Selective embolization for bleeding visceral artery pseudoaneurysms in patients with pancreatitis

BACKgROUND:Pancreatitis is associated with arterial complications in 4%-10%of patients,with untreated mortality approaching 90%.Timely intervention at a specialist center can reduce the mortality to 15%.We present a single institution experience of selective embolization as first line management of bleeding pseudoaneurysms in pancreatitis. METHODS:Sixteen patients with pancreatitis and visceral artery pseudoaneurysms were identified from searches of the records of interventional angiography from January 2000 to June *** visceral artery aneurysms and pseudoaneurysms arising as a result of post-operative pancreatic or biliary leak were excluded from the study. RESULTS:In 50%of the patients,bleeding complicated the initial presentation of *** was the offending agent in 10 patients,gallstones in 3,trauma,drug-induced and idiopathic pancreatitis in one *** 16 patients had a contrast CT scan and 15 underwent coeliac axis angiography. The pseudoaneurysms ranging from 0.9 to 9.0 cm affected the splenic artery in 7 patients:hepatic in 3,gastroduodenal and right gastric in 2 each,and left gastric and pancreatico-duodenal in 1 *** patient developed spontaneous thrombosis of the *** patients had effective coil embolization of the *** patient needed surgical exclusion of the pseudoaneurysm following difficulty in accessing the coeliac axis *** were no episodes of re-bleeding and no in-hospital mortality. CONCLUSIONS:Pseudoaneurysms are unrelated to the severity of pancreatitis and major hemorrhage can occur irrespective of their ***-existent portal hypertension and sepsis increase the risk of *** and selective coil embolization is a safe and effective way to arrest the hemorrhage.

小麦在高温烘焙或油炸等加工过程中易产生一种具有神经、遗传和生殖毒性的2A类致癌物质—丙烯酰胺,其引发的食品安全问题已广受关注。已有研究表明,小麦中游离的天冬酰胺是形成丙烯酰胺的主要前体物质,其籽粒中游离天冬酰胺的含量与高温加工过程中形成丙烯酰胺的含量呈极显著正相关。近年来,国内外对小麦中天冬酰胺积累的调控研究已取得一定进展,主要集中在栽培策略和分子育种等方面。一方面,禾谷类作物的栽培措施影响游离天冬酰胺的累积。增施氮肥和缺失硫肥均能导致游离天冬酰胺含量的积累。另一方面,国内外研究主要针对天冬酰胺合成途径中的关键酶-天冬酰胺合成酶(Asparagine Synthetase, ASN)开展了深入分子机理研究。天冬酰胺合成酶以氨或谷氨酰胺及天冬氨酸为底物直接催化生成天冬酰胺,但是由于天冬酰胺合成酶的不稳定性,粗提物中含有热稳定的抑制子和天冬酰胺酶的活性,使得该合成酶的提取、纯化及分析非常困难,从而限制了该方法的使用,最有效的途径是通过分离天冬酰胺合成酶的基因家族并研究其功能。前期已在小麦中分离出天冬酰胺合成酶家族基因TaASN1、TaASN2和TaASN3,比较了基因的结构,发现编码蛋白的氨基酸数目分别为585、581和591个,相互之间的相似性为74.6%～84.9%。依据酶促反应动力学,证实TaASN1和TaASN2基因编码的天冬酰胺合成酶催化合成天冬酰胺的效率较高。其中,TaASN1为响应硫胁迫控制天冬酰胺合成的关键基因,TaASN2为籽粒中特异性表达的基因,目前我们已经通过RNAi和Crispcas9技术对TaASN2基因进行了编辑,进一步验证其功能,同时,通过酵母单杂和竞争性凝胶迁移试验筛选获得了与TaASN1基因特异性结合的转录因子,以挖掘其他可能调控天冬酰胺合成的潜在基因。该研究为从农业生产源头降低小麦加工食品中的丙烯酰胺形成,培育符合食品安全要求的小麦品种提供重要线索。

Diagnostic accuracy of tests for Helicobacter pylori in an Alaska Native population

AIM:To evaluate the accuracy of two non-invasivetests in a population of Alaska Native persons. Highrates of Helico bacter pylori(H. pylori) infection,H. pylori treatment failure,and gastric cancer in this population necessitate documentation of infection status atmultiple time points over a patient's ***:In 280 patients undergoing endoscopy,H. pylori was diagnosed by culture,histology,rapidurease test,13C urea breath test(UBT) ,and immuno-globulin g antibodies to H. pylori in serum. The performances of 13C-UBT and antibody test were compared to a gold standard defined by a positive H. pylori testby culture or,in case of a negative culture result,bypositive histology and a positive rapid urease ***:The sensitivity and specificity of the 13C-UBT were 93% and 88%,respectively,relative to thegold standard. The antibody test had an equivalents ensitivity of 93% with a reduced specificity of 68%.The false positive results for the antibody test were as-sociated with previous treatment for an H. pylori infection [relative risk(RR) = 2.8]. High levels of antibodiesto H. pylori were associated with chronic gastritis and male gender,while high scores in the 13C-UBT testwere associated with older age and with the H. pyloribacteria load on histological examination(RR = 4.4) .CONCLUSION:The 13C-UBT out performed the anti-body test for H. pylori and could be used when a non-invasive test is clinically necessary to document treatment out come or when monitoring for reinfection.

The Influence of Avian Biodiversity and a Weedy Understorey on Canopy Arthropod Assembly

Identifying the factor causing species decline from a multitude of potential disturbances is essential for successful management. Invasive species are often drivers of decline, either through direct effects such as recruitment limitation, or through indirect effects such as habitat modification that facilitate biota changes in other taxa. In this study, we tested the importance of bird predation on arthropods in eucalypt canopies in wet sclerophyll forest that had been invaded by the understorey weed Lantana camara. A strong top-down effect is in agreement with studies that show elevated numbers of insectivorous birds, such as the despotic bell miner, Manorina melanophrys, reduce damaging herbivorous insect numbers and their effect on tree health. Abundance, order and family composition were compared among sites, feeding guilds, among the presence and absence of a despotic bird, and among presence and absence within sites, using a three-way nested Multivariate Analysis of Variance (MANOVA), and tested our representation of insect feeding guilds with rarefaction curves. In total, we found arthropods from 20 families on eucalypt trees, 16 where bell miners were present and 18 where they were absent (a sampling efficiency of 78% and 92% of families based on the Chao-I index). Overall, there was a difference in the abundance of insects in areas where bell miners were present (n = 181), and where they were absent (n = 67). There was also a difference in the families present in areas with bell miners and without them. Under some conditions, despotic birds may not change the insect canopy community. Further studies should examine the effects of a despotic bird on the insect canopy community in a variety of conditions and throughout their range.

On the mechanical behavior of the human biliary system

This paper reviews the progress made in understanding the mechanical behaviour of the biliary system. gallstones and diseases of the biliary tract affect more than 10% of the adult population. The complications of gallstones, i.e. acute pancreatitis and obstructive jandice, can be lethal, and patients with acalculous gallbladder pain often pose diagnostic difficulties and undergo repeated ultrasound scans and oral cholecystograms. Moreover, surgery to remove the gallbladder in these patients, in an attempt to relieve the symptoms, gives variable results. Extensive research has been carried out to understand the physiological and pathological functions of the biliary system, but the mechanism of the pathogenesis of gallstones and pain production still remain poorly understood. It is believed that the mechanical factors play an essential role in the mechanisms of the gallstone formation and biliary diseases. However, despite the extensive literature in clinical studies, only limited work has been carried out to study the biliary system from the mechanical point of view. In this paper, we discuss the state of art knowledge of the fluid dynamics of bile flow in the biliary tract, the solid mechanics of the gallbladder and bile ducts, recent mathematical and numerical modelling of the system, and finally the future challenges in the area.

Connecting muon anomalous magnetic moment and multi-lepton anomalies at LHC

In a previous paper by several of the authors a number of predictions were made in a study pertaining to the anomalous production of multiple leptons at the Large Hadron Collider(LHC). Discrepancies in multi-lepton final states have become statistically compelling with the available Run 2 data. These could be connected with a heavy boson, H, which predominantly decays into a standard model Higgs boson, h, and a singlet scalar, S, where mH≈270 geV and mS≈150 geV. These can then be embedded into a scenario where a two-Higgs-doublet is considered with an additional singlet scalar, 2HDM+S. The long-standing discrepancy in the muon anomalous magnetic moment,△aμ, is interpreted in the context of the 2HDM+S type-II and type-X, along with additional fermionic degrees of freedom. The 2 HDM+S model alone, with constraints from the LHC data, does not seem to explain the △aμ ***, adding fermions with mass of order O(100) geV can explain the discrepancy for sufficiently low values of fermion-scalar couplings.

Microvascular response to transfusion in elective spine surgery

AIM To investigate the microvascular(skeletal muscle tissue oxygenation; SmO_2) response to transfusion in patients undergoing elective complex spine *** After IRB approval and written informed consent, 20 patients aged 18 to 85 years of age undergoing > 3level anterior and posterior spine fusion surgery were enrolled in the study. Patients were followed throughout the operative procedure, and for 12 h postoperatively. In addition to standard American Society of Anesthesiologists monitors, invasive measurements including central venous pressure, continual analysis of stroke volume(SV), cardiac output(CO), cardiac index(CI), and stroke volume variability(SVV) was performed. To measure skeletal muscle oxygen saturation(SmO_2) during the study period, a non-invasive adhesive skin sensor based on Near Infrared Spectroscopy was placed over the deltoid muscle for continuous recording of optical spectra. All administration of fluids and blood products followed standard procedures at the Hospital for Special Surgery, without deviation from usual standards of care at the discretion of the Attending Anesthesiologist based on individual patient comorbidities, hemodynamic status, and laboratory data. Time stamps were collected for administration of colloids and blood products, to allow for analysis of SmO_2 immediately before, during, and after administration of these fluids, and to allow for analysis of hemodynamic data around the same time points. Hemodynamic and oxygenation variables were collected continuously throughout the surgery, including heart rate, blood pressure, mean arterial pressure, SV, CO, CI, SVV, and SmO_2. Bivariate analyses were conducted to examine the potential associations between the outcome of interest, SmO_2, and each hemodynamic parameter measured using Pearson's correlation coeffi-cient, both for the overall cohort and within-patients individually.
