Mycobacterium chimaera infections following cardiac surgery in Treviso Hospital, Italy, from 2016 to 2019: Cases report

BACKGRoUND An epidemic of Mycobacterium chimaera (M. chimaera) infections following cardiac surgery is ongoing worldwide. The outbreak was first discovered in 2011, and it has been traced to a point source contamination of the LivaNova 3T heater-cooler unit, which is used also in Italy. International data are advocated to clarify the spectrum of clinical features of the disease as well as treatment options and outcome. We report a series of M. chimaera infections diagnosed in Treviso Hospital, including the first cases notified in Italy in 2016. CASE SUMMARY Since June 2016, we diagnosed a M. chimaera infection in nine patient who had undergone cardiac valve surgery between February 2011 and November 2016. The time between cardiac surgery and developing symptoms ranged from 6 to 97 mo. Unexplained fever, psychophysical decay, weight loss, and neurological symptoms were common complaints. The median duration of symptoms was 32 wk, and the longest was almost two years. A new cardiac murmur, splenomegaly, choroidoretinitis, anaemia or lymphopenia, abnormal liver function tests and hyponatremia were common findings. All the patients presented a prosthetic valve endocarditis, frequently associated to an ascending aortic pseudoneurysm or spondylodiscitis. M. chimaera was cultured from blood, bioprosthetic tissue, pericardial abscess, vertebral tissue, and bone marrow. Mortality is high in our series, reflecting the poor outcome observed in other reports. Three patients have undergone repeat cardiac surgery. Five patients are being treated with a targeted multidrug antimycobacterial regimen. CoNCLUSIoN Patients who have undergone cardiac surgery in Italy and presenting with signs and symptoms of endocarditis must be tested for M. chimaera.

This paper presents a sampled-data control scheme for the disturbance rejection of nonlinear systems in the output feedback form. The continuous-time controller is designed first using a filtered transformation and the internal model technique. Using the emulation approach, the proposed sampled-data control uses the sampled output and a discrete- time implementation of the filter, and the internal model is involved. The proposed control is shown to render the overall system stable in a spirit of fast sampling. In particular, the disturbance is practically rejected in the sense that the ultimate bound of the output is allowed to be arbitrarily small by choosing appropriate gain parameters.

The effect of cathodic hydrogen charging on the tensile and constant load properties was deter- mined for an austenitic stainless steel weldment comprising a 304L steel in the solution treated condition as a base metal and a 308L filler steel as a weld metal. Part of the 304L solution treated steel was separately given additional sensitization treatment to simulate the microstructure that would develop in the heat affected zone. Tests were performed at room temperature on notched round bar specimens. Hydrogen charging resulted in a pronounced embrittlement of the tested materials. This was manifested mainly as a considerable loss in the ductility of tensile specimens and a decrease in the time to failure and threshold stress of constant load specimens. The 308L weld metal exhibited the highest, and the 304L solution treated steel the lowest, resistance to hydrogen embrittlement. Hydrogen embrittlement was associated with the formation of strain induced martensite as well as a transition from brittle to ductile fracture morphology onwards the centre of the specimens.

High Power Neutral Beam Injection in LHD

The results on high power injection with the ueutral bean, injection （NBI） system for the Large Helical Device （LHD） are reported. The system consists of three beam-lines with two hydrogen negative ion （H^- ion） sources installed in each beam-line. In order to improve the injection power, a new beam accelerator with a multi-slot grounded grid （MSGG） has been developed and applied to one beam-line. Using the accelerator, a maximum power of 5.7 MW was achieved in 2003 and 2004, and the maximum energy of 189 keV was reached. The power and energy exceeded the design values of the individual beam-line for LHD. The other beam-lines also increased their injection power up to about 4 MW, and the total injection power of 13.1 MW was achieved with three beam-llnes in 2003. Although the accelerator had an advantage in high power beam injection, extracted beams expand in the short side direction of the ground-grid slot. The disadvantage has been resolved by modifying the aperture shapes of the steering grid.
