Intraductal Prostatic Carcinoma: Epidemiological and Anatomopathological Aspects in Dakar

Introduction: Intraductal carcinoma is often associated with high-grade, high-stage adenocarcinoma. Its frequency is variable and it is considered a poor prognostic factor. In our context, when prostatic carcinoma is diagnosed, pathologists do not always report the presence of this anatomopathological entity. We therefore conducted a study to determine the epidemiological and anatomopathological profile of patients with this lesion in Dakar. Materials and Methods: This is a retrospective descriptive study covering a 1-year period from January to December 2022. It focused on cases of intraductal carcinoma diagnosed among prostatic carcinomas collected in the anatomopathology laboratories of Hôpital Général Idrissa Pouye (HOGIP) and Hôpital Militaire de Ouakam (HMO). It was based on archives of anatomopathological reports, blocks and slides. A total of 200 cases of prostatic carcinoma were collated and reviewed to identify those presenting with intraductal carcinoma according to the diagnostic criteria of Guo and Epstein. Results: 87 cases of intraductal carcinoma were found, representing 43.5% of prostatic carcinomas. The mean age was 71 years. Patients in their seventh decade were the most represented, i.e. 42.5%. The majority of samples examined were biopsies (72.4%). The mean PSA level was 965.91 ng/ml, with extremes ranging from 0.03 to 10,000 ng/ml. Histologically, 96.5% of cases (N = 84) were invasive prostatic carcinoma. Gleason score 8 (4 + 4) was the most common, accounting for 42.53% (N = 37). On average, the study found four (04) foci of intraductal carcinoma per specimen, with extremes ranging from 1 to 30. Dense cribriform architecture accounted for 78.16%, loose cribriform for 11.5%, solid for 8.04% and micropapillary for 2.3%. Six cases (6.9%) showed foci of comedonecrosis. Management of Acute Bowel Obstruction in Adults in Ziguinchor Hospital Establishments: About 163 Cases Followed Up

Acute intestinal obstruction represents a very frequent admission to surgical emergencies. The aim of our study was to make a descriptive analysis of the management of acute intestinal obstructions at the level of hospital establishments in Ziguinchor. Materials and Methods: This was a retrospective, descriptive, multicenter study from the period of January 1, 2017 to December 31, 2021. Patients over 15 years of age admitted for occlusive syndrome in the two surgical departments of the Ziguinchor regional hospitals were included in the study. Epidemiological, diagnostic, therapeutic and evolutionary data were studied. Results: Acute intestinal occlusions accounted for 8.3% of admissions. A total of 163 patients were enrolled, 61.9% were men, sex ratio of 1.6. The mean age was 48.7 years. The average consultation time was 74.8 hours (3.12 days). The total occlusive syndrome was in 48.5% of the cases. Abdominal pain was present in 87.1%, vomiting in 74.2%, cessation of matter and gas in 60.7% and meteorism in 36.2%. Biological lab tests reported hyperleukocytosis in 28.1% of the cases. Abdominal CT scans were performed in 71.2%, confirming the diagnosis in 90.5% and determining the etiology in 93.1%. The zone of the obstruction in the small bowel was 62.7%, and in the colon in 37.3%. Functional occlusions accounted for 22.7% of cases and mechanical for 77.3%, including 120 cases of strangulation occlusion (95.2%) and 6 cases of obstruction. Etiologies were dominated by adhesions and/or fibrous bands (61.2%). Medical treatment resolved the occlusive syndrome in 9.2% of cases, instrumental treatment in 17.1% and surgical treatment in 77.3%. Adhesiolysis/section of fibrous bands (55.7%), detorsion (14.3%) and resection anastomosis (11.9%) were the most common surgical procedures. Overall morbidity was 11.6%, dominated by surgical site infection (7.14%). The mean resumption of intestinal transit was 2.7 days, and the mean hospital stay was 7.9 days. Overall mortality was

Meckel Diverticulum Carcinoid Tumor Complicated by Ileal Invagination. About One Case at Vichy Hospital

Meckel’s diverticulum is a common pathology in children and rare in adults. We present a case of a 79-year-old patient in which a significant gastrointestinal bleeding, whose paraclinical explorations confirmed a fatty tumor of the terminal ileum. The exploratory coelioscopy revealed a tumor of the Meckel diverticulum complicated of intestinal invagination, resected at the same time with resection and extracorporeal anastomosis by mini coelio-guided laparotomy. Histology confirmed the presence of heterotopical tissue of the duodenal mucosa with Brunner cells and a carcinoid tumor. In conclusion, this clinical case shows that coelioscopy can be considered a very important diagnostic and therapeutic tool in this pathology especially in the elderly.

Results of the Management of Gastrointestinal Stromal Tumors (GIST): About 6 Cases at the Vichy Hospital Center (France)

The aim of our study was to analyze the results of surgical management of gastrointestinal stromal tumors (GIST) at the Vichy Hospital Center. Methodology: Between 2010 and 2020, the data of 6 patients operated at the Vichy Hospital Center for GIST were analyzed. The parameters studied were: age, sex, antecedents, discovery circumstances, imagery, surgical procedure, anatomopathological data, the follow-up and the morbidity-mortality. Results: There were 5 men and one woman with an average age of 72.16 years [58 - 80 years]. The average time of evolution was 8 months (0 - 14 months). The diagnosis was fortuitous in 2 cases. Atypical abdominal pain was the main symptom in 3 cases. One case was received in a clinical picture of peritoneal irritation syndrome. Echo-endoscopy with biopsy specimen histology made it possible to make the diagnosis in 5 cases and the surgical specimen in 1 case. The fusiform type was the predominant histological form. The stomach was the most common location. The average size of GISTs was 7.3 × 4 cm with a positive C Kit in all patients. Neoadjuvant chemotherapy was performed on one patient. Surgery was curative and was done laparoscopically on 4 patients. Adjuvant chemotherapy based on Imatinib at a rate of 400 mg/d in 3 patients was initiated. One patient presented a fistula of the esophagus-jejunal anastomosis on D6 post operation controlled by a drainage and an antibiotic therapy. Mortality was zero. During the surgery, all of the patients were followed up with surgeon, oncologist. Conclusion: GISTs are the most common mesenchymal tumors of the digestive tract with a preferential gastric location. Laparoscopic surgery, with advances in molecular biology and the introduction of targeted therapy has improved the management of these tumors in terms of morbidity and mortality.

Performance Analysis of the Reconfigurable Intelligent Surfaces Communication Systems

Reconfigurable intelligent surfaces (RIS) play a vital role in meeting the growing demand for higher data rates and reliability in wireless systems. This study focuses on analyzing the performance of RIS systems to gain a deeper understanding of their *** paper presents a mathematical analysis of the RIS system,deriving closed-form formulae that express its characteristics including signal-to-noise ratio(SNR)distribution,ergodic and effective capacity,outage,and error probability. The obtained formulae are newly derived and unconditionally valid solutions. Numerical results demonstrate a close agreement between the derived formulae and MonteCarlo simulation outcomes.

The First Cases of Laparoscopic Colectomy for Cancer in Ziguinchor Peace Hospital

Laparoscopic colectomy is a standardized technique for the curative treatment of colon cancer, based on the principles of carcinological surgery. It is rarely performed in our region but common practice in developed countries. We conducted a retrospective study of the first 4 cases of laparoscopic colectomy for cancer. The patients were 3 men and 1 woman, with a mean age of 54.2 years. Of the 4 cancers, 3 were localized to the sigmoid and one to the cecum. The main symptom was rectal bleeding. Total colonoscopy with biopsy was performed in 3 patients and the histological type was a lieberkühnian, infiltrative, moderately differentiated adenocarcinoma. Thoraco-abdomino-pelvic computed tomography (TAP CT) was performed in all the cases. Three patients were presented to the multidisciplinary team (MDT) prior to surgery. The operations were one right hemicolectomy and three left hemicolectomies. The mean operative time was 182.25 min [152 - 210]. Average blood loss was estimated at 200 cc. The average hospital stay was 4.71 days. Cancer staging was classified as stage I in 1 case (p T1N0M0) and stage III in 3 cases (pT3N1Mx, pT3N2Mx). Resection margins were healthy. The number of lymph nodes removed was 12, 14, 17 and 13 respectively. Conclusion: This is a safe and reproducible technique, but requires a much higher cost than laparotomy.

Critical assessment of R410A alternatives for mini-split air conditioners in the Egyptian market

In this study,the potential implementation of three different low-GWP refrigerants(R32,R452B,and R454B)as replacements for R410A was *** study was performed using a simulation tool developed by the authors called RACHP-Lab,which is a vapor compression system simulation tool developed based on physics-based simulation for typical mini-split air *** simulation study was carried out and validated using experimental performance data of 10 different air conditioning units available in the Egyptian *** units included fixed-speed or variable-speed compressors and operated in cooling or heating ***-in replace-ment with the new refrigerants was carried *** R32,the capacity increased between 4.9%and 13%for cooling cases,and 6.3%and 12.4%for heating ***,COP did not improve in all *** R452B and R454B with direct replacement,the capacity nearly remained the same,with an increase of COP between 1.6%and 8.0%.Soft optimization was also conducted on cooling cases where compressor suction superheat,condenser subcooling,and compressor volumetric speed were optimized to maximize COP while maintaining the original capacity of R410A.R32 showed an improvement of COP over R410A between 4.6%and 15.5%,while for R452B and R454B between 2.2%and 13.2%.

Advanced Optimized Anomaly Detection System for IoT Cyberattacks Using Artificial Intelligence

While emerging technologies such as the Internet of Things(IoT)have many benefits,they also pose considerable security challenges that require innovative solutions,including those based on artificial intelligence(AI),given that these techniques are increasingly being used by malicious actors to compromise IoT *** an ample body of research focusing on conventional AI methods exists,there is a paucity of studies related to advanced statistical and optimization approaches aimed at enhancing security *** contribute to this nascent research stream,a novel AI-driven security system denoted as“AI2AI”is presented in this ***2AI employs AI techniques to enhance the performance and optimize security mechanisms within the IoT *** also introduce the Genetic Algorithm Anomaly Detection and Prevention Deep Neural Networks(GAADPSDNN)sys-tem that can be implemented to effectively identify,detect,and prevent cyberattacks targeting IoT ***,this system demonstrates adaptability to both federated and centralized learning environments,accommodating a wide array of IoT *** evaluation of the GAADPSDNN system using the recently complied WUSTL-IIoT and Edge-IIoT datasets underscores its *** an impressive overall accuracy of 98.18%on the Edge-IIoT dataset,the GAADPSDNN outperforms the standard deep neural network(DNN)classifier with 94.11%***,with the proposed enhancements,the accuracy of the unoptimized random forest classifier(80.89%)is improved to 93.51%,while the overall accuracy(98.18%)surpasses the results(93.91%,94.67%,94.94%,and 94.96%)achieved when alternative systems based on diverse optimization techniques and the same dataset are *** proposed optimization techniques increase the effectiveness of the anomaly detection system by efficiently achieving high accuracy and reducing the computational load on IoT devices through the adaptive selection of active features.
