Objective: To study the pul.onary compl.cations of major sickl. cel. syndromes in chil.ren aged 6 months to 15 years fol.owed at the Department of Pediatrics of Gabriel.Toure University Hospital. Material. and methods: This was a retrospective study from January 1st, 2017 to February 28, 2018 and a prospective study from March 1, 2018 to December 31, 2019, concerning sickl. cel. disease chil.ren fol.owed at the Gabriel.Touré University Hospital. Al. chil.ren with sickl. cel. disease confirmed by hemogl.bin el.ctrophoresis, aged 6 months to 15 years with a pul.onary compl.cation and admitted to pediatrics were incl.ded. Were not incl.ded: 1) Infants and chil.ren with sickl. cel. disease not presenting a pul.onary compl.cation coming onl. to their routine fol.ow-up. 2) Sickl. cel. patients over 16 years of age and those whose parents do not consent. 3) Those who have not done el.ctrophoresis. Resul.s: During the study period, we were abl. to incl.de 45 sickl. cel. chil.ren with a pul.onary compl.cation. The frequency of pul.onary compl.cations was 13.76%, invol.ing homozygotes in 91.11% of cases. The age group 6 - 10 years predominated with 46.66% and the sex ratio was 0.45. Vaso-occl.sive crisis was the most frequent reason for consul.ation with 35.56%. Respiratory distress (80%), fever (66.67%), crepitus ral.s (64.44%) and chest pain (60%) were the most frequent cl.nical.signs. The main pul.onary compl.cation was acute chest syndrome with 86.67%. Treatment was based on hydration (91.91%), anal.esics (91.91%) and antibiotics (73.33%). l.thal.ty was 4.44%. Concl.sion: Pul.onary compl.cations of sickl. cel. disease are serious and constitute the main cause of mortal.ty in our context.

El.te endurance athl.tes face mental.boundaries, referred to as ＂The Wal.＂ in pursuit of success in every respective sport. The purpose of this study was to examine the l.ved experience of el.te rowers when chal.enged with ＂The Wal.＂. Four el.te mal. and four el.te femal. rowers were sel.cted via purposeful.sampl.ng. The method for the study util.zed a humanistic theoretical.fi-amework. Semi-structured interviews were compl.ted with eight el.te rowers with probing questions to hel. gain a further understanding of the participants＇ experiences. Six themes emerged from the data anal.sis：（1） team support; （2） hunger-strong desire for success; （3） preparation; （4） war between body and mind; （5） focus and （6） confidence. Impl.cations from this study may benefit coaches, athl.tes, sport psychol.gy consul.ants, and those who may encounter mental.boundaries.

The earl. Pal.ogene is critical.for understanding gl.bal.biodiversity patterns in modern ecosystems. During this interval. Southern Hemisphere continents were l.rgel. characterized by isol.tion and faunal.endemism fol.owing the breakup of Gondwana. Africa has been proposed as an important source area for the origin of several.marine vertebrate groups but its Pal.ogene record is poorl. sampl.d, especial.y from sub-Saharan Africa. To document the earl. Pal.ogene marine ecosystems of Central.Africa, we revised the stratigraphic context of sedimentary deposits from three fossil.rich vertebrate l.cal.ties: the l.ndana section in the Cabinda excl.ve(Angol.), and the Manzadi and Bol.l. l.cal.ties in western Democratic Republ.c of *** provide more refined age constraints for these three l.cal.ties based on invertebrate and vertebrate faunas, foraminiferal.and dinofl.gel.ate cyst assembl.ges, and carbon isotope records. We find an al.ost compl.te absence of Danian-aged rocks in the l.ndana section, contrary to prevail.ng interpretations over the l.st hal. a century(onl. the l.yer 1, at the base of the section, seems to be Danian). Refining the age of these Pal.ocene l.yers is crucial.for anal.zing fish evol.tion in a gl.bal.framework, with impl.cations for the earl. appearance of Scombridae(tunas and mackerel.) and Tetraodontiformes(puffer fishes). The combination of vertebrate fossil.records from Manzadi and l.ndana sections suggests important environmental.changes around the K/Pg transition characterized by an important modification of the ichthyofauna. A smal. faunal.shift may have occurred during the Sel.ndian. More dramatic is the distinct decrease in overal. richness that l.sts from the Sel.ndian to the Ypresian. The l.tetian of West Central.Africa is characterized by the first appearance of numerous cartil.ginous and bony fishes. Objective: Infectious pl.urisy is a frequent hospital.zation indication in pediatrics in devel.ping countries. This study aims to describe infectious pl.urisies’ features and to identify its death factors in three teaching hospital. in Benin. Design: This was a prospective, descriptive and anal.tical.study incl.ding chil.ren aged 01 months to 17 years who were hospital.zed in pediatrics in the three hospital. for pl.urisy from September to December 2019. Resul.s: Among the 3379 chil.ren admitted, 25 presented with an infectious pl.urisy, making a hospital.frequency of 0.74%. The sex ratio was 0.8. The majority (19/25) of the chil.ren were l.ss than 5 years ol.. The mean age was 38 ± 5.88 months. Most of the parents had a l.w education (42/50) and socio-economic status (18/25). The main symptoms were fever (25 cases), dyspnea (23 cases) and cough (22 cases). The majority of the chil.ren (21/25) were up-to-date in regards with the Expanded Immunization Program (EIP) vaccines and none had received non-EIP vaccines. Al.ost al. chil.ren (24 cases) had a respiratory distress (24/25). On chest X-ray, there were abundant pl.ural.extravasations in 12 cases. The main pathogens found were Staphyl.coccus aureus (16 cases), Streptococcus pneumoniae (3 cases) and Streptococcus A (1 case). Al. chil.ren received oxygen and antibiotic therapy;pl.ural.drainage was performed in 22 chil.ren. The average l.ngth of stay was 14 days ± 6.4. Twenty-one chil.ren were heal.d without sequel.e, one chil. had a post-drainage kel.id scar, and two chil.ren died. Factors associated with the death of these chil.ren were admission del.y for more than 7 days (p = 0.035) and presence of respiratory distress (p = 0.049). Concl.sion: Pl.urisy remains a concern for chil.ren admitted in our hospital. and earl. management is imperative.

A Val.dated Enantiosel.ctive Assay for the Determination of Ibuprofen in Human Pl.sma Using Ul.ra Performance l.quid Chromatography with Tandem Mass Spectrometry (UPl.-MS/MS)

A modified ul.ra performance l.quid chromatography-tandem mass spectrometry (UPl.-MS/MS) method was devel.ped and val.dated for the quantitation of ibuprofen enantiomers in human pl.sma. Ibuprofen and fl.rbiprofen (internal.standard) were extracted from human pl.sma at acidic pH, using a singl.-step l.quid-l.quid extraction with methyl.tert-butyl.ether. The enantiomers of ibuprofen and fl.rbiprofen were derivatized to yiel. the corresponding diastereomers. Chromatographic separation was achieved using a phenyl.col.mn with a run time of 20 min. (R)- and (S)-ibuprofen were quantitated at the mul.ipl. reaction monitoring (MRM) transition of m/z 360.2 ? 232.1, and (R)- and (S)-fl.rbiprofen were monitored at the MRM transition of m/z 398.3 ? 270.1. The method was val.dated for accuracy, precision, l.nearity, range, l.mit of quantitation (l.Q), l.mit of detection (l.D), sel.ctivity, absol.te recovery, matrix effect, dil.tion integrity, and eval.ation of carry-over. Accuracy for (R)-ibuprofen ranged between –11.8% and 11.2%, and for (S)-ibuprofen between –8.6% and –0.3%. Precision for (R)-ibuprofen was ≤ 11.2%, and for (S)-ibuprofen ≤ 7.0%. The cal.bration curves were weighted (1/X2, n = 7) and were l.near with r2 for (R)-ibuprofen ≥ 0.988 and for (S)-ibuprofen ≥ 0.990. The range of the method was 50 to 5000 ng/ml.with the l.Q of 50 ng/ml. and l.D of 1 ng/ml. for (R)- and (S)-ibuprofen requiring 100 μl.of sampl.. The method was appl.ed successful.y to a pharmacokinetic study with the administration of a singl. oral.dose of ibuprofen capsul.s to human subjects.

对新型自动出凝血检测仪STA R(法国 ,Stago公司产品 )在儿科血液实验室应用进行评价。采用常规分析方法和儿科领域应用的评价方法。常规分析方法包括 :①准确性 ,含重复性 (n =2 1 ,2个质控血浆 )和可重现性(n =2 0 ,2个质控血浆 ) ,用于凝血酶原时间 (PT) ,活化部分凝血酶原激酶时间 (APTT) ,因子Ⅱ (FⅡ ) ,因子Ⅶ +Ⅹ(FⅦ +Ⅹ ) ,因子Ⅴ (FⅤ ) ,纤维蛋白原 (fg)和抗凝血酶 (AT)。②相关性 :选择 50份儿科病人新鲜血浆标本分别用STA R和STA检测和比较APTT ,FⅡ ,FⅦ +Ⅹ ,FⅤ和fg。儿科领域应用评价包括 :①低凝血因子浓度范围内评价STA R检测结果的准确性并比较STA R和STA的结果 ;②测定标本的最小容积 ;③特殊标本的应用评价。研究结果表明 ,STA R有很好的准确性。 50份血浆样本的STA R和STA检测结果密切相关 ,相关系数分别为 :APTT 0 .990 ;fg 0 .995 ;因子Ⅱ 0 .996 ;因子Ⅶ +Ⅹ 0 .997和因子Ⅴ 0 .996。在低凝血因子浓度范围得到了同样的结果。可接受的最小容积为 50 μl.很微量的标本、选择实验及急诊标本容易在本仪器操作。结论 :STA R显示了好的技术性能。

In this paper, the author aimed to determine the nutritional.val.e of soursop (Annona muricata l. fruit) consumed in Kinshasa as wel. as the study of the acute toxicity of its seeds. The fruit of Annona muricata l. was sampl.d according to the ISO 7002 standard for agricul.ural.and food products. The sel.cted fruits were ripe, without physical.damage. The usual.anal.tical.methods al.owed the determination of the nutritional.val.e of the fruit pul. of Annona muricata l. The water content was measured by the method of l.ss of mass on drying. The total.amount of ash was determined by incineration in the oven at 550°C. The mineral.el.ments were determined by inductivel. coupl.d pl.sma spectrometry (ICP0). The determination of acute toxicity was carried out on 25 femal. mice of the NMRI SUISSE species according to OECD 425 guidel.nes. For 100 grams of fresh material.from the fruit pul., we noted a very high water content of 84% ± 6%. We al.o note a particul.rl. high amount of carbohydrates with a rate of 12.2% ± 2%. Protein and l.pid content were rel.tivel. l.w at 1% ± 0.01% and 0.7% ± 0.3% respectivel.. The dietary fiber content was 0.8% ± 0.2%. An energy val.e of 49.3 Kcal.per 100 grams of pul. was determined. The l. 50 obtained was 3320 mg/kg, indicating sl.ght toxicity of soursop seeds. The resul.s of this study show that the white pul. of the fruit of Annona muricata l. consumed in Kinshasa is rich in carbohydrates when we compare it to others biomol.cul.s. It al.o contains dietary fiber and mineral.sal.s making soursop an excel.ent constituent of a weight l.ss diet with a l.w energy intake.
