Evaluating the influence of a structured nursing protocol on targeted outcomes in rheumatoid arthritis patients

Objective:Rheumatoid arthritis(RA)requires comprehensive *** nursing protocols may enhance outcomes,but evidence is *** study evaluated the effect of a structured nursing protocol on RA *** and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments before and after a 12-week nursing protocol comprising education,psychosocial support,and self-management *** included clinical evaluation of joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self-Efficacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)for pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated significant improvements in both clinical-and patient-reported *** count decreased from 18.4±4.2 to 14.2±3.8(P<0.001),ESR from 30.1±6.8 mm/h to 25.5±6.8 mm/h(P<0.01),and CRP levels from 15.2±3.6 mg/L to 11.8±2.9 mg/L(P<0.01)***-reported outcomes showed a marked increase in ASES score from 140±25 to 170±30(P<0.001)and reductions in HAQ from 1.6±0.4 to 1.3±0.3(P<0.01),VAS pain score from 7.8±1.7 to 6.2±1.2(P<0.001),and HADS anxiety and depression scores from 11±3 to 8±2(P<0.05)and 10±2 to 7±1(P<0.05),***:A structured nursing protocol significantly improved clinical disease activity,physical functioning,pain,self-efficacy,and emotional well-being in RA patients.A multifaceted nursing intervention appears beneficial for optimizing RA outcomes.

Resistance to acid degradation,sorptivity,and setting time of geopolymer mortars

Experimental evaluations were conducted to determine the water sorptivity,setting time,and resistance to a highly acidic environment,of mortar with alkali-activated ground granulated blast furnace slag(GBS)binder and also of combinations of fly ash and GBS *** were activated using mixtures of NaOH and Na_(2)SiO_(3)*** molarity of NaOH in the mixtures ranged from 10 mol·L^(−1)to 16 mol·L^(−1),and the Na_(2)SiO_(3)/NaOH ratio was varied from 1.5 to *** samples were produced using three binder combinations:1)GBS as the only binder;2)blended binder with a slag-to-fly ash ratio of 3:1;and 3)mixed binder with 1:1 ratio of slag to fly *** samples were mixed and cured at(22±2)°C till the day of the *** impact of activator solution alkalinity,activator ratio Na_(2)SiO_(3)/NaOH,GBS content on the rate of water absorption were *** 7,28,and 90 d of immersion in a 10%sulfuric acid solution,the resistance of a geopolymer matrix to degradation was assessed by measuring the change in sample *** influence of solution alkalinity and relative fly ash content on setting times was ***-activated mortar with a slag-to-fly ash ratio of 3:1 had the least sorptivity compared to the two other binder combinations,at each curing age,and for mortars made with each of the NaOH alkaline activator *** sorptivity decreased with age and sodium hydroxide concentrations,suggesting the production of geopolymerization *** reduction in weight of sample occurred after immersion in the strong acid H_(2)SO_(4)solution for three months,regardless of binder *** was due to the synthesis of hydration and geopolymerization products in the presence of curing water,which outweighed the degradation of the geopolymer matrix caused by sulfuric acid.

Compressive strength and stability of sustainable self-consolidating concrete containing fly ash, silica fume, and GGBS

This paper presents the findings of an experimental program seeking to understand the effect of mineral admixtures on fresh and hardened properties of sustainable self-consolidating concrete （SCC） mixes where up to 80% of Portland cement was replaced with fly ash, silica fume, or ground granulated blast furnace slag. Compressive strength of SCC mixes was measured after 3, 7, and 28 days of moist curing. It was concluded in this study that increasing the dosage of fly ash increases concrete flow but also decreases segregation resistance. In addition, for the water-to-cement ratio of 0.36 used in this study, it was observed that the compressive strength decreases compared to control mix after 28 days of curing when cement was partially replaced by 10%, 30%, and 40% of fly ash. However, a fly ash replacement ratio of 20% increased the compressive strength by a small margin compared to the control mix. Replacing cement with silica fume at 5%, 10%, 15%, and 20% was found to increase compressive strength of SCC mixes compared to the control mix. However, the highest 28 day compressive strength of 95.3 MPa occurred with SCC mixes in which 15% of the cement was replaced with silica fume.

Transurethral Resection of Bladder Tumour: Safe Implementation of Bipolar Technique

Background: Traditionally, monopolar transurethral resection was the standard surgery for bladder tumors. After developing the bipolar technique, it was noticed that patients’ morbidity was reduced. We aim to compare bipolar and monopolar transurethral resection of bladder tumors in terms of efficacy, safety, and oncological outcomes. Patients and Methods: Clinical records of sixty patients with newly diagnosed bladder cancer who underwent either monopolar or bipolar transurethral resection between March 2019 and April 2021 were prospectively reviewed. Results: Thirty patients were included in each group. The mean age in monopolar and bipolar arms was 59.9 and 57.5 years, respectively. The obturator reflex occurred in 13.33% and 36.66% of the bipolar and monopolar arms, respectively (p = 0.032). Bladder perforation was reported in 2 cases in the monopolar arm. The resection time was significantly longer for the monopolar arm (25.45 Vs 22.85 minutes). Also, the monopolar arm reported longer mean irrigation (23.34 Vs 20.11, p = 0.039), with 1 TUR syndrome reported in the monopolar arm. The mean hemoglobin drop was more in the monopolar arm (0.9 Vs 0.5 gm/dl, p = 0.041). No statistical significance was reported regarding the histopathology results, and one patient in the monopolar group did not have muscle in his specimen. There was no statistically significant difference in recurrence and prognosis between both groups. Conclusions: The bipolar technique has demonstrated its reliability and efficiency as a surgical procedure, providing various advantages while minimizing the risk of complications.

Perception of Physicians to the Role of Clinical Pharmacists in United Arab Emirates (UAE)

The present study aims to investigate the perceptions and experience of physicians regarding the clinical role of the pharmacists. This is an observational study that was conducted during the period of September 2013 through March 2014. A self-administered questionnaire was designed to be distributed by personal interview to the physicians. The questionnaire population consisted of randomly selected physicians practicing in UAE. The questionnaire was piloted by 10 physicians for face and understandability. Demographic data, frequencies and cross tabulation between different variables were calculated. The Chi-square test was used to determine the significance of association between categorical variables (gender, length of medical practice, and specialty) and the perception of physicians. A total of 285 filled questionnaires were returned representing 285 physicians, with a male dominance (65.3%). The average age of the physician screened was 32.4 (SD = 10.38) years with a minimum one year experience and up to 24 years. General practitioners consumed the widest sector (21.8%) followed by cardiologists (15.8%), then internal medicine (10.1%). In the current study, two thirds of the physicians believed that pharmacists could act as a reliable source of general drug information and play an important role in discovering clinical related problems. It was found that the physicians who had fewer years in practice (less than 10 years) and recently graduated had more acceptances to the clinical role of the pharmacist and believed that there should be a clinical pharmacy services in their hospitals (p-value < 0.05). No other statistically significant differences were found from analyzing the data. In conclusion, results suggest that physicians in UAE appear comfortable with pharmacists providing the broad range of services but appear somewhat less comfortable with pharmacists’ provision of direct patient care.

Pharmacist Role in Pharmaceutical Care during Ramadan

Introduction: Ramadan is the ninth month in the Islamic Hijri calendar where Muslims are ex-pected to fast every day from dawn to sunset. The eating behavior changes during Ramadan and the abstinence of food affects the oral drugs administration and other convenience dosage forms. There is a need to highlight and illustrate the role of the pharmacists in medication adjustment challenges that the patients conquer in the Holy month of Ramadan. Objectives: To estimate the awareness of pharmacists’ role in providing pharmaceutical care during Ramadan;moreover, to explore pharmacists’ perspective on the importance of medication regimen adjustment along with the proper counseling required to optimize patients’ health throughout fasting. Method: This is an observational study that was done through dissemination of a cross-sectional survey among 130 pharmacists covering Abu Dhabi, Dubai, Sharjah, Ajman, and Northern Emirates regions. The targeted population consisted of practicing community pharmacists and hospital pharmacists across UAE. Results: Among pharmacists participated in the study 115 pharmacists (88.5%) were Muslims and 79 pharmacists (60.8%) were Arabs. 126 pharmacists (97%) reported performing one or more kinds of medication regimen adjustment around Ramadan for at least one patient. Changing the frequency had the highest percentage by the pharmacist compared to other methods of regimen adjustment (39%). Moreover, the results revealed that 46.9% of the pharmacists chose not to change the medication itself. Nineteen percent of Muslim pharmacists initiated the conversation about medication regimen adjustment (P = 0.0448) compared to non-Muslims, while14% of pharmacists were Arabs who showed no statistically significant difference. Conclusion: In conclusion, pharmacists are more qualified to adjust and manage medication regimens than what they are presently performing. A statistical approach to enhance the productivity of Streptomyces baarensis MH-133 for bioactive compounds

The goal of this study was to use statistical optimization to change the nutritional and environmental conditions so that Streptomyces baarensis MH-133 could make more active *** trials were used to screen for critical variables influencing productivity using the Placket-Burman Design *** MH-133 is significantly influenced by elicitation,yeast extract,inoculum size,and incubation period in terms of antibacterial activity.A total of 27 experimental trials with various combinations of these factors were used to carry out the response surface technique using the Box-Behnken *** analyses revealed that the model was highly significant(p<0.001),with a lack-of-fit of 0.212 and a coefficient determination(R2)of ***,the model predicted that the response as inhibition zone diameter would reach a value of 27 *** optimal conditions,*** MH-133 produced 18.0 g of crude extract to each 35L and was purified with column *** active fraction exhibiting antibacterial activity was characterized using spectroscopic *** MIC and MBC values varied between 37.5 and 300μg/ml and 75 and 300μg/ml,*** conclusion,the biostatistical optimization of the active fraction critical variables,including environmental and nutritional conditions,enhances the production of bioactive molecules by Streptomyces species.

Alzheimer’s disease (AD) is a neurodegenerative disorder that is characterized by progressive loss of memory, confusion, inability of speech and decline in the cognitive behavior. It is considered one of the most common forms of dementia. Clinical studies and preclinical data in the last decade proved that AD and Diabetes mellitus share a pathophysiological pathway, indicating that insulin resistance, oxidative stress and inflammatory response would increase the risks of developing AD in diabetic patients. This review presents briefly the etiology of AD and Diabetes, discusses the possible theories about the interplaying risk factors and the mechanism of action of anti-diabetic medications recommended for the treatment of AD. It is concluded that antidiabetics have good potential to improve dementia, especially in earlier AD stages. However, many of the underlying intricate molecular pathways are still unclear and thus thorough future research is required.

Severity of Coronary Artery Disease by Coronary CT Angiography in Relation to Left Ventricular Diastolic Function

Objectives:?The aim of this study is to evaluate the relationship between severity of CAD by coronary mutlislice computed tomography scan and the degree of diastolic function Methods:?The study consisted of 80 patients who underwent both coronary CTA and TTE within 1 year with no identifiable intervening cardiovascular event in National?Heart?Institute and Menoufia?University?Hospitals. All patients were subjected to detailed medical history, physical examination, full electrocardiography study including tissue Doppler imaging on mitral annulus, MSCT coronary angiography and different laboratory investigation as CBC, creatinine, lipid profile. Results:?From 80 patients who have done both echocardiography examination and MSCT coronary angiography, the CT scan results were 38 patients with non-obstructive coronary artery disease and 42 patients with obstructive CAD, higher number of patients with diastolic dysfunction with higher values of E/E’, medial e’, lateral e’, LAVI (left atrial volume index) and TRV (tricuspid regurge velocity) were found in the group with obstructive coronary artery disease.?Conclusion:?Our study showed that diastolic dysfunction is more prevalent in patients with obstructive coronary artery disease with higher grade of diastolic dysfunction.
