Absence of asymptomatic unruptured renal artery pseudoaneurysm on contrast-enhanced computed tomography after robot-assisted partial nephrectomy without parenchymal renorrhaphy

Objective:To assess the incidence of asymptomatic unruptured renal artery pseudoaneurysm(RAP)on contrast-enhanced computed tomography(CE-CT)after robot-assisted partial nephrectomy(RAPN)without parenchymal renorrhaphy.Methods:From May 2016 to December 2017,78 patients underwent RAPN for renal tumors.Inner suture was performed in the opened collecting system or renal sinus,whereas parenchymal renorrhaphy was not.For hemostasis,the soft coagulation system was used,and absorbable hemostats were placed on the resection bed.CE-CT was carried out within 7 days after surgery.Data on these patients were prospectively collected.A single radiologist determined the diagnosis of RAP.Results:Median(range)data were as follows:Patient age,65(19-82)years;radiographic tumor size,30(12-95)mm;operating time,166(102-294)min;warm ischemic time,16(7-67)min;and blood loss,15(0-4450)mL.One patient(1.6%)required a perioperative blood transfusion.No patient required conversion to open surgery or nephrectomy.CE-CT was carried out at median 6(3-7)days after surgery.CE-CT showed no RAP development in all 61 patients.Urinary leakage was not observed.One patient had acute cholecystitis,a postoperative complication classified as Clavien-Dindo grade higher than 3,which was treated with cholecystectomy.Positive surgical margin was identified in four patients(6.6%).Conclusion:RAPN using soft coagulation and absorbable hemostats without renorrhaphy appears to be feasible and safe.Our technique could eliminate the risk of RAP.

I’m Nervous about Sharing This Secret with You:Youtube Influencers Generate Strong Parasocial Interactions by Discussing Personal Issues

Purpose:Performers may generate loyalty partly through eliciting illusory personal connections with their audience,parasocial relationships(PSRs),and individual illusory exchanges,parasocial interactions(PSIs).On social media,semi-PSIs are real but imbalanced exchanges with audiences,including through comments on influencers’videos,and strong semi-PSIs are those that occur within PSRs.This article introduces and assesses an automatic method to detect videos with strong PSI potential.Design/methodology/approach:Strong semi-PSIs were hypothesized to occur when commenters used a variant of the pronoun“you”,typically addressing the influencer.Comments on the videos of UK female influencer channels were used to test whether the proportion of you pronoun comments could be an automated indicator of strong PSI potential,and to find factors associating with the strong PSI potential of influencer videos.The highest and lowest strong PSI potential videos for 117 influencers were classified with content analysis for strong PSI potential and evidence of factors that might elicit PSIs.Findings:The you pronoun proportion was effective at indicating video strong PSI potential,the first automated method to detect any type of PSI.Gazing at the camera,head and shoulders framing,discussing personal issues,and focusing on the influencer associated with higher strong PSI potential for influencer videos.New social media factors found include requesting feedback and discussing the channel itself.Research limitations:Only one country,genre and social media platform was analysed.Practical implications:The method can be used to automatically detect YouTube videos with strong PSI potential,helping influencers to monitor their performance.Originality/value:This is the first automatic method to detect any aspect of PSI or PSR.

“My ADHD Hellbrain”: A Twitter Data Science Perspective on a Behavioural Disorder

Purpose: Attention deficit hyperactivity disorder(ADHD) is a common behavioural condition. This article introduces a new data science method, word association thematic analysis, to investigate whether ADHD tweets can give insights into patient concerns and online communication needs. Design/methodology/approach: Tweets matching "my ADHD"(n=58,893) and 99 other conditions(n=1,341,442) were gathered and two thematic analyses conducted. Analysis 1: A standard thematic analysis of ADHD-related tweets. Analysis 2: A word association thematic analysis of themes unique to ADHD.Findings: The themes that emerged from the two analyses included people ascribing their brains agency to explain and justify their symptoms and using the concept of neurodivergence for a positive self-image. Research limitations: This is a single case study and the results may differ for other topics.Practical implications: Health professionals should be sensitive to patients' needs to understand their behaviour, find ways to justify and explain it to others and to be positive about their condition.Originality/value: Word association thematic analysis can give new insights into the(self-reported) patient perspective.

Is big team research fair in national research assessments? The case of the UK Research Excellence Framework 2021

Collaborative research causes problems for research assessments because of the difficulty in fairly crediting its authors.Whilst splitting the rewards for an article amongst its authors has the greatest surface-level fairness,many important evaluations assign full credit to each author,irrespective of team size.The underlying rationales for this are labour reduction and the need to incentivise collaborative work because it is necessary to solve many important societal problems.This article assesses whether full counting changes results compared to fractional counting in the case of the UK’s Research Excellence Framework(REF)2021.For this assessment,fractional counting reduces the number of journal articles to as little as 10%of the full counting value,depending on the Unit of Assessment(UoA).Despite this large difference,allocating an overall grade point average(GPA)based on full counting or fractional counting gives results with a median Pearson correlation within UoAs of 0.98.The largest changes are for Archaeology(r=0.84)and Physics(r=0.88).There is a weak tendency for higher scoring institutions to lose from fractional counting,with the loss being statistically significant in 5 of the 34 UoAs.Thus,whilst the apparent over-weighting of contributions to collaboratively authored outputs does not seem too problematic from a fairness perspective overall,it may be worth examining in the few UoAs in which it makes the most difference.

Fabrication of iron disilicide from multilayer by helicon plasma sputtering

Helicon plasma sputtering of Fe/Si multilayer followed by thermal annealing in Ar atmosphere was used to fabricate β-FeSi2 films. [Fe 0.5nm/Si 1.6nm] 120 multilayer was deposited on Si（100） substrate and then the samples were annealed at 900℃ for 2h or at 9001000℃ for 10s. Annealed samples were characterized by X-ray diffractometry（XRD）, Rutherford backscattering spectroscopy, scanning electron microscopy and optical absorption measurement. （202）/（220） oriented β-FeSi2 films are fabricated from the multilayer according to XRD patterns. It is found that there is a very small redistribution of Fe and Si components in the films during the thermal annealing, so that a good stoichiometry of FeSi2 can be obtained. It is also revealed by scanning electron microscopy that multilayer technology can result in a smooth surface for β-FeSi2 films. Optical absorption spectra demonstrate that the films have a direct band gap of 0.9eV at room temperature.

i-Gel Supraglottic Airway for Inexperienced Anesthesia Residents

i-gel (Intersurgical Ltd., Wokingham, UK) is a new supraglottic airway device with a cuff made of thermoplastic elastomer gel. We retrospectively studied easiness of insertion and troubles in ventilation when this device was used by less experienced anesthesiologists. Consecutive eleven cases undergoing surgery under general anesthesia were studied. i-gel was successfully inserted in 7 cases (63%) at the first attempt, 2 cases at the second attempt without changing the size. In one case, the size was changed at the second attempt, which resulted in the successful insertion. There was one case of failed insertion even at the second attempt. Fiberoptic observation of the glottis showed direct contact of the cuff to the arytenoids cartilage in cases with successful insertion, however, spastic glottis, defined as contact of vocal cords was observed in 4 cases, and down-folding of the epiglottis was observed in 2 cases. During maintenance of anesthesia, ventilation trouble was observed in 3 cases. We conclude that i-gel should only be used with sufficient knowledge, preparations, skills or support from experienced physician to solve problems related to the safe use of supraglottic airway devices.

BACKGROUND Biliary atresia(BA)is a rare pediatric disease.AIM To compare the outcomes of laparoscopic portoenterostomy(Lap-PE)with those of laparotomy(Open-PE)at a single institution.METHODS The surgical outcomes of PE were retrospectively analyzed for patients with a non-correctable type of BA from 2003 to 2020.RESULTS Throughout the assessment period,119 patients received PE for BA treatment,including 66 Open-PE and 53 Lap-PE cases.Although the operation duration was longer(medians:for Open-PE,242 min;for Lap-PE,341 min;P80%for both groups for the first half year post surgery,followed by a gradual decrease with time;there were no statistically significant differences in the native liver survival rates for any durations assessed.CONCLUSION Lap-PE could be a standard therapy for BA.

Examination of Experimental Method to Extract Characteristics of Lower Limb Movement in Emergency Braking Operation

The purpose of this study is to clarify the driving behavior and reaction pattern peculiar to the elderly. In this paper, we conducted a brake operation experiment with five young people, and aimed to create an evaluation index and an experimental method that can extract the characteristics of emergency brake operation from pedaling force and lower limb movement. The results showed that in order to operate the brakes strongly and quickly, the knee was firmly flection, and then the pedaling force was increased when the knee was extended. Furthermore, it was shown that if the brakes were operated without moving the ankle joint, the operation would be quicker.

Cdx2 expression and its promoter methylation during metaplasia-dysplasia-carcinoma sequence in Barrett's esophagus

AIM:To examine how the expression of caudal type homebox transcription factor 2(Cdx2) is regulated in the development of malignancy in Barrett's esophagus.METHODS:Cdx2,mucin(MUC) series(MUC2,MUC5AC and MUC6),p53 and E-cadherin expression in Barrett's esophagus and adenocarcinoma specimens were examined by immunostaining.Isolated clusters of cells from(1) MUC2 and Cdx2-positive intestinal metaplastic mucosa;(2) MUC5AC and MUC6-positive,and MUC2 and Cdx2-negative high-grade dysplasia(HD),or intramucosal adenocarcinoma(IMC);and(3) MUC5AC,MUC6 and Cdx2-positive poorly-differentiated invasive adenocarcinoma(PDA) were analyzed by methylationspecific polymerase chain reaction using sets of primers for detecting methylation status of the Cdx2 gene.RESULTS:Most of the non-neoplastic Barrett's esophageal mucosa showing intestinal-type metaplasia with or without low-grade dysplasia was positive for E-cadherin,MUC series and Cdx2,but negative for p53.A portion of the low-grade to HD was positive for E-cadherin,MUC5AC,MUC6 and p53,but negative for MUC2 and Cdx2.The definite IMC area was strongly positive for MUC5AC,MUC6 and p53,but negative for MUC2 and Cdx2.Methylation of the Cdx2 promoter was not observed in intestinal metaplasia,while hypermethylation of part of its promoter was observed in hot dipped and IMC.Hypermethylation of a large fraction of the Cdx2 promoter was observed in PDA.CONCLUSION:Cdx2 expression is restored irrespective of the methylation status of its promoter.Apparent positive immunohistochemical results can be a molecular mark for gene silencing memory.
