培哚普利治疗稳定性冠心病的耐受性研究

目的对稳定性冠心病患者,观察将培哚普利由4mg/d 调整至8mg/d 后的耐受性。方法开放性多中心研究。入组已经服用培哚普利4 mg/d 至少4周的稳定性冠心病患者,采用培哚普利8 mg/d 治疗2个月,观察增量后的安全性与耐受性。观察指标:不良事件和严重不良事件、患者依从性,记录基线状态、第4周和第8周的血压、心率、血钾、钠、肌酐和尿素氮。结果共入选109例符合入选标准的稳定性冠心病患者。其中102例患者能够很好耐受培哚普利8 mg/d 治疗,总耐受率为93.6%。共计18例患者(占16.5%)发生不良事件,无严重不良事件发生。仅1例患者(占0.9%)因不良事件退出。培哚普利增量使基线血压未达标患者的血压由(149.4±7.6)/(84.1±11.0)mm Hg(1 mm Hg=0.133 kPa)下降至(128.8±14.2)/(79.5±8.9)mm Hg(均 P<0.01),而对基线血压达标患者的血压无影响。观察期间患者心率由(72.3±9.4)次/min 下降至(69.8±7.8)次/min(P<0.05),血电介质、肌酐和尿素氮水平无变化。结论中国稳定性冠心病患者对培哚普利8 mg/d 的治疗剂量耐受性良好。

A Biobank for Long-term and Sustainable Research in the Field of Congenital Heart Disease in Germany

Congenital heart disease （CHD） is the most fi＇equent birth defect （0.8% 1% of all live births）. Due to the advance in prenatal and postnatal early diagnosis and treatment, more than 90% of these patients survive into adulthood today. However, several mid- and long-term morbidi- ties are dominating the follow-up of these patients. Due to the rarity and heterogeneity of the phenotypes of CHD, multicenter registry-based studies are required. The CHD-Biobank was established in 2009 with the aim to collect DNA from patients and their parents （trios） or fi＇om affected families, as well as cardiovascular tissues from patients undergoing corrective heart surgery for cardiovascular malformations. Clinical/phenotype data are matched to the International Paediatric and Congenital Cardiac Code （IPCCC） and the International Statistical Classification of Diseases and Related Health Problems 10th Revision （ICD-10）. The DNA collection currently comprises samples from approximately 4200 participants with a wide range of CHD phenotypes. The collection covers about 430 trios and 120 families with more than one affected member. The cardiac tissue collection comprises 1143 tissue samples [Yore 556 patients alter open heart surgery. The CHD-Biobank provides a comprehensive basis for research in the field of CHD with high standards of data privacy, IT management, and sample logistics.

The development of aircraft engine building and necessity for regular increase of the characteristics of superalloys for gas turbine engine blades require the improvement of the existing processes and the development of the advanced technological processes for the production of the turbine *** our country and abroad DS process is widely used for the production of the gas turbine engine blades. This process permitted to eliminate the lateral grain boundaries in the turbine blade body, which increased long-term strength of *** development of DS technology enabled SC casting of the turbine blades which permits to solve a problem of monocrystal strength completely and to improve fatigue properties of superalloys *** Bridgeman's method is widely used for the production of the turbine *** to this method a bottomless ceramic shell mounted on a water-cooled crystallizer is moved at the required rate from the heating zone and,as a result, DS blades are *** method permits to obtain SC blades using the selection or the seeds. In Russia this method is used in laboratory *** method furnaces (PMP type) and LMC furnace(UVNK-8P) are used for commercial production in *** PMP furnaces the DS process is carried out by horizontal movement of the ceramic shells in the heating zone done by descending along the height. Development and validation of a score predicting mortality for older patients with mitral regurgitation

OBJECTIVE To develop and validate a user-friendly risk score for older mitral regurgitation(MR)patients,referred to as the Elder-MR *** The China Senile Valvular Heart Disease(China-DVD)Cohort Study functioned as the development cohort,while the China Valvular Heart Disease(China-VHD)Study was employed for external *** included patients aged 60 years and above receiving medical treatment for moderate or severe MR(2274 patients in the development cohort and 1929 patients in the validation cohort).Candidate predictors were chosen using Cox’s proportional hazards model and stepwise selection with Akaike’s information *** Eight predictors were identified:age≥75 years,body mass index<20 kg/m^(2),NYHA class Ⅲ/Ⅳ,secondary MR,anemia,estimated glomerular filtration rate<60 mL/min per 1.73 m^(2),albumin<35 g/L,and left ventricular ejection fraction<60%.The model displayed satisfactory performance in predicting one-year mortality in both the development cohort(C-statistic=0.73,95%CI:0.69-0.77,Brier score=0.06)and the validation cohort(C-statistic=0.73,95%CI:0.68-0.78,Brier score=0.06).The Elder-MR score ranges from 0 to 15 *** a one-year follow-up,each point increase in the Elder-MR score represents a 1.27-fold risk of death(HR=1.27,95%CI:1.21-1.34,P<0.001)in the development cohort and a 1.24-fold risk of death(HR=1.24,95%CI:1.17-1.30,P<0.001)in the validation *** to EuroSCORE II,the Elder-MR score demonstrated superior predictive accuracy for one-year mortality in the validation cohort(C-statistic=0.71 vs.0.70,net reclassification improvement=0.320,P<0.01;integrated discrimination improvement=0.029,P<0.01).CONCLUSIONS The Elder-MR score may serve as an effective risk stratification tool to assist clinical decision-making in older MR patients.

减轻组织的机械性压力在治愈糖尿病相关的足溃疡所需的多种干预措施中最为重要。本文是2023年国际糖尿病足工作组(IWGDF)发布的减压治疗促进糖尿病相关的足溃疡愈合的循证指南,也是对2019年IWGDF指南的更新。我们遵循推荐、评估、发展和评价分级(GRADE)方法学,以患者-干预-比较-结局(patient-intervention-control-outcome,PICO)模式设计临床问题和重要结果,进行系统评价和Meta分析,制定评判表汇总,为每个问题编写推荐和理由。每项推荐都是基于系统综述中发现的证据,在无证据情况下则采用专家意见,以及对判断项目的GRADE等级总结进行仔细权衡,包括可预期和不可预期的影响、证据的确定性、患者价值、所需资源、成本效益、公平性、可行性和可接受性。为了治愈糖尿病患者神经性前足底或中足底溃疡,要使用不可拆卸的齐膝高减压装置作为首选减压干预措施。如果患者存在对不可拆卸减压装置的禁忌证或不能耐受,考虑使用可拆卸齐膝(或踝)高减压装置作为减压干预措施的第二选择。如果无可用的减压装置,考虑使用合适的鞋具结合毡制泡沫作为减压干预措施的第三选择。如果这种非手术减压治疗不能治愈前足底溃疡,可以考虑跟腱延长术、跖骨头切除术、关节置换术或跖骨截骨术。为了治疗继发于屈趾畸形的神经性足底或小趾远端溃疡,可行趾屈肌腱切开术。为了促进后足、非足底或并发感染或缺血的溃疡的愈合,提出了进一步的推荐。所有推荐都已在减压临床路径中进行了总结,以帮助促进本指南在临床实践中的实施。这些减压推荐将帮助医务人员为糖尿病相关的足溃疡患者提供最佳的治疗和预后,降低患者感染、住院和截肢的风险。
