AIM： Esophageal squamous cell carcinoma is generally sensitive to chemoradiotherapy （CRT）, but some cases are not. Using a retrospective analysis, we aimed to identify the predictors of the response by esophageal squamous cell carcinoma to definitive CRT. METHODS： The intensities of expression of p53, Ki67, Bci-2, Bax, olclin D1, VEGF, CDC25B, and metallothionein （MT） were evaluated immunohistochemically in the biopsy specimens obtained before CRT, and the intensities of their expression were tested for correlations with the clinical effects of CRT. RESULTS： The esophageal squamous cell carcinomas with negative p53, positive CDC25B, and negative MT expression were found to be significantly more sensitive to CRT. In addition, p53 positivity and CDC25B positivity respomd well to CRT. CONCLUSION： Esophageal squamous cell carcinomas with negative p53,positive CDC25B, and negative MT expressions respond well to CRT. AIM： TO investigate active cytomegalovirus （CMV） infection following the cydosporine A （CyA） treatment of steroid-refractory ulcerative colitis （UC）. METHODS： Twenty-three patients with severe UC not responding to steroid therapy （male 14, and female 9） enrolled at Nagoya University Hospital from 1999 to 2005. They received continuous intravenous infusion of CyA （average 4 mg/kg per day） for 1 mo. Serum and colonic biopsy samples were collected before CyA treatment and 4 d, 10 d, 20 d, and 30 d after treatment. Patients were evaluated for CMV by using serology （IgM antibody by ELISA）, quantitative real-time PCR for CMV DNA, and histopathological assessment of hematoxylin and eosin （HE）-stained colonic biopsies. CMV infection was indicated by positive results in any test. RESULTS： No patients had active CMV infection before CyA treatment. Eighteen of 23 UC patients treated with CyA were infected with active CMV （IgM antibody in 16/23 patients, 69.6%; CMV DNA in 18/23 patients, 78.2%; and inclusion bodies in 4/23 patients, 17.3%）. There was no difference in the active CMV-infection rate between males and females. Active CMV infection was observed after approximately 8 d of CyA treatment, leading to an exacerbation of colitis. Fifteen of these 18 patients with active CMV infection （83.3%） required surgical treatment because of severe deteriorating colitis. Treatment with ganciclovir rendered surgery avoidable in three patients. CONCLUSION： Our results suggest that active CMV infection in severe UC patients treated with CyA is associated with poor outcome. Further, ganciclovir is useful for treatment of CMV-associated UC after immunosuppressive therapy.

Right hepatectomy for giant cavernous hemangioma with diffuse hemangiomatosis around Glisson's capsule

Diffuse liver hemangiomatosis with giant cavernous hemangioma in adult is extremely rare. A 35 year-old woman presented to hospital with main complaint of epigastric pain and abdominal fullness. An enhanced computed tomography scan revealed a massive liver tumor in right lobe about 150 mm in size. There was contrast enhancement at the periphery of the mass consistent with a cavernous hemangioma. She underwent right hepatectomy. Histologically, it was diagnosed as a cavernous hemangioma. And also, hemangiomatous lesions were scattered around the Glisson’s capsule on the back ground liver. These hemangiomatous lesions were not recognized preoperatively. Even if we couldn’t diagnose hemangiomatosis around the main giant hemangioma preoperatively, we need to take enough surgical margins because the giant hemangioma has the potential to have small hemangiomatous lesions around the tumor. We reported right hepatectomy for giant cavernous hemangioma with diffuse hepatic hemangiomatosis without an extrahepatic lesion in an adult.

BACKGROUND Phosphoglyceride crystal deposition disease(PCDD)is a rare acquired disease in which phospholipid crystals deposit in bone and soft tissue long after surgery,trauma,or repeated *** SUMMARY A 60-year-old-woman was referred to our department because of multiple abdominal masses after open splenectomy for idiopathic thrombocytopenic purpura 29 years *** the masses showed marked fluorodeoxyglucose(FDG)uptake on ^(18)F-fluorodeoxyglucose positron emission tomography(^(18)F-FDGPET)and were strongly suspected to be malignant *** biopsies were performed,and the abdominal masses were found to be aligned vertically,three in a row,along the tissue layers cut in the patient’s previous *** finding of the specimens showed foreign body granuloma consisting of histiocytes and multinucleated giant cells accumulating around needle-like *** crystals were confirmed as phosphoglyceride by Raman spectroscopy,and PCDD was *** our knowledge,this is the first report of PCDD diagnosed by Raman *** We made a definitive diagnosis of PCDD in a patient with multiple tumors showing marked FDG uptake on ^(18)F-FDG-PET by incisional biopsy and composition analysis using Raman spectroscopy,a method that has not previously been reported for the diagnosis of PCDD.

Dear editor,The current treatment strategy for advanced non-small cell lung cancer(NSCLC)patients harboring epider-mal growth factor receptor(EGFR)mutations involves EGFR-tyrosine kinase inhibitors(EGFR-TKIs).Several reports on EGFR-TKI resistance have shown that the T790M mutation within the EGFR gene is present in approximately half of the patients who develop resist-ance to a first-or second-generation EGFR-TKI[1,2].In Japan,osimertinib monotherapy is the standard therapy for patients harboring the EGFR T790M mutation arising after the development of resistance to a first-or second-generation EGFR-TKI[3,4].Herein,we report the case of a patient who underwent four tumor biopsies over the treatment *** no detection of this mutation after developing resistance to the first-line EGFR-TKI,in the fourth biopsy specimen,which was collected after progression following EGFR-TKI re-challenge,EGFR T790M mutation was detected using the cobas®EGFR mutation test(Roche Molecular Systems,Pleasanton,CA,USA).Additionally,we analyzed the T790M allele frequency(AF)in longitudinal biopsy samples obtained at four different time points using droplet digital PCR(ddPCR)(PrimePCR™,Bio-Rad Laboratories,Inc.,Her-cules,CA,USA).We discuss the clinical usefulness of longitudinal assessment of the EGFR T790M AF,espe-cially for deciding the appropriate treatment(e.g.,osi-mertinib)in patients with EGFR mutations.

Minimum Regret Climate Policy with Act-Then-Learn Decision-A New Model Framework under Long-Term Uncertainties

The technology investment strategy under uncertainty is the key subject. However, the expected utility maximization often employed as the decision process fails to consider the high risk with low probability cases. On the other hand, the existing min-max regret strategy tends to be dominated by the ＂worst assumption＂ regardless of its probability. This research proposes a new framework by formulating the regret by the Minkowski＇s generalized distance. The authors then apply the formulation to the IAM （integrated assessment model） MARIA. This study focuses on the uncertainties of CCS （carbon capture and storage） costs and the global warming damages. This formulation is then extended to the multi-stage decision frame, known as ATL （act-then-learn） method. The simulation results suggest that the substantial changes in CCS and nuclear deployment strategies depending on the future uncertainty scenarios. The results also suggest that the minimum regret strategy favors the capital accumulation in the early stage.

A DC （data center） demands air-conditioning power as large as the 1/3-1/2 of total electricity consumption. Thus, energy saving of cooling power of DC yields considerable effect on both economic and environmental views. PV （Photovoltaic） and absorption refrigerator with CGS （cogeneration systems） or gas boiler are possible power saving options. The waste warm air from DC would be utilized for greenhouse heating when DC and greenhouse locate near in the suburbs. In this study, the authors develop an energy network model to assess the potential contribution of DC as a major electric power and chilled air consumer as well as the warm air supplier in a district to the energy efficiency improvement. The evaporation heat of LNG （liquefied natural gas） utilization is also considered as well as PV, CGS. This model is applied to the cases of the urban area in Tokyo which involves athletic center, shops and hospital and the suburbs including greenhouse and then compared.

Epidermal Growth Factor Enhances Orthovanadate-Induced Contraction via Src and Myosin Phosphatase Target Subunit 1 in Rat Vascular Smooth Muscle

Inhibition of protein tyrosine phosphatase by orthovanadate induces vasoconstriction, which is mediated by the Rho kinase-dependent inactivation of myosin light chain phosphatase (MLCP) via signaling downstream of Src-induced activation of the epidermal growth factor (EGF) receptor. The present study investigated the potential role of EGF in orthovanadate (OVA)-dependent vaso-constriction. OVA-induced aortic contraction significantly increased in the presence of EGF, and was abolished by inhibitors of Rho kinase (Y27632), extracellular signal-regulated kinase 1 and 2 (Erk1/2) (FR180204), Erk1/2 kinase (PD98059), EGF receptor (AG1478), and Src (PP2). Treatment of the rat endothelium-denuded thoracic aorta with either EGF or OVA augmented the phosphorylation of myosin phosphatase target subunit 1 (MYPT1) at Thr-853 and of the EGF receptor at Tyr-1173. The phosphorylation of MYPT1 was further increased by co-stimulation with EGF and OVA. EGF receptor phosphorylation at Tyr-845 was also increased by EGF or OVA;this effect was augmented by co-stimulation with EGF and OVA, and was abolished by Src inhibition. In addition, Erk1/2 was phosphorylated by EGF or by co-treatment with EGF and OVA;this was abolished by an EGF receptor inhibitor, but not by Src inhibition. These results suggested that OVA-induced EGF-related contraction was mediated by the Rho kinase-dependent inactivation of MLCP via two different signaling cascades: Src-dependent phosphorylation of the EGF receptor at Tyr-845 and EGF-dependent phosphorylation of Erk1/2.

Simultaneous Determination of Catechins and Caffeine in Green Tea-Based Beverages and Foods for Specified Health Uses

Catechins in green tea have various useful features including antioxidant activity and preventive effects on metabolic syndrome. Various beverages that are enriched with tea catechins are marketed as Foods for Specified Health Uses (FOSHU) in Japan. However, recent reports have indicated that excessive consumption of green tea extracts as a dietary supplement are associated with adverse health effects such as liver disorders. Various catechins and caffeine are constituents of FOSHU tea-based beverages. The amount of catechins in FOSHU products is displayed on labels as total catechin content, but the content of individual catechins are not provided. Although health hazards of FOSHU products have rarely been reported, precise information about the content and types of catechins in FOSHU products is needed to ensure safety. We used high-performance liquid chromatography with a photodiode array (HPLC/PDA) to simultaneously identify and quantify catechins and caffeine in green tea-based popular beverages and FOSHU beverages. This technique allowed simultaneous quantitation of five types of catechins and caffeine in green tea without complicated sample preparation. Epigallocatechin gallate (EGCG) and epigallocatechin EGC were the main catechins in various FOSHU beverages and the concentrations of almost all catechins were higher in FOSHU, than in popular green tea-based beverages. The concentrations of EGCG in green tea-based popular beverages and in FOSHU beverages were 5.4 - 7.3 and 10.2 - 41.9 mg/100mL, respectively, with the highest concentration being in a product named Healthya (approximately 147 mg/bottle). The simultaneous determination of compounds such as catechins and caffeine in FOSHU beverages can help to estimate beneficial and adverse effects to prevent deleterious effects on health and the excessive consumption of FOSHU beverages containing high concentrations of tea catechins should be avoided.

Kurozu is a traditional black rice vinegar in Japan. Kurozu-moromi is the solid sediment that arises during the production of Kurozu. Kurozu is thought to have blood thinning and antithrombotic activities but a specific effect has not been proven. Plasma plasminogen activator inhibitor 1 (PAI-1) inhibits fibrinolysis and when elevated, it is a risk factor for the development of thrombotic diseases. Here, we examined the effects of a diet containing Kurozu and Kurozu-moromi on LPS (lipopolysaccharide)-induced increases in plasma PAI-1 levels in mice and on TNF-α (tumor necrosis factor α) increased PAI-1 production in the medium of a cultured endothelial cell line. Mice were fed with a diet containing 0.25% (w/w) concentrated Kurozu or 0.5% (w/w) Kurozu-moromi for four weeks, and then subcutaneously injected with 0.015 mg/kg of LPS in saline and sacrificed three hours later. Orally administered concentrated Kurozu and Kurozu-moromi significantly suppressed the LPS-induced increase in PAI-1 antigen and its activity in mouse plasma. The ethanol extract of Kurozu-moromi inhibited TNF-α induced increases of PAI-1 in the culture medium of the ***926 endothelial cell line. The present findings showed that Kurozu and Kurozu-moromi contain natural products that decrease thrombotic tendencies by decreasing PAI-1 production in inflammatory states and have potential as antithrombotic foodstuffs.
