Mixed neuroendocrine-nonneuroendocrine neoplasms(Mi NENs)of the digestive tract are a rare heterogeneous group of tumors that present many challenges in terms of diagnosis and *** the years,the diagnostic criteria,classification,and clinical behavior of these tumors have been the subjects of ongoing debate,and the various changes in their nomenclature have strengthened the challenges associated with Mi *** review is performed to provide an understanding of the key factors involved in the evolution of the designation of these tumors as Mi NEN,highlight the current diagnostic criteria,summarize the latest data on pathogenesis and provide information on available ***,this work seeks to increase the awareness about these rare neoplasms by presenting the clinicopathological features and prognostic factors that play important roles in their behavior and discussing their different regions of origin in the gastrointestinal system(GIS).Currently,the Mi NEN category also includes tumors in the GIS with a nonneuroendocrine component and epithelial tumors other than adenocarcinoma,depending on the organ of *** is based on the presence of both morphological components in more than 30%of the ***,this value needs to be reconfirmed with further studies and may be a limiting factor in the diagnosis of Mi NEN by ***,available clinicopathological data suggest that the inclusion of amphicrine tumors in the definition of Mi NEN is not supportive and warrants further *** diagnosis of these tumors is not solely based on immunohistochemical *** are not hybrid tumors and both components can act independently;thus,careful grading of each component separately is *** addition to parameters such as the metastatic state of the tumor at the time of diagnosis and the feasibility of surgical resection,the aggressive potential of both components has paramount importance in the choice of treatment.R

Is portal vein cavernous transformation a component of congenital hepatic fibrosis?

Congenital hepatic fibrosis (CHF) is an autosomal recessive disorder that belongs to the family of fibropolycystic liver diseases. This family includes a spectrum of disorders which are usually found in combination with each other and are usually inherited. Clinically fibropolycystic diseases have three effects being present in different proportions, those of a space occupying lesion, of portal hypertension and of cholangitis. In most patients, the first manifestations of CHF are signs and symptoms related to portal hypertension such as splenomegaly and varices. Portal hypertension in these patients has been attributed to the hypoplasia or compression of the portal vein radicles in the fibrous bands. Cavernous transformation of the portal vein (CTPV) is a relatively rare condition resulting from extrahepatic portal vein obstruction with recanalization or collateral vein formation to bypass the obstruction. It has been found that patients with CHF having an accompanying CTPV have relatively large splenomegaly and suffers more frequent episodes of bleeding from esophageal *** believe that CTPV is a congenital component of CHF and also one of the important causative factors of portal hypertension in these patients.

Clinical characteristics of Caroli's syndrome

Caroli's syndrome is characterized by multiple segmental cystic or saccular dilatations of intrahepatic bile ducts associated with congenital hepatic fibrosis. The clinical features of this syndrome reflect both the characteristics of congenital hepatic fibrosis such as portal hypertension and that of Caroli's disease named as recurrent cholangitis and cholelithiasis. The diagnosis depends on both histology and imaging methods which can show the communication between the sacculi and the bile ducts. Treatment consists of symptomatic treatment of cholangitis attacks by antibiotics, some endoscopic, radiological and surgical drainage procedures and surgery. Liver transplantation seems the ultimate treatment for this disease. Prognosis is fairly good unless recurrent cholangitis and renal failure develops.

The changes in fatty acid compositions during nutritional interaction among almond Amygdalus communis Linnaeus （Rosales： Rosaceae）（host plant）, lacebug Monosteria unicostata （Mulsant and Rey）（Heteroptera： Tingidae） and its predator Piocoris luridus Fieber （Heteroptera： Lygaeidae） were determined by gas chromatography and gas chromatography-mass spectrometry analyses. The fatty acid profiles of phospholipids and triacylglycerols were substantially different. Unlike the general observations for virtually most terrestrial insects, arachidonic and eicosapentaenoic acids were detected in high proportions of phospholipid fractions in both insects, especially in P. luridus. Also the almond tissues provide very little oleic acid to the herbivore diet, yet both insect species developed high proportions of this component. Our data reveals instances of specific accumulation of fatty acid biosynthesis, elongation/desaturation, and not incorporating selected fatty acids into cellular lipids.

Background:Generally,advanced wound care resources are applied for complex wounds that pose a challenge to the medical and nursing *** this report,the use of leukocyte-and platelet-rich fibrin(L-PRF)is emphasized for complex wounds as an alternative,simple,inexpensive,time-saving process that does not require hospitalization and has a healing potential over that of bare soft tissue,including bone,tendon,and *** aim of this study is to extend the use of L-PRF in small-to-moderate-sized complex wounds of lower extremities in which L-PRF maintains the sensitive structures ***:Between January 2016 and December 2017,17 small-to-moderate-sized complex wounds of lower extremities treated with L-PRF were recruited from the plastic and reconstructive surgery clinic in Aydin State Hospital,*** treatment was administered twice per week in the outpatient *** on the size and extension of the complex wound,two to five blood samples were collected into 8.5 ml dry,glass vacuum tubes with no anticoagulant,and samples were immediately centrifuged at 1630×g for 5 min to obtain *** healing was defined as the day of complete wound ***:The median values of the initial wound size and wound duration were 12 cm^(2)(interquartile range,6 to 23 cm^(2))and 8 months before first admission(interquartile range,4 to 18 months),*** wounds showed significant improvements after L-PRF therapy and full closure after a median of 18months,with an interquartile range of 11 to 34 months of L-PRF *** were recurrences of wounds during the first 6 months after *** adverse events were ***:Our results add to the growing evidence that L-PRF treatments protect and maintain bare soft tissue structures viable,facilitate the formation of granulation tissue and epithelization,and remarkably reduce the need for additional soft tissue surgeries in small-to-medium-sized complex wounds.
