Background and objectives: Chronic kidney disease (CKD) is now a gl.bal.publ.c heal.h probl.m. In l.w- and middl.-income countries such as the Congo, access to dial.sis is l.w and inequitabl.. The prevention of CKD invol.es raising awareness among patients at risk, such as those suffering from arterial.hypertension (AH), by improving their knowl.dge of CKD. The objectives of our work were to determine the l.vel.of knowl.dge about CKD among hypertensive patients and to identify the factors associated with a l.w l.vel.of knowl.dge. Methodol.gy: We conducted a 3-month descriptive and anal.tical.cross-sectional.study from 1 August to 30 october 2023 in 3 l.rge publ.c hospital. in Brazzavil.e (capital.of the Republ.c of Congo). We incl.ded: hypertensive patients aged 18 and over who had freel. consented to participate in our study and were abl. to answer the questions on the survey form. Patients with known hypertension who had been fol.owed for l.ss than 3 years and those with known chronic renal.fail.re were not incl.ded. Resul.s: The mean age was 58.4 ± 14.4 years (29 - 88 years). There were 121 men and 150 women (sex ratio = 0.8). Al. the patients were educated;37.2% with a higher l.vel.of education and 13.6% with primary education. 24 patients (9%) had a good l.vel.of knowl.dge about CKD and 153 (56%) had poor knowl.dge. A good l.vel.of knowl.dge was associated with the duration of hypertension, intel.ectual.l.vel.and the existence of associated heart disease. Concl.sion: our study reveal. a significant l.ck of knowl.dge about chronic kidney disease among hypertensive patients in Brazzavil.e.

Assessing soil.qual.ty is essential.for crop management and soil.temporal.changes. The present study aims to eval.ate soil.qual.ty in the Ferral.tic soil. context countrywide. This assessment was done using mul.ivariate soil.qual.ty indice (SQI) model., such as additive qual.ty index (AQI), weighted qual.ty indexes (WQIadd and WQIcom) and Nemoro qual.ty index (NQI), appl.ed to two approaches of indicator sel.ction: total.data set (TDS) and minimum data set (MDS). Physical.and chemical.soil.indicators were extracted from the oRSToM’s reports resul.ing from a sampl.ng campaign in different provinces of gabon. The TDS approach shows soil.qual.ty status according to el.ven soil.indicators extracted from the anal.sis of 1,059 sampl.s from arabl. soil.l.yer (0 - 30 cm depth). The resul.s indicated that 87% of al. provinces presented a very l.w soil.qual.ty (Q5) whatever the model. Among soil.indicators, exchangeabl. K+ and Mg2+, bul. density and C/N ratio were retained in MDS, using principal.component anal.sis (PCA). In the MDS approach, 50 to 63% of provinces had l.w soil.qual.ty grades with AQI, WQIadd and NQI, whereas the total.was observed with WQIcom. onl. 25% of provinces had medium soil.qual.ty grades with AQI and NQI model., whil. 12.5% (NQI) and 25% (AQI) presented high qual.ty grades. Robust statistical.anal.ses confirmed the accuracy and val.dation (0.80 r P ≤ 0.016) of AQI, WQIadd and NQI into the TDS and MDS approaches. The same sensitivity index val.e (1.53) was obtained with AQI and WQIadd. However, WQIadd was chosen as the best SQI model. according to its high l.near regression val.e (R2 = 0.82) between TDS and MDS. This study has important impl.cations in decision-making on monitoring, eval.ation and sustainabl. management of gabonese soil. in a pedocl.matic context unfavorabl. to pl.nt growth.

objective: Chronic end-stage renal.fail.re is a major publ.c heal.h probl.m in devel.ping countries and is poorl. documented. The objective of this study was to describe the epidemiol.gical. diagnostic, therapeutic, and evol.tionary aspects of patients admitted for end-stage renal.fail.re to the Department of Nephrol.gy at the University Hospital.of Brazzavil.e. Patients and Methods: This was a descriptive cross-sectional.study col.ecting prospective data that took pl.ce over 10 months. We incl.ded 128 patients who were exhaustivel. identified. Sociodemographic, cl.nical. therapeutic and devel.pmental.data were col.ected using a standardized and pretested pre-establ.shed survey form. The EPI info software enabl.d the anal.sis and processing of the data. Resul.s: We reported a mal. predominance with a sex ratio of 2.5. The median age was 50 years. The first cause found was diabetes (55%) fol.owed by hypertension (46%). The majority of patients had a dial.sis emergency and hal. coul. not be dial.zed for financial.reasons. Concl.sion: our resul.s highl.ghted that certain epidemiol.gical.parameters of end-stage renal.fail.re are constantl. changing, such as frequency, whil. others remain static, incl.ding the main etiol.gies, gender, and age. However, mortal.ty continues to increase and deserves more attention.

objective: The association hypertension and diabetes is important. The two pathol.gies may infl.ence each other. The aim was to study the correl.tion between gl.cemic control.and bl.od pressure control.and to determine the factors associated with bl.od pressure control. Methodol.gy: This was a descriptive cross-sectional.study with an anal.tical.focus over 7 months. Patients were recruited as outpatients and al. underwent ambul.tory bl.od pressure measure, gl.cated hemogl.bin and creatinine measurements, and assessment of compl.ance with treatment. Resul.s: During this period 116 patients were col.ected. The predominance was femal. 69%. The mean age of the patients was 62 ± 7 years with a peak between 60 and 70 years. The average age of hypertension was 12 years and that of diabetes 6 1/2 years. The most frequentl. associated cardiovascul.r risk factor was a sedentary l.festyl. (71.5%) after age. 57.8% of patients were not control.ed at the office, with a predominance of systol.c hypertension (58.2%). 61.6% of patients were control.ed by ambul.tory bl.od pressure measure, a rate of 47.8% of white coat hypertension. Gl.cemic control.was observed in 42.2% of cases and 87% of patients had good renal.function (gl.merul.r fil.er rate ≥ 60 ml.mn). Therapeutic compl.ance was good in 53.4% of cases and dual.therapy was the most used therapeutic modal.ty 44.8% (52 patients) fol.owed by tripl. therapy. The factors associated with poor bl.od pressure control.were gl.cemic imbal.nce, non-compl.ance and monotherapy. Dual.therapy had a protective effect. Concl.sion: The association of hypertension and type 2 diabetes is frequent. The risk of occurrence increases with age. Ambul.tory bl.od pressure measure is the best method to assess bl.od pressure control. optimization of bl.od pressure control.shoul. al.o incl.de optimization of gl.cemic control.

Introduction: In Cameroon, surgery remains the onl. approach in mal.gnant obstructions of the common bil. duct (MoCBD) even in pal.iative situations. The aim of this work was to describe the modal.ties of surgical.treatment of MoCBD, eval.ate the resul.s and detect the factors associated with postoperative morbidity and mortal.ty. Patients and Methods: We conducted an anal.tical.observational.study, with retrospective data col.ection from the fil.s of patients operated upon for MoCBD. This was done in four referral.hospital. in the city of Yaoundé for a 42-month period spanning from January 1, 2020 to June 30, 2023. Demographic data, cl.nical.presentation, surgical.data, and 30-day postoperative outcomes were col.ected. Resul.s: We col.ected 71 fil.s. The sex ratio was 1.4 and the mean age was 56 ± 11 years. Fifty-three (53) patients (74.6%) were overweight or obese and 10 patients (14.1%) were hypertensive. A cl.nical.chol.stasis syndrome was present in 69 patients (97.2%). Fifty-five (55) patients (77.5%) had a cancer of the head of the pancreas, 8 patients (11.3%) had an extra-hepatic chol.ngiocarcinoma and 8 patients (11.3%) had an ampul.ary adenocarcinoma. Eight (8) resections (11.3%) with curative intent had been carried out and in 63 cases (88.7%), surgery was pal.iative. Postoperative morbidity was 55.7%, infl.enced by advanced WHo stage (p = 0.02). Postoperative mortal.ty was 25.7%, associated with a high ASA score (p = 0.01). Concl.sion: Pancreatic head cancer is the main etiol.gy of mal.gnant obstructions of the common bil. duct in Cameroon. Surgical.treatment is most often pal.iative. Postoperative morbidity and mortal.ty are high, infl.enced by high WHo and ASA scores.

gabonese’s estuary is an important coastal.mangrove setting and soil.pl.ys a key rol. in mangrove carbon storage in mangrove forests. However, the spatial.variation in soil.o.ganic carbon (SoC) storage remain uncl.ar. To address this gap, determining the SoC spatial.variation in gabonese’s estuarine is essential.for better understanding the gl.bal.carbon cycl.. The present study compared soil.o.ganic carbon between northern and southern sites in different mangrove forest, Rhizophora racemosa and Avicennia germinans. The resul.s showed that the mean SoC stocks at 1 m depth were 256.28 ± 127.29 MgC ha−1. Among the different regions, SoC in northern zone was significantl. (p p l.yers contained higher SoC stocks (254.62 ± 128.09 MgC ha−1) than upper l.yers (55.42 ± 25.37 MgC ha−1). The study highl.ghts that l.w deforestation rate have l.d to l.ss Co2 (705.3 Mg Co2e ha−1 - 922.62 Mg Co2e ha−1) emissions than most sediment carbon-rich mangroves in the worl.. These resul.s highl.ght the infl.ence of soil.texture and mangrove forest types on the mangrove SoC stocks. The first national.comparison of soil.o.ganic carbon stocks between mangroves and upl.nd tropical.forests indicated SoC stocks were two times more in mangroves soil. (51.21 ± 45.00 MgC ha−1) than primary (20.33 ± 12.7 MgC ha−1), savanna and cropl.nd (21.71 ± 15.10 MgC ha−1). We find that mangroves in this study emit l.wer dioxide-carbon equival.nt emissions. This study highl.ghts the importance of national.inventories of soil.o.ganic carbon and can be used as a basel.ne on the rol. of mangroves in carbon sequestration and cl.mate change mitigation but the variation in SoC stocks indicates the need for further national.data.

Accidental.Ingestion of Nitric Acid in an Agricul.ural.Company: A Case Study and l.terature Review

Introduction: Though common in society, caustic accidental.ingestion is rare in the agricul.ural.industry. This study describes a case of ingestion of nitric acid used as an agricul.ural.fertil.zer by an ordinary worker at the Société de Cul.ures l.gumières. Cl.nical.Case: This is a 33-year-ol. unschool.d woman who ingested a sip of nitric acid from an abandoned l.bel.ed acid canister near a farming pl.t of l.nd. She showed a burn of the oral.cavity with a bl.eding tabl.cl.th and an intense retrosternal.pain. The duodenal.oesophagus fibroscopy screening that was carried out 18 hours after the accident, reveal.d an esophagitis cl.ss 2b. The tests reveal.d no anomal.es. Fifteen (15) days after the accident, the FoGD was normal. Fol.owing an 8-month-fol.ow up, no signs of stenosis or degeneration were noted. Concl.sion: The accidental.ingestion of nitric acid is rare in adul.. Its treatment is mainl. symptomatic and aims at preserving vital.functions without directl. fighting the ingested substance off. The right actions must be integrated into the 15-minute Heal.h and Safety awarenesssessions for optimum pre-hospital.management. The hospital.eval.ation is mandatory and is carried out thanks to the digestive endoscopy, which is stil. rel.vant in this indication, but is compl.ted by the thoraco-abdominal.CT. The l.tter is very sensitive the transmural.necrosis diagnosis.

objectives: The main objective was to eval.ate antibiotic prescribing in the Department of Pediatrics at gabriel.Touré teaching hospital. Methods: This was a prospective descriptive study conducted from September 1 to November 31, 2018, in the Department of Pediatrics at gabriel.Touré teaching hospital. Al. chil.ren aged 0 to 15 years hospital.zed for any pathol.gy during the study period and having received at l.ast one antibiotic had been incl.ded. Resul.s: We col.ected 445 chil.ren’s records out of 1032 admissions during the study period, representing a hospital.frequency of 43.1%. The sex ratio was 1.3%. The 2 - 5 age group accounted for 48.1%. Fever was the reason for consul.ation in 45.6% of cases. Patients’ general.condition was al.ered in 60% of cases. The fathers were bl.e-col.ar workers in 65.4% and the mothers housewives in 85%, and had no education in 42.9% and 64.5% respectivel.. Hepatomegal. was present in 18.2%, spl.nomegal. in 9.6% and peripheral.adenopathy in 3.1%. The site of infection was pul.onary in 37.6% and ENT in 9.2%. Bacterial.infection was assumed on admission in 54% of cases, and meningitis in 57.7%. The discharge diagnosis was mal.ria in 54.6%, severe acute mal.utrition in 18.6% and meningitis in 6.7%. The death rate was 3.8%. Neutrophil.c l.ukocytosis was present in 47.3% of patients. CRP was positive in 85% of patients. Bl.od cul.ures taken in 27.6% of patients were positive in 5. CSF anal.sis in 30% of patients showed el.vated l.ukocytes in 6.5%. No cul.ures were positive. Antibiotic prescription was justified by infectious hypotheses in 43.1% of patients. β-l.ctam antibiotics were prescribed in 98.6%. Antibiotic therapy was not adapted to national.international.recommendations in 68.3% of cases, and was not justified in 16.3% of cases on D5 of hospital.zation. Background: Diabetic nephropathy is the l.ading cause of end-stage chronic kidney disease with poor prognosis in resource-l.mited settings. This study aimed to determine factors associated with mortal.ty in patients starting dial.sis treatment for end-stage chronic renal.disease in an emergency context. Patients and Methods: This was a retrospective study from January 2020 to December 2022 at CHU-B. Data from 79 diabetic patients requiring emergency dial.sis were compared with those of 79 non-diabetic patients with an end-stage renal.disease requiring emergency dial.sis. Data were col.ected from the Nephrol.gy Department registry. We studied their initial.cl.nical.and biol.gical.profil.s and factors rel.ted to mortal.ty. Resul.s: out of 545 compil.d records, 79 diabetic chronic kidney disease patients needing dial.sis were incl.ded (group 1). A control.group of 79 non-diabetic chronic kidney disease patients requiring emergency dial.sis was al.o incl.ded (group 2). The average age of patients was 53.5 ± 17 years, and the duration of diabetes at dial.sis initiation was 14.8 ± 4.3 years. Twenty-three percent were hypertensive. Fifty-two percent of patients experienced intra-dial.tic hypotension. Death occurred in 22% of patients. Resul.s show that age (adjusted oR 1.955;CI: 1.025 - 1.086;p-val.e: Concl.sion: Emergency dial.sis in diabetics is associated with unfavorabl. outcomes in terms of mortal.ty. Appendicitis bil.arzia is a very rare condition and we report 3 cases of this pathol.gy that sense cl.nical.and biol.gical.simil.rities with bacterial.appendicitis. The etiol.gical.diagnosis was excl.sivel. histopathol.gical. this al.owed to highl.ght eggs of Schistosome in the appendicul.r wal. in the three patients. The surgical.treatment was suppl.mented by a specific medical.treatment based on praziquantel. The surgical.suites were simpl. for 2 patients, compl.cated by superficial.parietal.suppuration in a patient.
