Susceptibility-Weighted mRI in the Evaluation of Gynecologic Diseases

Introduction: Owing to the advanced development of mRI science, it causes obvious great changes of many diseases that affect the female genital system and affect their fertility. Hemorrhagic gynecological diseases especially endometriosis affect young females and cause cyclic pain, in addition to infertility. So early detection is essential for proper treatment. Susceptibility-weighted (SWI) as one of the most recent newly created mRI sequences is highly sensitive to detect products of hemorrhage within different gynecologic disorders with 94.7% sensitivity being more meticulous than conventional mRI sequences as T1 and T2. Aim of the Work: A comparison between T1 and T2 as conventional mRI sequences with susceptibility-weighted images (SWI) in many gynecologic disorders by the detection of the presence of internal products of hemorrhage at any stage. Subjects and methods: 48 consecutive patients from Benha University clinics (age range, 17 - 60 years;mean age, 35.67 years). The patients included in the study were presenting with pelvic pain, irregular menses, Dyspareunia, and swelling. All with suspicious diagnosis of ovarian and extra-ovarian lesions. 38 patients out of the 48 patients were known to contain hemorrhagic disorder;all the patients underwent mRI routine pelvis protocol adding SWI sequence. Results: There was a greatly significant difference between SWI and conventional mRI sequences T1and T2 with sensitivity 94.7%, 57.9% and 33.3% respectively. Conclusion: SWI is a promising tool in the evaluation of hemorrhagic foci within different gynecological disorders. The great ability of detecting hemosiderin foci increases the value of SWI over conventional mRI or US.

Hypofractionated versus Conventionally Fractionated Radiotherapy in Post-mastectomy Breast Cancer Patients

Background & Objective: Hypofractionation has been used in curative setting in breast conservative surgery, but still no adequate information about its application in the adjuvant setting after mastectomy in breast cancer patients. The aim of this trial was to assess the efficacy and toxicity of hypofractionation radiotherapy (40 Gy in 15 fractions) in post mastectomy breast cancer patients and to compare these results with those of post mastectomy patients treated retrospectively by conventional radiotherapy (50 Gy in 25 fractions) as regard overall survival (OS), disease free survival (DFS), locoregional disease free survival (LDFS), and toxicities. Patients & methods: One hundred post mastectomy breast cancer patients were included into this study, they were divided into 2 groups, the 1st included 50 patients treated prospectively with hypofractionated radiotherapy regimen (40 Gy in 15 fractions), and the 2nd (control group) included 50 patients treated retrospectively with conventionally fractionated radiotherapy regimen (50 Gy in 25 fractions). Results: The 2 year overall survival were 96% & 94% respectively (p = 0.7), while the disease free survival were 91% & 89.8%, respectively (p = 0.9), and the LDFS were 95.8% & 93.3%, respectively (p = 0.9), G1 acute dermatitis was observed in 22 (44%) & 25 (50%) patients in group I & II respectively, G2 in 8 (16%) & 10 (20%) patients respectively, no G4 skin toxicity was detected. Radiation pneumonitis was observed in 2 patients (4%) only in group II. Conclusion: post-mastectomy hypofractionated radiation therapy achieved comparable survival and toxicity to the conventionally fractionated radiotherapy with the advantage of reducing overall treatment time, treatment burden & cost.

In this work, an extended Jacobian elliptic function expansion method is proposed for constructing the exact solutions of nonlinear evolution equations. The validity and reliability of the method are tested by its applications to the system of shallow water wave equations and modified Liouville equation which play an important role in mathematical physics.

Background: The emergence of carbapenemase producing Acinetobacter baumannii is increasingly reported nowadays and constitutes a major problem to the intensive care unit (ICU) patients with notable extensive-drug resistance ability. The study investigates carbapenemase producing A. baumannii strains exhibiting an extensively drug-resistant (XDR) phenotype, isolated from ICU patients in Khartoum. methods: A total of 100 nonduplicate Gram-negative coccobacilli strains were obtained from microbiology laboratory of ICU patients’ clinical isolates. molecular identification of A. baumannii was performed by targeting 16S rRNA gene using specifically designed primers. Then, XDR strains were determined by susceptibility testing (disc diffusion). For detection of carbapenemase genes Polymerase chain reaction (PCR) was carried out. Result: Of 100 ICU clinical isolates, 38 (38.0%) was confirmed A. baumannii strains, those strains showed 100% carbapenem resistance and 60.5% extensive drug resistance to the antibiotics tested. The frequency of carbapenemase producer was 57.9% (22/38) of carbapenem resistance A. baumannii (CRAB). The most common carbapenemase associated with resistance was blaOXA gene followed by blaNDm and blaGES A. baumannii isolates. The co-occurrence of blaOXA-48-like and blaNDm, blaOXA-23-like and blaOXA-51, and blaNDm-1 and blaOXA-51 was detected in 22.7%, 18.2% strains and 4.5% respectively. A unique characteristic of our findings was the coharbouring of the genes blaNDm-1, blaOXA-23-like, blaOXA-51 and blaOXA-143 in 9.1% strains (2/22), and this was the first report in the Khartoum city, Sudan. Conclusion: We have demonstrated for the first time a high prevalence of XDR-carbapenemase producing A. baumannii clinical isolates from ICU patients in Khartoum. Also an emergent blaOXA-143 was reported as High-Risk Clones. This highlights the routine mentoring of XDR-carbapenemase producing A. baumannii to avoid clone dissemination in our region hospitals.

Effect of Lion’s Foot (Alchemilla vulgaris) on Liver and Renal Functions in Rats Induced by CCl4

The present work aims to study the influence of antioxidants activity of lion’s foot (Alchemilla vulgaris) leaves at different concentrations to give more protection against chronic liver disease. Results indicated that dried lion’s foot leaves had rich in total polyphenolic and flavonoids content (395.65 and 183.10 mg/100g, respectively). These results were reflected to the antioxidant activity (DPPH);it’s noticed that the antioxidant activity of dried lion’s foot leaves was high (131.74%). The major polyphenolic components were benzoic acid (1084.63 ppm) followed by ellagic acid, catechol, and catechin (614.16, 580.54, and 566.53 ppm, respectively) then salicylic acid and protocatechuic acid (479.71 and 444.43 ppm, respectively). On the same trend, flavonoids fractions indicated the highest content in luteo-6-arabinase 8-glucose, apig. 6-rhamnase 8-glucose, acatein, narengin and luteolin (40.01;15.04;8.07;6.64 and 6.42 ppm, respectively). Fifty-six male albino rats were used in biological experiments. Rats fed on basal diet for two weeks before the performance of the experiment. At the beginning, rats divided into eight main group were fed on diets for 45 days as follows: Negative control group (first group) was fed on basal diet. Forty nine rats were fed on basal diet and induced by CCl4, in paraffin oil (50% v/v, 2 ml/Kg) twice weeks subcutaneous injection to induce chronic damage in the liver, then divided into 7 groups numbered from group 2 to group 8. Positive control group rats fed on basal diet till final experiment (second group). Group 3 and 4 rats treated with 50 and 100 ppm ethanolic leaves extracts, respectively. Also, group 5 and 6 treated with 50 and 100 ppm aqueous leaves extracts, respectively. All extracts were fed on orally every day. While, rats in group 7 treated with 1% and 2% dried lion’s foot leaves. At the end of the experimental period, serums were collected to determine liver and renal functions. Hartree-Fock Calculations of 16O and 40Ca Nuclei Using Fish-Bone Potential

Hartree-Fock calculations are carried out to describe some properties of 16O and 40Ca nuclei using the two forms of fish-bone potential (I and II). A computer simulation search program has been introduced to solve this problem. The Hilbert space was restricted to three dimensional variational space spanned by single spherical harmonic oscillator orbits. Binding energies, root mean square radii and form factors are found to have a good fit with experimental data.

A Validated High-Performance Thin-Layer Chromatography (Hptlc) method for the Quantitative Determination of Tricin in Two SpergulariaSpecies

A simple, sensitive, precise and robust high-performance thin-layer chromatographic (HPTLC) method for analysis of tricin has been developed and validated for the determination of tricin in genus Spergularia. Analysis of tricin was performed on pre-coated TLC aluminum plates with silica gel as the stationary phase. Linear ascending development was carried out in double twin trough glass chamber saturated with a mobile phase consisting of chloroform: methanol (37:3 v/v). Spectro-densitometric scanning was performed by TLC scanner III (CAmAG) in absorbance mode at the wavelength of 270 nm. The system was found to give compact spots for tricin (Rf value of 0.34 ± 0.04). The linear regression analysis data for the calibration plots showed a good linear relationship with r2 = 0.9981 in the concentration range 2-15 μl with respect to peak area. According to the International Conference on Harmonization (ICH) guidelines, the method was validated for precision, recovery, and robustness. The tricin content quantified from Spergularia marina and S. daiandra was found to be 0.398 mg/100g and 1.642 mg/100g fresh weight, respectively. Statistical analysis of the data showed that the method is reproducible and selective for the estimation of tricin.
