Functional outcomes of traumatic and non-traumatic rotator cuff tears after arthroscopic repair

AIM To compare the functional outcomes of traumatic and non-traumatic rotator cuff tears after arthroscopic repair. METHODS Eighty-seven patients with rotator cuff tears following arthroscopic treatment were divided into traumatic and non-traumatic tear groups. Postoperative muscle strength and outcomes using the modified University of California, Los Angeles score were evaluated. Sex, age, affected limb and dominant limb were correlated between groups. Muscle strength of the repaired and unaffected shoulders was compared. Rotator cuff injury size was measured. RESULTS Of the 87 patients who underwent rotator cuff repairs, 35 had traumatic tears and 52 had non-traumatic tears. In patients with non-traumatic tears, the average age was 59 years, 74.5% were female, 96.1% were righthand dominant and 92.3% had their dominant shoulder affected. Patients with traumatic tears were 59.5 yearsold on average, 51.4% were female, 91.4% were righthand dominant and 88.5% had their dominant shoulder affected. No difference existed in the mean modified University of California, Los Angeles score between patients with traumatic tears(33.7) compared with those with non-traumatic tears(32.8). No strength differences were observed between groups: The strength difference between the non-affected and affected sides was 1.21 kg in the non-traumatic group and 1.39 kg in the traumatic group(P = 0.576), while the strength ratio between the non-affected/affected sides was 0.805 in the nontraumatic group and 0.729 in the traumatic group(P = 0.224). CONCLUSION The functional results of traumatic rotator cuff repairs are similar to non-traumatic tears. Both outcomes are satisfactory.

BACKGROUND Shoulder maneuvers and magnetic resonance imaging(MRI)are performed to diagnose supraspinatus tendon tears regardless of arthroscopy *** there are many studies on this subject,there is a lack of studies comparing the sensitivity(Se)and specificity(Sp)of shoulder maneuvers and MRI to arthroscopic findings(intact,partial,or full thickness supraspinatus tendon tear).AIM To compare the diagnostic values of shoulder maneuvers with MRI for supraspinatus tendon tears in patients undergoing shoulder *** A total of 199 consecutive patients from four orthopedic centers met the eligibility criteria of shoulder pain persisting for at least four *** were prospectively enrolled in this study from April 2017 to April *** clinical tests(full can,empty can,drop arm,Hawkins’,painful arc,Neer’s sign and resisted external rotation)and MRI were performed,and all were compared with surgical *** can,empty can and resisted external rotation tests were interpreted as positive in the case of pain and/or *** assessed the Se,Sp,accuracy,positive predictive value(PPV)and negative predictive value(NPV),positive and negative likelihood ratio and diagnostic odds ratio for overall,partial and fullthickness supraspinatus *** MRI had the highest Se for overall(0.97),partial(0.91)and full-thickness(0.99)tears;moreover,MRI had the highest NPV:0.90,0.88 and 0.98 for overall,partial and full-thickness tears,*** overall supraspinatus tears,the Se and PPV were:Painful arc(Se=0.85/PPV=0.91),empty can(pain)(Se=0.80/PPV=0.89),full can(pain)(Se=0.78/PPV=0.90),resisted external rotation(pain)(Se=0.48/PPV=0.87),drop arm(Se=0.19/PPV=0.97),Neer’s sign(Se=0.78/PPV=0.93)and Hawkins’(Se=0.80/PPV=0.88).MRI had the highest PPV(0.99).The Hawkin’s test had the highest false positive rate in patients with intact tendons(0.36).The Sp of the empty can and full can(both tests positive for pain and weakness),drop arm and MRI were:0.93,

Effect of Specific Verbal Instructions on the Identification of Pain Location during a Passive Straight Leg Raise Test

Musculoskeletal pain is common. Because pain is subjective, objectively describing it is crucial. However, pain assessment may cause distress in patients;therefore, physical therapists (PTs) should conduct these tests quickly and accurately. Simple and clear instructions are recommended for pain assessment. However, few studies have provided evidence to support this hypothesis. Correspondingly, this study aimed to confirm the effectiveness of specific verbal instructions for pain location during five consecutive Passive Straight Leg Raise (PSLR) tests. The 28 asymptomatic participants (age 27.4 ± 9.6 years) who provided informed consent received five consecutive PSLR tests: three without and two with specific verbal instructions to ascertain pain intensity, quality, and location. The participants drew pain locations on a body chart and described the pain intensity and quality after each test. All participants were interviewed regarding the differences they noted in the presence and absence of specific verbal instructions. Each pain location was classified into one of ten areas for statistical analysis. The proportion of participants who changed the pain location was compared between the tests using McNemar’s test, and the kappa coefficient was confirmed for consistency of pain location. There was a significant difference in the proportion of participants who changed their pain location between the second and third tests and from the third to the fourth test (McNemar’s test: p = 0.003). Kappa coefficients had low consistency (κ = 0.28) just after receiving the specific verbal instructions in the fourth test compared to the third test. Consistency improved in the fifth test (κ = 0.57);93% of the participants answered that the pain location had become clearer. This study revealed the effects of specific verbal instructions in identifying pain locations. ＜正＞This paper presents a joint system called the "multilink spherical joint", which is a novel mechanism that allows tri-axial rotation of members. It can form ideal structure systems of variable geometry without small offsets between the rotation centers of the links. We also propose an analytical method for finding the shape of these structures in unstable states. The method is based on a combination of the stabilizing process and the steepest descent method. The proposed method was compared with the other two methods, and found to be optimal for numerical analysis with a fixed tolerance.

本田间试验在ＨｏｋｋａｉｄｏＫｉｔａｍｉ农业试验站进行了三年，旨在确定能否在收获前预测冬小麦籽粒蛋白质含量（ＧＰＣ）。供试材料为适于加工日本面条的冬小麦品种Ｃｈｉｈｏｋｕｋｏｍｕｇｉ。当分级施Ｎ时，根据抽穗末期倒二叶（旗叶下面的那片叶）的颜色可以准确地预测出ＧＰＣ。为了评价这一试验的有效性，在３年试验期间同时调查了Ｈｏｋｋａｉｄｏ东部地区的９５块生产麦田。结果表明，在环境条件不同的许多地点，用倒二叶叶色预测ＧＰＣ的准确性较低，但叶色可以作为一个指数来判断ＧＰＣ相对于加工要求是低还是高。当在抽穗末期用叶绿素仪测得的倒二叶叶色值小于４０时，ＧＰＣ可能低于加工标准。这一指数可以在Ｈｏｋｋａｉｄｏ东部地区采用，但泥炭土田块除外。

Mutual Adsorption of Lead and Phosphorus onto Selected Soil Clay Minerals

＜正＞ The upper reservoir of the Kyogoku Project, which is being constructed by Hokkaido Electric Power, will be a pool-type reservoir lined with asphalt facing for sealing. This reservoir is located in one of the coldest regions in Japan and also has much snowfall. For designing the asphalt facing and the asphalt mixtures, it is important to consider the severe weather conditions. Because of the limited time available for civil engineering work, it is important to do the work efficiently and accurately. For the asphalt facing, we used a thick lift placing of a single 8-cm thick layer for the upper impervious layer, the first time this has been done in Japan. Also, the cold-lay foamed asphalt system was used for the base layer during the paving work, which is the first time it was used in the world. An information technology （IT） construction system was used on every phase of construction, including the machine control and navigation system.

Mutual adsorption of lead (Pb) and phosphorus (P) at pH 5 onto three soil clays materials (kaolinite, montmorillonite, and allophane) was studied to know interaction of the anion and the cation at surface of the clays. Adsorption of Pb was determined on montmorillonite, kaolinite and allophane with the following pretreatments;1) untreated clay (control), 2) phosphate treated clay (P-clay) and 3) clay pre-treated with both P and Pb (P-Pb-clay). Adsorption of P was determined on montmorillonite, kaolinite and allophane with the following pretreatments;1) control 2) Pb treated clay (Pb-clay) and 3) P-Pb-clay. The adsorption of Pb on the untreated clays was in the order: montmorillonite > allophane > kaolinite. On allophane and kaolinite Pb adsorption was in the order P-clay > P-Pb-clay > control. For montmorillonite, the trend was: P-Pb-clay = control > P-clay. Phosphorus adsorption was in the order Pb-clay = P-Pb-clay > control for montmorillonite and kaolinite, Pb-clay > control > P-Pb-clay for allophane. The findings suggested that pre-treatment with phosphate increases Pb adsorption on kaolinite and allophane, and decrease on montmorillonite, while pretreatment with Pb increases phosphate sorption on all clays, and both Pb and P increased adsorption on montmorillonite and kaolinite and decrease on allophane.
