Scenario-Based, Single Best, Multiple-Choice Questions (SB-SB-MCQs) in Basic Medical Sciences: An Exploratory Study about the Staff Awareness, Knowledge and Difficulties Encountered

Background: Multiple Choice Questions (MCQs) are well known and widely used assessment tool. They can be used to measure the different levels of educational outcomes: knowledge, understanding, judgment and problem solving. Traditional MCQs (stand-alone MCQs) often are used as a tool for facts recall. Advantages of using Scenario-based questions (SBQs) include more focus on learning objectives and ability to assess higher levels of learning. Shifting to scenario-based questions can increase the level of difficulty and measure higher levels of cognition. Purpose: This study explores the current knowledge and overall awareness of the undergraduate teaching staff about the use and difficulties of scenario-based single-best answer, multiple-choice questions (SB-SBA-MCQs) in assessments of the basic medical sciences. Method: We used an e-Likert scale questionnaire to explore this issue. The questionnaire covered the current knowledge, experience of staff in writing (SB-SBA-MCQs), courses or postgraduate degrees they attended and difficulties they face or anticipate in writing (SB-SBA-MCQs). Results: The majority (86%) are familiar with courses or workshops related to MCQs writing and assessment in general, a small minority have not attended any. The majority (86%) had some experience in writing MCQs. Only, a small percentage have not tried writing this type of MCQs. Nearly 60% think it takes time to construct, the majority (96%) of those researched are in support of shifting to scenario-based MCQs in basic medical sciences. Conclusion: The study has shown most of the teachers of basic medical sciences are aware of and with good knowledge in (SB-SBA-MCQs). It also highlighted the importance and need for regular training courses and workshops on the same.

高压直流输电是一种成熟的技术,主要用于远距离传输大容量电力,连接异步网络或海底电缆系统。由于其与现有系统的互连和新技术的应用,电力系统的复杂性逐渐增加,同时,许多经济和其他限制因素迫使电力公司在最大稳定性极限附近运行其系统,并以最低的成本提供可靠和清洁的电力[1]。电力需求的持续增长迫使需要远距离、大容量的高压直流系统进行电力传输。此外,人们越来越有兴趣将可再生能源纳入电网,再次依赖高压直流输电链路。因此,高压直流输电技术的应用是克服这些问题的必要手段。基于线路换相换流器的高压直流（LCC-HVDC）已经证明了它们在远距离传输大容量电力方面的有效性。LCC-HVDC的主要缺点之一是换相故障发生的风险很高。换相故障主要发生在逆变器侧,由于交流系统干扰或控制故障。当发生换向失败故障时,可能会威胁到系统的稳定性,因为它会导致传输功率的扰动冲击。此外,它还会增加逆变器的无功功耗,这在弱交流系统的情况下是不可取的。为了限制换相失败的影响,提出了几种方法。无论是在改进一次设备方面,还是在改进控制系统方面。改进控制系统是采用最多的,因为它具有可行性,并且可靠性高。在通过改进控制系统来预防换相故障时,有两种主要方法可用于提高系统性能,无论是通过修改触发角,还是使用单独的控制系统来控制熄弧角。但是,为了能够制定有效的预防策略,应充分利用控制系统,以避免应用预防策略和增加系统的无功功率,这是不希望的。尽管不对称故障是换相失败的主要原因[1],但迄今为止,用于分析和预防LCC-HVDC换相故障的模型并没有准确地解决相位移问题,无论是在检测策略方面还是在换流阀级控制下发出触发角。这将限制对换相失败故障的检测和预防。在本论文中,提出了有效的策略来有效地检测和防止换相故障。本论文的贡献可以总结如下:（1）提出一个准确的模型:在不对称故障期间,系统将发生相位移。在本论文中,开发了一个基于序分量的模型,以获取系统上可能发生的不同相位移。相位移尤其会影响系统的触发脉冲。如果在晶闸管不正向偏置时施加触发脉冲,则会发生换相故障。因此,需要随时检测系统的相位角,以有效地控制系统。（2）提出强化策略:基于提出的精确模型,开发有效利用控制系统的有效方法。为了提高PLL的性能,并能够在晶闸管正向偏置时提供触发脉冲,使用提出的数学模型提供电压,并使用基于Tan-Sun的PLL来提供触发脉冲。通过使用这种方法,即使故障持续一段时间,也可以有效地防止连续换相失败故障,而无需增加换相面积。为了验证所提出的抑制换相失败的方法,使用PSCAD/EMTDC进行了仿真。在逆变器终端施加不同类型的故障,以研究系统性能。故障电感在较大的范围内变化,以验证所提出的方法。此外,还考虑了不同强度的系统,以确保该方法在不同条件下的有效运行。

Performance investigation of LAMBDA and bootstrapping methods for PPP narrow-lane ambiguity resolution

Precise point positioning with ambiguity resolution(PPP-AR)is a powerful tool for geodetic and time-constrained applications that require high *** performance of PPP-AR highly depends on the reliability of the correct integer carrier-phase ambiguity *** this study,the performance of narrow-lane ambiguity resolution of PPP using the Least-squares AMBiguity Decorrelation(LAMBDA)and bootstrapping methods is extensively investigated using real data from 55 IGS stations over one-month in *** PPP with 24-,12-,8-,4-,2-,1-and½-h sessions using two different cutoff angles(7°and 30°)was conducted with three PPP modes:***-float and two kinds of ambiguity-fixed PPP using the LAMBDA and bootstrapping methods for narrow-lane AR,*** results show that the LAMBDA method can produce more reliable results for 2 hour and shorter observation sessions com-pared with the bootstrapping method using a 7°cutoff *** a 30°cutoff angle,the LAMBDA method outperforms the bootstrapping method for observation sessions of 4 h and *** long observation times,the bootstrapping method produced much more accurate coordinates compared with the LAMBDA method without considering the wrong fixes *** results also show that occurrences of fixing the wrong integer ambiguities using the bootstrapping method are higher than that of the LAMBDA method.

Risk factors for fever and sepsis after percutaneous nephrolithotomy

Objective:Percutaneous nephrolithotomy(PCNL)is commonly used in the management of large renal *** infections are one of the most common complications of this *** present study is to determine and assess the factors that may increase the risk to develop fever and urinary sepsis after ***:A total of 60 patients(38 males and 22 females)with a mean age of 40.25 years enrolled in this study in Sulaimania Teaching *** had renal stone disease need operation with different socioeconomic status,body mass index and different type and size of stones were included in this *** with preoperative positive urine culture and sensitivity were *** investigations done for all *** Patients received prophylactic antibiotic gentamicin intravenously at the induction of *** pelvis urine sample were taken from all patients after puncturing the pelvicalyceal system and send for culture and *** were monitored closely in the postoperative period for the development of fever and ***:Mean duration of the operations was 77.08 min ranged 40e120 *** patients had postoperative nephrostomy ***(28.33%)patients developed post PCNL fever and the statistically significant factors for post PCNL fever were diabetes mellitus(DM)(p Z 0.001),stone burden(p Z 0.001),number of the stones(p<0.001),degree of hydronephrosis(p Z 0.001),duration of the operation(p<0.001),residual stones(p Z 0.001)and number of tracts(p Z 0.038).Three(5.00%)patients developed post PCNL sepsis,and the statistically significant risk factors for post PCNL sepsis were duration of the operation(p Z 0.013)and intraoperative blood loss,postoperative drop in haemoglobin(HB)level(p Z 0.046).Conclusion:DM,staghorn stones,degree of hydronephrosis,duration of the operation and number of tracts are risk factors for post PCNL fever,while number of stones,intraoperative blood loss,duration of the operation

The fast-paced growth of artificial intelligence applications provides unparalleled opportunities to improve the efficiency of various *** as the transportation sector faces many obstacles following the implementation and integration of different vehicular and environmental aspects *** congestion is among the major issues in this regard which demands serious attention due to the rapid growth in the number of vehicles on the *** address this overwhelming problem,in this article,a cloudbased intelligent road traffic congestion prediction model is proposed that is empowered with a hybrid Neuro-Fuzzy *** aim of the study is to reduce the delay in the queues,the vehicles experience at different road junctions across the *** proposed model also intended to help the automated traffic control systems by minimizing the congestion particularly in a smart city environment where observational data is obtained from various implanted Internet of Things(IoT)sensors across the *** due preprocessing over the cloud server,the proposed approach makes use of this data by incorporating the neuro-fuzzy ***,it possesses a high level of accuracy by means of intelligent decision making with minimum error *** results reveal the accuracy of the proposed model as 98.72%during the validation phase in contrast to the highest accuracies achieved by state-of-the-art techniques in the literature such as 90.6%,95.84%,97.56%and 98.03%,*** far as the training phase analysis is concerned,the proposed scheme exhibits 99.214% accuracy. The proposed prediction modelis a potential contribution towards smart cities environment.

Radon Dosimetry in Erbil City Hospitals

The aim of the study was to compare radon concentrations and effective dose equivalent in state hospitals inside the Erbil city having different periods of building. Effective doses were estimated in different floors of the five selected hospitals by using the (CR-39) solid state nuclear track detectors with the guide of ICRP Publication 66. The hanged dosimeters were left for about 60 days and after etching the radon concentration, annual effective dose and effective dose equivalents were calculated. Annual effective doses were estimated as < 1 mSv, so the results reveal the present concentrations to be below the standard values in the studied hospitals. Therefore, no healthy risks concerning the radon activities are existing in the hospital staff and patients in these hospitals.

Sonographic Evaluation of Obstructive Jaundice

Purpose: This study was done to assess the role of ultrasound in diagnosis of obstructive jaundice and to identify the most common causes and symptoms. Materials and methods: The study took place in different Khartoum clinical centers and hospitals in the period between Jan 2014 and July 2014. A total of 150 patients with obstructive jaundice were examined using ultrasound machine, 3.5 - 7 MHz probe. Results: The study revealed that 65.33 of the study population with obstructive jaundice were female and 34.67% was male. The most common affected groups were 46 - 65 years old and 25 - 45 years old. The common causes of obstructive jaundice were biliary stones (61.33%) which were common in females and abdominal masses (32.67%) which were common in females, too. Nausea, yellowish discoloration and abdominal pain were the most common symptoms (98%, 97.33% and 96.67% respectively). The causes of obstructive jaundice showed significant difference between males and females (P-value = 0.002). Conclusion: Ultrasound was superior diagnostic tool in detecting and assessing biliary system obstruction, because it was easy, available, accurate and noninvasive. The ageing and gender were considered as risk factors of obstructive jaundice.

Objective: Acute kidney disease develops 16% - 30% of patients with preoperative impaired renal functions. Our aim of this study was to compare postoperative renal outcomes using two open heart surgery techniques, on-pump beating heart (OPBH) and conventional on-pump (COP), in patients who have preoperative low glomerular filtration rate (eGFR) as an indicator of creatinine clearance. Methods: From 2004 to 2015, 341 patients with preoperative creatinine clearance were lower than 90 ml/min/1.73m2 were selected for this study. On-Pump beating heart was performed in 111 patients (Group I). Conventional on-pump technique was used in 200 patients. In the remaining patients, we measured postoperative Tumor Necrosis Factor Alpha (TNF-alpha), cardiac troponin I (cTn-I), Brain natriuretic peptid (NT-Pro- BNP), creati-nine (Cr), blood urea nitrogen (BUN) and postoperative eGFR daily until day four after surgery. Results: There were no differences in baseline levels of TNF-alpha, NT-Pro-BNP, BUN, cTn-I, Cr levels between the groups. Cardiopulmonary bypass (CPB) time were much longer and cumulative inotrope use was significantly higher in patients underwent COP (P Conclusion: Our study showed that on-pump beating heart CABG technique provides a morbidity benefit and is associated with a lower risk of peroperative LOS in patients with mild renal disfunction. Our study results suggest that OPBH is superior to the COP in terms of postoperative renal injury and cardiac mediators.

The aquafeed industry continues to expand in response to the rapidly growing aquaculture ***,the identification of alternative protein sources in aquatic animal diets to replace conventional sources due to cost and sustainability issues remains a major ***,insects have shown tremendous results as potential replacers of fishmeal in *** present study aimed to review the utilization of insects in aquafeeds and their effects on aquatic animals’growth and feed utilization,immune response and disease resistance,and fish flesh quality and *** many insect species have been investigated in aquaculture,the black soldier fly(Hermetia illucens),and the mealworm(Tenebrio molitor)are the most studied and most promising insects to replace fishmeal in ***,insect rearing conditions and biomass processing methods may affect the product’s nutritional composition,digestibility,shelf life and required insect inclusion level by aquatic ***,insect-recommended inclusion levels for aquatic animals vary depending on the insect species used,biomass processing method,and test ***,while an appropriate inclusion level of insects in aquafeed provides several nutritional and health benefits to aquatic animals,more studies are needed to establish optimum requirements levels for different aquaculture species at different stages of development and under different culture systems.
