AIM: To compare two different anti-vascular endothelial growth factor(anti-VEGF) treatment regimens'-a priori pro re nata(PRN) and PRN regimen following^(th)e loading phaseanatomical and functional results in neovascular agerelated macular degeneration(n AMD) patients. METHODS: Totally 544 n AMD patients followed and treated with aflibercept(n=135) and ranibizumab(n=409)at 9 different centers between 2013 and 2015 were enrolled into^(th)is retrospective multicenter study. Patients with initial best corrected visual acuity(BCVA) interval of 1.3-0.3(log MAR) and a minimum follow-up of 12 mo were included. Patients under two different regimens-a priori pro re nata(1+PRN) or 3 consecutive intravitreal injections followed by a PRN regimen(3+PRN)-were compared in BCVA at 3^(th), 6^(th) and 12^(th) months, and in central macular^(th)ickness(CMT) at 6^(th) and 12^(th) months. The total study group, intravitreal ranibizumab(IVR) and intravitreal aflibercept(IVA) groups were evaluated separately. RESULTS: The mean CMT decreased in^(th)e 1+PRN(n=101) regimen from 407 to 358 and 340 μm and in^(th)e 3+PRN(n=443) group from 398 to 318 and finally to 310 μm at months 6 and 12, respectively. Anatomically,^(th)e CMT reduction at 6^(th) month(48.5 vs 76.4;P<0.05) was statistically significant in favor of 3+PRN group. BCVA changed in 1+PRN group from 0.77 to 0.78, 0.75 and 0.75;in 3+PRN group from 0.81 to 0.69, 0.72, and 0.76 at months 3, 6, and 12, respectively. Visual gain was statistically better in 3+PRN group at 3^(th) month(-0.01 vs 0.12;P<0.001). In IVR group, CMT reduction was in greater in 3+PRN at 6^(th)(44 vs 72) and 12^(th) month(61 vs 84), but statistically insignificant. The 3+PRN group revealed statistically better visual results at 3^(th) month(-0.02 vs 0.11, P<0.05). In IVA group, although statistically insignificant, CMT reduction(61 vs 89, 6^(th) month;85 vs 97, 12^(th) month) and visual gain(0.02 vs 0.16;0.02 vs 0.14;0.05 vs 0.11) was found in favor of 3+PRN group

Helicobacter pylori (H. pylori) is a prevalent, worldwide, chronic infection. Choice of treatment can be modified according to antibiotic-resistance rates of H. pylori. The ideal therapeutic regimen for H. pylori infection should achieve an eradication rate of ≥ 80%. In some countries, triple therapy with a proton-pump inhibitor (PPI), clarithromycin, and amoxicillin or metronidazole is still the best option. Bismuth-containing quadruple therapy consisting of bismuth salts, tetracycline, metronidazole and PPI, may be the preferred option in countries with clarithromycin resistance > 20%. Sequential therapy including a PPI and amoxicillin given for the first 5 d, followed by triple therapy including a PPI, clarithromycin, and nitroimidazole antimicrobial (all twice daily) for the remaining 5 d, can be another option for the first-line treatment of H. pylori. Recent data suggest that treatment with PPI, levofloxacin, and amoxicillin for 10 d is a good choice for second-line therapy. Concomitant therapy consisting of PPI, amoxicillin, clarithromycin and metronidazole is another option for second-line treatment. If second-line treatment also fails, it is recommended to culture H. pylori from biopsy specimens and perform antimicrobial susceptibility testing. Rescue treatment should be based on antimicrobial susceptibility.

We report a 12 years old female patient with an overlap syndrome involving autoimmune hepatitis （AIH） and systemic lupus erythematosus （SLE）. The patient presented with jaundice, hepatosplenomegaly, malaise, polyarthralgia, arthritis and butterfly rash on the face. Laboratory tests revealed severe liver dysfunction, Coombs positive hemolytic anemia and a positive ANA/ anti-dsDNA test. Renal biopsy showed class IIA kidney disease, while liver biopsy showed chronic hepatitis with severe inflammatory activity. The patient satisfied the international criteria for both SLE and AIH. Clinical symptoms and laboratory findings of SLE improved with high dose treatment with corticosteroids and azathioprine, however, remission of the liver disease could not be achieved. Repeat biopsy of the liver after three years of therapy revealed ongoing chronic hepatitis with high level of inflammatory activity. The present case indicates that children with liver dysfunction and SLE should be investigated for AIH. There is much diagnostic and therapeutic dilemma in patients with AIH-SLE overlap syndrome.

Aim： To investigate whether the autonomic nervous system （ANS） components are suitable biological markers for representing well-being in patients with erectile dysfunction （ED）. Methods： The present study included 74 male patients who had applied for check-ups in the cardiology outpatient clinic at Kirikkale University （Kirikkale, Turkey） and who had been diagnosed as having hyperlipidemia. Of these patients, 26 had an additional diagnosis of ED and made up the patient group. The remaining 48 patients formed the control group. Well-being was assessed with shortform 36 （SF-36）. The International Index of Erectile Function （IIEF） was used as a measure of libido and erectile function. Quantitative assessment of the ANS was made based on the analysis of heart rate variability by means of 24-h holter monitorization. Results： Comparisons between the ED and control groups showed significant differences only in energy scale of SF-36. The ED group also had significantly higher values of sympathetic activity. Except for the general health score of SF-36, which was found to be correlated with parasympathetic activity only in ED group, there were similar correlation patterns within the groups. Although well-being and sympathetic activity were correlated negatively, parasympathetic activity and well-being were correlated positively. Conclusion： Quantitative assessment of the ANS by heart rate variability analysis might be a suitable marker for well-being of patients with ED.

The Goshgarchay Cu-Au deposit is located in the central part of the northwest flank of the Murovdagh region in the Lesser *** Goshgarchay Cu-Au deposit is associated with Middle Jurassic volcanic and Late Jurassic-Early Cretaceous high-K calc-alkaline intrusive *** Cu-Au mineralization is commonly related to quartz-sericite-chlorite alteration dominantly composed of chalcopyrite,gold,sphalerite,pyrite,bornite,hematite,covellite,chalcocite,malachite,and *** Goshgarchay copper-gold deposit,which is 600 m wide and approximately 1.2 km long,is seen as a faultcontrolled and vein-,stockwork-and disseminated type *** Goshgarchay Cu-Au deposit predominantly comprises Cu(max.64500 ppm)and Au(max.11.3 ppm),while it comprises relatively less amounts Zn(max.437 ppm),Mo(max.47.5 ppm),Pb(max.134 ppm),and Ag(max.21 ppm).The homogenization temperatures and salinities of fluid inclusions in quartz for stage Ⅰ range from 380℃ to 327℃,and 6.9 wt% to 2.6 wt% NaCl eq.,*** salinities in quartz for stage Ⅱ range from 304℃ to 253℃,and 7.6 wt% to 3.2 wt% NaCl eq.,*** calculated δ^(34)S_(h2s)values(-1.5‰ to 5.5‰)of sulfides and especially the narrow range of δ^(34)S_(h2s) values of chalcopyrite and bornite(between -0.07‰ and +0.7‰)indicate that the source of the Goshgarchay Cu-Au mineralization is *** on the mineralogical,geochemical,fluid inclusion,and sulfur isotopic data,the Goshgarchay Cu-Au deposit represents a late stage peripheral magmatic-hydrothermal mineralization probably underlain by a concealed porphyry deposit.

Clinical outcomes of 25-gauge vitrectomy surgery for vitreoretinal diseases: comparison of vitrectomy alone and phaco-vitrectomy

AIMTo compare the clinical outcomes of combined 25-gauge pars plana vitrectomy (PPV) and phacoemulsification/posterior chamber intraocular lens (PC-IOL) implantation with vitrectomy alone surgery in patients with various vitreoretinal *** total of 306 eyes (145 with PPV alone and 161 with phaco-vitrectomy) were enrolled in this retrospective analysis. The surgical approach was 25-gauge PPV combined with phacoemulsification and PC-IOL implantation at the same time in eyes in phaco-vitrectomy group and only PPV in eyes in vitrectomy alone surgery group. The main outcome measures were postoperative clinical outcomes included anterior chamber inflammation, changes in intraocular pressure (IOP) and best corrected visual acuity (BCVA).RESULTSThe most common postoperative complication was anterior chamber reaction which has higher incidence in phaco-vitrectomy group (Pst day IOP of vitrectomy alone group was significantly lower than that of phaco-vitrectomy group (16.3±5.8 mm Hg vs 17.8±8.1 mm Hg, respectively, P=0.02). Hypotony (IOP≤8 mm Hg) was not different between groups in the postoperative 1st day (P>0.05). The mean preoperative visual acuity was not different between groups (1.6±0.9 logMAR vs 1.8±0.9 logMAR, respectively, P>0.05). However, the mean visual acuity was decreased in vitrectomy alone group at the final visit compared to phaco-vitrectomy group (1.2±0.8 logMAR, 0.9±0.7 logMAR, respectively P<0.05).CONCLUSIONTwenty-five gauge PPV combined with phacoemulsification surgery is a safe and efficient procedure, which can be preferred in phacic patients with a variety of vitreoretinal diseases compared to vitrectomy alone. Despite improved outcomes, this approach is not free of limitations as anterior chamber complications especially with combined surgery.

AIM:To adapt the low vision-related quality of life(LVQOL) instrument into Turkish language and to assess its validity and ***:The study was conducted in 387 patients attending the Centre of Low Vision Rehabilitation,Faculty of Medicine,Ankara *** statistical analyses,the Spearman's correlation coefficient,Cronbach's alpha coefficient and Confirmatory Factor Analysis(CFA) were ***:According to results of CFA,the item in the 'Adjustment' subscale because of having the factor loading below 0.40,was excluded from the *** reliability of the questionnaire was assessed according to Cronbach's alpha *** reliability of the 'Distance Vision,Mobility,and Lighting' subscale was ?琢=0.863;of the 'Adjustment' subscale was ?琢=0.694;'Reading and Fine Work' was ?琢=0.791,and 'Activities of Daily Living' was ?琢= *** these results indicate that the questionnaire is reliable to measure the vision related quality of life of low-vision *** correlations between the subscales were also analyzed,and the correlation between 'Adjustment' and 'Reading and Fine Work' was found to be the lowest(rs=0.336,P <0.001),whereas the strongest correlation was found between the 'Reading and Fine Work',and 'Activities of Daily Living'.Additionally,the 'Adjustment' dimension showed the strongest correlation with only 'Distance Vision,Mobility,and Lighting' ***:After removing the last item in the second dimension,the Turkish adaptation of all dimensions of the LVQOL has been shown to be reliable,valid and suitable for use in patients with low vision in Turkey.
