Ankylosing Spondylitis in a West African Hospital

Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by predominant axial and peripheral (enthesitis, sacroiliitis) involvement affecting young subjects aged 30 to 40 years, 80% to 98% of cases are associated with HLA-B27. Objective: To determine the epidemiological profile of ankylosing spondylitis in the rheumatology department of the Ignace Deen National Hospital in Conakry (Guinea). Materials and Methods: This was a descriptive cross-sectional study carried out within the said department over a period of 18 months from July 1, 2018 to December 31, 2020, including all patients seen in consultation and/or hospitalized in the department in which the diagnosis of ankylosing spondylitis had been retained according to the modified New York criterion. The parameters studied were sociodemographic, clinical, paraclinical and therapeutic. Result: We collected 73 cases or 4.1% of ankylosing spondylitis out of a total of 1781 patients seen during the study period. The male gender was represented with 54.8% for a sex ratio of 1.2 M/F. The average age of our patients was 32.18 ± 12.44 years with extremes ranging from 17 to 54 years. Axial involvement was present in 89.9% of cases with a lumbar predominance (95.2%), followed by the sacroiliac seat (35.5%), cervical (14.5%) and dorsal at 4.8%. The pain was chronic in 93.2% of cases. The most common drug treatment was taking analgesics and NSAIDs (100%) followed by cortisone infiltration (41.1%), corticosteroids (30%), and physiotherapy (21.9%). Ankylosing spondylitis represents 83% of spondyloarthritis followed by undifferentiated spondyloarthritis (9.1%) and juvenile spondylitis (3.4%) were the most common conditions.

Impact of Medical Treatment of Lower Urinary Tract Symptoms Related to Benign Prostatic Hyperplasia on Male Sexuality

Introduction: Benign prostatic hyperplasia (BPH) is a benign neoplasm that develops from the constituent elements of the prostate. It is a common age-related condition, with more than 50% of men over 50 years old exhibiting symptoms indicative of BPH. It is the main cause of lower urinary tract symptoms (LUTS). Materials and Methods: This was a prospective, descriptive and longitudinal study over a six-month period from December 15, 2023, to May 15, 2024. All patients admitted for BPH and who received medication treatment during this period were included in the study. Results: The average age of patients was 65.4 years, with the 60 to 69-year age group being the most represented (37.18%). There was no statistical link between the level of education and the occurrence of erection dysfunction ED. 66.67% of patients suffered from ED before treatment. Age was a major risk factor. 94.87% of patients were treated with Alpha-blockers due to their tolerance and effectiveness. 14.10% of patients had a history of inguinal herniorrhaphy, often due to the strain of urination and physical labor. 46.16% of patients had hypertension. No significant link was found between comorbidities and ED. 38.45% of patients consumed tobacco or alcohol. Tobacco was a significant risk factor for ED. 57.67% of patients suffered from ED after treatment, indicating an improvement compared to 66.67% before treatment. However, 24.36% did not ejaculate during sexual intercourse. Conclusion: Medication treatment is the first-line treatment for BPH. However, it can lead to retrograde ejaculation, negatively impacting ejaculatory function. The results showed that the treatment improves patients’ sexuality (IIEF-5 score), but age and tobacco consumption increase the risk of sexual dysfunction.

Mucosal Prolapse of the Urethra in Little Girls about 11 Cases at the Urology Department of the Sino-Guinean Friendship Hospital in Conakry (Guinea)

Introduction: Urethral prolapse, a rare benign lesion of the female urethra, is considered unusual in children, but it is far from exceptional in girls aged 18 to 24. African origin. The diagnosis is clinical but can be overlooked or confused with other pathologies. Treatment remains controversial. The aim of this work is to study the epidemiological, clinical and therapeutic data of urethral mucosal prolapse in young girls in our department. Materials and methods: Retrospective and descriptive study on urethral mucosal prolapse in little girls identified from the hospitalization records of the Sino-Guinean Friendship Hospital in Conakry. The study period was 5 years. Patients were selected randomly. Outcome assessment focused on the occurrence of recurrences and urinary incontinence. The mean follow-up duration was 24 months. The parameters studied were: Age, clinical and histological findings, therapeutic data and patient outcomes. Results: The incidence of urethral mucosal prolapse in girls in the urology department was 2.2 cases/year. The mean age of the patients was 6.7 years (with extremes of 5 months and 10 years). The age group of 7 to 10 years was the most represented, observed in 63.63% of patients. The most frequent reason for consultation was vulvar bleeding in 7 patients, or 63.63%. Malpighian hyperplasia with a congested chorion was the most observed histological type, in 4 patients. Surgical treatment consisting of complete excision of the prolapsed part had given excellent results. Conclusion: Urethral prolapse is a rare pathology, observed mainly in young black girls. Its diagnosis is based mainly on clinical examination. Surgical treatment gives satisfactory results, both clinically and aesthetically.

Introduction: Cardiac rehabilitation is defined as a comprehensive, multidisciplinary approach to secondary prevention. Its main components include patient assessment (exercise capacity, ventricular function and the existence or risk of arrhythmias), therapeutic education and control of cardiovascular risk factors, optimization of drug therapy, psychosocial support and, of course, personalized physical training. Method: The Basse Terre hospital department served as the setting for this study. All patients admitted to the department for cardiac rehabilitation were our target population. All patients operated on and admitted to the cardiology department for cardiac rehabilitation constituted our study population. A pre-established survey form and patient records were used to collect data. Ethical principles and patient confidentiality were paramount. Results: The prevalence was 13%. The mean age of our patients was 59.66 ± 12.9 years. Valve replacement was 25%;coronary artery bypass grafting was 75%;Kardégic 83.33%, CHBT protocol 83.33%, muscle fatigue 66.66%. 58.33% had 3 - 8 Watts at initial assessment and ≥15 Watts at final assessment. 8.33% atrial dilatation, and 16.66% segmental hypokinesia. 8.33% VO2 and ESV and Q necrosis. Conclusion: Exercise testing is a reliable means of assessing patients’ functional capacity.

Introduction: Foreign bodies (FB) in the lower airways (LAs) constitute a potentially life-threatening emergency requiring immediate management. The objective of our study was to describe the patient pathway and management of foreign bodies in the lower airways. Methodology: This was a descriptive study with retrospective data collection conducted over a 6-year period (January 1, 2014, to December 31, 2019) in the ENT and Head and Neck Surgery Department of Donka National Hospital. Results: We observed a prevalence of 1.79%. The average age was 5.71 years, with a sex ratio of 1.2. However, 82.61% of the cases originated from rural areas. We noted that 78.83% of patients were referred after visiting 1 to 2 healthcare facilities (75.36%). The consultation delay was 3 days. Penetration syndrome was present in 98.56% of cases. The foreign body incidents occurred during play in 54.84% and during meals in 29.09% of cases. Radiographs were performed in 93.9% of cases. Endoscopy was used for both diagnostic and therapeutic purposes in all patients. The foreign bodies were located in the trachea (37.68%), larynx (26.08%), and bronchi (21.73%). Non-organic foreign bodies were predominant (52.17%). Postoperative outcomes were uncomplicated in 95.98% of cases, and the lethality rate was 2.85%. Conclusion: The pathway of patients with foreign bodies in the airways remains unpredictable in our context due to parental hesitations. Early management reduces morbidity and mortality rates.

Psychosocial Aspects of Male Infertility at the Service of Urology-Andrology of the Hospital of the Sino-Guinean Friendship

Objective: Describe the psychosocial aspects of male infertility at the hospital of the Sino-Guinean Friendship. Patients and method: It is a prospective study of a descriptive type covering a period of 6 months. The study covered 17 patients, all received for a desire to conceive after at least one year of regular sexual intercourse without contraception. The data were collected from patient interviews using a pre-established questionnaire. Results: The average age of the patients was 32.07 years with extremes of 23 years and 42 years. During this study, 64.70% of patients were no longer participating in community ceremonies. The patients’ relationships with their spouse and family deteriorated in 52.94% and 47.06%, respectively. Conversely, relations with the family of origin remained unchanged in 70.59 percent of cases. The reduction in economic activity was by 13 patients (76.48%). Conclusion: Male infertility causes a real psychic earthquake in men with its corollaries of negative feelings. The rather complex moral repercussions of male infertility affect not only the individual, his/her partner, and family, but also economic activity.

Case Report: A Severe Hypernatremia with Multiple Organ Failure, about a Clinical Observation

A hypernatremia severity is often associated with a poor prognosis, especially if it is associated with multiple organ failure. In a country with very limited resources, the prognosis may be favorable in the absence of renal replacement. We report the case of a 63 years old woman, hypertensive and diabetic, admitted to the ICU for unconsciousness. Clinically, neurological examination notes a Glasgow of 8/15 (Y2, V2, M4) and a left hemiplegia. Temperature was 39°C. Diuresis was 100 ml during the first 24 hours. Blood pressure was 90/60 mmHg, tachycardia at 133/min. Cardiac auscultation is normal. Vascular axes were weakly perceptible. Oxygen saturation was 95%. The skin examination notes a dry skin and mucous membranes, a flattening of the superficial veins, sunken eyes and a persistent skin fold. There is no hepatomegaly or splenomegaly, or jaundice. Biological point of view note natremia: 176 mmol/L;osmolarity: 390 mosmol/kg;creatinin: 300 μmol/L;glycemia > 6 g/L;transaminases 217 UI. Diagnostics: malignant hypernatremia with a high plasma osmolarity associated with an acute anuric renal failure, hydro electrolytic disorders, an abnormal liver function, a fever of central origin and a stroke. The treatment consisted of a correction of the electrolyte disorders by infusion of isotonic and hypotonic fluids with insulin. In a country with very limited resources, the severe hypernatremia prognosis associated with anuric acute renal failure may be favorable in the absence of renal replacement.

Urogenital Complications of Groin Hernia Surgery: A Multicentre Study in Guinea

Context and Objective: Groin hernia is a common pathology in visceral surgery (2nd rank after appendicitis), which affects approximately 4.6% of the African population. Restoring the normal anatomy of the groin region is one of the most benign interventions. However, uro-andrological complications are possible. This study aimed to contribute to the improvement of the management of urogenital complications of groin hernia surgery. Patients and Method: We carried out a prospective study of descriptive type with a duration of 6 months from 1 August 2021 to 31 January 2022. The data were collected using a pre-established survey sheet. The study covered several sites (public hospitals and private clinics) in Guinea. Results: The urogenital complications of the surgery of the hernia of the groin represented 15.22% or 14 cases out of 92 patients. The average age was 37.00 years with extremes of 20-69 years. Polygamists occupied the first place. The reason for consultation was dominated by decreased testicular volume (42.86%), followed by a desire to conceive (21.43%). Unilateral testicular atrophy represented 63.64%, bilateral 27.27%. We performed a left orchidectomy in one patient and a cystorraphy in another simple operative follow-up. The other cases consisting of testicular atrophy, due to lack of a suitable technical platform, did not benefit from any specific therapeutic treatment. Conclusion: Urogenital complications of groin hernia surgery are relatively common. Testicular atrophy was the main clinical complication. The left orchidectomy and cystorraphy were the therapeutic procedures performed.

Soldiers’ Hypertension Prevalence in Military Garrisons in the City of Conakry

High blood pressure (HBP) is a public health problem worldwide. The objective of this study is to determine the prevalence of hypertension in military garrisons in Conakry, the capital. Material and Methods: The study was done in military garrisons. The data concern 1025 soldiers. This prospective and cross-sectional study took place from 13 November 2014 to 13 February 2015. Only the military who had agreed to participate in the study were included. Sociodemographic, clinical data and urine dipsticks were collected. Results: Among 1025 soldiers, 222 cases of HPB were observed, that was 21.66%. They were 193 men (86.94%) and 29 women (13.06%). The mean age was 46.05 years (range: 18 - 77 years). HPB cases were discovered in 155 soldiers (69.82%) in random observations. Following the grade of hypertension: 124 were of grade I, 62 of grade II, 35 of grade III and 1 case of isolated systolic hypertension was identified. Urinary dipstick tests showed that, 3 patients had proteinuria, 26 patients suffered from leucocyturia, 11 patients suffered from glycosuria and 1 case of hematuria was also identified. More than half of the soldiers (41%) were in the army. Officers were strongly affected (80.18%), followed by non-commissioned officers (17.12%) and finally enlisted men (2.70%). Risk factors were multiple: 78.82% ate very salty food;emotional stress;physical inactivity was observed in over half of the cases. Conclusion: This study gives an overall picture of the prevalence of hypertension in the military in Guinea. The discovery of the symptom lies on the systematic measurement of blood pressure.

Evaluating the Influence of the Prostate Weight on the Morbidity and Mortality of Transvesical Open Prostatectomy: Prospective Analysis of 108 Cases in the Urology-Andrology Department of the Ignace Deen National Hospital in Conakry

Introduction: Open transvesical prostatectomy remains today one of the most effective approaches for the management of benign prostatic hyperplasia despite the fact that, this method is associated with multiple complications. The objective of this study was to evaluate the influence of prostate weight on the morbidity and mortality of transvesical prostatectomy for adenoma in the urology-andrology department of the Ignace Deen National Hospital. Materials and Methods: This was a prospective, longitudinal and analytical study lasting 6 months, from March 1, 2022 to August 31, 2022 including patients admitted and operated on by open transvesical prostatectomy by assessing the influence of prostate weight on the morbidity and mortality of transvesical adenomectomies. Results: 108 patients were included in our study, the average age of our patients was 70 ± 7.7 years, cultivators were the most represented profession with 38.89%, and hypertension was the most represented comorbidity with 75%. 33.06% of cases became complicated and surgical wound infection was the main complication with a frequency of 17.40%. Statistical analysis did not conclude that, the prostate weight does not have a statistically significant influence on the morbidity and mortality of transvesical open prostatectomy for benign prostatic hyperplasia. Conclusion: Prostate weight has no influence on the morbidity and mortality of transvesical prostate adenoma.
