Prospective evaluation of the cause of acute pancreatitis,with special attention to medicines

AIM: To investigate the cause of acute pancreatitis(AP) by conducting a thorough investigation of drugs and their possible etiological ***: We investigated the cause of AP in a large retrospective cohort of 613 adult patients admitted with AP at the Akershus University Hospital, Norway, from 2000 until 2009, who were evaluated with standard ward investigations. This group was compared with a prospectively evaluated group(n = 57) admitted from January 2010 until September 2010 who investigated more extensively using medical history and radiological ***: The groups were comparable with regards to gender, age, comorbidity and severity. The most common etiology was bile stones and alcohol, occurring in 60% in both groups. The prospective group was examined more thoroughly with regards to the use of alcohol and medicines. An increased number of radiological investigations during hospital stay and at follow-up were also performed. A more extensive use of radiological evaluation did not increase the detection frequency of bile stones. In the prospective group, more than half of the patients had two or more possible causes of pancreatitis, being mostly a combination of bile stones and drugs. No possible cause was found in only 3.5% of these patients, compared with 29.7% in the retrospective ***: A detailed medical history and extensive radiological evaluation may determine a possible etiology in almost all cases of AP. Many patients have several possible risk factors, and uncertainty remains in establishing the definitive etiology.

rong>Purpose:rong> To identify clinical predictors for reduced long-term survival and describe the cause of death after surgical treatment for rectal cancer. Methods: A retrospective follow-up study of 442 consecutive, unselected patients treated for rectal cancer at a tertiary centre from 1990 until 2000 and followed for 17 years or until death. Predictors for death were assessed by Cox regression analysis. The cause of death was obtained from the Norwegian Cause of Death Registry. Results: 254 men and 188 women with a median age of 71 years (21 - 95 years) were resected for rectal cancer with low anterior resection (n = 266), abdominoperineal resection (n = 125), Hartmann’s procedure (n = 19) or diverting stoma only (n = 32). Median follow-up was 5 years (0 - 17 years). The relative five-year survival rates for stages I, II, III and IV was 83.9%, 65.2%, 41.1% and 9.3%, respectively. The proportion of deaths due to recurrence from colorectal cancer in stages I, II, III and IV was 23.5%, 55.8%, 72.3% and 98.0%, respectively. rong>Background:rong> Clinical predictors of death and survival in surgical treatment of colon cancer are easily confounded by the modern adjuvant and neo-adjuvant chemotherapy. This study focuses on lethality and survival during implementation of ultra-radical surgery for colonic cancer prior to multimodal therapy. Methods: Retrospective observational follow-up study of 824 consecutive, unselected patients resected for Stage I, II, III and IV colon cancer from 1990 until 2000 at one tertiary centre, with a median follow-up of 45 months (0 - 202 months). Predictors for death were assessed by Cox regression analyses and log-rank test. The cause of death was obtained from the Norwegian Cause of Death Registry. Results: The relative survival rates were 86.3%, 71.9%, 50.3% and 6.6% in Stage I, II, III and IV, respectively. In 28.7% of the patients, the cause of death was other than colorectal cancer recurrence. The adjusted Cox regression model showed that higher age (1.04 (95% CI: 1.03;1.05)), male gender (1.37 (1.14;1.66)), emergency surgery (1.52 (1.21;1.93)), left vs. right hemicolectomy (1.39 (1.03;1.87)), and perioperativerdana;"> blood transfusion (1.25 (1.01;1.55)) were predictors of reduced survival. Long term recurrence,pain and patient satisfaction after ventral hernia mesh repair

AIM: To compare long term outcomes of laparoscopic and open ventral hernia mesh repair with respect to recurrence, pain and ***: We conducted a single-centre follow-up study of 194 consecutive patients after laparoscopic and open ventral hernia mesh repair between March 2000 and June 2010. Of these, 27 patients(13.9%) died and 12(6.2%) failed to attend their follow-up appointment. One hundred and fifty-three(78.9%) patients attended for follow-up and two patients(1.0%) were interviewed by telephone. Of those who attended the follow-up appointment, 82(52.9%) patients had received laparoscopic ventral hernia mesh repair(LVHr) while 73(47.1%) patients had undergone open ventral hernia mesh repair(OVHr), including 11 conversions. The follow-up study included analyses of medical records, clinical interviews, examination of hernia recurrence and assessment of pain using a 100 mm visual analogue scale(VAS) ruler anchored by word descriptors. Overall patient satisfaction was also determined. Patients with signs of recurrence were examined by magnetic resonance imaging or computed tomography ***: Median time from hernia mesh repair to follow-up was 48 and 52 mo after LVHr and OVHr respectively. Overall recurrence rates were 17.1% after LVHr and 23.3% after OVHr. recurrence after LVHr was associated with higher body mass index. Smoking was associated with recurrence after OVHr. Chronic pain(VAS > 30 mm) was reported by 23.5% in the laparoscopic cohort and by 27.8% in the open surgery cohort. recurrence and late complications were predictors of chronic pain after LVHr. Smoking was associated with chronic pain after OVHr. Sixty point five percent were satisfied with the outcome after LVHr and 49.3% after OVHr. Predictors for satisfaction were absence of chronic pain and recurrence. The importance of water to our lives cannot be overemphasized as it is the source of all living things. Water scarcity is a concern in many Middle Eastern nations due to war and politics. Many people lose their essence due to actions made by their rulers, such as the Marsh Arabs. Water and biodiversity should be considered, and governments should be held accountable for attaining a more balanced lifestyle and reducing immigrant levels. Many academics have also explored the hydro-immigration of marshland residents. They suffer more hazards due to a lack of money, poor education, and social instability than their colleagues who remain in their own country. Furthermore, they have insufficient health care and have difficulty integrating into the new community. This implies that not all nations that receive such migrants can implement programs to assist the immigrants. This research reveals how wars, hydro-politics, and immigration issues have drastically affected architecture and social structures in the Arab Marshes region, leading to ecological destruction, mass migrations, and cultural loss. Therefore, this work focuses on the need to critically investigate all factors covered within the context of this research.

Progression of Corneal Astigmatism after Phaco-Alternative at the University Hospital Center of the Oueme-Plateau Department in Porto-Novo

Objective: To study the postoperative evolution of corneal astigmatism following phaco-alternative at the University Hospital Center of Ouémé-Plateau (UHC-OP) in Porto-Novo. Method: This was a prospective, cross-sectional study with a descriptive and analytical purpose conducted from April 19 to September 20, 2021;a period of 6 months. The study included all patients presenting with senile or non-senile cataracts without a history of corneal trauma and who underwent phaco-alternative during the study period. results: A total of 62 eyes underwent phaco-alternative. The average age was 63 ± 12 years with a male predominance of 59.7%. Phaco-alternative was performed with 74.2% linear incision and 25.8% smile incision. Perioperative incidents were mainly capsular ruptures with vitreous loss occurring in 8.1% of cases. The mean preoperative astigmatism was 1.29 ± 1.5 D with an axis of 180˚ ± 20˚, indicating with-the-rule astigmatism. The mean postoperative astigmatism was 2.2 ± 1.5 D with an axis of 90˚ ± 20˚, indicating against-the-rule astigmatism. The mean induced astigmatism at Day 30 was 1.4 ± 1.2 D for smile incision and 1.8 ± 1.2 D for linear incision. Conclusion: Phaco-alternative yields good results with few complications but remains astigmatogenic. The smile incision appears to be less astigmatogenic.

EVOKE： A VALUE-DrIVEN CONCEPT SELECTION METHOD FOr EArLY SYSTEM DESIGN

The development of new technologically advanced products requires the contribution from a range of skills and disciplines, which are often difficult to fred within a single company or organization. requirements establishment practices in Systems Engineering （SE）, while ensuring coordination of activities and tasks across the supply network, fall short when it comes to facilitate knowledge sharing and negotiation during early system design. Empirical observations show that when system-level requirements are not available or not mature enough, engineers dealing with the development of long lead-time sub-systems tend to target local optima, rather than opening up the design space. This phenomenon causes design teams to generate solutions that do not embody the best possible configuration for the overall system. The aim of this paper is to show how methodologies for value-driven design may address this issue, facilitating early stage design iterations and the resolution of early stage design trade-offs. The paper describes how such methodologies may help gathering and dispatching relevant knowledge about the ＇design intent＇ of a system to the cross-functional engineering teams, so to facilitate a more concurrent process for requirement elicitation in SE. The paper also describes EVOKE （Early Value Oriented design exploration with KnowledgE maturity）, a concept selection method that allows benchmarking design options at sub-system level on the base of value-related information communicated by the system integrators. The use of EVOKE is exemplified in an industrial case study related to the design of an aero-engine component. EVOKE＇s ability to raise awareness on the value contribution of early stage design concepts in the SE process has been further verified with industrial practitioners in ad-hoc design episodes.

Palynology and stratigraphy of the Upper Miocene Chad Formation, Bornu Basin,northeastern Nigeria

Both lithological and palynological analyses were employed to decipher sedimentary stacking patterns and to date the sediments within the interval 50—325 m in Kemar-1 well,Bornu Basin,northeastern *** different lithofacies units deposited in various environments were *** are:(1) the grayish sandy claystone,rich in organic matter and associated with lignite,deposited in a continental to lacustrine environment;(2) the micaceous claystone,rich in muscovite flakes and organic matter,deposited in a lacustrine environment;(3) the lithified claystone which intercalates the dark gray shale,and deposited in a marine setting;(4) the shale;(5) the micaceous sandstone,which is characterized by presence of muscovite,silty to granular grain sizes,well sorted and deposited in a meandering setting;(6) the poorly-sorted *** is poorly sorted,poorly-graded,and coarse-grained,with erosive surfaces to underlying facies,and deposited in a fluvial environment;(7) the heterolithic clayey sandstone,poorly graded,grayish,with a degree of organic richness,suggesting an anoxic lacustrine environment;and(8) the grayish claystone which is nonlithic and rarely ferruginized marking the Upper Miocene/Eocene *** main palynological zone,i.e.,the Echitricolporites spinosus assemblage zone,has been *** base of the studied section at 385 m is characterized by the last up-hole occurrence of Grimsdalea magnaclavata suggestive of Eocene boundary while the overlying sediments are characterized by Late Miocene marker species such as ***,Elaeis guineensis,Anthocerus sp.,Nymphaea lotus,and *** *** Chad Formation is dated as Late Miocene age unconformably overlying the Eocene Kerri-Kerri Formation which indicated that the Chad Formation was probably deposited during an alternation of lacustrine and continental settings,due to climatic change.
