Influence of Psychiatric Problems on Clinical Outcomes during the First 12 Months after Primary Total Knee Arthroplasty

Background: Total knee arthroplasty (TKA) is a useful treatment option for advanced knee osteoarthritis. Excellent clinical outcomes after TKA have been widely recognized, but the influence of psychiatric problems on them has not been focused on until quite recently. This study aimed to assess the influence of psychiatric problems on clinical outcomes after TKA in Japanese patients using two assessment scales developed in Japan because the Japanese cultural lifestyle is specifically characterized by bending to the floor and standing up. Methods: Clinical outcomes and psychiatric problems were evaluated using the Japanese Knee Osteoarthritis Measure (JKOM) and Brief Scale for Psychiatric Problems in Orthopaedic Patients (BS-POP), respectively. A total of 115 TKA patients were evaluated preoperatively and at 3, 6, and 12 months after TKA. The patients were classified into four groups (groups A-D) based on the BS-POP score. The JKOM scores were then compared between the two groups (groups A and D) with the worst and least psychiatric problems. The JKOM improvement rate between pre- and postoperative status in both groups A and D was also calculated. Results: The total JKOM score was significantly poorer in group A than in group D preoperatively and at 3, 6, and 12 months after TKA. The improvement rate showed no significant difference between groups A and D. Conclusion: Psychiatric problems influence both the poorer post- and preoperative clinical outcomes. However, a similar improvement rate in both groups A and D has indicated that TKA can be an effective treatment even for patients with psychiatric problems.

Aeolian Tone from a Semi-Circular Cylinder in a Stream

Aeolian tone from a semi-circular cylinder in a uniform flow is studied experimentally for various angles of attack. It is found that the peak sound spectrum of the Aeolian tone from the semi-circular cylinder is smaller than that from the circular cylinder and the lowest sound is observed around the zero angle of attack. This is due to the reduction in the fluctuating lift force on the semi-circular cylinder compared to that of the circular cylinder. This result suggests the validity of the analogy between the sound pressure level and the fluctuating lift force on a semi-circular cylinder in a stream. The flow visualization study also supports these results.

Eye-Movement While Generating a Trajectory Using the Upper Limb

This paper describes the experimental analysis of eye-movement during the generation of a trajectory by the human upper limb, carried out to elucidate the human mechanism for visual-information recognition. The results showed that the subjects adopted an eye-movement pattern called the subgoal travel method （fixating in the vicinity of the fingertip while drawing/tracing） for the complete circle. Subsequently, when the target trajectory was one of incomplete shapes, some subjects continued following the subgoal travel method by drawing the missing part of the target trajectory, while others followed two other eye-movement patterns. The first is called the center-point fixation method, in which subjects consider the center point as the most important point for generating the target-image trajectory, and therefore, fix their gaze at the center point throughout the experiment. The second is called the point-to-point travel method, in which the subjects＇ gazes shift between the center point of a displayed image and their fingertip. Further, the results confirmed that the eye-movement pattern, movement accuracy, and drawing speed are correlated. Additional experiments clarified the conditions for which different eye-movement patterns are suitable： the subgoal travel method is suitable for high-accuracy drawing; the center-point fixation method is suitable for higher-sneed drawing

Fatigue and Depression from Early Postpartum to 1 Month among Postpartum Women with Mental Disorders

Objective: This study aims to ascertain the prevalence of fatigue and depression from early postpartum to 1 month after delivery among postpartum women with mental disorders. Methods: The participants of this study were postpartum women who had delivered a child after a full-term pregnancy and were suffering from a mental disorder. We administered the Postpartum Fatigue Scale (PFS) on days 1, 3, and 4 (to multiparas), and 1, 3, and 5 (to primiparas), and 1 month after delivery to all participants. The Edinburgh Postnatal Depression Scale (EPDS), Japanese version, was also administered at the same time intervals as PFS except on day 3 after delivery. Results: A total of 7 primiparas and 9 multiparas participated in this study. The average age was 29.0 ± 7.0 years in primiparas and 32.0 ± 4.0 years in multiparas. The PFS scores were higher among the primiparas and peaked at day 1, and more than half the primiparas scored more than 9 points in EPDS. In addition, the EPDS score at 1 month had a strong correlation with the PFS subscale “mental stress situation” (r = 0.818/p = 0.047). In multiparas, the percentage of women who scored more than 9 points decreased. However, their EPDS scores at 1 month were related to the total scores of the PFS, “physical stress situation”, “mental stress situation”, and “sleep deprivation situation”. Especially, the score of “sleep deprivation situation” was higher than those during hospitalization. Conclusion: There was more fatigue 1 month after the discharge among postpartum women with mental disorders. The percentage of women whose EPDS scores were more than 9 points was high in both primiparas and multiparas. This study suggests assisting primiparas and multiparas by controlling the former’s disorder during childcare, and using family support for the latter, so that they get time to rest.

Identification and Expression Analysis of Regulatory Genes Induced by Near-Ultraviolet Irradiation in <i>Bipolaris oryzae</i>

Bipolaris oryzae is the causal agent of brown leaf spot disease in rice, and its asexual spore (conidium) formation is known to be induced by near-ultraviolet (NUV) irradiation. In order to reveal the photomorphogenic response and to identify new genes upregulated by NUV irradiation, suppression subtractive hybridization (SSH) was carried out in B. oryzae. To confirm the differential gene expression in NUV-irradiated mycelia, quantitative real-time PCR (qRT-PCR) analysis was performed among 301 genes arbitrarily chosen from 1170 cDNA clones. The expression of 46 genes (named NUV01 to NUV46) was found to be significantly enhanced (>4-fold) by NUV irradiation. Sequence analysis revealed that 23 out of the 46 sequences (50%) showed significant matches to known fungal genes. The 46 genes were categorized as either BLR1-dependent or BLR1-independent expression groups using the BLR1-deficient mutant, which presumably lacks the blue/UVA-absorbing photoreceptor. This finding demonstrates that NUV irradiation can induce gene regulation, and that this response may be mediated by both a blue/UVA-absorbing photoreceptor and an as-yet-unidentified photoreceptor in B. oryzae.

Changes in the levels of CAM kinase II and synapsin I caused by oxidative stress in the rat brain, and its prevention by vitamin E

To define whether oxidative stress and aging induce abnormal dissociation of neurotransmitter-enclosing synaptic vesicles in rat brain nerve terminals, we assessed the activation of Ca+/calmodulin dependent protein kinase II (CAM kinase II) and changes in the levels of synapsin I, which is a synaptic vesicle-associated protein involved in the modulation of neurotransmitter release. Assessment of young rats subjected to hyperoxia-induced oxidative stress and normal aged rats revealed that synaptic CAM kinase II in the rat brain was markedly activated through oxidative stress and aging. In accordance with the activation of CAM kinase II, the levels of phosphorylated synapsin I increased significantly in nerve terminals. Furthermore, it was found that vitamin E prevents these oxidative stress-induced abnormal processes in rat nerve terminals. These results suggest that oxidative stress and aging facilitate the mobilization of neurotransmitter-enclosing synaptic vesicles from the reserve pool in the nerve terminal, thereby inducing abnormal accumulation of synaptic vesicles in the synapse, and that vitamin E inhibits this process in the brain through its antioxidative action.

Use of central venous saturation monitoring in a patient with pediatric cardiac beriberi

The patient was a 1-year-and-4-mo-old boy. He had drunk about 1 L of an isotonic drink for infants daily since about 10 mo after birth. He was examined by a local doctor due to anorexia and vomiting, found to have cardiomegaly, and transported to our hospital with suspected myocarditis. After admission, the patient showed polypnea, a decreased level of consciousness, and marked metabolic acidosis and lapsed into circulatory insufficiency, requiring catecholamine administration, endotracheal intubation, and extracorporeal membrane oxygenation. Initially, low-output heart failure due to acute myocarditis was suspected, but the central venous oxygen saturation was high, at 82%. Considering high-output heart failure to be more likely, we evaluated its cause and noted, by urinary organic acid analysis, increases in lactate, pyruvate, 3-OH-butyrate, acetoacetate, metabolic products of branched-chain amino acids, 2-ketoglutarate, 2-OHglutarate, 2-keto-adipate, and 2-OH-adipate. Since the vitamin B1 level was reduced to 12 ng/m L(normally 20-50 ng/m L), a diagnosis of cardiac beriberi due to vitamin B1 deficiency was made. When unexplained heart failure is observed in children, cardiac beriberi must be excluded as a differential diagnosis of myocarditis and cardiomyopathy. The measurement of the central venous oxygen saturation may be useful for the diagnosis.
