Implants made of magnesium(Mg)are increasingly employed in patients to achieve osteosynthesis while degrading in *** Mg implants and Mg^(2+)have been suggested to possess anti-inflammatory properties,the clinically observed soft tissue inflammation around Mg implants is ***,using a rat soft tissue model and a 1-28 d observation period,we determined the temporo-spatial cell distribution and behavior in relation to sequential changes of pure Mg implant surface properties and Mg^(2+)*** to nondegradable titanium(Ti)implants,Mg degradation exacerbated initial *** of Mg degradation products at the tissue-implant interface,culminating at 3 d,actively initiated chemotaxis and upregulated mRNA and protein immunomodulatory markers,particularly inducible nitric oxide synthase and toll-like receptor-4 up to 6 d,yet without a cytotoxic *** vascularization was demonstrated morphologically,preceded by high expression of vascular endothelial growth *** transition to appropriate tissue repair coincided with implant surface enrichment of Ca and P and reduced peri-implant Mg^(2+)*** implants revealed a thinner fibrous encapsulation compared with *** detailed understanding of the relationship between Mg material properties and the spatial and time-resolved cellular processes provides a basis for the interpretation of clinical observations and future tailoring of Mg implants.

Evaluation of the 8th AjCC staging system for pathologically versus clinically staged pancreatic adenocarcinoma: A time to revisit a dogma?

Background: The 8 th edition of the American joint Committee on Cancer(AjCC) staging system for pancreatic exocrine adenocarcinoma has been released. The current study seeks to assess the 7 th and 8 th editions among patients registered within the surveillance, epidemiology and end results(SEER) ***: SEER database(2010–2013) has been accessed through SEER*Stat program and AjCC 8 th edition stages were reconstructed utilizing the collaborative stage descriptions. Kaplan–Meier analysis of overall survival and pancreatic cancer-specific survival analyses(according to both 7 th and 8 th editions and according to whether pathological or clinical staging were conducted) has been performed. Multivariate analysis of factors affecting pancreatic cancer-specific survival was also conducted through a Cox proportional hazard ***: A total of 18 948 patients with pancreatic adenocarcinoma were identified in the period from 2010–2013. Pancreatic cancer-specific survival among pathologically staged patients and according to the 8 th edition showed significant differences for all pair wise comparisons among different stages(P < 0.0001) except for the comparison between stage IA and stage IB(P = 0.307) and the comparison between stage IB and stage IIA(P = 0.116). Moreover, P value for stage IA vs IIA was 0.014; while pancreatic cancer-specific survival according to the 7 th edition among pathologically staged patients showed significant differences for all pair wise comparisons among different stages(P < 0.0001) except for the comparison between IA and IB(P = 0.072), the comparison between stage IIA and stage IIB(P = 0.065), the comparison between stage IIA and stage Ⅲ(P = 0.059) and the comparison between IIB and Ⅲ(P = 0.595).Among clinically staged patients(i.e. those who did not undergo initial radical surgery), the prognostic performance of both 7 th and 8 th stages for both overall survival and pancreatic cancer-specific survival was ***: Th

Cover crops have long been proposed as an alternative soil management for minimizing erosion rates in olive stands while providing additional ecosystem ***,the trade-off between these benefits and the competition for water with the trees makes the definition of optimal management practices a challenging task in semiarid *** work presents an improved version of OliveCan,a process-based simulation model of olive orchards that now can simulate the main impacts of cover crops on the water and carbon balances of olive *** simple in its formulation,the new model components were developed to deal with different cover crop management *** are presented for simulation runs of a traditional olive orchard in the conditions of southern Spain,evaluating the effects of different widths for the strip occupied by the cover crop(Fcc)and two contrasting mowing *** revealed that high Fccresulted in lower olive yields,but only when mowing was applied at the end of *** this regard,late mowing and high Fccwas associated with lower soil water content from spring to summer,coinciding with olive flowering and the earlier stages of fruit *** also negatively correlated with surface runoff irrespective of the mowing *** the other hand,net ecosystem productivity(NEP)was substantially affected by both Fccand mowing *** simulations under future climate scenarios comparing the same management alternatives are also presented,showing substantial yield reductions by the end of the century and minor or negligible changes in NEP and seasonal runoff.

Critical assessment of R410A alternatives for mini-split air conditioners in the Egyptian market

In this study,the potential implementation of three different low-GWP refrigerants(R32,R452B,and R454B)as replacements for R410A was *** study was performed using a simulation tool developed by the authors called RACHP-Lab,which is a vapor compression system simulation tool developed based on physics-based simulation for typical mini-split air *** simulation study was carried out and validated using experimental performance data of 10 different air conditioning units available in the Egyptian *** units included fixed-speed or variable-speed compressors and operated in cooling or heating ***-in replace-ment with the new refrigerants was carried *** R32,the capacity increased between 4.9%and 13%for cooling cases,and 6.3%and 12.4%for heating ***,COP did not improve in all *** R452B and R454B with direct replacement,the capacity nearly remained the same,with an increase of COP between 1.6%and 8.0%.Soft optimization was also conducted on cooling cases where compressor suction superheat,condenser subcooling,and compressor volumetric speed were optimized to maximize COP while maintaining the original capacity of R410A.R32 showed an improvement of COP over R410A between 4.6%and 15.5%,while for R452B and R454B between 2.2%and 13.2%.

Pancreaticoduodenectomy:A study from India on the impact of evolution from a low to a high volume unit

AIM To analyse the impact of turning of our department from a low to a high volume provider of pancreaticoduodenectomy(PD) on surgical *** A retrospective collection of data was done for patients who underwent *** to the number of PDs undertaken per year,we categorized the volume into low volume( 25 PDs/year) *** From 2002 to 2013,200 patients underwent *** annual number of PD increased from 4 in 2002 to 34 in *** mean operative time,operative blood loss and need for intraoperative blood transfusion decreased considerably over the volume categories(P < 0.001,P < 0.001 and P < 0.001,respectively).Increased procedural volume was associated with a lower morbidity(P = 0.021) and shorter length of hospital stay(P < 0.001).Similarly the rate of mortality dropped from 10% for the low volume group to 2.2% for the medium volume group and 0.0% for the high volume group(P = 0.007).CONCLUSION The transformation from a low volume to a high volume provider of PD resulted in most favourable outcomes favouring the continued centralization of this high risk procedure.

Dear editor, We report a case of a 47-year-old female who presented with a toxic bupropion ingestion leading to cardiac arrest. She initially exhibited a loss of brainstem reflexes in conjunction with burst-suppression pattern on EEG. Burst suppression is an EEG waveform pattern of alternating isoelectric suppressions and high voltage bursts, Our patient ultimately made a full neurologic recovery a few days later. While there are two other cases in the literature of bupropion overdose resulting in EEG burst-suppression and loss of brainstem reflexes, we believe this is the only reported adult case complicated by cardiac arrest.

Gastric acid inhibitory and gastric protective effects of cannabis and cannabinoids

Cannabis sativa has long been known for its psychotropic effect. Only recently with the discovery of the cannabinoid receptors, their endogenous legends and the enzymes responsible for their synthesis and degradation, the role of this ‘endocannabinoid system’ in different pathophysiologic processes is beginning to be delineated. There is evidence that CB1 receptor stimulation with synthetic cannabinoids or Cannabis sativa extracts rich in ?9-tetrahydrocannabinol inhibit gastric acid secretion in humans and in experimental animals. This is specially seen when gastric acid secretion is stimulated by pentagastrin, carbachol or 2 deoxy-D-glucose. Cannabis and/or cannabinoids protect the gastric mucosa against noxious challenge with non-steroidal anti-inflammatory drugs, ethanol as well as against stress induced mucosal damage. Cannabis/cannabinoids might protect the gastric mucosa by virtue of its antisecretory, antioxidant, anti-inflammatory, and vasodilator properties.

The intermolecular interaction between poly(methyl methacrylate) (PMMA) and polystyrene (PS) intetrahydrofuran (THF) and N,N'-dimethyl formamide (DMF) solvents was studied at 28℃ using a dilute solution viscometrymethod. Solvent is believed to play a key role in characterizing the viscosity behavior of the polymer solution. The intrinsicviscosity and viscosity interaction parameter were experimentally measured for the binary (solvent/polymer) and for theternary systems in two solvents. The compatibility of the polymer mixture was discussed in terms of the sign of △b_m. Theresults show that the compatibility of PMMA/PS blend in DMF is larger than that in THF.

An assessment of the concentrations of particulate polycyclic aromatic hydrocarbons (PAHs) in the aftermath of a chemical store fire incident

PM 10 airborne particles and soot deposit collected after a fire incident at a chemical store were analyzed in order to determine the concentrations of polycyclic aromatic hydrocarbons(PAHs). The samples were extracted with 1∶1 hexane-dichloromethane by ultrasonic agitation. The extracts were then subjected to gas chromatography-mass spectrometric(GC-MS) analysis. The total PAHs concentrations in airborne particles and soot deposit were found to be 3.27±1.55 ng/m 3 and 12.81±24.37 μg/g, respectively. Based on the molecular distributions of PAHs and the interpretation of their diagnostic ratios such as PHEN/(PHEN + ANTH), FLT/(FLT + PYR) and BeP/(BeP + BaP), PAHs in both airborne particles and soot deposit may be inferred to be from the same source. The difference in the value of IP/(IP + BgP) for these samples indicated that benzo[g,h,i]perylene and coronene tend to be attached to finer particles and reside in the air for longer periods. Comparison between the molecular distributions of PAHs and their diagnostic ratios observed in the current study with those reported for urban atmospheric and roadside soil particles revealed that they are of different sources.
