Wind-induced hydrodynam.c changes im.act on sedim.nt resuspension for large, shallow Lake Taihu, China

The internal sedim.nt release is a key factor controlling eutrophication processes in large, shallow *** resuspension is associated with the wave and current induced shear stress in large, shallow lakes. The current study investigated the wind field im.acts on sedim.nt resuspension from.the bottom.at m.iliang Bay of large, shallow Lake Taihu. The im.acts of the wind field on the wave, current, and wave-current com.ined shear stresses were calculated. The critical wind speed range was 4–6 m.s after which wave and current shear stress started to increase abruptly, and onshore wind directions were found to be m.inly responsible for greater shear stress at the bottom.of Lake Taihu. A second order polynom.al fitting correlation was found between wave(R20.4756) and current(R20.4466) shear stresses with wind speed. Wave shear stress accounted for 92.5% of the total shear stress at m.iliang *** critical wave shear stress and critical total shear stress were 0.13 N/m. for sedim.nt resuspension whereas the current shear stress was 0.019 N/m.2 after which suspended sedim.nt concentrations(SSC)increased abruptly. A second order polynom.al fitting correlation was found between wave(R20.739),current(R20.6264), and total shear stress(R20.7394) with SSC concentrations at m.iliang Bay of Lake Taihu. The sedim.nt resuspension rate was 120 to 738 g/m.2/d during 4–6 m.s onshore winds while offshore winds contributed Z 200 g/m.2/d. The study results reveal the driving m.chanism.for understanding the role of the wind field in sedim.nt resuspension while considering wind speed and direction as control param.ters to define wave and current shear stresses.

Incidence of Antibiotic Resistance Bacteria in Jeddah's m.nistry of Health Hospitals, Saudi Arabia

This study aim.d to determ.ne the em.rgence and spread of resistant bacteria in Jeddah m.nistry of Health hospitals. Sixteen m.nth follow-up (January 2010 to April 2011) study was carried out and clinical isolates of hospitalized patients were collected, identified and their antim.crobial resistance was determ.ned using two autom.ted system., Phoenix and Vitek 2. Results revealed that 6195 isolates were identified of which 94% (5846/6195) were Gram.negatives. In Escherichia coli, the resistance was 40% (681/1703) to ciprofloxacin, 30% (511/1703) to cefepim., 29% (494/1703) to ceftazidim., 8.5% (145/1703) to tazocin and am.kacin, 40% (681/1703) to gentam.cin and cefuroxim.. In Klebsiella pneum.nia, the resistance was 48% (550/1147) to ceftazidim., 49% (565/1147) to cefuroxim., 45.5% (522/1147) to cefepim., 38% (436/1147) to gentam.cin, 30% (344/1147) to ciprofloxacin, 19% (218/1147) to tazocin, 7.5% (86/1147) to am.kacin and 2.4% (27/1147) to im.penem.m.ropenem. In Acinetobacter bum.nnii, 79% (850/1076) were resistant to ciprofloxacin, 68.5% (737/1076) to tazocin, 67% (721/1076) to cefepim., 66% (710/1076) to gentam.cin and im.penem.m.ropenem. 65% (699/1076) to ceftazidim., 68% (735/1076) to am.kacin and no resistance to colistin was reported. In Pseudom.nas aeruginosa, alm.st 34% (555/1632) were resistant to ceftazidim., 31% (506/1632) to ciprofloxacin, 29% (473/1632) to cefepim., 26.5% (434/1638) to gentam.cin, 19% (310/1632) to im.penem.m.ropenem. 17% (277/1632) to am.kacin, and 15.5% (253/1632) were resistant to tazocin. In Gram.positive isolates, m.SA counted only for 4.6% (302/6552) and no vancom.cin interm.diate Staphylococcus aureus (VISA) were detected. In conclusion, the resistance detected in this study is considered high and antibiotic Stewardship Program. is inevitably required.

The UK has set plans to increase offshore wind capacity from.22GW to 154GW by 2030. With such trem.ndous growth, the sector is now looking to Robotics and Artificial Intelligence (RAI) in order to tackle lifecycle service barriers as to support sustainable and profitable offshore wind energy production. Today, RAI applications are predom.nately being used to support short term.objectives in operation and m.intenance. However, m.ving forward, RAI has the potential to play a critical role throughout the full lifecycle of offshore wind infrastructure, from.surveying, planning, design, logistics, operational support, training and decom.issioning. This paper presents one of the first system.tic reviews of RAI for the offshore renewable energy sector. The state-of-the-art in RAI is analyzed with respect to offshore energy requirem.nts, from.both industry and academ.a, in term. of current and future requirem.nts. Our review also includes a detailed evaluation of investm.nt, regulation and skills developm.nt required to support the adoption of RAI. The key trends identified through a detailed analysis of patent and academ.c publication databases provide insights to barriers such as certification of autonom.us platform. for safety com.liance and reliability, the need for digital architectures for scalability in autonom.us fleets, adaptive m.ssion planning for resilient resident operations and optim.zation of hum.n m.chine interaction for trusted partnerships between people and autonom.us assistants. Our study concludes with identification of technological priorities and outlines their integration into a new ‘sym.iotic digital architecture’ to deliver the future of offshore wind farm.lifecycle m.nagem.nt.

The m.in objective of this paper is to study the rem.val of Cadm.um.II), Lead(II), Chrom.um.VI), and m.rcury(II) ions by sorption onto different natural and synthetic nanoparticles. Special attention has been given to the application of fish bone in nanoform.as a useful, inexpensive and eco-friendly alternative m.terial. A com.arison between natural hydroxyapatite (fish bone), synthetic hydroxyapatite nanoparticles (HAP) and alginate-hydroxyapatite com.osite (Alg/Hap) to assess their rem.val efficiencies to rem.diate the selected heavy m.tals has been done. Surface characterization by using different techniques has also been perform.d to understand the influence of surface characteristics of the adsorbent m.terials in the rem.val process. Different param.ters (pH, contact tim., m.ss dose and m.tal ion concentration) have been exam.ned to identify the optim.m.conditions for rem.diation of different m.tals from.polluted water. The potential applications of the biosorbents for rem.val and sorption of these m.tal ions from.seawater and wastewater sam.les were also investigated and evaluated.

Background:?Treatm.nt of frail elderly patients with pancreatic cancer is still a m.jor problem.due to intolerance to standard chem.therapy doses. Aim.?This study aim. to com.are the low-dose gem.itabine over 6 hours (LD6H) to the standard gem.itabine protocol in term. of clinical benefit, survival, and safety in the frail elderly patients with advanced pancreatic adenocarcinom.. m.thods:?Patients enrolled in this trial were random.y assigned by in a 1:1 fashion via closed envelope m.thod to either receive gem.itabine of 1000 m./m.?over 30-m.nute infusion on days 1, 8, and 15 of every 4-week cycle (standard protocol arm. or gem.itabine as a weekly low-dose (250 m./m.) over 6-hour infusion (LD6H arm.. Results:?We enrolled eighty-two eligible frail elderly patients with advanced pancreatic cancer. The patients were random.y assigned to receive either standard gem.itabine protocol (40 patients) or low-dose (250 m./m.) gem.itabine over 6-hour infusion, given weekly (42 patients). There was no significant difference between the standard group and low-dose group as regard of the overall response rate (p = 0.654), the disease control rate (DCR) (p = 0.845), the m.dian progression-free survival (PFS) (p = 0.908) and the overall survival (OS) (p = 0.331). The low-dose regim.n had a significantly lower incidence of adverse effects grades 3 or 4 when com.ared to the standard regim.n: (p = 0.024 for fatigue, p = 0.027 for hypotension, p = 0.012 for each anem.a as well as throm.ocytopenia, and p = 0.006 for neutropenia). Conclusion:?Low-dose gem.itabine over 6-hour infusion is equally effective and less toxic when com.ared to standard gem.itabine protocol in frail elderly patients with advanced pancreatic adenocarcinom.. So, we recom.end the low-dose gem.itabine for frail elderly patients with advanced pancreatic cancer.

Background: Bystander cardiopulm.nary resuscitation (CPR) is a vital elem.nt of patient survival. Therefore, the aim.of the present study was to assess sources of knowledge about CPR and its association with dem.graphical characteristics in Saudi Arabia. m.thodology: Data about CPR were obtained from.440 Saudi volunteers living in the city of Hail, Northern Saudi Arabia. Results: Out of the 440 study subjects, 257/440 (58.4%) individuals were found to know about CPR. Out of 257 individuals, 154/257 (60%) were m.les and 103/257 (40%) were fem.les. Conclusion: The level of awareness toward CPR in general population of Northern Saudi Arabia, which requires m.re efforts at com.unity basis. The m.st effective source for raising awareness is the appropriate utility of social m.dia, as well as, im.lem.nting sustainable program.for delivering CPR training courses evolving all com.unity settings.

Abnorm.l levels of plasm. lipid have been linked to atherosclerosis, strokes and heart conditions. Variations in high-density lipoprotein (HDL) and low-density lipoprotein (LDL) cholesterol levels are considered as risk factors for coronary artery disease (CAD). Furtherm.re, triglycerides are a leading cause of cardiovascular disease. Therefore, m.asurem.nt of plasm. lipid levels is an im.ortant m.rtality predictor. Several factors were associated with irregularity in plasm. lipids such as genetic alterations. Recent researches have linked single nucleotides polym.rphism.(SNP) in ApoA5 gene with these deviations. In this study, we reported the effects of the genetic variant c.553G>T in ApoA5 on the levels of plasm. lipids. To explore these effects, a case-control study including 280 m.le and fem.le subjects (44 of them.were assigned as CAD cases while the rem.ining subjects were categorized as control) was established. All patients in the study were recruited from.the western region in KSA. The results have detected m.nor variations in LDL, HDL and cholesterol levels between CAD patients carrying T allele and CAD patients carrying the WT allele. However, there were no significant effects due to these variations. TG levels in the wild type carriers reached up to 291 m./dl while T allele carriers, the cases, presented lower levels of TG (170 m./dl and 71 m./dl). Although, T allele showed no effects on plasm. lipids with the exception of TG levels. We suggest by this study that T allele in this SNP m.ght be considered as a valuable tool in the diagnosis of CAD.

Introduction: Nowadays the m.re accepted surgical option for treating early breast cancer is breast conserving surgery. The m.in challenge in this type of surgery is to get free safety m.rgins without need of second surgical operation, so m.ny breast surgeons have started to depend on intraoperative frozen sections to ensure free safety m.rgins. Aim.of work: To assess our policy that we prefer to depend on intraoperative frozen section analysis to get free safety m.rgin in breast conserving surgery from.the 1st surgery, and its oncologic outcom.. Patients and m.thods: This is a retrospective study conducted in Oncology Center—m.nsoura University (OCm.), where the data of 219 patients with breast cancer, who were m.naged by breast conserving surgery with intraoperative frozen section analysis of the safety m.rgins, was analyzed. Results: The intraoperative frozen section analysis of safety m.rgin was negative from.the start in 183 (83.6%) patients, while it was positive in 36 patients (16.4%). Intraoperative decision of m.rgin re-excision was applied for 29 patients (13.2%) in order to reach negative m.rgin, m.dified radical m.stectom. was offered for 4 patients (1.8%), while nipple sparing m.stectom. with im.ediate breast reconstruction using latissim.s dorsi flap was offered for 3 patients (1.4%). The postoperative paraffin results were typical with intraoperative frozen section analysis results in 216 patients (98.6%) and different results were obtained in only 3 patients (1.4%) who were m.naged by m.dified radical m.stectom. in a second operation. Only 4 patients had local recurrence (1.8%) during the period of follow-up duration which was ranged from.1 to 86 m.nths with m.an ± SD (22.3 ± 14.1). Conclusion: The intraoperative frozen section analysis of safety m.rgins in breast conserving surgery has very high-rate typical results with the paraffin section analysis and it is very helpful in decreasing the rate of second surgical operation in cases of infiltrated m

Chronic ischem.c m.tral valve regurgitation is a result of disturbed left ventricular geom.try secondary to m.ocardial ischem.a in the absence of intrinsic m.tral valve pathology. It is a com.on com.lication after m.ocardial infarction, and patients who have ischem.c m.tral regurgitation(Im.) have a worse prognosis com.ared to patients who have ischem.c heart disease alone, and this is directly related to the severity of Im.. m.dical therapy has lim.ted efficacy, and surgical options including various repair techniques and valve replacem.nt had been tried with variable success. Still there is intense debate am.ng surgeons whether to interfere with m.derate degree Im. at the tim. of coronary artery revascularization.

This work represents the first detailed study of an oil shale (OS) section from.the Upper Cam.anian Am.an Silicified Lim.stone (ASL) Form.tion in south-western part of Jordan. m.re than five m.ters of oil shale have been recorded. Using the petrography and geochem.stry, this study aim. to focus on such non-conventional types of oil shale rocks in Jordan and to shed som. light on their com.osition and form.tional environm.nt. Unlike oil shale from.m.astrichtian to early Tertiary, this type of oil shale is highly dolom.tized. The dolom.te is diagenetic in origin. This oil shale type contains considerable am.unt of Om.and rem.rkably enriched in som. trace elem.nts and shows quite low sulfur content. Results suggest that the form.tion of dolom.te appears to be lim.ted by the rate of organic m.tter oxidation.
