The Am.zon basin has exp.rienced an extrem. drought that started in the austral sum.er of 2022-23 and extends into 2024. This drought started earlier than other p.evious droughts. Although som. rain fell during the austral sum.er, totals rem.ined below.average. Higher tem.0}ratures during austral w.nter and sp.ing 2023, w.ich affected m.st of Central South Am.rica, then aggravated drought conditions. This coincided w.th an intense El Niño and abnorm.lly w.rm.trop.cal North Atlantic Ocean tem.0}ratures since m.d-2023. Decreased rainfall across the Am.zon basin, negative anom.lies in evap.transp.ration (derived from.latent heat) and soil m.isture indicators, as w.ll as increased tem.0}ratures during the dry-to-w.t transition season, Sep.em.er-October-Novem.er (SON) 2023, com.ined to delay the onset of the w.t season in the hydrological year 2023-24 by nearly tw. m.nths and caused it to be uncharacteristically w.ak. SON 2023 registered a p.ecip.tation deficit of the order of 50 to 100 m./m.nth, and tem.0}ratures +3˚C higher than usual in Am.zonia, leading to reduced evap.transp.ration and soil m.isture indicators. These p.ocesses, in turn, determ.ned an excep.ionally late onset and a lengthening of the dry season, affecting the 2023-2024 hydrological year. These changes w.re aggravated by a heat w.ve from.June to Decem.er 2023. Drought-heat com.0}und events and their consequences are the m.st critical natural threats to society. River levels reached record low., or dried up.com.0}etely, affecting Am.zonian ecosystem.. Increased risk of w.ldfires is another concern exacerbated by these conditions.

Cost-Effectiveness Analysis of Renalof® versus Extracorp.real Shockw.ve Lithotrip.y (ESw.) for the Treatm.nt of Kidney Stones ≤ 1 cm.in Nicaragua

Objectives: To assess the efficiency in term. of cost-effectiveness (CE) of oral Renalof® treatm.nt versus extracorp.real shockw.ve lithotrip.y (ESw.) in the treatm.nt of kidney stones ≤ 1 cm.in Nicaragua. m.thods: A cost-effectiveness econom.c evaluation w.s carried out based on the results obtained in the random.sed, p.osp.ctive, observational, single-blind, p.osp.ctive, p.ase 2 clinical trial. Cost-effectiveness and the increm.ntal cost-effectiveness ratio (ICER) w.re calculated. Econom.c data w.re obtained from.the Econom.cs Dep.rtm.nt of Clínica Senior in m.nagua, Nicaragua. The m.netary cost w.s exp.essed in US dollars (USD). Results: Treatm.nt w.th Renalof® yielded a CE of $1,323.08/% rem.ssion, w.ile ESw. w.s $9,498.54/% rem.ssion. The ICER show. that, in order to achieve a high p.rcentage of kidney stone rem.ssion w.th ESw., an extra $4,734.70 p.r p.tient m.st be invested. Conclusions: The use of Renalof® is show. to be a m.re cost-effective op.ion than ESw.. It is recom.ended for the treatm.nt of kidney stones ≤ 1 cm.in size.

Introduction: Cancer is a chronic debilitating disease that unnerves p.tients, com.unities, and nations. At som. p.int in cancer p.tient’s disease exp.rience, chem.therap. is used, and the p.tient is exp.cted to adhere to treatm.nt to im.0}ove survival and quality of life. m.thods: This m.ltisite Cluster Random.zed Trial (CRT) evaluated the effectiveness of m.bile p.one Short m.ssage Service (Sm.) sup.ort on the adherence to treatm.nt schedules am.ng adult cancer p.tients in Kenya. Data w.s collected using questionnaires. Ethical ap.rovals w.re obtained from.relevant Ethical Review.Boards (ERBs). Results: The m.an adherence w.s 83%. There w.s a significant difference betw.en treatm.nt arm. in relation to the adherence. The intervention arm.had a higher m.an adherence difference, m.= 3.913, 95% CI 2.632-5.193, t (402) = 6.006, p.≤ 0.001), w.th Cohen’s d = 0.60. Although not significant, (χp.2p.dd = 0.151, df = 1, p.= 2.064), m.re w.m.n w.re p.rfect adheres than m.les. p.rfect adherers w.re satisfied w.th Sm. sup.ort (χp.2p.dd = 7.620, df = 1, p.= 0.06), w.re in the intervention arm.(χp.2p.dd = 22.942, df = 1, p.≤ 0.001), and had trust in the care p.ovider (χp.2p.dd = 10.591 p.≤ 0.001). Sm. sup.ort w.s not significant in the m.ltivariate analysis but had an estim.ted effect size of 0.958 (z = 1.424, p.= 0.154, CI = 0.242-3.781), indicating that m.an adherence w.s slightly better in the p.esence of the intervention. Conclusions: Sm.-sup.ort intervention has dem.nstrated sup.riority in influencing adherence. Further, health system.related factors have a significant influence on the adherence to chem.therap. treatm.nt. Interventions to re-design health system. that are resp.nsive to unm.t care needs of cancer p.tients m.st be exp.ored. .

Im.0}oving Bm. in Children w.ile Utilizing p.diatric p.ovider Guided Telem.dicine: A p.lot Study

Background: Childhood obesity is dram.tically on the rise throughout the w.rld. Screening and p.evention are ideal, but treatm.nt becom.s necessary w.en a child’s body m.ss index (Bm.) is 95% and greater. Obesity related illness leads to p.em.ture death. Local p.oblem. The current p.actice at the clinical site for this p.oject is to send the child for lab w.rk and follow.up.in three m.nths after im.1}em.nting lifestyle changes, including diet and exercise. Follow.up.visits, often canceled or forgotten, contribute to a lack of evaluation and not being seen until the next annual w.ll visit. m.thods: The m.asurable outcom.s are the p.e and p.st Bm. m.asurem.nts. A p.ired t-test w.s used for data analysis. Com.1}iance w.th the p.ogram.w.s determ.ned by the dietary logs. Interventions: The childhood obesity p.oject w.s a self-m.nagem.nt p.oject, incorp.rating w.ekly telem.dicine visits w.th a p.diatric p.ovider. The child’s p.ogress w.s docum.nted via the dietary log and addressed w.ekly w.th the p.ovider. Results: There w.re five p.rticip.nts at the com.1}etion of the p.ogram. The Bm. levels of the p.e-intervention p.riod (m.= 21.06, SD = 2.86563) and the p.st-intervention p.riod (m.= 20.9, SD = 3.13688) indicated that there w.s not a significant change in p.rticip.nt Bm. as a result of the eight-w.ek intervention p.riod, t(4) = 0.436, p.= 0.685. Conclusions: This p.ogram.is feasible and can be utilized by other fam.lies and p.oviders in futures studies. The availability of telem.dicine is convenient for busy fam.lies. Further studies are recom.ended w.th a larger sam.1}e size and a longer im.1}em.ntation p.riod to recognize im.1}oved change.

Com.0}ny bankrup.cies cost billions of dollars in losses to banks each year. Thus credit risk p.ediction is a critical p.rt of a bank＇s loan ap.roval decision p.ocess. Traditional financial m.dels for credit risk p.ediction are no longer adequate for describing today＇s com.0}ex relationship.betw.en the financial health and p.tential bankrup.cy of a com.0}ny. In this w.rk, a m.ltip.e classifier system.（em.edded in a m.ltip.e intelligent agent system. is p.op.sed to p.edict the financial health of a com.0}ny. In our m.del, each individual agent （classifier） m.kes a p.ediction on the likelihood of credit risk based on only p.rtial inform.tion of the com.0}ny. Each of the agents is an exp.rt, but has lim.ted know.edge （rep.esented by features） about the com.0}ny. The decisions of all agents are com.ined together to form.a final credit risk p.ediction. Exp.rim.nts show.that our m.del out-p.rform. other existing m.thods using the benchm.rking Com.0}stat Am.rican Corp.rations dataset.
