Gastrointestinal stromal tumors (GIST) account for 2% of all tumors of the digestive tract. They are most frequently located in the stomach followed by the small intestine and most often present with mucosal ulceration and erosion associated with gastrointestinal bleeding. Bleeding from the small intestine can be difficult to diagnose and angiography becomes a useful diagnostic and therapeutic procedure in these patients. We present a case of a GIST located in the ileum which presented with a lower gastrointestinal bleeding (lGIB). The patient was stabilized by performing an arteriography and selective embolization of the bleeding point, followed by elective surgery.

＜正＞ The MAPEC study was designed to investigate whether normalizing the circadian blood pressure （BP） profile towards a more dipper pattern reduces cardiovascular risk. This prospective study investigates 2643 subjects （1328 men）, 51.9±14.1 years of age. At inclusion and yearly thereafter, BP was measured at 20 - min intervals from 07:00 to 23: 00h and at 30 - min intervals at night for 48 hours. The median time of follow up so far was 3.2 years. Based on the baseline BP profile, cardiovascular morbidity was similar for extreme-dippers （1.23 events/100 patient-years） and dippers （1.14）, but significantly higher for non-dippers （2.81） and mainly for risers （8.70）. When morbidity was analyzed on the basis of the BP profile closer to the event, results indicate a diminished morbidity in extreme-dippers （0.38） and dippers （0.89）, and an increased morbidity in non-dippers （3.23） and risers （10.70）. The probability of event-free survival is markedly correlated with the day/night BP ratio. Most important, results suggest that increasing this ratio towards a more dipper pattern by Chronotherapy decreases cardiovascular risk, while decreasing the day/night BP ratio is associated with increased morbidity and mortality.

The aim of this review is to compare the effectiveness of percutaneous vertebroplasty and kyphoplasty to treat pain and improve functional outcome from ver-tebral fractures secondary to osteoporosis and tumor conditions. In 2009, two open randomized controlled trials published in the New England Journal of Medicine questioned the value of vertebroplasty in treating ver-tebral compression fractures. Nevertheless, the prac-tice of physicians treating these conditions has barely changed. The objective of this review is to try to clarify the most important issues, based on our own experi-ence and the reported evidence about both techniques, and to guide towards the most appropriate choice of treatment of vertebral fractures, although many ques-tions still remain unanswered.

Recurrent acute lymphoblastic leukaemia(All) is a common disease for pediatric oncologists and accounts for more deaths from cancer in children than any other malignancy. Although most patients achieve a second remission, about 50% of relapsed All patients do not respond to salvage therapy or suffer a second relapse and most children with relapse die. Treatment must be tailored after relapse of All, since outcome will be influenced by well-established prognostic features, including the timing and site of disease recurrence, the disease immunophenotype, and early response to retrieval therapy in terms of minimal residual disease(MRD). After reinduction chemotherapy, high risk(HR) patients are clear candidates for allogeneic stem cell transplantation(SCT) while standard risk patients do better with conventional chemotherapy and local therapy. Early MRD response assessment is currently applied to identify those patients within the more heterogeneous intermediate risk group who should undergo SCT as consolidation therapy. Recent evidence suggests distinct biological mechanisms for early vs late relapse and the recognition of the involvement of certain treatment resistance related genes as well cell cycle regulation and B-cell development genes at relapse, all providing the opportunity to search for novel target therapies.

AIM： To address the role of CD209 in celiac disease （CD） patients. Non-human leukocyte antigen （HlA） genetic factors in CD predisposition are poorly understood, and environmental factors like infectious pathogens may play a role. CD209 is a dendritic and macrophage surface molecule involved in pathogen recognition and immune activation. Recently, a functional variant in the promoter of the CD209 gene （-336A/G） has been shown to affect the transcriptional CD209 activity in vitro and it has been associated with a higher susceptibility to/or severity of infection. METHODS： The study population was composed of two case-control cohorts of 103 and 386 CD patients and 312 y 419 healthy controls as well as a panel of 257 celiac families. Genotyping for the -336A/G CD209 promoter polymorphism was performed using a TaqMan 5＇ allelic discrimination assay. HlA-DQ was determined by hybridization with allele specific probes. RESUlTS： Initially, the case-control and familial studies did not find any association of the -336 A/G CD209 genetic variant with CD susceptibility. However, the stratification by HlA-DQ2 did reveal a significant association of CD209 promoter polymorphism in the HlA-DQ2 （-） group （carrier A vs GG in DQ2 （-） vs DQ2 （＋） patients （P = 0.026, OR = 3.71）. CONClUSION： The -336G CD209 allele seems to be involved in CD susceptibility in HlA-DQ2 （-） patients. Our results might suggest a possible role of pathogens in the onset of a minor group of CD patients.

Unraveling the factors that determine variation of diversity in tropical mountain systems is a topic for debate in plant *** is especially true in areas where topography is complex due to volcano elevational gradients and where forests are vulnerable to human *** this study we used a set of climatic(temperature,rainfall,and radiation solar),topographic(elevation,slope aspect,and slope orientation)and human disturbance variables to determine their effect on diversity and composition patterns of a tree community,considering three slope aspects of a tropical volcano in southeastern *** sampled trees in seventy 0.1-ha plots distributed on three slope aspects of the Tacanávolcano along an elevational gradient of 1500 to 2500 *** determined diversity patterns(general tree richness,exponential of Shannon index,and pioneer species richness)with linear regression models,and for beta diversity,we used a dissimilarity index(within and between elevational bands 100 m wide).The effect of a set of environmental and human disturbance variables on tree diversity and community composition was analyzed with general linear models and multivariate analyses,*** registered 2,949 individual trees belonging to 176 species and *** average species richness and alpha diversity per plot were 13(standard deviation±6)and 9(±5),*** tree richness and alpha diversity increased in the middle part(unimodal patterns)of the elevational gradient,but pioneer species richness decreased linearly with *** variance explained by general linear models was greater in richness(32%)than in alpha diversity(25.3%).The most important predictor variables were temperature(elevational gradient),which explained the unimodal pattern(richness and alpha diversity increase at intermediate levels of temperature),and slope orientation,which explained the increase in richness and alpha diversity toward the geographic *** temperature had a signific

为探究团簇nico_(2)S_(4)的磁学性能,本文依据密度泛函理论(DFT)在B3lYP/lan12dz水平下对团簇nico_(2)S_(4)进行优化计算,确定了三重态下的6种优化构型,并从轨道成单电子数、自旋布居数、磁矩、自旋密度差、轨道态密度图多个角度对构型进行分析。研究表明:团簇nico_(2)S_(4)的6种构型中,构型2^((3))性能最优异,而热力学稳定性最好的构型1^((3))在磁学性能方面表现一般,其余构型磁学性能相近。对于团簇nico_(2)S_(4)的磁性强度,在原子轨道上,s、p轨道对团簇nico_(2)S_(4)整体的磁性强度贡献较小,d轨道对整体的磁性强度贡献最大;Ni原子和自旋向上的α成单电子是团簇整体磁性强度的主要贡献者,其中,s轨道成单电子数的主要贡献者是S原子,p、d轨道主要是金属原子,Ni原子d轨道主要受α成单电子影响,Co、S原子既受α成单电子的影响又受β成单电子的影响。团簇nico_(2)S_(4)作为磁性材料时采用构型2^((3))作为模型基础,可以通过增大金属Ni比例来增强磁性。

The management of patients with advanced non-small cell lung carcinoma(NSClC)has undergone major changes in recent *** the one hand,improved sensitivity of diagnostic tests,both radiological and endoscopic,has altered the way patients are *** the other hand,the arrival of new drugs with antitumoral activity,such as targeted therapies or immunotherapy,has changed the prognosis of patients,improving disease control and prolonging ***,the development of radiotherapy and surgical and interventional radiology techniques means that radical ablative treatments can be performed on metastases in any location in the *** of these advances have impacted the treatment of patients with advanced lung cancer,especially in a subgroup of these patients in which all of these treatment modalities *** poses a challenge for physicians who must decide upon the best treatment strategy for each patient,without solid evidence for one optimal mode of treatment in this patient *** aim of this article is to review,from a practical and multidisciplinary perspective,published evidence on the management of oligometastatic NSClC *** evaluate the different alternatives for radical ablative treatments,the role of primary tumor resection or radiation,the impact of systemic treatments,and the therapeutic *** short,the present document aims to provide clinicians with a practical guide for the treatment of oligometastatic patients in routine clinical practice.
