Controlled Release of Naproxen Sodium from Supermacroporous Cryogels

Cryogels are gel matrices that have interconnected macropores and are formed in freezing-thawing systems. These interconnected macropores give elasticity to cryogels. Transdermal controlled-release systems can be used to deliver drugs with short biological half-life and can maintain plasma levels of very potent drugs within a narrow therapeutic range for prolonged periods. In this study, cryogels have been used in a different area--transdermal controlled-release system, to obtain controlled drug release medium. For this purpose, naproxen sodium has been selected as a model drug. Naproxen, a propionic acid derivative, is a NSAID （nonsteroidal anti-inflammatory drug）. For controlled releasing of naproxen sodium, cryogels that have naproxen sodium in macropores have been prepared in sheet form for local application. Acrylamide based cryogel bands have been synthesized by flee radical cryogelation process. These cryogel bands have different pore size and includes naproxen sodium in their pores. This cryogel material has been characterized by swelling tests and SEM. Then, releasing ofnaproxen sodium from cryogels has been investigated at two different pH values, 7.4 and 5.5. According to experimental data, it has seen that these cryogel matrices that including naproxen sodium in macropores could be used in controlled drug releasing systems as bandages or other transdermal controlled releasing agents at room temperature

Diffusion Weighted MRI: A Practical and Quick Approach to Evaluate Brain Death

It is significantly important to define brain death with greater precision in terms of timing and accuracy. While in the past determination of brain death is simply based on conventional angiography, now with major technological advances the Diffusion-weighted MRI is a new method sensitive to cerebral ischemia which gives on the molecular level the deeply ischemic nature of the changes. Its value in brain death has been shown in various studies. In our study, we did a comparative overview of diffusion-weighted imaging (DWI) with and magnetic resonance angiography (MRA) considering the contribution of ADC measurements from brain parenchyma, in the patients diagnosed with brain death by clinical criteria. We studied 16 brain deaths in serial studies, in which there is a prominent difference between the white and gray matter ADC values on diffusion MRI. In the postmortem brains, ADC values comparing with the normal brain parenchyma, were reduced 65% in white matter and 42% in gray matter. Also, the patients’ ADC values of gray and white matter were significantly lower than those of irreversible brain-ischemia patients’ in ADC values. In comparison to most of the other studies, in our study population studied is large, in which is a comprehensive study that results consistent with the literature. As a result we propose that in the definition of brain death Diffusion MRI and ADC measurements are reliable to show diffuse ishemic changes of brain parenchyma.

目的：通过测量随访12mo后的中央黄斑厚度（CMT）和视力，评估玻璃体腔注射bevacizumab治疗视网膜分支静脉阻塞（BRVO）继发黄斑水肿（ME）的效果。方法：被诊断为BRVO继发ME的患者行眼科检查、CMT测量、荧光素血管造影。排除荧光素血管造影出现黄斑缺血患者，其他疾病继发的其他部位新生血管化患者，有眼内治疗史（激光治疗、玻璃体腔注射或眼科手术）患者。CMT 〉250μm的32例患者给予bevacizumab（Altuzan，0.125mg/0.05mL）注射治疗，并随访12mo。分析最佳矫正视力（BCVA）logMAR数据和CMT控制参数。使用Minitab15.0软件统计分析配对t检验，P〈0.05有统计学意义。结果： BCVA logMAR数据和CMT控制参数的平均值较注射前有明显改变（P〈0.01）。平均最佳矫正视力增量为0.477±0.235，平均CMT较注射前下降257.906±88.865。10例（31%）患者对单次注射有阳性反应，平均12.6±0.66mo未复发ME。5例（15.6%）患者接受两次注射，17例（53%）3次以上。单眼平均注射量2.18±0.91（1～4）。第一组ME复发时间为2.45±0.63mo，第二组为2.58±0.66mo，第三组为3.17±0.48mo。5例（15.6%）患者需要多次注射以减轻ME，视力并未随ME的减轻而增加。结论：玻璃体腔注射bevacizumab是常规治疗BRVO继发ME的方法，有效、快速、安全。为了使疗效持久，需加强后续处理，通过激光或长效药物保持无水肿状态。视网膜静脉循环和ME须通过荧光素血管造影观察，而不能采取频繁注射。是否需再次注射必须根据ME的临床表现和视力的预测来判断。

Prognostic Factors and Treatment Outcome in 178 Locally Advanced Cervical Cancer Patients

Background: To evaluate local control, survival, radiation side effects and treatment outcome in locally advanced cervical cancer patients. Materials and Methods: Among 2006-2011, 178 patients with locally advanced cervical cancer were treated with chemoradiotherapy +/-?radiotherapy and high dose rate (HDR) brachytherapy. Follow-up was complete for all patients. Concomitant chemotherapy was not administered in 44 patients due to renal impairments and ECOG of 2 - 3. Results: The median follow-up period was 34.5 months (range, 5 to 93) and 42 months (range, 14 to 93 months) for alive patients. Five years local-regional control, progression-free survival and overall survival rates were 87.8%, 58.9% and 67.3% in all patients, respectively. In this retrospective study young age, tumor diameter, stage, presence of residual tumor and administration of chemotherapy were effected in survival analysis. The parameters which affected the complete response of patients were defined as presence of concomitant chemotherapy and number of courses <5. Central region recurrence rate was defined higher in the group with treatment duration of 9 weeks and higher (p = 0.044). Conclusion: Primary chemoradiotherapy +/-?radiotherapy achieved a satisfactory rate of local control and survival rates with acceptable complications in locally advanced cervical cancer. Concomitant chemotherapy and treatment duration were the important prognostic factors for completed response locally advanced cervical cancers.

Hormone receptor status of primary tumor as a prognostic factor in patients with liver metastases from breast cancer treated with transcatheter arterial chemoembolization

We read with great interest the article by XP et al. They reported the results of their experience with transcatheter arterial chemoembolization （TACE） and systemic chemotherapy for forty-five patients with liver metastases from breast cancer and evaluate the prognostic factors. In their study, the response and survival rates were significantly better in TACE group than in chemotherapy group. The lymph node status of the primary cancer, the clinical stage of liver metastases, the Child-Pugh grade, loss of weight were found to be significantly associated with survival in both univariate and multivariate and analyses. However, they did not mention hormone receptor status of the patients that might have an effect on the survival rate. Elias et al. in their study evaluated 54 breast cancer patients with liver metastases as the sole site of metastatic disease （except for bone metastases in 3 patients） that underwent hepatectomy. They showed that the only factor influencing survival in both the univariate and multivariate analyses was the hormone receptor status （P = 0.03）, and the relative risk of death increased by 3.5-fold when hormone receptor was negative. Moreover, Mack et al reported excellent local tumor control and survival rates achieved by laser induced interstitial thermotherapy （LITT） in breast cancer patients with liver metastases. Regarding the prognostic and predictive factors related to primary tumor, they found no statistically significant difference in terms of mean survival between the patients with N0-N1 lymph nodes and N2-N3 lymph nodes. However, they found that the hormone receptor status is a significant （P〈0.05） prognostic factor on both mean and median survival. Mean Platelet Volume as an Inflammation Marker, Possible Biomarker of Tumor Detection in Prostate Biopsy

Background: In the diagnosis of prostatic diseases, the need for markers other than prostate specific antigen (PSA) has been increasing in recent years. So, we aimed to determine the predictive value, the neutrophil lymphocyte ratio, platelet-to-lymphocyte ratio and mean platelet volume before prostate biopsy in predicting the results of pathology. Transrectal ultrasound-guided biopsy of the prostate was performed because of high PSA values and compared values of these parameters to predict of pathology results. Methods: 2715 patients who underwent 10 - 12 quadrant transrectal ultrasound-guided prostate biopsies between January 2008 and January 2018 have been evaluated retrospectively. Patients were divided into groups according to the biopsy pathology results by benign (group 1), atypical small acinar proliferation (ASAP) (group 2) and prostate cancer (group 3). A total of 204 patients who were benign prostate hyperplasia in 71 patients (34.8%), atypical small acinar proliferation in 80 (39.21%) and prostate adenocarcinoma (PCa) in 53 patients (25.98%) were included in the study by systematic sampling. Before the biopsy total PSA (tPSA), free PSA (fPSA), rate of percentage of free to total prostate specific antigen (f/tPSA) rate, PSA density (PSA-D), white blood cell (WBC) count, blood neutrophil count (NC), blood lymphocyte count (LC), neutrophil lymphocyte ratio (NLR), mean platelet volume (MPV), platelet count (PLT) and platelet-to-lymphocyte ratio (PLR) were measured and compared in all groups. Differences in continuous variables were assessed using the ANOVA. Logistic regression was used to analyze the linear relationship between predictive variables and pathology results. P < 0.05 was considered statistically significant. Results: NLR and PLR values were lower in group 1 than group 2 and were found statistically significant between in group 1 and group 2 (p: 0.03 and p: 0.02, respectively). ＜正＞ XLas （Extra Large Alpha S） is an alternatively spliced variant of Gas. the G protein alpha subunit that couples various extracellular stimuli to the activation of adenylyl cyclase. XLas is identical with Gas except for its long amino terminus and appears to share its functional properties such as receptor selectivity, bg binding and adenylyl cyclase activation. In this study, coupling properties of Gas and XLas to b2 adrenergic receptor （β2AR） and adenylyl cyclase are investigated in transfected HEK-293 cells. Adenylyl cyclase activity measurements show that, compared to Gas. Xlas exhibits a high basal and agonist-induced activity, both of which are enhanced by the increased expression of β2AR. Thus, spontaneous receptor activity of the β2AR seems to contribute to the basal activities of both Gas and XLas. However, while the basal activity of Gas can be inhibited by inverse agonists that suppress the spontaneous activity of the β2AR, the basal activity of XLas is insensitive to these ligands. This suggests that the conformation induced by these ligands is sensed as an inactive state by Gas but not by XLas. This study is supported partly by the research grants A. Biological subtypes of breast cancer: Prognostic and therapeutic implications

Breast cancer is a heterogeneous complex of diseases, a spectrum of many subtypes with distinct biological features that lead to differences in response patterns to various treatment modalities and clinical outcomes. Traditional classification systems regarding biological characteristics may have limitations for patient-tailored treatment strategies. Tumors with similar clinical and pathological presentations may have different behaviors. Analyses of breast cancer with new molecular techniques now hold promise for the development of more accurate tests for the prediction of recurrence. Gene signatures have been developed as predictors of response to therapy and protein gene products that have direct roles in driving the biology and clinical behavior of cancer cells are potential targets for the development of novel therapeutics. The present review summarizes current knowledge in breast cancer molecular biology, focusing on novel prognostic and predictive factors.

Ovarian stem cells:From basic to clinical applications

The field of reproductive biology has undergone significant developments in the last decade. The notion that there is a fixed reserve pool of oocytes before birth was established by Zuckerman in 1951. However, in 2004, an article published in nature challenged this central dogma of mammalian reproductive biology. Tilly's group reported the existence of ovarian germline stem cells(GSCs) in postnatal ovaries of mice and suggested that the bone marrow could be an extragonadal source of ovarian GSCs. These findings were strongly criticized; however, several independent groups have sincesuccessfully isolated and characterized ovarian GSCs in postnatal mice. The ovarian GSCs are located in the ovarian surface epithelium and express markers of undifferentiated GSCs. When transplanted into mouse ovaries, mouse ovarian GSCs could differentiate and produce embryos and offspring. Similarly, in a recent study, ovarian GSCs were found to be present in the ovaries of women of reproductive age. Conversely, there is increasing evidence that stem cells responsible for maintaining a healthy state in normal tissue may be a source of some cancers, including ovarian cancer. Cancer stem cells(CSCs) have been found in many tissues, including ovaries. Some researchers have suggested that ovarian cancer may be a result of the transformation and dysfunction of ovarian GSCs with self-renewal properties. Drug resistant and metastasisgenerating CSCs are responsible for many important problems affecting ovarian cancer patients. Therefore, the identification of CSCs will provide opportunities for the development of new therapeutic strategies for treatments for infertility and ovarian cancer. In this article, we summarize the current understanding of ovarian GSCs in adult mammals, and we also discuss whether there is a relationship between GSCs and CSCs.
