Hydrocephalus had been managed by ventriculoperitoneal shunt (VPS) or endoscopic third ventriculostomy (ETV) since several years. But these two different technics had some complications that must be managed promptly to avoid eventual fatal evolution. Chronic subdural hematomas after ETV is among these complications and is a very rarely situation observed in our department. This rare event associated with malaria in a child is considered to have a high mortality. Here we report a rare case of bilateral chronic subdural hematoma occurring in a 4-month-old boy after ETV and we discuss the likely pathogenesis and the difficulties of management.

Evaluation of Results of the Furlow’s Modified Technique in the Management of Cleft Palate in the Teaching Hospital of Conakry

Introduction: Cleft palate (CP) is a congenital dysmorphosis that results from a failure of the palatal processes of the maxillary buds and nasal septum to merge. The objectives of this study were to determine the frequency of complications of the modified Furlow technique and to evaluate its anatomical and functional outcomes. Materials and Method: This was a retrospective study that took place over a period of 4 years (from January 1st, 2015 to December 31th, 2018) in the Department of Odontostomatology and Maxillofacial Surgery at the National Hospital of Donka (Conakry, Guinea). Data retrieval consisted of records from patients operated on for cleft palate according to Furlow’s modified technique only. The operative technique consisted of marginal incisions of the cleft according to the technique of the double Z-plasty of Furlow. Subsequently, the defect created between the 2 flaps and the anterior edge of the cleft was filled by two fat masses of Bichat sutured to each other with Vicryl 3-0. Four years after surgery, the patients were recalled for an evaluation of the anatomical and functional results. Results: 13 patients underwent surgical loading according to the modified Furlow technique. The average age of the patients was 6.9 years with an age range of 7 months and 17 years. Females were the most affected, with 61.54% of cases and a sex ratio of 0.62. The primary repair rate was 84.61% of cases. Immediate operative follow-up was simple with epithelialization of the flaps in all patients. Four (4) years after the surgeries, 12 patients were reevaluated (92.30%), among them one case

Introduction: Rhinosinusitis is one of the most common ENT diseases. Its main complications are of three types: cranial, endo-cranial and ophthalmological. The aim of this study was to contribute to the study of ophthalmological complications of rhino-sinusitis in our context. Material and Methods: This was a prospective study, of descriptive type, of six (6) months (July-December 2017), concerning the patients received and treated for ophthalmological complications of rhino-sinusitis at the National IgnaceDeen Hospital. Results: During our study period, we collected 32 ophthalmological complications of rhinosinusitis, i.e. 65.31% of cases. The average age of our patients was 34.25 ± 15.15 years with extremes of 8 and 76 years and a sex ratio of 0.45. A history of rhinitis and/or rhino-sinusitis was reported in all our patients. Housewives were the most affected socio-professional group, accounting for 34.37% of cases. The average consultation time was 20 ± 52 days. Tearing, palpebral oedema and purulent rhinorrhea were present in all our patients. Preseptal cellulitis and abscess were the most common complications, accounting for 56.25% and 28.12% of cases respectively. The treatment was medical-surgical. Drainage of a subperiosteal abscess of the ethmoid involved 53.12% of cases. Surgical treatment of the maxillary sinus and/or ethmoid by endoscopy was sometimes associated. The evolution was favourable in all cases. Conclusion: Ophthalmological complications of rhino-sinusitis are still frequent in our developing countries. Their management must be multidisciplinary.

Influence of Copper and Arsenic on Gold Recovery in the Yalea Deposit, Western Mali

Gold recovery process is well known. The following paper presents the problematic related to the influence of Copper and Arsenic on the recovery of gold in the Yalea deposit. Multielement tests (Au, Cu, As) carried out on 37 blocks made it possible to understand that there is a correlation between these elements. This correlation has been observed since the analysis of block models (the block model for Copper, block model for Arsenic and the block model for gold. These models have shown that the Yalea deposit areas with a high gold content correspond to areas of high copper content and arsenic. Those who made it clear that copper and Arsenic are tracing elements of Gold in the Yalea deposit. In this paper, the mineralurgical tests carried out on 28 blocks revealed that the copper and the arsenic content in the ore penalize the recovery of Gold (146 ppm for copper and 4710 ppm for Arsenic). The Yalea deposit was emplaced by several hydrothermal phases that reactivated the structures. These phases are responsible for the establishment of large quantities of copper sulphides. Copper and Arsenic are elements that have a considerable influence on the gold recovery in the Yalea deposit.

Objective: Evaluate pyeloplasty according to Küss-Anderson-Hynes at the urology-andrology department of the Sino-Guinean Friendship Hospital. Patient and Method: This is a 3-year prospective descriptive study from January 1, 2018 to December 31, 2020. It focused on a sample of 21 patients, who had undergone pyeloplasty according to Küss-Anderson-Hynes. Results: The averages age of the patients was 24.24 years. Lumbar pain was the main reason for consultation in 71.43% of cases. pyeloplasty according to Küss-Anderson-Hynes alone was performed in 76.20% of cases. It was associated with lower pole vessel uncrossing in 14.29% of cases and in 9.52% of cases with pyelolithotomy. The main Postoperative complications consisted of surgical site infections (23.81%) and fistula of the pyelo-ureteral junction (9.52%). The result of the pyeloplasty evaluated after three years, was qualified as good in 13 patients (86.67%), conversely the result was declared bad in 2 patients or 13.33%. During the follow-up period, postoperatively, we had lost sight of 6 patients. Conclusion: Pyeloplasty according to Küss-Anderson-Hynes in addition to its excellent results reported by the literature was the only therapeutic alternative performed during this study. However, its indications are considerably reduced with the advancement of laparoscopy.

To improve the understanding of the relationship between silica source and organic matter accumulation,the origin of silica and its implications for organic matter enrichment in the Upper OrdovicianLower Silurian(O_(3)w-S_(1)l)black shale in western Hubei Province in the middle Yangtze area,were investigated through geochemical and petrological *** results show that the O_(3)w-S_(1)l black shale is mainly composed of five lithofacies with varying graptolite abundance,total organic carbon(TOC),and silica *** silica and terrigenous siliciclastic input constitute the main silica sources in O_(3)w-S_(1)l black shale and they exhibit an upward inverse variation trend interpreted to be related to sea-level ***,with the increase in biogenic silica content or decrease in terrigenous siliciclastic input,TOC values in black shale initially rise and then fall,which is different from originally expected simple linear *** infer that organic matter enrichment and the distribution of silica from different origins are controlled by sea-level changes and variations in terrigenous input in a continental shelf with little hydrothermal *** environment with appropriate sea level and terrigenous input should be most favorable for organic matter accumulation,rather than one with excessive high sea level and less terrigenous input.

Survey of Patient Knowledge, Attitudes and Practices Regarding ENT Pathologies in a Tertiary Health Care Facility in West Africa

The ENT pathologies are not well known to the populations of our developing countries. This lack of knowledge leads certain subjects to have attitudes and practices that favor their occurrence and/or their aggravation. The objective of this study was to assess the knowledge and describe the attitudes and practices of patients facing ENT pathologies at the Conakry Hospital and University Center (CHU). This was a prospective, descriptive study lasting six months (June-November 2018). Among the 1410 patients who had consulted in the department during the study period, 1102 had participated in the survey, or 78.16% of cases. The mean age of the patients was 39.3 ± 17.3 years, predominantly female, for a sex ratio of 0.75. The majority of patients were of university education (41.02%) and civil servants (38.66%) represented the dominant socio-professional layer. Rhino-sinusitis (30.85%) and ear infections (20.14%) were the most common pathologies. Almost a third (32.85%) of respondents had no knowledge of behaviors harmful to ENT health. More than half (52.45%) of the patients did not know of any factors favoring the occurrence of ENT pathologies. The therapeutic orientation had been made towards modern medicine in 63.52% against 4.54% for traditional medicine. The choice of self-medication concerned 31.94% of patients. This survey showed that patients have limited knowledge of ENT pathologies as well as the attitudes and practices likely to favor their occurrence. However, a multicenter study involving a larger proportion of patients will confirm these data.

Traumatic Spinal Cord Injuries in Gold Miners in Mali: Preliminary Study on 76 Cases

Introduction: Gold panning is the research and artisanal exploitation of gold in the auriferous areas (rivers or others). Traumatisms arising from these activities are due to severe spinal cord injury and generally affect young people in full activity. They constitute a real social handicap in Mali and even in Africa. The aim of our study is to share our expertise in the management of these vertebro-medullary traumasin these miners who currently escape to all controls in Mali. Material and Methods: From 2013 to 2016, 76 Traumatic Spine Cord Injuries (TSCI) were treated in the department of Neurosurgery of “Hôpital du Mali”. There were 73 males (96.05%) versus 03 Females (3.95%), aged between 21 - 43 years old. Results: All of our patients were admitted with deficit differing from paraparesis to tetraplegia. According to Frankel scale, we found 59 patients with complete deficits (A grade, 77.63%), and incomplete deficits in 17 cases (12 grade B, 04 grade C, 01 grade D). The X-ray standard radiographies were done in 24 cases (31.58%) from which 02 patients had been operated with these data. CT scan centered on the injured spinal segment was performed in 74 cases (97.37%). The lumbar spine was affected in 39 cases, followed by thoracolumbar junction in 19 cases and thoracic spine in 16 cases. The cervical spine was only interested in two cases. Fractures dislocations are the most frequently lesions encountered in our series. The surgical approach was performed in 74 patients and 02 patients received conservative treatment. Conclusion: Traumatic Spinal Cord Injuries (TSCI) among miners are a real disaster in Mali and continue to plague thousands of families or increase the number of disabled people in our society. It is the only work, where the person concerned is making of his own grave. And according to witnesses, several miners remained at the bottom of the wells. 疟疾是现代世界卫生方面的突出问题,也是流行最广、危害最大的疾病之一。人参是中国的一种传统的草药,本文研究了人参总皂甙和原人参二醇型皂甙对疟疾的治疗和预防效果。利用ICR小鼠,进行了剂量范围为5-50mg/kg每天小鼠体内的预防与治疗实验。在感染的不同时期测试了药物对疟原虫的抑制活性。分别以10和50mg/kg人参总皂甙对疟原虫进行4、6和8天的预防实验,显示50mg/kg条件下,6天和8天有预防效果,与对照相比有极显著的差异,抑制率分别达到53.12%和57.52%。原人参二醇型皂甙在治疗和预防实验中都显示了有效的抗疟活性。当药量从5mg/kg-50mg/kg每天持续增加时,2-6天的治疗实验中给原人参二醇型皂甙4天,显示治疗效果最好,而到第6天却有降低。预防实验中,10mg/kg的原人参二醇型皂甙的预防效果到第6天时达到最高,第8天略有降低,抑制率分别为41.22%和35.49%,均有极显著差异(P＜0.01);而50mg/kg原人参二醇型皂甙随着时间延长,预防效果逐渐增加,显示出了时间的依赖性。

The Jarque-Bera’s fitting test for normality is a celebrated and powerful one. In this paper, we consider general Jarque-Bera tests for any distribution function (df) having at least 4k finite moments for k ≥ 2. The tests use as many moments as possible whereas the JB classical test is supposed to test only skewness and kurtosis for normal variates. But our results unveil the relations between the coeffients in the JB classical test and the moments, showing that it really depends on the first eight moments. This is a new explanation for the powerfulness of such tests. General Chi-square tests for an arbitrary model, not only normal, are also derived. We make use of the modern functional empirical processes approach that makes it easier to handle statistics based on the high moments and allows the generalization of the JB test both in the number of involved moments and in the underlying distribution. Simulation studies are provided and comparison cases with the Kolmogorov-Smirnov’s tests and the classical JB test are given.
