Clinical features in proven and probable invasive fungal disease in children and adolescents at a pediatric referral center: a 5-year experience

Background There is limited information concerning the overall epidemiology of invasive fungal disease (IFD) in *** aim of this study was to clarify the clinical features of IFD in a tertiary pediatric care *** Patients diagnosed with proven or probable IFD at our hospital between 2011 and 2015 were retrospectively *** and probable IFD were defined according to the European Organization for Research and Treatment of Cancer/Cancer/Invasive Fungal Infections Cooperative Group and the National Institute of Allergy and Infectious Diseases Mycoses Study Group *** with possible IFD were *** The incidence of proven or probable IFD was 26 of 20,079 hospitalized patients (0.13%).The predominant underlying disease was malignancy (54%) and congenital anomaly (27%).The most common diagnosis was candidemia among the patients with proven IFD (8 of 13,62%).All the isolated pathogens in the candidemia patients were non-albicans Candida *** most common site of infection was the lungs in patients with probable IFD (11 of 13 patients,85%).In probable IFD episodes,positive β-D-glucan and galactomannan were found in 12 of 13 (92%) and 5 of 13 (38%) patients,*** but one patient (96%) received empirical antifungal *** patients underwent surgical resection of residual *** overall mortality was 23% and the attributable mortality of IFD was 12%.Conclusion Our results suggest the emergence of non-albicans Candida species as important pathogens in childhood IFD.

Differences in the Histopathology and Cytokine Expression Pattern between Chronological Aging and Photoaging of Hairless Mice Skin

Skin photoaging is a complex, multifactorial process resulting in functional and structural changes of the skin, and different phenotypes from chronological skin aging are well-recognized. Ultraviolet (UV)-irradiated hairless mice have been used as a skin photoaging animal model. However, differences in morphology and gene expression patterns between UV-induced and chronological skin changes in this mouse model have not been fully elucidated. Here we investigated differences in histopathology and cytokine expression between UV-irradiated and non-irradiated aged hairless mice to clarify the factor(s) that differentiate photoaging from chronological skin aging phenotypes. Eight-week-old HR-1 hairless mice were divided into UV-irradiated (UV-irradiated mice) and non-irradiated (control mice) groups. Irradiation was performed three times per week for 10 weeks. In addition, 30-week-old HR-1 hairless mice were reared until 70 weeks of age without UV irradiation (aged mice). Histopathologies revealed that the flattening of dermal-epidermal junctions and epidermal thickening were observed only in UV-irradiated mice. Decreases in fine elastic fibers just beneath the epidermis, the thickening of elastic fibers in the reticular dermis, and the accumulation of glycosaminoglycans were more prominent in UV-irradiated mice as compared to non-irradiated aged mice. Quantitative PCR analyses revealed that UV-irradiated mice showed an increase in the expression of IFN-γ. In contrast, aged mice exhibited proportional up-regulation of both pro-inflammatory and anti-inflammatory cytokines. The IFN-γ/IL-4 ratio, an indicator for the balance of pro-inflammatory and anti-inflammatory cytokines, was significantly higher in UV-irradiated mice as compared to control and non-irradiated aged mice. An elevated IFN-γ/IL-4 ratio was also observed in aged senescence-accelerated mouse-prone 1 (SAMP1) mice, a spontaneous skin photoaging model we recently reported. Extraction Method and Determination of Sudan I Present in Sunset Yellow FCF by Isocratic High-Performance Liquid Chromatography

A method to extract and analyze Sudan I present in Sunset Yellow FCF (SYF) products was developed and validated. The method included the simple extraction of Sudan I from the SYF product using water, acetonitrile, and ethyl acetate and high-performance liquid chromatography (HPLC) analysis with isocratic elution using acetonitrile:water (7:3) with a photodiode array detector at 485 nm. This method was found to remove most of the excess SYF colorant and other impurities before injection to the HPLC instrument, making it easy to maintain precision control in routine laboratory tests for Sudan I in the SYF colorant. The detection limit of Sudan I in SYF products was 0.2 μg/g. A survey conducted to determine Sudan I in 13 commercial SYF samples from Japanese manufacturers from 1970 to 2010 showed that the levels of Sudan I ranged from 0.3 to 1.9 μg/g in products manufactured from 1970 to 1996 and were below the limit of detection in products manufactured after 2005.
