Lung Ultrasound Protocol for Diagnosis of 2019-nCoV Pneumonia and Remote Consultation( 1st Edition)

Since December 2019, the emergence of a new type of coronavirus (SARA-CoV-2) pneumonia (COVID-19) in Wuhan, Hubei Province, has become a national public health emergency. The disease diagnosis is currently confirmed to be based on nucleic acid test and high-resolution CT (High Resolution CT, HRCT). Lung ultrasound (LUS) has been used in clinical practice in the differential diagnosis of lung exudative inflammation lesions, and in the evaluation of thoracic effusion, pneumothorax and the evaluation of patients with dyspnea and acute respiratory failure, as well as in the treatment monitoring to guide the treatment more efficiently. The clinical LUS application has been formed a number of domestic and foreign expert consensus. As a fast, convenient, radiation-free and easy-to-implement lung disease visualization technology at the bedside, LUS plays a role in the treatment of critically ill patients of COVID-19 . In view of the problems of LUS performance in different lung diseases, LUS has not been widely used in front-line clinic and ICU and needs to be fully mastered by clinicians in routine practice. Starting from the increasing number of severe patients with COVID-19 pneumonia, the urgent need for immediate dynamic and effective image monitoring of lung lesions in severe patients bedside and the actual clinical needs of the accurate disease changes assessment and the evaluation of treatment timely, in line with the principle of ""problem-oriented, information technology support, model innovation"", the integration of various resources, and put forward a targeted short-term can achieve a wide range of efficient applications of LUS technology of the COVID-19 pneumonia, an enhenced remote diagnosis consultation platform should be established. To promote the clinical application of LUS in the routine diagnosis and treatment of COVID-19 pneumonia in various regions, it is recommended to use the 5G remote consultation platform to achieve cross-institutional, cross-regio

新型冠状病毒感染后间质性肺炎患者肺移植受体评估和术后管理中国专家共识

自2019年12月新型冠状病毒肺炎（coronavirus disease 2019,COVID-19）大流行开始，中国感染的人群已超过85%，新型冠状病毒（severe acute respiratory syndrome coronavirus 2,SARS-CoV-2）最主要累及呼吸系统，尤其是肺，重症感染患者死亡率高，6%～10%患者最终会发展为严重COVID-19相关的急性呼吸窘迫综合征（COVID-related acute respiratory distress syndrome,CARDS），需要机械通气和体外膜肺氧合支持，部分急性肺损伤幸存患者后续会出现持续的COVID-19后肺纤维化（post COVID-19 pulmonary fibrosis,PCPF）。充分治疗后的CARDS及严重的PCPF都是适宜的肺移植候选者，由于感染病程较为特殊，故需要采用不同于普通终末期肺病患者的筛查评估策略。COVID-19患者肺移植术后需要特殊的监测及治疗，包括规范进行SARS-CoV-2核酸检测、合理使用免疫抑制药物及规范使用抗病毒药物，这些均使肺移植术后管理面临较大的考验。本共识基于国内外大量关于SARS-CoV-2感染、治疗及肺移植手术的循证医学证据，并经过专家多次研讨达成一致意见，为SARSCoV-2感染后间质性肺炎患者的肺移植评估及术后管理提供参考。

It has been found that some patients with coronavirus disease 2019(COVID-19)have clinically relevant heart injuries,and the incidence of myocarditis among patients with COVID-19 was reported to be 12%in hospitals and 32%in intensive care units in Wuhan,China.[1]Acute myocardial injuries or myocarditis is a very important clinical issue due to the risk of rapid disease progression to severe arrhythmia,heart failure,cardiac shock,etc.[2,3]The bedside echocardiography examination is the first-line preferential imaging option for the evaluation of myocarditis and other complications,such as acute right heart failure specially due to pulmonary artery embolism,stress cardiomyopathy,acute myocardial infarction and coexisting fundamental chronic cardiovascular disease.

电视胸腔镜下胸交感神经切断术（endoscopic thoracic sympathictomy/sympathotomy,ETS）是目前国内外治疗手汗症效果最佳的微创手术。其在我国胸外科已广泛开展，而且越来越多的医疗机构将ETS列为日间手术，但目前国内尚无ETS日间手术医疗质量控制的指南或共识。为此，中国医师协会胸外科医师分会手汗症学组、吴阶平医学基金会交感神经外科专家委员会和福建省海峡医药卫生交流协会手汗症专业委员会组织国内数十位专家，以国内外文献为主要依据，经反复磋商和充分讨论，形成《手汗症ETS微创技术优化与日间手术质量控制中国专家共识》。旨在为我国胸外科同行手汗症临床诊疗提供参考，以提升其管理水平和工作效能，进而实现规范化质量控制。

Interpretive Criteria of Grated Exercise Test Results (Draft)

Now the exercise test is not simply a method to determine the likelihood of CAD, but to cstimate prognosis, instruct therapy even more. It is used for assessing the functional severity of CAD and the prognosis or potential risk for future cardiac events, determining exercise prescription and effects of therapy for rehabilitative cardiology. In order to meet the needs of reference for cardiovascular rehabilitation medicine, formulate this interpretive criteria in accordance with progress recently in grated exercise test.
