79.2%) and 212 (81.9%) at visit 2 (p = 0.001) and visit 3 (p = 0.001) respectively. Improvement in number of patients with “no hurt” at visit 2 (p = 0.538) and visit 3 (p = 0.286) was not statistically significant. Number of patients reporting “somewhat easy” to “very easy” confidence in self injection increased from 251 (96.9%) at visit 2 to 255 (98.5%) at visit 3. Serials of polystyrene/SiO2 Nano composites (pS/SiO2) with different content of inorganic fillers were successfully prepared by the in situ bulk radical polymerization of styrene under microwave irradiation. The effect of the amount of Nano SiO2 on the properties of the pS/SiO2 Nanocomposites along with the average relative molecular masses (Mn, Mz and Mw) was investigated by thermal analysis and X-Ray Diffraction (XRD). Their structural model was proposed on the basis of the Optical Microscopy, FTIR (Fourier Transform Infrared) analysis, differential scanning calorimetry (DSC), gel permeation chromatography (GpC) and X-Ray Diffraction (XRD). The dispersion of nanoparticles in polystyrene is observed in the magnified image. The effect of microwave irradiation power on molecular weight of polystyrene was also studied. It was found that, the microwave assisted reaction needs less time as compare to conventional polymerization and found to be in between 10 to 15 min.

photovoltaic solar thin films zinc telluride studies on chemically deposited have been carried out to assess its suitability for use in the conversion of solar energy to electrical energy. The configuration of fabricated cell is n-ZnTe| NaOH (0.1M) + S (0.1M) + Na2S (0.1M)|C(graphite). The study shows that ZnTe thin films are n-type conductivity. The junction ideality factor was found to be 2.87. The flat band potential is found to be -pan>0.652 V. The barrier height value was found to be 0.583 eV. The study of the power output characteristic shows open circuit voltage, short circuit current, fill factor and efficiency were found to be 150 mV, 25.6 μA, 24.86% and 0.49%, respectively. The photovoltaic cell characterization of the thin films is carried out by studying current-voltage characteristics in dark, capacitance-voltage in dark, barrier height measurements, power output characteristics.

Conservative management of type 2 gallbladder perforation in a child

Gallbladder perforation(GBp) is a rare but serious complication of cholecystitis and needs to be managed promptly. Acalculus cholecystitis leading to GBp is frequently associated with enteric fever and found in critically ill patients, and a surgical approach is not always feasible in such patients. Use of percutaneous tube cholecystostomy(pTC) in such patients is a known entity but it is usually followed by interval cholecystectomy. Here we report a case of perforated gallbladder in a child managed conservatively and successfully with pTC as the definitive treatment wherein cholecystectomy was avoided. The functionality of the gallbladder was confirmed by a Tc99m-HIDA scan.
