Assessment of Knowledge and Attitude in a Sample of Patients with Rheumatoid Arthritis and Its Association with Disease Activity and Severity: A Cross-Sectional Study

Background: Rheumatoid arthritis (RA) is a chronic and disabling disease that has a major impact on the lives of patients. Objectives: To test the patients’ knowledge about their disease, its treatment, its complications, and if this affects severity of the disease as measured by the disease activity index (DAS 28). Patients and Methods: A sample of 100 patients with RA who met the Criteria of American College of Rheumatology for RA agreed to participate in this study over the period September 2011-March 2012. Patients’ data were obtained by personal interview. The questionnaire included demographic characteristics and the patients’ knowledge about their disease. The disease activity was measured using standard (DAS28). Results: Thirty three percent of the patients didn’t have an idea about their disease, 20% didn’t know the reason for the investigations, 49% didn’t know the treatment and 40% didn’t know the side effects of their medications. Most patients had a high disease activity index, and there was a poor correlation between patients’ educational level and the disease activity. Conclusions: Neither the educational level nor the frequency of hospital admissions had effects on the knowledge about this disease. As most patients had a high disease activity, a better knowledge may improve disease control and prevent complications.

成功建立了人增生性瘢痕细胞和正常皮肤成纤维细胞的原代培养,并利用热休克蛋白(HSP47)和成纤维细胞特异蛋白(FSP)标记物进行了鉴定。研究发现,经过壳聚糖衍生物处理,人增生性瘢痕成纤维细胞和正常皮肤成纤维细胞在培养中均出现了不同类型的蛋白表达。多功能转录因子蛋白(CTCF)在壳聚糖衍生物处理的增生性瘢痕成纤维细胞中出现表达上调;在聚糖衍生物处理的正常皮肤成纤维细胞中数量无变化。YB-1结合蛋白在经壳聚糖处理的正常皮肤成纤维细胞与人增生性瘢痕细胞中的表达几乎无异,但在未经壳聚糖处理的细胞中表达不同。C-MYC和P53蛋白在壳聚糖衍生物处理的增生性瘢痕纤维细胞中表达上调,但在正常皮肤成纤维细胞中,无论是否经过壳聚糖衍生物处理,这两种蛋白都没有表达。上述4种蛋白在人增生性瘢痕细胞和正常皮肤成纤维细胞中表现出不同的表达方式,这种新型壳聚糖衍生物可能在控制人增生性瘢痕细胞和正常皮肤成纤维细胞生长和增殖过程中起着重要作用。这些蛋白因子的表达机制目前还不是完全清楚,有待于进一步研究。

Internal Jugular Vein Graft after Inadvertent Severing of the Internal Carotid Artery during Carotid Endarterectomy and an Urgent Re-Exploration for Immediate Post-Operative Wound Site Bleeding: A Case Report

Carotid endarterectomy is a well-established treatment for preventing stroke in selected patients. Although there is debate over whether patch angioplasty or primary closure should be used to reconstruct the bifurcation after carotid endarterectomy, there is growing evidence in the literature in favor of patch angioplasty. When compared to primary closure, patch angioplasty during conventional carotid endarterectomy is suggested to lower the incidence of restenosis and recurrent ipsilateral stroke. Various materials have been used as a patch in this procedure, including the saphenous vein, synthetic patches, or less frequently, an internal jugular vein patch where extensive narrowing of the internal carotid artery is evident. In our case, we used an internal jugular vein graft after inadvertent severing the internal carotid artery (ICA) during carotid endarterectomy after the failure of reconstruction with a saphenous vein patch. We also encountered immediate postoperative reactionary hemorrhage following anesthetic reversal, necessitating an urgent re-exploration. The purpose of this case report is neither an attempt to suggest all patients need angioplasty nor to state that an internal jugular vein patch or graft is superior to synthetic material or saphenous veins;rather, it is an attempt to emphasize a potentially effective rescue way to reconstruct inadvertent extensive vascular injury during carotid endarterectomy.

IgG4 related disease is a relatively recently recognized systemic entity, that has been observed in nearly every organ system, with similar histopathological features across all identified organs. IgG4 is an antibody molecule accounting for less than 5% of the total IgG count in a healthy person. The molecule is implicated in numerous autoimmune diseases;however its role in IgG4 related disease, whether primary or secondary, remains unknown. The gold standard in the diagnosis of this condition is demonstration of organ involvement from biopsy specimens. In this case report, the patient had exhibited symptoms related mainly to the gastrointestinal tract for 12 years prior to his diagnosis of IgG4 related disease. After this patient’s diagnosis of IgG4 related disease, he was commenced on Rituximab, a monoclonal antibody, which was successful in subsiding his symptoms and he has remained symptom free for the past two years of follow up since commencing treatment and hadn’t developed any complications of the disease nor any complications related to treatment. IgG4 related disease is a systemic autoimmune disease that involves multiple organs 60% - 90% of the time, therefore clinicians should be aware that IgG4 related disease can mimic some autoimmune rheumatic diseases and place it as a differential diagnosis when multi-organ disease with systemic features is involved.

Are potassium levels in children with hemolytic uremic syndrome predictive of outcome?

Objective: To ascertain the role of serum potassium levels in predicting clinical outcomes in diarrhea-associated hemolytic uremic syndrome (HUS D+). Methods: We reviewed clinical and laboratory data from HUS D+ patients at our tertiary care institution from 2001 to 2008. Serum potassium concentration at presentation and during the acute phase of acute renal failure were recorded and related to laboratory parameters and clinical outcomes. Results: 15 HUS D+ cases were identified. E. coli 0157:H7 was found in 9/15 cases (70%). Potassium levels were not predictive of clinical outcomes. normal serum potassium levels were found in the majority of patients. Potassium levels nor the presence of renal failure resulted in clinically significant changes in serum potassium levels.

Generating thermal energy for temperatures more than 100 degrees centigrade, especially for high capacities, is not easy nor cost efficient. Absorbing chillers with liquid absorber could be used for this application yet there is a limitation for the thermal source which should be in a high temperature. Thus, the desired situation is to use absorbing chillers with least limitations so that the cost for cooling systems would significantly decrease. To this end, we substituted "solid absorbers" instead of liquid absorbers in the absorbing chillers. Beside the advantages of the proposed chillers, e.g. the required temperature for this chiller is low hence the costs would be less, there are some draw backs in implementing the solid absorbers. Hence, the Scientific Air conditioning institute of Iran Technical ＆ Vocation University – Mohajer center, started to work on reducing the absorbing cycle problems. The results of five years researches in this institute came to devising new absorbing chiller using ‘solid absorber’. Main novelties of this system could be mentioned as: the type of the used absorber, the increase of controlling accuracy, efficiency coefficient, the life time, and also the decrease in running time of cycle. In this paper we discuss absorbing chillers with solid absorbers thoroughly. Moreover, the mechanical cycle, thermodynamic cycles and thermodynamic processes are numerically analyzed. We compared our results with the commonly used chillers. The design method, advantages and disadvantages of cycle are illustrated briefly.

A Novel Y-Suture Transfixation Technique to Improve Graft Adherence in High-Risk Descemets Stripping Automated Endothelial Keratoplasty

Purpose: To report a novel Y-suture transfixation technique to improve success rate of high-risk Descemets stripping automated endothelial keratoplasties (DSAEK). Design: Retrospective non-comparative case series. Participants: Twenty five high-risk patients undergoing DSAEK at one tertiary care institution. Methods: A retrospective evaluation of consecutively enrolled high-risk DSAEK cases performed by three surgeons at one institution during a 3-year period (2007-2010). Main outcome measures were graft adherence and suture-related complications. Results: Highrisk factors were defined as followed: prior incisional glaucoma surgery (58%), presence of vitreous in the anterior chamber (15%), anterior chamber intraocular lens (8%), graft dislocation (8%), previous DSAEK (8%), and iris trauma/aphakia (3%). Twenty-five high-risk patients underwent DSAEK with Y-suture transfixation. Postoperative follow-up ranged from 3 to 27 months. One patient had dislocation due to hypotony from prior trabeculectomy (4%), requiring repeat DSAEK. Neither primary graft failure nor suture-related complication was observed. Conclusion: The Y-suture transfixation technique is associated with improved success rate of high-risk DSAEK transplant.

This case report aims to report an uncommon case of bee sting keratopathy with retained stinger.A 55-year-old man presented with left cornea bee sting while gardening two days prior to first *** complained of severe eye pain with redness,tearing and blurring of *** examination,his right eye visual acuity was 6/6 and in left eye was hand *** was generalized conjunctival hyperemia and cornea showing significant descemet striae.A bee stinger with surrounding infiltration noted at 2 o'clock was associated with striate *** was deeply seated at the posterior third of cornea stroma near to paracentral *** was mid-dilated with absence of relative afferent pupillary *** was neither hypopyon nor *** posterior segment could not be visualized due to severe corneal ***,B-scan ultrasound was *** stinger was removed under local anaesthesia on the day of ***-operatively,patient was administered with topical moxifloxacin and topical non steroidal anti-inflammatory *** weeks later,there was resolution of cornea infiltrate with improvement of striate keratitis and his vision was improved to 1/***,cornea edema did not regress but ended up with bullous *** patient has undergone descemet-stripping automated endothelial keratoplasty and his vision was improved to 6/*** recommend early stinger removal to reduce the possible sequelae of bee sting toxicity for better visual outcome.

Effect of Plumeria alba＇s Bioactive Extract against Colletotrichum gleosporoides in Vitro and in Vivo

Plant species in the genus Plumeria of Apocynaceae family are well known for their uses in traditional medicine antimicrobial activity. Plant materials from P. alba were extracted using Soxhlet apparatus and screened for the control of anthracnose disease caused by Colletotrichum gleosporoides on fruit during post harvest storage. For in vitro, the crude extract of P. alba (stem bark and leaves) were completely inhibited conidial germination (sporulation test method) of Colletotrichum gleosporoides at all concentration tested except the lowest concentration (4.4 uL). In vivo studies show that percentage of infection on fruit after treated with both stem bark (11.11 ? 3.8) and leaves (15.55 ? 3.8) part, at 32,000 mg/L have no significant differences during seven days of storage at ambient temperatures. These results proved that P. alba extracted (stem bark and leaves) were found to be promising as an antifungal agent and to control anthracnose disease in fruits tested.
