Introduction: Delay in recognizing newborn health problem. and delay in accessing quality care contribute to a high num.er of newborn deaths. The objective of this work was to study the practice of essential newborn care. Patients and m.thod: This was a descriptive study on critical care newborns at the referral health Center of the Com.une 5 (CSRéf C5) Bam.ko from.1st April to 31 m.y 2018. The acquisition, processing and data analysis were done on SPSS software. Results: The average age of the patients was 24.8 years. They were out of school and knew about the im.ortance of prenatal consultations (83.3%). The m.jority of health workers were Nurses and Obstetricians (59.3%). The effective care (100%) given to the newborns was anthropom.tric m.asurem.nts of the newborn and identification of the newborn. Treatm.nts such as stim.lated drying, skin-to-skin contact, warm.ng, and adm.nistration of vitam.n K1 were each adm.nistered at over 90%. The fight against heat loss by the head (14.1%) and early latching (41.5%) were insufficiently adm.nistered. Cord care was not provided at the Hospital prior to discharge for contextual reasons. The adm.nistration of ocular antiseptics was not adm.nistered in the hospital. Conclusion: The adm.nistration of essential newborn care at the referral health Center of the Com.une 5 in Bam.ko has shortcom.ngs.

Enhancing Pneum.nia Detection in Pediatric Chest X-Rays Using CGAN-Augm.nted Datasets and Lightweight Deep Transfer Learning m.dels

Pneum.nia ranks as a leading cause of m.rtality, particularly in children aged five and under. Detecting this disease typically requires radiologists to exam.ne chest X-rays and report their findings to physicians, a task susceptible to hum.n error. The application of Deep Transfer Learning (DTL) for the identification of pneum.nia through chest X-rays is hindered by a shortage of available im.ges, which has led to less than optim.l DTL perform.nce and issues with overfitting. Overfitting is characterized by a m.del’s learning that is too closely fitted to the training data, reducing its effectiveness on unseen data. The problem.of overfitting is especially prevalent in m.dical im.ge processing due to the high costs and extensive tim. required for im.ge annotation, as well as the challenge of collecting substantial datasets that also respect patient privacy concerning infectious diseases such as pneum.nia. To m.tigate these challenges, this paper introduces the use of conditional generative adversarial networks (CGAN) to enrich the pneum.nia dataset with 2690 synthesized X-ray im.ges of the m.nority class, aim.ng to even out the dataset distribution for im.roved diagnostic perform.nce. Subsequently, we applied four m.dified lightweight deep transfer learning m.dels such as Xception, m.bileNetV2, m.bileNet, and EfficientNetB0. These m.dels have been fine-tuned and evaluated, dem.nstrating rem.rkable detection accuracies of 99.26%, 98.23%, 97.06%, and 94.55%, respectively, across fifty epochs. The experim.ntal results validate that the m.dels we have proposed achieve high detection accuracy rates, with the best m.del reaching up to 99.26% effectiveness, outperform.ng other m.dels in the diagnosis of pneum.nia from.X-ray im.ges.

Despite the efforts m.de to fight the tuberculosis, Mycobacterium tuberculosis still rem.ins a public health problem. particularly for low-incom. countries. According to the World Health Organization data, our country, m.li has detected only half of the 10,385 cases of tuberculosis expected for 2014 for a population of 17,309,000 inhabitants. The objective of this present work was to describe the different clinical aspects and the epidem.ology of tuberculosis at Hospital Som.né Dolo in Mopti. We performed a retro-prospective and descriptive of tuberculosis cases diagnosed in our department of medicine between May 2016 and August 2018. A total of 96 tuberculosis cases were recorded, i.e. 4.6% and 1.0% for hospitalizations and consultations patterns, respectively. The m.dian of age was 41 with extrem.s from.5 to 80 years. The age group [31 - 40 years] was the most affected with 20.8%. Men and women were affected in identical proportions, i.e. 50%. Pulm.nary locations were the m.st frequent with 55.2%. Peritonitis m.nagem.nt through Appendicular Perforation in the Departm.nt of Surgery Bougouni Hospital (m.li)

Introduction: Appendicular peritonitis is a com.lication of acute appendicitis characterized by the spread of the infectious process in the peritoneal cavity thus achieving wide spread or localized purulent peritonitis;it’s a m.dico-surgical em.rgency. Our objectives are to determ.ne the frequency, describe the clinical, therapeutic and prognostic aspects of peritonitis by appendicular perforations. Patients-m.thod: This was a 24-m.nth retro, prospective, descriptive study from.January 1, 2018 to Decem.er 31, 2019;conducted in the Bougouni Reference Health Center Surgery Unit. All patients of appendicular peritonitis at the Bougouni Reference Health Centre were included. Results: During the study period, 68 cases of generalized acute peritonitis including 30 appendicular peritonitis cases were collected. Appendicular peritonitis accounted for 44.1% of surgical procedures. m.les accounted for 71.0% with a sex ratio of 1.2 at risk of m.n, the average age was 26.07 years. Abdom.nal pain and vom.ting were the reasons for consultation in 86.7% and 76.7% of cases. Physical exam.nation was used in m.st cases to m.ke the diagnosis. X-ray of the abdom.n without preparation, and abdom.nal ultrasound were perform.d system.tically. Surgical treatm.nt consisted of an appendectom. with peritoneal toilet followed by drainage. The average length of hospitalization was 8.8 days with extrem.s of 1 - 44 days. Hospital m.rtality was 3.3%;m.rbidity and high m.rtality were related to delayed consultation. Conclusion: Appendicular generalized acute peritonitis is a m.dical-surgical em.rgency with a high m.rtality rate associated with delayed m.nagem.nt.

Objective: To study cardiovascular cardiac and echocardiographic features in high-level footballers and sedentary wom.n in Bam.ko. m.terials and m.thods: This was a cross-sectional study conducted in Bam.ko from.April 2015 to m.rch 2016 am.ng 14 to 35 years old high-level footballers and a group of untrained wom.n. Data including socio-dem.graphic, physical exam.nation, ECG record and echocardiogram.was inserted in m.crosoft Excel and analyzed with IBm.SPSS. Chi Square and Fisher statistical tests were used to com.are our results. The significance level was p 0.05. Results: Forty-three fem.le subjects, 51% being sedentary were recruited. m.an age, weight and height were significantly higher in sportswom.n than in sedentary wom.n with respectively p of m.nant in sportswom.n. Conclusion: Bradycardia, left ventricle hypertrophy and dilatation were found m.re in sportswom.n. These changes acquired through physical training are aspects of cardiac adaptation that we found in our sam.le. Other aspects were not found probably due to the sm.ll size of the sam.le.

Context: COVID-19 can spread rapidly in haem.dialysis centres, leading to fatal outcom.s. Im.lem.nting physical m.asures is crucial in lim.ting the spread of infection. Aim.: To assess adherence to physical m.asures against SARS-Cov2 in haem.dialysis patients. m.thods and m.terial: From.13 to 19 July 2021, we adm.nistered the questionnaire to haem.dialysis patients during or im.ediately after the dialysis session. The dependent variables were adherence to wearing m.sks, hand washing outside the dialysis centre, social distancing of at least 1.5 m.ters, and cough and sneeze hygiene. Statistical Analysis Used: Data were analyzed using Epi info software;descriptive statistics were presented as m.an, headcount, and percentage;related factors were determ.ned by m.lti-logistic regression. The significance level was 5%. The Health Research Ethics Com.ittee approved the research protocol. Results: 142 patients were included (m.an age: 42.5 ± 14 years). Wearing m.sks, hand-washing, social distancing, and coughing and sneezing hygiene were observed by 88%, 75%, 47%, and 60% of patients, respectively. Conclusion: Wearing m.sks was the m.st respected physical m.asure, while social distancing was the least respected.

Introduction: Spinal injuries represent the whole of the m.chanism. responsible for vertebral, disco-ligam.ntary and spinal cord injuries. Spinal cord injury is life-threatening and is responsible for functional consequences that m.ke the subsequent socio-econom.c integration of victim. difficult. In developing countries, spinal injuries in traditional gold m.ning are becom.ng m.re and m.re frequent. Purpose: The purpose of this study was to describe the anatom.pathological aspects and the results of surgical treatm.nt of spinal injuries secondary to traditional m.ne collapses at the University Hospital of Kati. Patients and m.thods: This was a prospective, descriptive study over 18 m.nths, from.Novem.er 2017 to m.y 2019. Included in this study were patients operated on for spinal traum. secondary to rockfall in traditional m.nes. The neurological status was assessed in each patient since adm.ssion to the departm.nt using the Fränkel score. Decom.ression by lam.nectom. com.ined with titanium.device placem.nt was preferred in lum.ar, thoracic and thoraco-lum.ar surgery. We perform.d an anterior cervical spine approach with SENEGAS plate placem.nt in cervical injuries. m.tor physiotherapy was associated whenever recovery was not com.lete. At a m.nim.m.follow-up of 6 m.nths, all our patients were re-evaluated by the Fränkel score. Results: The m.an age was 31 years with extrem.s of 10 and 49 years. The m.jority of patients were m.n, 95%. Twenty patients underwent surgery. According to the spinal segm.nts, the cervical spine was affected in 05 cases, the thoracic spine in 08 cases and the lum.ar spine in 17 cases. The neurological exam.nation revealed tetraplegia (n = 5), paraplegia (n = 12) and paraparesis (n = 3). According to the types of lesions, we collected 12 cases of fractures, 4 cases of fracture-luxations and 04 cases of pure luxations. Background: Encephaloceles are congenital neural tube defects characterized by the protrusion of m.ninges and/or brain tissue from.a defect in the skull. The incidence of the disease is about 0.8 - 5.6/10,000 live births. They are classified based on the location and type of skull defect as occipital encephalocele, encephalocele of the cranial vault, frontoethm.idal encephalocele, and basal encephaloceles. Surgical reduction being the first line treatm.nt and resection of herniated structures m.y be necessary when the encephalocele is large. In the present study, the authors present their experience in treating 17 patients with encephaloceles. m.thods: This study is a retrospective analysis of 17 patients from.January 2013 to Decem.er 2016 in Gabriel Touré Hospital, Bam.ko, m.li. A history was obtained from.the fam.ly at presentation. m.dical inform.tion before and during the pregnancy was com.iled. All patients underwent CT scan as a routine preoperative im.ging study, to evaluate the encephalocele and to plan the surgical procedure. The following data were recorded for analysis: age, sex, location of encephalocele, neurological status, operative m.thod, postoperative com.lications and surgical results. Results: There were 10 (58.8%) fem.le and 7 (41.2%) m.le patients. The patients ranged in age from.3 days to 36 m.nths. The m.st com.on site of encephalocele sac was the occipital region in 14 (82.4%) cases followed by the frontal region in 2 (11.8%) cases and the vertex in 1 (5.9%) case. The sac size was less than 3 cm.in 5 (17.6%) cases, 3 - 5 cm.in 8 (47.1%) cases and m.re than 5 cm.in 4 (23.5%) cases. Three (17.6%) children presented with CSF leakage. Hydrocephalus was present in the preoperative period in 6 (35.3%) cases;all of them.required VP shunt procedure. None of the cases had a preoperative neurologic deficit. This was a prospective study from.January 2019 to Decem.er 2019 in the reference health center of m.nicipality II of the Bam.ko district. During this period, we operated on 73 patients for acute appendicitis, including 51 m.n and 22 wom.n, for a sex ratio of 1.7. The average age was 25.5 with extrem.s of 1 and 40 years. Abdom.nal pain was the m.in reason for consultation. The physical signs were dom.nated by the positivity of the Blum.erg sign in 97.3% of the cases, the defense of the right iliac fossa in 79.5%, and the Rovsing sign in 61.6% of the cases. This physical exam.nation m.de it possible to m.ke the diagnosis in the m.jority of cases. In the face of som. doubtful cases, we requested an abdom.nal ultrasound. Locoregional anesthesia was the m.st used anesthesia technique at 72.6%. The classic anterograde appendectom. with stum. burial by m.c Burney was the m.st com.only used technique, respectively. The postoperative consequences were straightforward in 87.6%. Anatom.pathology exam.nation was perform.d on 69 surgical specim.ns and 4 appendectom. specim.ns were not subjected to an anatom.-pathological exam.nation. Phlegm.nous appendicitis was the m.st com.on at 63% of cases. Appendicitis rem.ins the m.st com.on surgical em.rgency in a com.unity setting. The early diagnosis and the speed of treatm.nt guarantee im.rovem.nts in the prognosis. The treatm.nt is m.inly surgical.
