近年来,基于表面等离子体共振(Surface Plasmon Resonance,SPR)的光子晶体光纤(Photonic crystal fiber,PCF)生物传感器引起了很多研究人员的关注。光纤等离子共振生物传感器被广泛用于病原体、毒素、抗体和蛋白质的检测中,在健康诊断、生物医学、生化分析和环境监控等领域发挥重要作用。相比于基于Kretschmann大棱镜的传统表面等离子体共振生物传感器相比,光纤等离子共振生物传感器可在光纤内部实现折射率检测。现有的光纤等离子共振生物传感器一般利用微弱的光纤消逝场激发等离子共振效应,从而导致光纤等离子共振生物传感器灵敏度差等问题。此外,传统光纤生物传感器在光纤外部使用微流体设备来进行传感实验,因此其尺寸较大,结构也更加复杂。本文提出了一种双芯光纤等离子共振生物传感器。在光纤内部沉积了一层薄银(Ag)膜(20 nm)作为一种等离子体激发金属。与传统的光纤等离子共振生物传感器相比,本文提出的传感器将待测物质引入到光纤的中心空气孔内。这样种结构不仅可实现更小的传感器尺寸,还可以增加激发等离子共振效应的光场能量,从而传感器获得更高的灵敏度。通过仿真表明,当待测物质折射率变化范围为1.340至1.346时,通过波长解调法本文设计的双芯光纤等离子共振生物传感器可达到80,000nm/RIU灵敏度和1×104RIU的分辨率。通过幅度解调法在相同的折射率变化范围内,双芯光纤等离子共振生物传感器的灵敏度值为 997/RIU,分辨率为 1×10-4RIU。相比与传统光纤等离子共振生物传感器,改进后双芯光纤等离子共振生物传感器具有更便携的体积和更高的灵敏度。仿真表明传统基于消逝场方法的光纤等离子共振生物传感器灵敏度为10,000nm/RIU。与其相比,本文提出的双芯光纤等离子共振生物传感器最大灵敏度为80,000nm/RIU。灵敏度的提高可以提升生物传感器对分析物的检测能力。通过对各传感器各指标的比较,如结构、传感方法和折射率范围等,本文有效验证了所设计双芯光纤等离子共振生物传感器具有良好的性能。这种双芯光纤等离子共振生物传感器有望在生化检测、生物医药、环境检测等方面发挥作用。

AIM: To review the clinical presentation and surgical management of complicated hydatid cysts of the liver and to assess whether conservative surgery is adequate in the management of complicated hydatid cysts of ***:The study was carried out at Sher-i-Kashmir Institute of Medical Science,Srinagar,Kashmir,*** nine patients with hydatid disease of the liver were surgically managed from April 2004 to October 2005 with a follow up period of three *** included 27 men and 42 women with a median age of 35 *** abdominal ultrasound,computed tomography and serology established *** with jaundice and high suspicion of intrabiliary rupture were subjected to preoperative endoscopic retrograde *** with infection,rupture into the biliary tract and peritoneal cavity were categorized as complicated *** patients(26%)had complicated cysts and formed the basis for this ***:Common complications were infection(14%),intrabiliary rupture(9%)and intraperitoneal rupture(3%).All the patients with infected cysts presented with pain and *** the patients with intrabiliary rupture had jaundice,while only four with intrabiliary rupture had pain and only two had *** procedures performed in complicated cysts were:infection-omentoplasty in three and external drainage in seven;intrabiliary rupture-omentoplasty in two and internal drainage in four *** patients with intraperitoneal rupture underwent external *** was no *** postoperative morbidity was 50%in complicated cysts and 16%in uncomplicated ***:Complicated hydatid cyst of the liver can be successfully managed surgically with good long term results.

This work is comprised of anatomical study of fossil wood collected from Chnji formation (72°22'E, 32°41'N) of Miocene age exposed at Chinji National Reservoir. The material consists of a single piece of petrified wood dark brown in colour. The three dimensional sections were prepared by ground thin section technique. Microscopic examination of the characters was compared with modern and fossil wood and found comparable with the family Leguminosae. The closest resemblance of our fossil wood is with the wood of Ougenia. This fossil wood shows close resemblance in respect of vessels, fibers, parenchyma and xylem rays character. Therefore it is considered as new species and it is named Ougenioxylon chienjiensis sp. nov. The specific epithet refers to Chinji formation to which fossil wood belongs.

The deployment of Relay Nodes (RNs) in 4G LTE-A networks, mainly originating from the wireless backhaul link, provides an excellent network planning tool to enhance system performance. Better coordination between the base station and relays to mitigate inter-cell interference becomes an important aspect of achieving the required system performance, not only in the single-cell scenario, but also in multi-cell scenarios. In this paper, we model and analyze two basic approaches for designing a 4G LTE-A tri-sectored cellular system. The approaches are based on Antenna Selection Sectored Relaying (ASSR) and Beam Selection Sectored Relaying (BSSR). The main purpose of the proposed schemes is to enhance system performance by improving the quality of the wireless relay backhaul link. In this technique, antenna selection takes into consideration Non-Line-Of-Sight (NLOS) communication, whereas BSSR considers the case of Line-Of-Sight (LOS) communication using heuristic beam forming approach. The resource allocation problem has also been investigated for relay based cooperative LTE-A trisectored cell in the downlink. The best possible location for relay node in the sector, power allocation and MIMO channel modeling is formulated as an optimization problem with the aim of maximizing the end to end link rate and the Signal to Interference plus Noise Ratio (SINR) of 4G LTE-A systems. Power allocation/optimization has been solved by means of the duality equation of the stationary Karush-Kuhn-Tucker (KKT) cond让ion and is used to derive optimal values for the beam forming vector on both the relay as well as the access link. The performance of the proposed scheme is verified through simulations carried out using MATLAB software. The simulation results show a significant improvement in the SINR, throughput capacity, and coverage area of the 4G LTE-A cell, while guaranteeing better quality of service.

Objective: To find out the changes in lipid metabolism among patients suffering from chronic liver disease secondary to hepatitis C. Study Design: Hospital based observational study. Setting: Medical Unit-I, Ward–5, Jinnah Postgraduate Medical Centre, Karachi. Duration: July 2013 to December 2013. Patients and Methods: About 110 patients admitted in Medical Unit-I with a diagnosis of chronic liver disease were included in the study. Patients suffering from DM, HTN, CKD were excluded from the study. Fasting lipid profile was done in all cases. Results and Observations: There were 44 (40%) male and 66 (60%) female patients. Mean age of the patients was 50.18 (±11.7) years. Total cholesterol was decreased in 76 (69.09%) patients. Normal range was present in 34 (30.91%) patients. None of the patient had hypercholesterolemia. Serum triglyceride levels were low in 14 (12.72%) patients, normal in 82 (74.54%), borderline high in 7 (6.36%) and hypertriglyceridemia was seen in 7 (6.36%). HDL-c was below normal in 26 (23.63%) cases, normal in 78 (70.91%), and high in 6 (5.45%). LDL was near optimal/above optimal in only 5 (4.5%) patients. Mean TC/HDL ratio was 2.53 (±1.02). Mean LDL/HDL ratio was 1.23 (±0.73). Mean TC of HCV +ve patients was 130.5 mg/dl as compared to that of HCV –ve patients which was 82.85 mg/dl (P-value: 0.011). Mean TGs of HCV +ve group was 151.5 mg/dl while that of HCV –ve was 79.9 mg/dl (P-value: 0.025). Mean HDL & LDL levels were 43.67 mg/dl and 39.78 mg/dl in HCV group while 34.83 mg/dl & 64.67 mg/dl in the other group with P-value of 0.026 and 0.081 respectively. Conclusion: When it comes to its relationship with lipid metabolism, HCV is a remarkable virus. Its interaction with lipoproteins and its ability to induce massive steatosis are quite unique and idiosyncratic. This paper is based on the results of an ethnomedicinal research work conducted in the semi-tribal mountainous area lying between District Mianwali (PunjabProvince) and District Karak (KhyberPakhtunkhwaProvince),Pakistanduring June 2009-July 2011. During field survey, questionnaires were used to interview the local inhabitants of the area. The ethnomedicinal data consists of 131 plant species (2 Pteridophytes, 7 Monocots, 122 Dicots) belonging to 48 families (2 Pteridophytes, 4 Monocots, 42 Dicots). The most commonly represented families were Amaranthaceae (9 spp.), Mimosaceae (8 spp.), Asteraceae and Papilionaceae (7 spp.) each;Solanaceae, Euphorbiaceae and Scrophulariaceae (6 spp.) each, followed by Boraginaceae, Brassicaceae, Lamiaceae and Polygonaceae (5 spp.) each. Plant specimens were identified, preserved and vouchers were deposited in the Plant Taxonomy Lab. of Department of Biological Sciences,University of Sargodha,Pakistanfor future reference. The data collected through interviews with the local people indicates that these species are mostly used in respiratory tract infections (RTI), urinary tract infections (UTI), digestive disorders, sexual problems, skin diseases, liver ailments, wound healing, blood vomiting, malaria, piles, epilepsy, jaundice, gummosis, round worms, hepatitis etc. in the area. Due to lack of scientific approach regarding harvesting and utilization of this valuable medicinal flora, many plants are quickly going towards extinction;therefore, further work is needed to create awareness in the community conservation of the precious medicinal plants.

Does Cash Incentive Effect TB Case Notification by Public Private Mix-General Practitioners Model in Pakistan?

Setting: Private healthcare providers including solo practitioners and laboratories enrolled with Public Private Mix General Practitioners model in Pakistan who were enrolled with the program and maintained participation during 2014 to 2016. Objective: To assess the change in case notification by private healthcare providers as well as effect on sputum smear positivity rate at private laboratories after introduction of incentives. Design: A comparative cross-sectional study based on routinely maintained National TB Control Program data collected before and after the introduction of incentives (2014 and 2016). Results: In 2016, TB case notification increased by 71% after cash incentives was introduced. Among the 618 private healthcare providers who enrolled and maintained participation in the Public Private Mix program, only 1% of the GPs contributed to 19% increase in case notification, whereas 27% GPs had reported a reduced number of cases compared to the pre-intervention period. After incentives, slide positivity rate for diagnostic declined by 1.9% and follow-up increased by 0.6%. Number of slides for diagnostic and follow-up testing increased by 82% and 72% respectively. Conclusion: Engaging the private healthcare sector, by providing cash incentives to private health care Provider’s is an effective strategy for providing a substantial increase in National TB case notification.
